  • Today, with the development of modern civilization, the change of industrial structure and the increase of traffic volume and population make the number of maxillofacial injury patients also increase. Especially, the fracture possibilities of mandibular condyle have been increased because of mandibular position and shape. I experienced the case that mandibular condyle fracture caused by gun-shot injury(Bullet had been packed at the opposite condyle of input site) was treated by foreign body removal and condyle open reduction. I will represent that case with the journal review.

  • It has been established that disk displacement of the temporomandibular joint(TMJ) can cause mandibular asymmetry in growing subjects. One of the causes of internal derangement of TMJ seems to be the result of poor positioning of the joint structure in unilateral cross bite, and the subsequent occlusal trauma transferred to the functioning unit of the mandible, the joint and disc. Transverse discrepancy of the maxillary and the mandibular posterior dentoalveolar units was often found in mandibular asymmetric subjects. Most of the asymmetry in growing subjects becomes worse with further growth if left untreated. However once sufficient posterior overjet is gained through orthodontic treatment, many cases have shown improvement in facial asymmetry. Furthermore, the position of condyles in computed tomogram(con-beam CT) changed from anatomically unfavorable position to more concentric position.

  • The author intended to observe the developmental pattern in mandibular bone and condyle following the loss of function of molar with 5 week-old rats b y means of removing the crown of molar. The bjects were observed everyweek during six weeks. The results were as follows ; 1. Bone apposition was observed at the root apex and interadicular area of alveolar bone. 2. Development of bone substance in mandible tended to increase in the narrow bone trabeculae and fibrous bone substance. 3. Development of condylar head showed little difference from that of control group and development of calcified zone appeared more or less poorly.

  • The author examined bone changes from 1274 poly tomographic images of 182 temporomandibular joints which showed symptoms of temporomandibular disorder and the following results were obtained; 1. The number of temporomandibular joints which showed bone changes were 64 (35.2%) among 182 joints. 2. The age and sex distribution of 64 joints which had bone changes showed the prevalence of female (90.6%) and third decade (25.0%) followed by fourth (21.2%) and second decade (17.2%). 3. The 252 images which showed bone changes consisted of 56 images from lateral side (22.2%). 118 images from center (46.8%) and 78 images from medial side (30.9%). 4. The most frequently observed bone changes were flattening (22.7%) followed by sclerosis (19.3%) and cortical unsharpness (19.3%)

A Study on the Mandibular Condylar Movement and the Temporomandibular Joint Sound Effected by the Stabilization Occlusal Splint (교합안전장치가 하악과두운동 및 악관절 잡음에 미치는 영향에 관한 연구)

  • The purpose of this study was to evaluate the effects of stabilization occlusal splint by using a simplified condylar path recorder and a dental sound checker. For this study, 11 subjects (10 men and 1 woman) with TMJ disorder were selected from students at Chosun University, School of Dentistry. And they were treated with the stabilization occlusal splint. The condylar movement and the TMJ sound of each subject were recorded and analyzed by using a simplified condylar path recorder and a dental sound checker. The obtained results were as follows: 1. No statistically significant reduction of reduced PRI scores occurred before and immediately after wearing of stabilization occlusal splint. 2. The reduced PRI scores after wearing of stabilization occlusal splint showed statistically significant reduction with the lapse of time. 3. After stabilization occlusal splint therapy, Fisher angle and Bennett angle had almost no change. 4. TMJ sound disappeared in 4 out of 11 subjects.

  • This is to report functional treatment, as mechano-therapy, of condylar fracture in cases of growing patients. Benoist's appliance was used in 3 cases condylar fractures, provides extraoral elastic forces to external pterygoid muscle. The appliance providing guided exercise of masticatory muscles induces transformative growth of trauma-tic condyles. The results were as follow; 1. Early exercise preventes ankylosis of fractured condyle. 2. Optimal physical exercise therapy induces the normal growth pattern by muscular training and bony remodeling of fractured condyle in children.

  • Temporary reconstruction of the mandibular continuity defect resulting from the ablative tumor surgery with a reconstruction plate can be used for the preservation of normal facial contour and oral function and for periodic follow up of recurrence. Reconstruction plates are adapted to the bone before the resection and provisionally fixated with some screws. Accurate contouring and adaptation are very important for the prevention of displacement of bony stumps and decubituous skin ulcer. However, if there is large expanding buccal tumor mass in mandible, it is very difficult or even impossible to contour the plate before resection. I, therefore, introduce the reconstruction plate application technique using a simple condylar repositioning miniplate after segmental mandibular resection.

  • The present study reports a case of eosinophilic granuloma of the mandibular condyle. Eosinophilic granulomas on the mandibular condyle are very rare, but there are several common clinical and radiographic presentations. The clinical presentations involve swelling on preauricular area, limitation of opening, TMJ pain, etc. The radiographic presentations involve radiolucent lytic condylar lesion with or without pathologic fracture. Sometimes new bone formations are observed. The purpose of the article is to add new cases to the literatures.

  • To determine the difference in cephalometric measurements between centric relation and centric occlusion, 60 patient-20 patients for each malocclusion group-were examined with the cephalograms in centric relation and in centric occlusion. The results are as follows: 1. There are differences in cephalometric measurements with the mandible in the two different position, and some measurements are statistically significant. 2. No clinically useful prediction may be made from cephalometric radiographs concerning the amount of mandibular deflection from centric relation to centric occlusion. 3. If, the large differences in the cephalometric measurements with the mandible in the two different position, the analysis with the cephalogram in centric occlusion is needed to be compensated with the other clinical approach.

  • The auther has evaluated the panoramic image of the mandibular condyle according to its horizontal codylar angle(0°, 10°, 20°, 30°, 40°) and intercondylar distance( 9.5, 11.0, 12.5, 14.0㎝). The Results were as follows: 1. In all horizontal condylar angle, the farther the intercondylar distance was, the smaller the horizontal length in radiogram was. 2. The increased the horizontal condylar angle was, the longer in horizontal direction and the shorter in vertical direction the image of superior condylar surface was. 3. In case of large horizontal condylar angle, the real mesial surface of condyle was projected to the posterior surface on radiograph.
