  • Purpose Precocious Puberty is defined as the development of secondary sexual characteristics in girls younger than 8 years, and boys 9 years. Cause premature closure of the epiphysis is a disease that eventually decreases the final adult height. In this study, we retrospectively analyzed to evaluate the diagnostic difference the GnRH (Gonado-tropin-releasing Hormone) stimulation test results with medical records of precocious puberty in girls. Materials and Methods From February 2015 to December 2015 it was enrolled in the girls 118 people who visited the Seoul National University Bundang Hospital, Pediatrics, Endocrinology Internal Medicine. True precocious puberty group (n=57), early puberty group (n=39), were divided into Premature thelarche (n=22) group. A Tanner stage, chronological age, bone age, height, body weight for each group was determined by examining the mean${\pm}$standard deviation. GnRH test result was compared LH (Basal, 30 min, 45 min, 60 min), FSH (Basal, 30 min, 60 min) for each group, Each group LH, FSH Peak value distribution, the mean${\pm}$standard deviation was calculated for the peak LH/LH basal ratio, peak LH/Peak FSH ratio. The significance probability (P-value) between the value of each third group was determined. Results The average height of the true precocious puberty group $131{\pm}14.85$, the mean weight was $28.80{\pm}4.93$, the average chronological age $7.1{\pm}0.81$, the mean bone age was $9.9{\pm}0.9$, The average height of early puberty group was $134{\pm}5.10$, the average weight $28.50{\pm}4.43$, the average chronological age $8.05{\pm}0.03$, the mean bone age was $10.0{\pm}0.62$, The average height of Premature thelarche $129{\pm}6,01$, the average weight was $28.65{\pm}5.98$, the average chronological age $7.02{\pm}0.58$, the mean bone age was $8.04{\pm}1.29$. There was no significant difference when compared to the height and weight. There was a significant difference between the groups in the chronologic age and bone age difference (P <0.0002) True precocious puberty group showed peak LH levels at 30'(82.5%), 45'(12.3%), 60'(5.3%), in Peak FSH 30'(8.8%), 60'(91.2%). Early Puberty group showed high values in Peak LH at 30'(79.5%), 45'(17.9%), 60'(2.6%), in peak FSH levels at 30'(7.7%), 60'(92.32%). In Premature thelarche Group it showed the Peak LH levels at 30'(30%), 45'(59%), 60'(9.09%), Peak FSH levels at 30'(0%) 60'(100%). When compared with the The Peak LH/basal LH ratio, True precocious puberty group was $19.09{\pm}17.15$, early puberty group was $15.23{\pm}10.88$, Premature thelarche group showed significant differences between the three groups as $4.93{\pm}4.36$.(P <0.0001) LH Peak/FSH Peak ratio, true precocious puberty group was $1.222{\pm}0.77$, early puberty group was $1.34{\pm}1.23$, Premature thelarche group showed significant differences between the three groups as $0.3{\pm}0.09$(P <0.0001) Conclusion In order to diagnose the true precocious puberty have a diagnostic value when the LH peak after GnRH stimulation is increased by more than two to three times compared to baseline or a predetermined level or more than 5~10 IU/L increases. GnRH Test is a test for a long time and the patient discomfort due to repeated blood sampling, but the hypothalamus-pituitary gland- gonad axis activity evaluate and is the most basic accurate test in the differential diagnosis of precocious puberty disorders.

  • Recently, As the prevalence of precocious is increased in Korea This study was evaluated the physical growth and female sex hormone characteristics of girls with Idiopathic precocious puberty. Retrospective analysis about the medical record of 113 girls for evaluation of signs of precocious puberty except for organic causes was done. Physical growth features and Female sexual hormone were analyzed. Height SDS and Weight SDS were significantly increased in true sexual precocity compared to pseudo sexual precocity. Hormone studies showed that the level of basal LH, FSH, E2 was significantly higher in true precocious puberty. The finding of this study suggest that we need to develop intervention about physical and psychological problems for precocious puberty girls.

  • Objectives The purpose of this study is to examine the relationship between skeletal maturity and heart rate variability (HRV) based on the bone age and HRV parameters. Methods 103 children from 6 years to 17 years of age, who do not have any disease-related symptom, and visited ${\bigcirc}{\bigcirc}$ oriental medicine hospital, are measured based on their bone age and short-term spectral analysis of HRV. Results 1. Skeletal maturity was significantly correlated with HRV indices: mean HRT and SDNN. As the skeletal maturity increases, the mean HRT was decreased and the SDNN was increased. 2. When classifying according to the skeletal maturity score, the mean HRT was higher in the 'below -0.4' group compare to '-0.3~0.7' group and '0.8~1.9' group. SDNN was higher in the '0.8~1.9' group compare to '-0.3~0.7' group, 'below -0.4' group 3. When classifying according to the sex, age and secondary sexual characteristics, as the skeletal maturity was increased, the mean HRT was significantly decreased and the SDNN was significantly increased only in the boys who did not develop secondary sexual characteristics. Conclusions Skeletal maturity could be statistically significant with HRV indices, especially to the boys and the children than the girls and the teenagers.

  • Objectives The study was designed to find out the relationships between low weight and growth, skeletal maturity among children by analyzing body composition and bone age. Methods Subjects were composed of 336 children from six years to seventeen years of age, without any other diseases related to growth, who visited the department of oriental pediatrics East West Neo Medical Center at Kyung Hee University and were measured their body composition and bone age. Results There were significant correlations between decreasing of RH-MPH(%) and low weight. 1. As obesity index decreased, the RH-MPH was also decreased. 2. The RH-MPH(%) of the low weight group according to the obesity index classification was significantly decreased than that of the normal weight group. 3. The skeletal maturity was more decreased in the low weight group. However, the differences between the two group was not significant. 4. The RH-MPH(%) was increased in children who developed secondary sexual character. Skeletal maturity was decreased in children who developed secondary sexual character. 5. The RH-MPH(%), and skeletal maturity was not significantly different between males and females. 6. Only in the case of children without development of secondary sexual character, low weight caused significant RH-MPH(%) decrease. Conclusions Low weight children might be smaller than what it supposed to be. Also, low weight could affect body development more to the children than to the teenager.

  • Objectives : The purpose of this study was to investigate correlation between obese degree, result of hair tissue mineral analysis and bony maturation. And we also wanted to compare the differences between early bony maturation group(EBM) and late bony maturation group(LBM). Methods : 146 subjects who visited growth clinic were measured by BMI, PBF(percent body fat), percent BMI, bone age esimation and HTMA(hair tissue mineral analysis). The patients were classified into two groups - EBM, LBM group - according to the gap of bone age and chronological age. It was analysed that the correlation of bony maturation and obese degree, nutritional elements, heavy metals, significant ratio of nutritional elements. Also, analysed the differences between groups. Results : 1. BMI, percent BMI had a correlation with bony maturation, PBF however didn't have a correlation with it. 2. Ca, Mg, Zn and P had a positive correlation with bony maturation, and also K, Cr and Mo had a negative correlation. 3. U, As and Cd had a negative correlation with bony maturation. 4. Ca/P, Na/K, Ca/K and Zn/Cu had a positive correlation with bony maturation, and also Na/Mg, Ca/Mg had a negative correlation. 5. Percent BMI, Ca, Mg, Mn and Ca/p were higher in EBM group. 6. K, Mo and Hg were lower in EBM group. Conclusion : According to this study it could be suggested that maintaining proper percent BMI and accumulation of nutritional elements, heavy metals to prevent early bony maturation.

  • This study explored the effect of calcium levels and/or ovariectomy on bone composition and its related factors using the female Sprague-Dawley rats which achieved peak bone mass in normal(0.5%) calcium intake during growth period. The rats were randomly divided into six groups and fed 0.1%, 0.5% and 1.5% calcium diets for 8 weeks after ovariectomized and sham operation. The results indicated that body weight gain was higher in ovariectomized groups than sham groups, regardless of dietary calcium levels and food intakes. Serum Ca and P concentrations were normal level regardless of dietary calcium levels and operation. Estrogen concentration was decreased in ovariectomized rat groups. Serum alkaline phosphatase activity and urinary hydroxyproline were increased in ovariectomized groups. When rats were fed normal Ca diet during growing period, weight, length and breaking force of femur were not significantly different in all groups but increased the same level. Generally, lipid contents in lumbar and femur were lower in low calcium groups and this effect was more pronounced in femur. In case of femur, the significant decrease in Ca contents of bone was observed in a relatively short period of feeding low Ca diet, even though it appeared th desirable peak bone mass had achieved through the growing period by supplementing the adequate amount of calcium. The marked decrease of estrogen levels after ovariectomy did not seem to influence greatly bone parameters measured except bone wet weight. Therefore, this study indicated that lower intake of Ca might be more important determinating factor against osteoporosis than postmenopausal state stimulated by ovariectomy in female rats. With normal or high intake of Ca it appears possible to prevent bone loss in postmenopausal period, and this might apply only in case of achieving peak bone mass in the growing with the adequate intake of calcium. • Evaluation of a fever of unknown origin (FUO) is complex. Recently, PET scanning has been approved for screening in FUO evaluation. We treated three cases of FUO associated with increased FDG uptake in the bone marrow of the femur and tibia on the fusion PET/CT; all three had the same pattern of uptake. Bone marrow biopsies revealed mature lymphocyte and histiocyte infiltration and myxoid changes in one case, and cortical bone involvement in another case. The cases were all young females who had fever with neutropenia and relative lymphocytosis that lasted for several weeks and then remitted spontaneously. Even though the results of the studies were not diagnostic, the unique uptake pattern on PET/CT and the histology might be related to the cause of the illness and should be studied further to assess the association with classic FUO.

  • To investigate the relationship between the calcification stages of mandibular canines and the skeletal maturity stage of the hand-wrist in subjects with normal occlusion and Class III malocclusion, hand-wrist radiographs and panoramic radiographs were taken from subjects of normal occlusions(94 males, 88 females) and Class III malocclusions(75 males, 76 females) who had no systemic diseases and no history of orthodontic or prosthodontic treatment. Fishman's method for the skeletal maturity stages of the hand-wrist and Demirijian's method for the calcification stages of mandibular canines were used and analyzed. The results were as follows : 1. In subjects with normal occlusion and Class III malocclusion, skeletal maturity of the hand-wrist and calcification of mandibular canines at various ages occured earlier in females than in males(p<0.05). 2. Comparing the skeletal maturity stages of the hand-wrist and the calcification stages of mandibular canines between subjects with normal occlusion and Class III malocclusion, there were no significant differences between the groups. 3. The correlation coefficient between the calcification stages of mandibular canines and the skeletal maturity stages of the hand-wrist. in subjects with normal occlusion and Class III malocclusion showed a high association(p<0.01). 4. In stage 4 of the skeletal maturity of the hand-wrist, the frequency distribution of calcification G stage among the various calcification stages was highest both in normal occlusion and in subjects with Class III malocclusion. However, there was no significant difference in the frequency distribution of calcification stages between the groups.
