일부 폐경전 성인여성의 골밀도와 신체계측 및 식이인자에 관한 연구

  • 본 연구는 폐경전 성인여성들의 골격상태를 알아보고 신체계측 및 식이섭취실태가 골밀도에 미치는 영향을 평가하기 위해서 성인여성 61명을 대상으로 신체계측을 실시하고 24시간 회상법으로 영양소 및 주요 식품군의 식품섭취실태를 조사하고 DEXA를 사용하여 요추와 대퇴부 3부위의 골밀도를 측정하였다. 1) 대상자들의 평균 연령은 37세였으며, 평균 신장과 체중은 158.17cm, 54.55kg였으며, 평균 BMI와 WHR은 21.82, 0.78로 대상자들의 비만도는 정상이었다. 2) 평균 골밀도는 요추(L24) 1.04 g/$\textrm{cm}^2$, 대퇴경부 0.76g/$\textrm{cm}^2$, 대퇴전자부 0.66g/$\textrm{cm}^2$, 와드삼각부 0.69g/$\textrm{cm}^2$이었다. 3) T-score로 판정시 요추(L24)의 경우 골다공증군 3.28%, 골감소증군 14.75%, 대퇴경부의 경우 골다공증군 9.84%, 골감소증군 52.46%, 대퇴전자부의 경우 골다공증군 1.64%, 골감소증군 34.43%, 와드삼각부의 경우 골다공증군 6.56%, 골감소증군 45.90%였다. 4) 영양소중 칼슘(78.75%), 철분(69.75%) 및 비타민 A(92.17%) 영양소의 1일 평균 섭취량은 영양권장량보다 낮은 수준이었다. 5) 신체계측 및 영양소섭취량과 골밀도와의 상관관계분석에서 신체계측에서 체중(p < 0.05), 엉덩이둘레(p < 0.01), 제지방함량(p < 0.05), 총수분함량(p < 0.05)이 높을수록 요추골밀도가 유의하게 높았으나, 영양소섭취량은 골밀도와 유의한 상관성을 보이지 않았다. 6) 주요 식품군의 식품섭취실태와 골밀도와의 상관관계분석에서 버섯군의 식품섭취횟수(p < 0.05, p < 0.05)가 많을수록 요추(L3, L24)의 골밀도가 각각 유의하게 높았으며, 유지류군의 섭취식품수(p < 0.05)가 많을수록 와드삼각부의 골밀도가 유의하게 높았으나 곡류군의 섭취식품수(P < 0.01, p < 0.05, p < 0.05)가 많을수록 대퇴부 3부위 (대퇴경부, 대퇴전자부 및 와드삼각부)의 골밀도가 각각 유의하게 낮은 것으로 나타났다. 이상의 결과에서 폐경전 성인여성의 경우 요추보다는 대퇴부 3부위의 경우 골다공증이나 골감소증의 비율이 상대적으로 높았으며, 특히 대퇴경부에 있어서 골다공증이나 골감소증의 비율이 가장 높았음을 알 수 있다. 또한 요추와 대퇴부 3부위의 골밀도는 영양소섭취량과 유의한 상관관계를 보이지 않았으나 체중, 엉덩이둘레 등의 신체계측요인과 곡류군, 버섯군 및 유지류군 등의 주요 식품군의 식품섭취실태와 밀접한 상관관계를 보이고 있음을 알 수 있다. 따라서 성인여성에 있어서 골밀도손실을 예방하기 위해 평상시 적절한 체중유지와 아울러 식생활지침에 있어서 주요식품군별로 적절한 섭취식품의 수, 섭취량 및 섭취빈도에 대한 영양교육이 필요한 것으로 생각된다.

The Relationship Between Lumbar Spine Bone Mineral Density and Cardiovascular Risk Factors in Premenopausal and Postmenopausal Women (폐경 전후 여성의 요추 골밀도와 심혈관 위험인자와의 연관성)

  • Recent studies suggest a possible pathogenic linkage between the osteoporosis and atherosclerosis. We investigated the relationship between cardiovascular risk factors, including high sensitivity C-reactive (hs-CRP), hs-CRP and bone metabolism in females. Anthropometric measurements were performed on 300 women, and cardiovascular risk factors, including fasting blood glucose, lipid profiles and hs-CRP. An atherogenic index was calculated using the serum total cholesterol level divided by the high-density lipoprotein cholesterol level. The lumbar spine bone mineral density was measured using dual X-ray abosorptiometry. By bivariate analysis, the lumbar spine BMD showed negative correlations with age, systolic and diastolic blood pressures, serum total cholesterol, high-density lipoprotein cholesterol. The Age, BMI and high-density lipoprotein cholesterol showed significant correlations with the lumbar spine BMD. From linear regression analyses of all the study subjects, age, high-density lipoprotein cholesterol were found to be determinants of the lumbar spine BMD($R^2=0.272$).

The Relationship of Age, Body Mass Index, and Individual Habit to Bone Mineral Density in Adults (성인의 연령, 체질량지수 및 생활습관과 골밀도의 관계)

  • We studied the change of bone mineral density (BMD) by age, body mass index (BMI), coffee, carbonated drink, alcohol, smoking, and exercise in adults who checked in health center. The number of study subjects was total 268 persons (women of 136 persons and men of 132 persons). The BMD was determined in lumbar spine and femoral neck by dual energy x-ray absorptiometry. And we got some results as below : 1. In women, mean body height was $155.8{\pm}6.0cm$, mean body weight was $56.8{\pm}7.9kg$, and mean BMI was $23.4{\pm}3.1kg/m^2$. In men, mean body height was $169.1{\pm}6.0cm$, mean body weight was $69.0{\pm}9.5kg$, and mean BMI was $24.1{\pm}2.7kg/m^2$. 2. BMD decreased as age increased, and the age was the most determinant factor for BMD (p<0.01). Women's BMD decreased rapidly in the groups aged $\geq$50s, while men's BMD decreased gradually with age. In addition, for both sex, lower BMD was measured in lumbar spine than in femoral neck. 3. BMD increased in high BMI, and BMD with BMI increased distinctly in the group aged 50s. But their relationship was not significant. 4. In view of the distribution by three BMD categories, women's BMD was mostly normal in the groups aged $\geq$40s, but the rate of osteopenia and osteoporosis was similar in the group aged 50s, and the rate of osteoporosis was the highest in the groups aged 60s and 70s. Men's BMD was mostly normal through all groups except the group aged 70s. 5. Coffee and carbonated drink were not influenced in BMD. But alcohol-drinking group showed higher BMD than non-drinking group, and alcohol was statistically significant determinant for BMD (p<0.05). Smoking and exercise were not statistically significant determinant of BMD.

A Study on the Change of Bone Mineral Density(BMD) by Life Habit and Physical Condition (성인의 신체조건 및 생활습관에 따른 골밀도 변화에 대한 연구)

  • Purpose: To evaluate the correlation between BMD and life habit such as drinking. exercise smoking or physical condition such as age, sex, height, weight, body mass index(BMI). Materials and Methods: I evaluated the BMD of the femoral neck and L2-L4 spines of 321 persons who took a regular health screening in Woosuk university oriental medical hospital from February to April in 2006 by dual energy bone mineral densitometry. Results: The age of persons ranged from 20 years to 75 years(mean $45.10{\pm}11.54$) and there were 160 males and 161 females. In males, BMD of the femoral head was highest at 2nd decade, BMD of the spine was highest at 4th decade, and BMD of both femoral head and lumbar spine was lowest at 6th decade. In fenales, BMD of both femoral head and lumbar spine was highest at 4th decade and lowest at 6th decade. Among the various physical conditions, only height of persons showed significant correlation with BMD in both males and females. BMD was increased according to increasing height. In males, BMD of persons who had habit such as drinking, exercise or smoking did not show significant change statistically. But in females, drinking group showed high BMD relative to non-drinking group in both femoral head and lumbar spine. Conclusion: BMD was different according to age, sex, height and life habit. Especially aged people showed osteoporotic change progressively. More persistent effort is needed to find out the factors decreasing BMD for prevention of problems by osteoporosis.

  • 본 연구는 안전하면서 효율적으로 비만을 개선시키는데 도움이 될 수 있는 식품소재로 목이버섯을 선정하여 복부 비만이 있는 중년 여성의 골밀도 및 혈청지질농도에 미치는 영향에 대하여 알아보고자 하였다. 30~50대의 복부비만 여성 30명에게 4주 동안 대조군은 목이버섯을 섭취시키지 않았고, 실험군은 목이버섯을 식사와 함께 섭취하도록 하였다. 실험 전 후 골밀도(T-score, Z-score), 총 콜레스테롤, HDL-콜레스테롤, LDL-콜레스테롤, 중성지방을 측정하였다. 연구결과, 목이버섯 섭취군과 대조군 간에 T-score와 Z-score는 유의한 차이를 보이지 않았지만, 목이버섯 섭취군의 혈청 총 콜레스테롤, LDL-콜레스테롤 및 중성지방 수준이 대조군에 비해 유의하게 감소되었다.

Relationships of Changing Social Atmosphere, Lifestyle and Bone Mineral Density in College Students (대학생의 골밀도와 사회분위기 및 생활습관과의 관계)

  • The decrease of bone mineral density gives rise to the outbreak of osteopenia and makes the possibility of a bone fracture. It makes health problems in society. It's very important to prevent osteopenia in advance. Also it's critical to prevent and take care of it in adolescent because it's the most developing period comparing to middle ages because that bone mineral density decreases. There are genetic, physical and enviromental factors that affect bone mineral density. Recently, a lifestyle and eating habits are also changing as the society atmosphere is gradually doing. This study have shown that 134 women and 75 men was chosen and responded to the survey of measuring bone mineral density and investigating a lifestyle. The measure of bone mineral density is to use Dual energy X-ray absorptiometry(DEXA) and check femoral neck and lumbar spine. Also questionaries was required to pre-made survey about their lifestyles. Analysis of data was done with SPSS program. Multiple regression analysis was used for the relation of bone mineral density, the heigths and BMI. The sample of Groups are checked for drinking, smoking or excercising about differences by t-test. The results of the experiments were; first, there is statistically significant differences in the comparisons between BMD and BMD. But there isn't any special correlation between drinking, smoking and BMD. Secondly, bone mineral density becomes low related to an intake of caffeine. Particularly, this is statically significant on women. Also there is statically significant correlation between femoral neck and quantity of motion for both men and women. Third, there is significant relation between eating habits and bone mineral density on women's lumbar spine. However, there is no significant relation between men's lumbar spine and women's one. Therefore, to prevent osteopenia, it's good to abstain from intaking caffeine within an hour after a meal. In addition, it's helpful to walk or run regularly and have a balanced meal.

Effects of combined intervention of isoflavone supplementation and exercise on bone metabolism in growing rats (이소플라본 보충과 운동의 병행이 성장기 쥐의 골격대사에 미치는 영향)

  • Purpose: This study examined the effects of combined intervention of isoflavones and exercise on bone mineral density, bone mineral content, and biochemical bone markers. Methods: Forty rats were divided into four groups; Control, Isoflavones (IF), Isoflavones + Running (IFR), and Isoflavones + Swimming (IFS). All of the rats in this study were fed an experimental diet and deionized water ad libitum for nine weeks. Bone mineral density (BMD) and bone mineral content (BMC) were estimated using PIXImus (GE Lunar Co, Wisconsin.) in spine and femur nine weeks after feeding or training. Results: The combined intervention did not affect weight gain, mean food intake, or food efficiency ratio. The serum concentrations of ALP and osteocalcin were not significantly different by combined intervention. The urinary DPD crosslinks values were not significantly different by combined intervention. There were no significant differences in serum PTH, calcitonin, and estradiol among all groups. Spine BMD, spine BMC and femur BMC were not significantly different by combined intervention. However, femur BMD was significantly higher in the IFR group than in the control group. Compared with the control group, spine BMD, spine BMC, and femur BMD per weight were markedly increased in the isoflavones supplementation and combined intervention group. In addition, femur BMC per weight was significantly higher in the IFS group than in the control group. Compared with the isoflavones supplemented group, BMD and BMC were not significantly different by combined intervention. Conclusion: It can be concluded that isoflavones supplementation or combined intervention of isoflavone and exercise had a beneficial effect on spine and femur peak bone mass in growing female rats.

Effects of Soy Protein on Bone Mineral Content and Bone Mineral Density in Growing Male Rats (콩단백질이 성장기 수컷흰쥐에서 골함량과 골밀도에 미치는 영향)

  • The purpose of this study was to determine to which differences in the source of protein (soy vs casein) and of isoflavones in soy protein are responsible for differential effects of bone mineral density and bone mineral content. Thirty 21-d-old Sprague-Dawley young rats were divided into 3 groups: the control group was find a casein-based diet, the soy concentrate group was fed soy protein with totally reduced isoflavones content (isoflavone 0.07 mg/g protein), and soy isolate group was fed with a higher isoflavone content (isoflavone 3.4 mg/g protein) than normal. The animal was scanned to determine the BMD and BMC using dual energy X-ray absorptiometry (DEXA, Lunar Corporation, Madison, WI). The soy concentrate group had significantly higher total body calcium/weight and total mineral content/weight than the casein group. The soy isolate group had significantly greater total bone mineral density/weight, spine bone mineral density/weight, and femoral bone mineral density (in g/$\textrm{cm}^2$ than the control and soy concentrate group. The findings of this study suggest that soy protein and isoflavones in soy protein are beneficial for bone-formation in growing male rats. Therefore exposure to these soy protein and isoflavones early in life may have long-term health benefits for bone diseases such as osteoporosis.

Analysis of Bone Mineral Density Status in Korean Mother : In the Center of One Obstetric Hospital (국내 산모의 골밀도 현황 분석 : 일개 산부인과 전문병원중심으로)

  • This study was conducted to provide basic data for prevention of osteoporosis by examining the factors affecting bone mineral density in pregnant mothers. This investigation was a descriptive research study that analyzed a total of 137 medical records. Differences in T-score between general and obstetric characteristics were assessed by independent t-tests and one way ANOVA followed by Scheffe's post-hoc test. The mean age of the subjects was 32.4 years and the mean T-score was 0.34. The BMI measured at 4 weeks after delivery was significantly higher in the underweight group than in the normal group and the overweight group (F=11.935, p<.001). The BMD was significantly higher in subjects with sufficient vitamin D than in subjects with insufficient vitamin D, in subjects with insufficient vitamin D than in subjects with deficiency vitamin D (F=4.906, P=0.009). BMD was significantly higher in cases without experience of abortion than those with more than one experience of abortion (t=4.264, p<.001), and the normal delivery was significantly higher than that of cesarean section (t= 2.019, p=.046). Based on the results of this study, it is necessary to actively prevent the factors affecting BMD during pregnancy and delivery. The findings presented herein can be used as basic data for the management of osteoporosis.
