RECONSTRUCTION WITH A VASCULARIZED FREE ILIAC OSTEOMUSCULAR FLAP AFTER HEMIMANDIBULECTOMY FOR OSTEOSARCOMA OF THE MANDIBULAR CONDYLE REGION : A CASE REPORT (하악과두에서 발생한 악성 골육종 환자에서 외과적 절제 후 유리장골이식을 이용한 재건)

  • Osteosarcoma of the jaw is a rare malignant bone tumor which usually leads to a poor prognosis. It commonly occurs in young patients, especially in male. The tumor can involve mandible or maxilla with same frequency. The swelling in the involved area and facial deformity are common clinical findings. The pain and sensory changes are also complained by the patients. Although radical surgery plays an important role in the management of this tumor, the adjuvant chemotherapy or radiotherapy is used to enhance local control and to prevent distant metastases. We treated a 22-year-old male patient who had osteosarcoma in the left condylar region. The radical surgery which consisted of hemimandibulectomy and total parotidectomy, was done and an immediate mandibular reconstruction was performed with a vascularized free iliac osteomuscular flap. The obtained results, both esthetic and functional, were satisfactory. The patient was received postoperative chemotherapy. This is a case with reviews of the literatures.

A CLINICAL STUDY ON FRACTURES OF THE MANDIBULAR CONDYLES: EFFECTIVE EXTRACORPOREAL FIXATION TECHNIQUE (하악 과두골절에 관한 임상적 연구: 효과적인 체외고정법)

  • Purpose: Fractures of the mandibular condyle accounts for 15% to 30% of all the mandibular fractures and lots of complications are reported. Among these complications, condylar resorption is the most important one to be taken into account. The purpose of this study is evaluating condylar resorption pattern in relation to extracorporeal fixation technique. Patients & methods: Thirty four patients with 42 mandibular condylar fractures were retrospectively examined with an average follow-up of 18.5 months (6-66 months). Male/female ratio is 27 males (7 bilateral cases) and 7 females (1 bilateral case). Applicated surgical techniques were extracorporeal fixation via submandibular approach and extracorporeal fixation via intraoral approach. We compared radiographic views of post-operative periods with that of most long term follow up in 34 operated sites respectively. Result: There were 2 generalized condylar resorption sites and 3 partial resorption sites with clinical implications. Others (29 joints) showed good healing or satisfactory adaptive state of temporomandibular joints clinically. Radiologically, signs of partial condylar resorption and/or osteoarthrosis were diagnosed in 11 patients. But, 8 out of the 11 patients didn't show significant clinical symptoms, which means successful adaptive remodeling of the surgically located condylar head. Conclusion: From the above results, intentional ramal osteotomy, temporary detachment of the fractured condyle and extracorporeal fixation technique seems to be effective and useful in those cases of condylar fractures in which reduction of the dislocated condyle is very difficult or virtually impossible.

RADIOGRAPHIC STUDY ON THE INTERRELATION BETWEEN BONE DEF ORMANS AND CONDYLAR HEAD POSITION IN THE TMJ ARTHROSIS (악관절증에서의 골형태이상과 과두위변화와의 상호관계에 관한 방사선학적 연구)

  • The author analysed the interrelation between the morphologic changes of bone Structures and the position of condylar head from the routine radiographs of 134 cases of the temporomandibular joint arthrosis. The frequencies of coincidence between the site of bone defrmity and condylar head positional change were examined. Also, the positional changes of condylar head and the direction of condylar movement in relation to the kind of bone deformities were observed. The results obtained were as follows; 1. In 52.65 per cent of total cases, the site, of positional change of condylar head was coincided with the site of bone deformans. The frequencies of the coincidence between these in the five items among seven items examined were above 53 per cent. From the results, it seems that the positional changes of condylar head were related with the morphological change of bone structure. 2. Eburnation and erosion. revealed frequently positional changes in the opening and closing position of the mouth, although in the early stages of the TMJ arthrosis. 3. In the bone deformans, during opene position of the mough 44.81 per cent of total cases revealed backword movement and 37.74 per cent showed forward movement. In closed position of the mouth, downward movement was revealed in 35.23 per cent of total cases and upward movement 28.41 per ,cent of total cases. 4. In the cases showing eburnation, the frequencies of coincidence between the site of positional change and bone deformans were 58.57 per cent of the total cases. that means it was high in the early stages of the TMJ arthrosis.

A ROENTGENOGRAPHIC CEPHALOMETRIC STUDY OF CONDYLAR HEAD ON THE SUBMENTOVERTEX CEPHALOGRAM (두부축방향방사선규격사진에 있어서 하악과두의 두부방사선계측학적 연구)

  • The purpose of this study was to observe the anatomic variation of condylar head and the positional relationship between condylar head and other anatomic structures of head by means of cephalometry using the submentovertex cephalogram from each person in centric occlusion for producing a good quality of temporomandibular joint radiograph. The 100 submentovertex cephalograms of 100 Korean adults consisted of 50 females ranged from 20 to 24 years age and 50 males ranged from 22 to 30 years age, were studied and analyzed statistically. The results were as follows; 1. The mean of the horizontal angulation of condylar head to the transmeatal line (EE-LA) was 18.5° (S.D. 7.9°), and all measurements that indicate the horizontal angulation of condylar head showed considerable differences among individuals. 2. In the comparison of male vs. female in the measurements, statistically significant differences were found in the majority of measurement items, and with exception of R-LA and LA-LA, the rest of these measurements were larger in male than in female. 3. In the comparison of left vs. right in the measurements, statistically significant differences were found in the majority of measurement items, and with exception of PC-CC, the rest of these measurements were larger in right than in left. As above, because the majority of measurements that involve the horizontal angulation of condylar head varied among individuals, between male and female, and between left and right, the condition of various temporomandibular joint radiography should be determined by means of cephalometry using the submentovertex cephalogram for producing the accurate radiographic image of temporomandibular joint.

A CONFUTER ANALYSIS ON THE ARTICULAR EMINENCE AND THE CONDYLAR PATH OF THE EDENTULUS PATIENT IN MANDIBULAR PROTRUSIVE MOVEMENT (무치악자의 하악전방운동시 관절융기와 과두운동로에 관한 컴퓨터 분석)

  • The objective of this study was to compare the condylar path and the anterior angle of articular fossa and to analyze the pattern of condylar path in edentulus patients. Nineteen male and female edentulous patients with normal masticatory system ranging in age 42 to 78, without present symptoms and any history of TMJ disturbance were selected for this study. On the computer analysis on the transcranial radiographs of the TMJ, the angle of slope of articular eminance and condylar path to the Frankfort Horizontal Plane and the height of glenoid fossa was measured respectively, and stuied their interrelationship comparatively. Obtained results were asfollows. 1. The angle of the slope of articular eminence averaged 37.28 degree. and there was no significant difference between the right and left side. 2. The condylar path angle averaged 29.05 degree and there was no significant difference between the right and left side. 3. The height of the glenoid fossa averaged 8.11 mm and there was no significant difference between the right and left side. 4. The sequence of the frequence of condylar movement patterns were concavex curve(39.5% ), 'S' shape curve(34.2%), reverse 'S' shape(15.8%) and convex curve(10.5%). 5. The horizontal distance of the point of the changed curve of the condylar path averaged 2.91 mm. 6. The height of glenoid fossa was highly correlated to the slope of articular eminence and relatively highly correlated to tile condylar path and the condylar path was closely correlated to the slop of articular eminence.

Study of the Plating Methods in the Experimental Model of Mandibular Subcondyle Fracture (하악골 과두하부 골절 실험모델에서 견고정을 위한 플레이트 고정방법 연구)

  • Purpose: This study examined the biomechanical stability of four different plating techniques in the experimental model of mandibular subcondyle fracture. Methods: Twenty standardized bovine tibia bone samples ($7{\times}1.5{\times}1.0cm$) were used for this study. Each of the four sets of tibia bone was cut to mimic a perpendicular subcondyle fracture in the center area. The osteotomized tibia bone was fixed using one of four different fixation groups (A,B,C,D). The fixation systems included single 2.0 mm 4 hole mini adaption plate (A), single 2.0 mm 4 hole dynamic compression miniplate (B), double fixation with 2.0 mm 4 hole mini adaption plate (C), double fixation with a 2.0 mm 4 hole mini adaption plate and 2.0 mm 4 hole dynamic compression miniplate (D). A bending force was applied to the experimental model using a pressure machine (858 table top system, $MTS^{(R)}$) until failure occurred. The load for permanent deformation, maximum load of failure were measured in the load displacement curve with the chart recorder. Results: Double fixation with a 2.0 mm 4 hole mini adaption plate and a 2.0 mm 4 hole dynamic compression miniplate (D) applied to the anterior and posterior regions of the subcondyle experimental model showed the highest load to failure. Conclusion: From this study, double fixation with an adaption plate and dynamic compression miniplate fixation technique produced the greatest biomechanical stability. This technique may be considered a useful means of fixation to reduce the postoperative internal maxillary fixation period and achieve early mobility of the jaw.

Magnetic Resonance Evaluation of the Articular Disc Configuration in the Patients with Internal Derangement of the Temporomandibular Joint (악관절 내장증 환자에서 자기공명영상사진상의 관절원판 형태의 평가)

  • 이 연구의 목적은 악관절 내장증 환자에서 관절원판의 형태를 자기공명영상사진으로 평가하고 임상적 증상과의 관계를 알아보고자 하는 것이다. 악관절 내장증의 증상을 가진 90명의 환자에서 169개의 관절을 자기공명영상사진으로 관절원판과 하악과두의 관계를 기준으로 정상군, 정복성 관절원판 변위군, 비정복성 관절원판 변위군으로 분류하고 관절원판의 형태를 5가지로 나누었다. 임상검사는 측두하악관절통과 관절음의 유무를 확인하여 그 상관관계를 알아보았다. 실험 결과에서 측두하악관절통은 각 군에서 관절원판의 형태와는 정상군을 제외하고는 통계적인 유의성이 없었으며, 염증성 삼출액과 유의한 관계를 보였다. 관절음은 정복성 관절원판 변위환자에서 관절원판형태와 통계적으로 유의성을 보였으며, 비정복성 관절원판 변위환자에서도 상당수 관절음이 발생하였음을 알 수 있었다. 이 결과는 측두하악관절의 통증은 관절원판의 형태가 많이 변형될수록 통증이 심해지는 경우가 많았지만, 다른 여러 가지 원인들이 존재할 수 있으며, 관절음은 정복성 관절원판 변위환자에서 관절원판의 변형이 심할수록 관절원판이 정복할 때 발생될 수 있다는 것을 의미한다. 따라서, 임상적인 검사와 더불어 자기공명영상사진으로 정확한 진단을 함으로써 관절원판 변위의 적절한 치료가 행해질수 있으리라 생각된다.

Histological Observation of Osteochondrosis Dissecans Occurred in Mandibular Condyle (하악과두에 생긴 박리성 뼈연골증의 조직학적 관찰)

  • A 57 years old female complained of severe pain on the right temporomandibular joint (TMJ) area. Her right condyle had been partly resected under surgical operation 13 years ago due to condyle hypertrophy, thereafter she felt dull pain on TMJ area and recently the lesion became severely swelled and painful leading to cancer phobia. The present radiological views showed slightly enlarged and sclerosed condyle with increased radiopacity, but its articular sliding function was almost disable during mouth opening. The patient's TMJ lesion was carefully managed with conservative physiotherapy and pain treatment. The microsection of condyle head obtained from the previous operation was re-evaluated histologically, and it was finally diagnosed as osteochondrosis dissecans (OCD), exhibiting hyperplastic proliferation of cartilage in condyle head and marked vascular dilatation in epiphyseal zone. This abnormal cartilage tissue was distinguishable from normal cartilage tissue found in the peripheral cartilaginous cap of the same microsection. The involved cartilage cap showed thick hypertrophic chondrocyte zone with horizontal and vertical clefts accompanying diffuse hyaline degeneration. The superficial fibrous zone of cartilage cap was thickened and frequently peeled off, while lower hypertrophic zone of cartilage cap was highly cellular and proliferative. Consequently, the endochondral ossification became aberrant and resulted pre-mature apoptosis of many hypertrophic chondrocytes, followed by diffuse and mild inflammatory reaction in the underlying marrow tissue. Therefore, it was suggested that this hypertrophic condyle lesion, OCD, be differentiated depending on radiological and histological features from ordinary condyle hyperplasia, osteochondroma, and osteoarthritis, and that the pathological confirmation of OCD may provide a reliable modality for dental and medical treatment of chronic and painful TMJ lesion.

Full Mouth Implant Rehabilitation in Facial Asymmetric Patient (안면비대칭 환자의 전악임플란트 수복)

  • Patients who miss teeth partially or fully show many changes which make them lose function and esthetics.From the esthetic point of view, loosing teeth makes lower face unharmonized. There are various changes of lower and whole face according as how much change oral cavity is. Restoring the multiple teeth missing properly can make patient's face harmonized. Especially full mouth implant restorations can cause drastic occlusal change affecting masticatory muscles. Because all the muscles are connected closely, the masticatory muscles which is part of lower facial muscles can cause whole muscle change. In full mouth implant restoration case, I will show the whole face muscle change harmonized by meticulous occlusal treatment process. Full mouth restorations installed in right way show whole face muscle changes extending to head and neck muscles.

RELATIONSHIP BETWEEN CO-CR DISCREPANCY AND FACIAL SKELETAL TYPE (안면골격 형태와 중심교합위-중심위 변위간의 상관성에 관한 연구)

  • The present study was performed to prove the relationship between CO-CR discrepancy and facial skeletal type. In this study, 242 subjects were randomly selected and devided into 9 groups(devided into class I, II, III by ANB and each one devided into dolicho-, brachy-, mesofacial skeleton by Ricketts' vertical index). Lateral cephalometric radiographs with the mandible in centric occlusion were taken and measured and CO and CR bites were registered on all subjects. Diagnostic casts were mounted on Panadent articulator using an estimated face-bow and centric relation bite registration. The amount and direction of CO-CR discrepancy present was recorded using a Condylar Position Indicator(CPI) and a centric occlusion wax bite registration. CPI measurements and cephalometric measurements were statistically analyzed. The finding of this study can be summerized as follows : 1. There is little correlation between right and left sides for magnitude or direction of CO-CR discrepancies. The correlation between the magnitude of CO-CR discrepancy of left A-P and right A-P is higher than that of left S-I and right S-I. 2. Correlation of Class II malocclusion group was higher than that of the other groups between the magnitude of CO-CR discrepancy of left CPI and right CPI. 3. There is no difference between the pattern of CO-CR discrepancy of 9 malocclusion groups. 4. There is very little, if any, correlation between Skeletofacial measurements and CO-CR discrepancy. 5. In Class II brachyfacial skeleton and Class III mesofacial skeleton there was Lateral cephalometric measurements by that we predict CPI measurements was detected. That was overbite, overjet, upper genial angle, lower genial angle, saddle angle, articular angle, convexity of point A, ANS-Me/Na-Me, PCBL/RH, Posterior FH/anterior FH.
