A Single Case Study on the Psychological Analysis of a Middle-aged Woman Suffering from Animal Phobia (동물공포증을 겪는 중년여성의 심리분석 단일사례연구)

  • This study is an instrumental case study based on a theoretical proposition on animal phobia as a single case, Six categories were derived for the purpose of psychological analysis of the father of a female research participant suffering from the earthworm phobia. They are 'a fearful existence', 'problems with the woman who is always by her father's side', 'secret adultery and shock', 'fear of punishment', 'symbol for earthworm and fear displacement', and 'earthworm phobia'. Anxiety about punishment from a fearful father was the cause of the phobia. For the study participant, the earthworm phobia was the displacement of her father's fear of earthworm, and the earthworm phobia was interpreted as a substitute for her father. In other words, earthworm phobia should be understood as an escape and defense process as it replaces the re-emergence of long-repressed fearful feeling toward her father, and its symbolism can be seen as a metaphorical transformation of the pathological family atmosphere.

  • The goals of this study are twofold: To investigate non-phobics'and Phobics'Physiological response in virtual environments. and to analyze the trend of phobics' Physiology during virtual reality treatment. As a measure of Physiology. heart rate, skin resistance. and skin temperature were acquired. The data of two group subjects were analyzed: twenty-two non-Phobic subjects (M=32.94 years). thirty-six subjects with fear of flying (M=40.12 yearn) who met the DSM-lV criteria for a fear of flying. As a result. skin resistance showed significant differences between non-Photics and phobics. T(56) =2.978, P〈0.01 And the physiological responses of 33 subjects among the Phobics. who succeed to fly without medicine after virtual reality treatment, showed a gradual trend toward the non-Phobics Physiological responses as therapy sessions went on. In this study. Physiological monitoring. skin resistance appeared to be useful both in understanding the Physiological state of Phobic individuals and in evaluating the results of treatment in virtual reality Psychotherapy.

Convergence Study on the Relationship between Kinesiophobia and Fear of Falling in Patients with Stroke (뇌졸중 환자의 운동공포증과 낙상공포 사이의 관계에 대한 융합 연구)

  • This study aimed to investigate the convergence relationship between kinesiophobia and fear of falling in patients with stroke. A total of 113 patients with stroke participated in this study. Participants underwent inpatient rehabilitation and completed surveys with three different questionnaires including the fall efficacy scale (FES), translated Tampa Scale for Kinesiophobia 13 (TSK-13), and activity-specific balance confidence scale (ABC). TSK-13 and FES showed weak negative correlation (r=-0.226), and TSK-13 and ABC showed moderate negative correlation (r=-0.300). FES had a very strong positive relationship compared with ABC (r=0.838). Faller showed significantly low FES and ABC scores compared with non-faller (p<0.05). These results present that patients with stroke had mild kinesiophobia, and kinesiophobia is related to fear of falling. It is necessary to evaluate kinesiophobia in stroke rehabilitation.

  • Virtual reality technology is one of the important technologies that will affect our life in many ways. This novel technology will draw new paradigm into the medical field due to its advantages in physical safety and environment that are fully controlled. Phobia treatment using VR technology has been implemented and its feasibility has been proved through a number of researches in many institutions. This study has observed the current progress of its technology environment and the trend of research. Also, This study has analyzed the results from domestic and international data. Analysis shows that other countries are ahead of korea in all aspects of the phobia treatment using virtual reality method. Although the authors limited the kinds of journals, the amount of quantity in international researches are two times more than domestics. Also, The researchers in other countries concentrate on the multi-modal studies. To use virtual reality in the phobia treatment, we need to understand the needs of the society members and the government has responsibility to support what the researchers need.

  • We dentists perform many of routine dental procedures and must deal with much of the population's dental anxiety and fear. Dentists have used many non-pharmacological modalities initially to overcome anxiety and fear, including distraction, empathy, desensitization, and so on. However, certain patients need pharmacological backup of anxiety and fear to conquer dental treatment. This article presents four cases to introduce the methods from nitrous oxide and oxygen inhalation to general anesthesia for difficult adult dental phobic patients to deal with.

  • It is our purpose to evaluate the validity & interrater reliability of CIDI-social phobia, Korean Version. Method : CIDI-Social phobia Korean Version was evaluated by 50 Korean social phobia patients who met diagnostic criteria of DSM-IV. The authors calculated diagnostic concordance rate between clinical diagnosis & diagnosis by using the CIDI-social phobia Korean Version for evaluating validity. We also calculated the interrater diagnostic concordance of 20 social phobia patients for evaluating interrater reliability. Results : Validity of the CIDI-social phobia Korean Version was 0.74, & Interrater reliability(kappa) was 0.75. Conclusions : CIDI-Social phobia, Korean version had a good reliability & validity. That was proved a good instrument for diagnosing social phobia for general population and clinical patients

A Study of Functional Impairment in Social Phobia (사회공포증 환자의 기능장애에 관한 연구)

  • This study was aimed to investigate the nature of functional impairment in social phobia and the correlation between the functional impairment and the duration of the disease, and the severity of the symptoms. In 66 patients with social phobia, the functional impairments were assessed with the SDS(Sheehan's disability scale) and the LSRDS(Liebowitz self-rated disability scale), and the severity of the symptoms was assessed with the SPS(social phobia scale). The results were summarized as follow : 1) Most of the social phobic patients reported moderate to severe impairment in their lives in areas of education, employment, family relationships, romantic relationships, social networks, and other interests, etc. 2) There was a significant correlation between the severity of the symptoms and the disability in their lives. 3) The offensive social phobic patients reported that they had more disability in their lives and more severe symptoms than non-offensive patients did. 4) There was no significant correlation between the duration of disease and the disability in their lives. Conclusively, we confirmed that social phobia was associated with the functional impairment in most areas of their lives.

  • Virtual Reality(VR) is a new technology which makes humans communicate with computer. It allows the user to see, hear, feel and interact in a three-dimensional virtual world created graphically. In this paper, we introduced VR into psychotherapy area and developed VR system for the exposure therapy of acrophobia. Acrophobia is an abnormal fear of heights. Medications or cognitive-behavior methods have been mainly used as a treatment. Lately the virtual reality technology has been applied to that kind of anxiety disorders. A virtual environment provides patient with stimuli which arouses phobia, and exposing to that environment makes him having ability to over come the fear. In this study, the elevator stimulator that composed with a position sensor, head mount display, and audio system, is suggested. To illustrate the physiological difference between a person who has a feel of phobia and without phobia, heart rate was measured during experiment. And also measured a person's HR after the virtual reality training. In this study, we demonstrated the subjective effectiveness of virtual reality psychotherapy through the clinical experiment.

Noise Phobia-Induced Relative Polycythemia in a Dog (개에서 소리공포증에 의해 발생한 상대적 적혈구증가증 증례)

  • A 6-year-old castrated male Miniature Pinscher dog was referred due to shaking, panting and inappropriate elimination during thunderstorms. The dog had noise phobia after a car accident two years ago. The intensity of the fear of noise, especially with thunderstorms, worsened during the past 3 months (thunderstorm season). Physical examination revealed hyperthermia, tachypnea (panting), mild tachycardia, and an elevated systolic blood pressure. Laboratory examination revealed mild polycythemia with a lower oxygen pressure and saturation. Based on the history, physical examination, and laboratory tests, the dog was diagnosed as a noise phobia concurrent with relative polycythemia. Treatment was initiated with behavior modification with desensitization, counter-conditioning, and medication. Music therapy was also used and appeared to be beneficial. Clinical signs including polycythemia are improved. This case indicates that relative polycythemia can be occurred by chronic mental stress, such as noise phobia in a dog.
