Plasma Transforming Growth Factor-$\beta$1 Levels of Cancer Patients (암 환자의 혈장 Transforming Growth Factor-$\beta$1 농도)

  • To evaluate the usefulness of transforming growth factor-$\beta$1 (TGF-$\beta$1) as a new tumor marker, we determined the plasma TGF-$\beta$1 levels using sandwich ELISA assay in cancer patients. Patients with three most common adult cancers in Korea (stomach, liver and breast cancer) and children's cancers (leukemia and two kinds of solid tumor) were enrolled for the study. Furthermore, 39 individuals were subjected to age and sex-stratified plasma TGF-$\beta$1 analysis. No statistical difference was demonstrated with respect to age or sex. The mean plasma TGF-$\beta$1 level (16.0 ng/ ml) of stomach cancer patients was significantly higher than that (8.3 ng/ml) of controls. However, there was no difference among the mean plasma TGF-$\beta$1 levels of liver, breast cancer patients and controls. Seven of 16 patients (43.7%) with stomach cancer, one of 8 (12.5%) with liver cancer, and one of 7 (14.3%) with breast cancer showed higher TGF-$\beta$1 levels compared to controls. Plasma TGF-$\beta$1 concentrations of five leukemic children remained in the normal range regardless of the remission state. In contrast, initial high TGF-$\beta$1 levels from two children with solid tumors returned to normal range on surgical resection of tumors. From the above results, we could conclude that plasma TGF-$\beta$1 levels of apparently healthy individuals seem to be rather constant irrespective of difference in age or sex, and the plasma TGF-$\beta$1 has the limited value as a screening test for the diagnosis of aforementioned adult cancers because of its low sensitivity. Finally, additional studies need to be pursed for the large number of stomach cancer and pediatric solid tumor patients in order to reach a secure conclusion on the usefulness of plasma TGF-$\beta$1 as a tumor marker in these patients.

  • Photodynamic therapy (PDT) is one of the expectable current cure operation methods. Tumor tissue is treated by abundant oxygen in a body and generated singlet or free radical from exterior laser diode and photosensitizer. Current problem of PDT is the low penetration power of the light beam in a deep seated large tumor and solid tumor thus results in low treatment outcome. In the study, we tried to develop interstitial photodynamics therapy treatment to solve this problem. As the accurate determination of light dosimetry in biological tissue is one of the most important factors affecting the effectiveness of PDT, parameters used in this study are the optical property of biological tissue. Since biological tissues have large scattering coefficient to visible light the penetration depth of a biological tissue in visible light region is only $15\~20$ mm. We showed that it is possible to measure fluence rate and penetration depth within the biological tissues by Monte Carlo simulation very well. Based on the MC simulation study, the effectiveness of interstitial photodynamic therapy on tumor control in solid tumor was proved through in vivo animal experiment.

Effect of Mycelia Extracts of Mushroom-Cultured Ginseng By-product on Proliferation in Cancer Cell Lines (인삼박으로 배양된 버섯 균사체 추출물이 암세포 증식에 미치는 영향)

  • This study was conducted to investigate the effect of mycelia extracts of mushroom-cultured ginseng by-product on electron donating ability and proliferation of hepatic cancer cell (Hep3B) lines and sarcoma 180(S-180). The ginseng by-product was obtained from ginseng residues generated in processing of ginseng water extract. Mushroom strains used for preparation of mushroom mycelia cultured with ginseng residue were Phellinus linteus, Ganoderma lucidum, Coriolus versicolor and Lentinus edodes. The electron donating abilities of the test samples were increased in a dose-dependent manner in the range of 500ppm to 10,000ppm, and Coriolus versicolor extract showed the most potent activity among four mycelia extracts. In an anti-cancer test using Hep3B cells, ethanol extract showed higher antiproliferating effect than water extract. Ethanol extract of Lentinus edodes showed growth-inhibitory effect of 99.1% at 5,000ppm. All of mycelia extracts of mushroom showed the tumor suppressive effect in mice injected with S-180 cells. The growth­inhibitoy rates against tumor cells were 59% for Phellinus linteus, 61% for Ganoderma lucidum, 65% for Coriolus versicolor, 56% for Lentinus edodes. In conclusion, these results suggest that mycelia extracts of mushroom cultured with ginseng by-product have an antiproliferating effect against Hep3B cell and S-180 tumor cells.

A Test for Psychobiologic Entropy Model on Cancer Related Fatigue among Patients with Solid Tumors (고형암 환자의 암성피로에 대한 정신생리학적 엔트로피 모델 검증)

  • Purpose: The purpose of this study was to test a Winningham's psychobiologic entropy model (PEM) on cancer related fatigue (CRF) among patients with solid tumors. Methods: Participants consisted of 213 patients with solid tumors recruited from December, 2012 through June, 2013, in a university hospital, in Hwasun, South Korea. Primary symptoms, adjustment, physical activity, status of nutrition and fatigue were measured using structured questionnaires. Collected data were analyzed using SPSS 21.0 and AMOS 21.0 programs. Results: The modified model tested provided a reasonable fit to the data ($x^2=65.80$ [df=30, p<.001], TLI=.92, CFI=.95, RMSEA=.08, SRMR=.07). Primary symptoms (dyspnea, anxiety, depression and insomnia) had direct positive effects on CRF. Adjustment and status of nutrition showed indirect negative effects on CRF. However, the impact of physical activity was not significant. These variables explained 49.2% of the variance of CRF among solid tumor patients. Conclusion: The findings demonstrate that the tested model explain some CRF among solid tumor patients and warrant future research considering the cancer-related clinical factors of the given population.

Antitumor Effect of Persimmon Leaves in vivo using Sarcoma-180 Cells (Sarcoma-180 세포를 이용한 in vivo에서 감잎의 항암효과)

  • Antitumor effects of hexane, chloroform fractions and tannins from persimmon leaves were studied by using sarcoma-180 tumor cells. The solid tumor growth was inhibited most effectively when 0.5mg/kg of hexane fraction was administerated to the Balb/c mouse. The life prolongation effects were 35.6%, 11.5% and 29.9%, respectively when hexane fraction, chloroform fraction and tannins from the persimmon leaves were administerated to the mouse. Spleen index, a marker for immunological activity, was increased in mice administrated hexane fraction, chloroform fraction and tannins of persimmon loaves, compared with the control group.

Anti-tumor effect of Inonotus obliquus in xenograft animals with EBV+human gastric carcinoma (Inonotus obliquus (차가버섯) 추출물의 EBV 양성 인간위암에 대한 in vivo 항종양 효능 연구)

  • Inonotus obliquus is a medicinal mushroom with a variety of biological activities. It has reported to have strong anti-cancer, antioxidant and anti-inflammatory properties. EBV+ gastric carcinoma is one of the most common EBV-associated cancers that were caused by latent EBV infection. In this study, we investigated the anti-cancer effects of ethanol extract of I. obliquus using in vivo xenograft animal models implanted with EBV+ human gastric carcinoma (SNU719). We also explored the molecular mechanisms responsible for its anti-cancer activity. The result indicated that the extract of I. obliquus had an anti-cancer effect in in vivo xenograft mice with EBV+ gastric carcinoma (SNU719). Extract of I. obliquus also showed a great effect on inducing the expression of p53, p21 and Bax in tumor tissue derived from EBV+ human gastric carcinoma, and these were correlated with increased expressions of the cleaved forms of caspase-9 and Parp. Also, I. obliquus attenuated the expression of viral proteins, BZLF-1 and LMP-2 in tumor tissue from EBV+ human gastric carcinoma.

In vitro and in vivo Antitumor Activity of the Fruit Body of Phellinus linteus (상황버섯 자실체의 in vitro 및 in vivo 항암활성)

  • This study was undertaken to investigate immunochemotherapeutic activity against tumor growth and cytotoxic activity against tumor cell lines. Phellinus linteus (PL), which was artificially cultured in Morus alba, prolonged significantly the survival rate of mice intraperitoneally implanted with sarcoma 180 and inhibited solid tumor growth on mice subcutaneously implanted with sarcoma 180. The acetone precipitate of water extract of PL was better than its water extract. However, PL showed little cytotoxic activity against tumor cell lines.

  • Radiotherapy and chemotherapy can effectively control cancer but can also cause new second cancers to develop as long-term complications especially in childhood cancer. We experienced two patients with second malignant solid neoplasm who had been treated with radiation and chemotherapy for childhood cancers One female patient with rhabdomyosarcoma of the right popliteal fossa was treated with radiotherapy at total dose of 54Gy Three years and seven months later, osteosarcoma developed in the field of radiation therapy The other male patient with non-Hodgkin's lymphoma of the small bowel was treated with radiotherapy and leiomyosarcoma developed in the field of radiotherapy 18 years later. We reviewed the literature of the second malignant neoplasm in children in respect of risk factors. The risk for a second primary cancer following radiotherapy or chemotherapy emphasizes the need for life long follow-up of patients receiving such treatments, particularly patients treated for childhood cancers.

Safety and Anticancer Effects of Platycodon grandiflorum Extracts (도라지 추출물의 안전성 및 항암 효과)

  • This study investigated the antimutagenic and anticancer effects of Platycodon grandiflorum extract (PGE) and its fractions against carcinogenic N-nitrosodimethylamine (NDMA) and genotoxicity. The Ames Salmonella mutagenicity test employing histidine mutants of Salmonella Typhimurium TA98 and TA100 was used to examine the mutagenicity of PGE and its fractions. Bacterial reversion assay with S. Typhimurium TA98 and TA100 did not show a significantly increased number of revertant colonies. The same test was used to examine the ability of PGE and its fractions to prevent acquisition of N-methyl-N'-nitro-N-nitrosoguanidine- and 4-introquino-line-1-oxide-induced mutations. PGE and its fractions inhibited mutagenesis in a dose-dependent manner. Among the fractions, ethyl acetate fraction from PGE (PGEA) exhibited a higher antimutagenic effect than other fractions. PGE and its fractions suppressed the growth of cancer cell lines, including human cervical adenocarcinoma, human hepatocellular carcinoma, human breast adenocarcinoma, human lung carcinoma, and transformed primary human embryonic kidney cells. In addition, we evaluated the antitumor activity of PGEA and its fractions in sacorma-180 solid tumor-bearing mice. In vivo anticancer activity results showed that PGE and its fractions could more effectively suppress tumor growth than the control. PGEA showed higher in vitro and in vivo anticancer effects than PGE and other fractions, and PGEA inhibited NDMA formation. Thus, we showed that PGEA has antimutagenic and anticancer activities, making it a candidate anticancer material under these experimental conditions.

Effect of Oral Care Protocol on Oral Cavity Status and Self-care Performance in Solid Cancer Patients Receiving Chemotherapy (구강관리 프로토콜 적용이 항암화학요법을 받는 고형암 환자의 구강상태 및 자가간호수행에 미치는 효과)

  • The purpose of this study was to identify the effectiveness of oral care protocol including individual oral care education, cryotherapy and benzydamine gargling on oral cavity status and self-care performance in solid cancer patients receiving chemotherapy. Thirty-one patients were enrolled by convenience sampling in this study and allocated to control (n=18) and experimental group (n=13). In the intervention group, individual oral care education, oral cryotherapy and benzydamine gargling were applied, while the control group received pre-existed oral care. Oral cavity status and self-oral care behavior were measured in five periods. The data were analyzed using the Mann-Whitney test, Friedman test, and Chi-square test. There was no significant difference in oral cavity status between the groups. But the two groups showed the poorest oral cavity status on $14^{th}$ day. Self-care behavior was significantly higher in the intervention group than the control group. This effect had maintained for 4months after completion of study. The results showed that individual oral care education by oncology nurse is effective to encourage patients to do self-care behavior and a further study is needed to explore the effect of cryotherapy and benzydamine gargling on oral cavity status of patient with solid tumor.
