Calcific Tendinitis of Shoulder Associated with Intraosseous Involvement -A Case Report- (골내 침범을 동반한 견관절 석회화 건염 - 증례 보고 -)

  • Purpose: We present an atypical case of calcific tendinitis of the shoulder with intraosseous loculation. Materials and Methods: A 59 year-old female complained of acute exacerbation of chronic left shoulder pain and restricted range of motion. Simple radiographs showed a subacromial calcific deposit and magnetic resonance imaging revealed cortical erosion with intraosseous extension of calcific material mimicking infection or tumor. She was managed with arthroscopic excision of the calcific deposit, curettage of the intraosseous lesion and subsequent rotator cuff repair with a suture anchor. Results: Her acute pain promptly subsided. Her rehabilitation was uneventful and she gained full range of motion. Radiographs five months after the operation showed no recurrence of calcific material. Conclusion: Calcific tendinitis of the shoulder can present with a variety of images involving the adjacent bone. The correct recognition of this disorder may avoid unnecessary investigation and treatment.

Resection of Ganglion Cyst of the Posterior Cruciate Ligament using Arthroscopic Posterior Trans-septal Approach (관절경하 후격막 통과 도달법을 이용한 후방십자인대 결절종의 치료)

  • Purpose: We reviewed the effectiveness and the result of arthroscopic resection for ganglion cyst of posterior cruciate ligament(PCL) through the posterior trans-septal approach. Material and Method: Seven patients with symptomatic ganglion cyst of the PCL were treated with arthroscopic resection using posterior trans-septal approach. We analyzed advantages of the posterior trans-septal approach and presence of intra articular problem. Knee function was assessed using the lysholm knee scale. Results: The overall result for the 7 knees were very satisfied in all. We found that four of seven patients had intra articular problems, and past history of trauma could be found in 3 cases. The Lysholm knee scale was increased from average 93.3 points preoperatively to average 97.8 points postoperatively. Conclusion: Arthroscopic posterior trans-septal approach shows an effective technique for complete resection of cyst and evaluation of the combined posterior compartment injury.

Arthroscopic Ankle Arthrodesis (관절경하 족근관절 고정술)

  • Purpose : This study was conducted to analyze the results of arthroscopic ankle arthrodesis and to verify the advantages of the technique compared to open ankle arthrodesis. Materials and Methods : Between October 1992 and August 1996, the arthroscopic ankle arthrodesis had been performed in five patients(six ankle joints): two patients with seropositive rheumatoid arthritis(one patient surgically treated bilaterally), two with osteoarthritis and one with tuberculous arthritis. There were one man and 4 women. Average age was 48 years ranging from 38 to 65 years. Follow up period was average 45 months(range, $12\~80$). Results : All patients were successfully treated with ankle joint arthrodesis under arthroscopic control. The mean time to fusion was 10 weeks(range, $6\~15$). There was a $100\%$ fusion rate without any complication. Conclusion : The arthroscopic ankle arthrodesis was successful in all cases with less morbidity and short hospital stay. It was technically feasible with excellent predictability.

  • 견갑하근 건의 파열의 흔한 원 인은 대개 외상이다. 극상근 건의 파열에 비해 드문 것으로 알려져 왔으나 최근 견관절의 관절내시경 시술이 증가함에 따라 관절면의 부분 파열이 많이 보고되고 있다. 본 증례는 40세 남자 환자로 상완골 소결절에 발생한 골극에 의한 견갑하근 건의 관절면의 부분층 파열에 대해 관절경하 골극의 제거 및 견갑하근 건의 변연절제술 후 양호한 결과를 보였다

Arthroscopic Direct Removal of Popliteal Cyst by Posteromedial Portal (관절경하 후내측 도달법을 이용한 슬와 낭종의 직접적 제거술)

  • Purpose: To evaluate the effectiveness of arthroscopic direct cystic removal of popliteal cyst using 70 degree arthroscopy and posteromedial portal. Materials and Methods: From January 2003 to January 2004, 21 patients(23 cases) with popliteal cyst have been treated by direct cystic decompression with arthroscopy. The average age of the 21 patients was 54 years(range 35 to 78 years). There were 5 males and 16 females. Of the 23 cases,8 cases were occured in right side and 11 cases were occured in left side. 4 cases were both sides. In all cases, preoperative MRI were performed to detect combined intraarticular pathology. At 6 months and 1 year postoperatively, follow-up ultrasonography were performed to detect recurrence of cyst. We used Rauschning and Lindgren criteria for clinical evaluation. Results: All cases had no recurrence and no complaints of pain, swelling, or functional impairment at 1 year after surgery. At ultrasonography, no recurrence were founded. One complication was occurred. Postoperatively, Rauschning and Lindgren criteria were more than grade 1. Conclusion: Arthroscopic direct cystic removal using arthroscopy and posteromedial portal is an effective method in popliteal cyst surgery.

Snow-Man Shaped Nodular Tenosynovitis in the Knee - Case Report - (슬관절내에 발생한 눈사람 형태의 결절성 건활액막염 - 증례보고 -)

  • The authors describe one case of a 25-year-old male with nodular tenosynovitis in left knee joint who did not have a history of joint trauma. He had joint pain and restricted flexion of the left knee at 100 degrees. After arthroscopic excision of mass, symptom was relieved completely and the patient had gained full range of motion of the left knee. No recurrence of symptom and loss of motion of the left knee were noticed during follow up period for 12 months.

Arthroscopic Treatment for Degenerative Elbow Contractures (주관절 퇴행성 강직의 관절경적 처치)

  • Purpose : To describe long-term clinical results and serial changes in the postoperative range of motion(ROM) after arthroscopic treatment for a limitation of motion(LOM) of the elbow. Materials and Methods : The subjects who visited chosun university hospital from December, 1996 to January 2000 were twenty-one patients ranging from 37 to 54 years of age, and the average age was 43.2 years. The chief complaints were painful limitation of motion of the elbow and average ROM showed that flexion contracture were 17 degrees and further flexion were 87 degrees. Results : The total ROM was $70^{\circ}$ preoperatively. 2 months after postoperatively the mean flexion contracute improved from $17^{\circ}\;to\;3^{\circ}$ with further flexion from $87^{\circ}\;to\;122^{\circ}$ degrees. One year after postoperatively the mean flexion contracture were $5^{\circ}$ and further flexion were $113^{\circ}$. All patients reported a decresement in pain level as well as improvement in motion. There was no complication in this series. Conclusion : Arthroscopic surgery appear to be satisfactory management modality for degenerative elbow contractures.

견관절의 결절종 제거수술후 발생한 SLAP병변 -증례보고 1예-

  • 29 year-old male was suffered from deep shoulder pain and muscle weakness 2 years ago. Recently 2 month ago lancinating shoulder pain and night pain was progressed. physical examination and MRI and EMG finding showed suprascapular nerve compression syndrome due to ganglion cyst in the shoulder joint. Arthroscopic ganglion cyst excision was performed and his symptoms was improved. 2 years later shoulder pain was developed in abduction and external rotation position and mild discomfort was continued. In second operation, SLAP lesion was found and arthroscopic SLAP repair was performed. Our case presented SLAP lesion occurred arthroscopic ganglion excision in the shoulder joint 2 years later.

Subcoracoid impingement After the Fixation of the Fractured Coracoid Process -A Case Report- (견관절 오구돌기 골절의 고정 후 발생한 오구돌기하 충돌증후군 - 증례 보고 -)

  • Subcoracoid impingement resulting from abnormal contact between the anterosuperior humerus and the anterior coracoacromial arch represents an uncommon source of anterior shoulder pain. Certain operative procedures can also alter the relationship between the coracoid and the lesser tuberosity, leading to impingement of the interventing soft tissue, including the subscapularis and the bursa. We describe an unique case of subcoracoid impingement with the tear of subscapularis tendon after the internal fixation of the fractured coracoid process with cannulated screw due to crowding of the coracohumeral space. Arthroscopic removal of the screw and repair of the subscapularis in our patient resulted in successful resolution of his symptoms. Although subcoracoid impingement is a rare cause of shoulder pain, failure to diagnose and treat this condition may represent a significant cause of failed shoulder surgery.
