Case study: Seeking for an ultimate solution for Knee Osteoarthritis based on Evidence-based medicine research (임상사례 : 골관절염에 대한 최소한의 치료적 방법)

  • Evidence based physical therapy interventions for this patient was applied in 15 clinical sessions. Considering the patient's two main impairments, two physical therapy interventions were delivered with WOMAC index outcome measurement. From this case report, I observed that a combined physical therapy interventions consisting of manual therapy, therapeutic exercise, and TENS Unit in patients with knee osteoarthritis may result in decreased pain, stiffness and increased physical function.

Prediction Factors of Fatigue in Patients with Fibromyalgia (섬유조직염환자의 피로 예측 요인)

  • Purpose: The purpose this research was to provide with basic data in the control of the fatiguer found in the patients with fibromyalgia by analysing the factors that predict that. Method: At three university medical center, appointed 245 out-patients diagnosed of fibromyalgia according to the conditions by American College of Rheumatology (1990). The research instruments used in this study were graphic rating scale(Anxiety, sleep disturbance, pain, joint stiffness and depression), physical activity, the number of tender points, life satisfaction and Self-efficacy scale. In data analysis, SPSS 12.0 program was utilized and data were analyzed using descriptive statistics, Pearson's correlation coefficient and multiple regression. Result: The factors that predict the fatigue of patients with fibromyalgia were sleep disturbance, life satisfaction, pain, joint stiffness, illness duration, and anxiety which explained 50.1% of the fatigue. Conclusion: It has been confirmed that the regression equation model of this research may serve as a fatigue prediction factors in patients with fibromyalgia.

The Clinical and Radiographic Features of Temporomandibular Joint Dysfunction in Patients with Rheumatoid Arthritis (류마티스성 관절염 환자에서 나타나는 측두하악관절장애의 임상적 및 방사선학적 특징)

  • The aims of this study was to investigate the clinical and radiographic features of temporomandibular dysfunction in the patients with RA to elucidate whether the RA patients would be a risk group for TMD. The 35 patients with temporomandibular joint dysfunction were included for this study, of which 15 had rheumatoid arthritis diagnosed by a rheumatologist, and other 20 was control group and they didn't have any history or clinical signs related to it. Clinical symptoms and signs of temporomandibular joint disorders, radiographic and MRI findings of temporomandibular joint were investigated. The results were compared between two groups. In RA group bilateral pain, morning stiffness, reduced opening range, and crepitations were more frequently reported than control group. Radiologic findings such as sclerosis and flattening of condylar head, marginal proliferation, and erosion of glenoid fossa were more frequently observed in RA group than control group. Disk destruction, cortical bone erosion, and intraarticular enhancement were more prominenet in RA group. From theses results, it can be concluded that many RA patients will develop TMD symptoms and the structural changes of TMJ is more extensive than the usual TMD cases.

Dynamic Threshold Model of Spasticity that Can Predict Various Pendulum Motions (다양한 진자운동을 재현가능한 경직의 동적 역치 모델)

  • The objective of this work is to develop the knee joint model for representing various pendulum motions and quantifying the spasticity. Knee joint model included the extension and flexion muscles. The joint moment consists of both the active moment from the stretch reflex and the passive moment from the viscoelastic joint properties. The stretch reflex was modeled as nonlinear feedback of muscle length and the muscle lengthening velocity, which is Physiologically-feasible. Moreover, we modeled the spastic reflex as having dynamic threshold to account far the various pendulum trajectories of spastic patients. We determined the model parameters of three patients who showed different pendulum trajectories through minimization of error between experimental and simulated trajectories. The simulated joint trajectories closely matched with the experimental ones, which show the proposed model can predict pendulum motions of patients with different spastic severities. The predicted muscle force from spastic reflex appeared more frequently in the severe spastic patient, which indicates the dynamic threshold relaxes slowly in this patient as is manifested by the variation coefficient of dynamic threshold. The proposed method provides prediction of muscle force and intuitive and objective evaluation of spasticity and it is expected to be useful in quantitative assessment of spasticity.

  • 본 연구는 뇌손상(뇌졸중, 외상성 뇌손상, 뇌성마비 등)으로 인하여 손의 능동적 움직임이 결여되어 발생하는 관절의 구축, 근육의 단축, 근육의 탄력성 저하 등의 문제점을 분석하여 인체 역학적 모델에 따른 과학적 설계를 기반으로 환자의 손 기능 회복을 위하여 로봇 기술과 스마트폰의 융합을 통한 재활 로봇 보조 치료기를 설계하고 구현하였다. 제안된 시스템은 일반적인 근 경직을 치료하는 방법을 응용하여 IT 기술과 로봇기술을 융합하여 치료사들의 부담을 덜어 주고, 환자들에게 오랫동안 정확한 운동을 반복적으로 할 수 있도록 하는데 목적이 있다. 하나의 구동기로 2자유도의 움직임을 조절 할 수 있는 링크 매커니즘과 링크의 길이를 조절하여 신전(extension)과 과신전(Hyperextension)의 범위 조절이 가능하도록 로봇 플랫폼을 설계하였다. 또한 환자의 재활정도 및 상태에 적합한 운동속도, 운동반복횟수 등을 손쉽게 조작할 수 있는 등의 개인 맞춤형 재활훈련이 가능한 사용자 인터페이스를 설계 및 구현하였다.

The Effects of Trunk Muscle Strengthening Exercises on Balance Performance of Sitting Posture and Muscle Tone of Children with Cerebral Palsy (체간근력 강화 운동이 뇌성마비 아동의 앉은 자세 균형 수행력과 근 긴장도 변화에 미치는 영향)

  • This study aims to investigate the effect of trunk muscle strengthening exercises on balance performance of sitting posture and muscle tone, targeting the children with spastic diplegic cerebral palsy. 16 children with spastic diplegic cerebral palsy in IV phase of GMFCS were sampled at random and the tests were conducted for 6 weeks, 3 times per week. For experimental groups, only trunk muscle strengthening exercises were conducted and for control groups, only basic physical therapy was conducted. The trunk muscle strengthening exercise consisted of 2 exercises to strengthen abdominal muscles and back muscles. BPM(Balance Performance Monitor) was used to measure balance performance and MMAS(Modified Modified Ashworth Scale) was used to measure muscle tone. As a result, the changes of sitting balance performance in experimental groups and control groups show significant difference in the changes of sway path and sway velocity(p<.05), the comparison of changes in sitting balance performance in between experimental groups and control groups show significant difference in the changes of sway path and sway velocity(p<.05). Knee flexor muscles and hip adductor muscles in both groups show no significant difference in changes of the muscle tone(p>.05). Therefore, the balance performance in both experimental groups and control groups was improved. However, the experimental group had more significant improvement in balance performance and no change in muscle tone was detected in both groups. Thus, these findings show trunk muscle strengthening exercises are effective in improving the balance performance of sitting posture for the children with spastic diplegic cerebral palsy without changing muscle tone.

Effects of Laughter Therapy on Joint Pain, Functional Status of Knee Joint and Depression in Elderly with Degenerative Knee Arthritis (웃음요법이 퇴행성 슬관절염 노인의 관절통증, 관절기능상태, 우울에 미치는 효과)

  • This study examined the effects of laughter therapy in the elderly with degenerative arthritis who had joint pain, joint dysfunction, and depression. Through a single-blind, randomized assignment, an experimental group of 26 subjects and a control group of 26 subjects were recruited among the elderly with degenerative knee arthritis, who were over 65 years of age. From January 8 to March 11 of 2019, a total of 16 laughter therapies were administered two times a week, 60 minutes per session, in eight weeks. The pain threshold (p=.012) and joint function status (p<.001), which represents the pain, joint stiffness, and physical function, and depression (p<.001), in the experimental group improved after the eight weeks laughter therapy compared to the control group. Laughter therapy is a therapeutic intervention that can be delivered through physical or intellectual activities. This therapy is a non-invasive procedure and has the advantage that nurses can practice easily with a little training. It can be used as an intervention within community and clinical settings to relieve pain and improve depression in the elderly with arthritis.

  • The purpose of this study is to evaluate the possibility of identifying joint damping property through commercially available isokinetic ergometer (BIODEX). The proposed method is to estimate the damping torque of the knee joint from the difference between the external joint torque for maintaining isokinetic movement and the gravity torque of the lower leg. The damping torque was estimated at various joint angular velocities, from which the damping property would be derived. Measurement setup was composed of the BIODEX system with an external force sensor and Labview system. Matlab was used in the analysis of the damping property. The experimental result showed that the small variation in angular velocity due to acceleration and deceleration of the crank arm resulted in greater change of inertial torque than the damping torque. Therefore, the estimation of damping property from the isokinetic movement is difficult.

The Effect of Repetitive Transcranial Magnetic Stimulation on H-Reflex Inhibition and Fascilitation of Range of Motion of Spastic Ankle Joint in Chronic Stroke Patients (만성 뇌졸중 시 반복경두개자기자극에 의한 경직성 발목관절의 관절가동 범위 향상 및 H-반사 억제 효과)

  • Purpose : The purpose of this study was carried out to investigate the effect of repetitive transcranial magnetic stimulation on increase of H-reflex inhibition and fascilitation of range of motion of spastic ankle joint in chronic stroke patients. Methods : 30 chronic stroke patients were randomly divided into three groups, a control group(placebo rTMS group), 5 Hz rTMS group and manual therapy group. The MAS and ROM of ankle joint and H-reflex inhibition of soleus muscle were evaluated on each group. Results : The rTMS group decreased MAS of ankle joint and increased H-reflex inhibition of soleus muscle, and ROM of ankle joint than manual therapy group. The placebo rTMS group did not affected the change of MAS, ROM of ankle joint and H-reflex inhibition of soleus muscle. Conclusion : The rTMS was a good therapeutic tool to improve the foot drop in the chronic stroke patients.

The Effects of leg length difference on Low Back Pain and joint stiffness (하지길이 차이가 요통과 관절경직에 미치는 영향)

  • Human body balances right and left leg centering around pelvis and spine. Therefore, imbalance of lower extremity means disequilibrium of the body. The difference of lower extremity length can cause a number of clinic symptoms including scoliosis, low back pain, sacroiliac pain, and sports injury. In this study, we tried to analyze low back pain and joint stiffness resulting from the difference of lower extremity length. The subjects were 80 male students who are 20-25 years old. The results of this study were as following: 1. Low back pain depending on the difference of lower extremity length One group which the difference of lower extremity length is above 12mm showed average different length as 18.0mm, the other group which one is below 12mm showed as 6.3mm. A group of above 12mm had more severe low back pain than a group of below 12mm. 2. Joint stiffness depending on the difference of lower extremity length A group of above 12mm had more severe joint stiffness than a group of below 12mm.
