Effects of an Experiences Sharing Self-management Program for Hypertension and Diabetes Patients in Primary Public Health Center (보건진료소 중심 고혈압, 당뇨병 환자의 경험공유 자가관리 프로그램 효과)

  • This study examined the effects of an Experience Sharing Self-management Program (ESSP) on physiological indexes, knowledge, and self-management implementation among hypertension and diabetes patients in a primary public health center in G city. A quasi-experimental research was conducted with a non-equivalent control group pretest-posttest design. The experimental group (n=30) participated in a 12-week ESSP, and the control group (n=25) received only general services of a primary public health center between May 1 and October 31 in 2018. Data were analyzed with x2 test (Fisher's exact test), Mann-Whitney test, and Ranked ANCOVA using SPSS/WIN 22.0 program. The results showed significant differences in systolic blood pressure (F=14.56, p<.001), diastolic blood pressure (F=7.05, p=.012), knowledge (F=14.30, p=.001), self-management implementation (F=29.73, p<.001) of hypertension patients, knowledge (F=7.92, p=.010), and self-management implementation (F=4.54, p=.044) of diabetes patients between the two groups. Therefore, the ESSP was effective for reducing the blood pressure of hypertension patients and improving the knowledge and self-management implementation among hypertension and diabetes patients in a primary public health center.

Risk of Stroke according to the Duration of Diabetes Mellitus with Hypertension (고혈압을 동반한 당뇨병에서 유병기간에 따른 뇌졸중 위험도)

  • Stroke is a major cause of death and disorder, and diabetes mellitus (DM) is a significant risk factor of stroke. This study examined the importance of target blood pressure (<140/<90 mmHg) management in patients with DM. Based on the data of the 6th National Health and Nutrition Examination Survey, a total of 16,389 patients aged 18 to under 80 were assessed through regression analysis to estimate the risk of stroke through blood pressure control. The prevalence of stroke according to the presence or absence of DM was higher in the diabetic group, and a longer duration of DM indicated a higher risk of hypertension and stroke. The hazard ratio of stroke was lower in the maintenance group (0.57) of target blood pressure (<140/<90 mmHg) compared to the high blood pressure group(>140/>90 mmHg). Despite the accompanying treatment for DM and hypertension, the frequency of hypertension increased, and the prevalence of stroke increased after five years of DM. Hence, effective blood pressure management at this time is important. The risk of stroke should be reduced by continuously managing blood pressure in DM.

A Comparative Study on Knowledge of Hypertension and Blood Parameters Between a Well-Controlled Hypertension Group and an Uncontrolled Hypertension Group Commuting to a Public Health Center (보건소의 혈압이 조절되는 고혈압 환자와 조절되지 않는 고혈압 환자의 고혈압관련 지식 및 혈액성상)

  • The purpose of this study was to compare knowledge on hypertension and blood parameters through blood pressure control of hypertensive patients commuting to a public health center. Hypertensive subjects were classified according to their blood pressures into a well-controlled group (SBP<140 mmHg and DBP<90 mmHg, n=44) and an uncontrolled group ($SBP\geq140mmHg\;or\;DBP\geq90mmHg$, n=41). All subjects were interviewed for clinical characteristics, effort for health maintenance and knowledge on hypertension and nutrition. Biochemical measurement of blood urea nitrogen, creatinine, triglyceride, total cholesterol, HDL-cholesterol and fasting blood glucose (FBG) were obtained from subjects. The knowledge about hypertension and effort for health maintenance were not significantly different between the groups. However, the nutrition knowledge score of the well-controlled group was significantly higher (p<0.05) than that of the uncontrolled group. On the test of nutrition knowledge, 97.7% of the well-controlled group answered correctly to the item, 'Medication is an easy way to control blood pressure'. This result was significantly higher (p<0.05) than that of the uncontrolled group of whom 82.9% answered correctly. Participants in both groups had only a little understanding about the importance of a balanced diet. Blood parameters were in the normal range and there was not a significant difference between the groups. However total cholesterol, triglyceride and fasting blood glucose levels of the uncontrolled group were higher than those of the well-controlled group. Therefore, education programs about the continuous control of hypertension through careful monitoring, medication, and exercise are required. The uncontrolled group was especially in need of strengthening education. Also more effective nutrition education programs about low salt, high fiber, low cholesterol, and balanced diets are required for hypertensive patients commuting to public health center.

  • The purpose of this study was to identify the effectiveness of the 8-week telephone counseling hypertension self-management program provided through welfare center nurses and nursing student volunteers for hypertensive patients with physical disabilities and brain lesions who use the welfare centers for the disabled. A quasi-experimental with non-equivalent control group pretest-posttest design was implemented. From December 2019 to May 2020, data from 28 controls and 29 experimental groups who completed pre and post-investigation were analyzed using the SPSS program. As a result of the main study, after the intervention, the knowledge of hypertension, seriousness and sensitivity among the health beliefs, and quality of life EQ-VAS indicated significant difference between the experimental group and the control group. Therefore, efforts to improve self-care behaviors and quality of life by applying various self-care programs to hypertensive patients with the disabled which require long-term health care will be needed continuously.

  • In order to study on the Na, K, Ca, Mg content of low sodium diet used in hospital the mineral content of low sodium diet and general diet was measured at C Hospital in Taegu during June 26~28 in 1990. Results were as follows; 1)Energy per day was basis on 2300 Kcal in low sodium diet and general diet. 2) Na content per day of low sodium diet and general diet were 2510.0mg and 3580.3mg 3) K content per day of low sodium diet and general diet and general diet were 676.1mg and 1132.9mg. 4) Ca content per day of low sodium diet and general diet were 662.9mg and 605.4mg 5) Mg content per day of low sodium diet and general diet were 443.1mg and 278.7mg.

Comparison of Anger between Patients with Essential Hypertension and Normal Controls (본태성 고혈압환자들과 정상인들 간의 분노의 비교)

  • A comparison was made regarding the extent of anger between 50 hypertensives and 50 normotensives, using anger scale. In addition, correlation was investigated between the extent of anger and biological variables such as systolic and diastolic blood pressure, serum total cholesterol, high density lipoprotein(HDL) and triglyceride in each of both the groups. The two groups were matched to each other regarding age, sex, the level of education and body mass index(BMI). The hypertensives scored significantly higher than normotensives in anger suppression, whereas normotensives scored significantly higher than hypertensives in anger expression. In hypertensives, scores of anger suppression and anger expression did not significantly correlate with blood pressure, serum total cholesterol, HDL and triglyceride, respectively. In normotensives, however, scores of anger suppression correlated positively with systolic blood pressure. Male hypertensives scored significantly higher than female hypertensives regarding anger expression, whereas male normotensives scored significantly higher than female normotensives regarding anger expression and anger total subscale, respectively. These results suggest that hypertensives suppressed more anger than normotensives, and that anger suppression in normotensives was more likely to increase blood pressure, supporting the previous reports that suppressed anger may be related to hypertension.

Tumor Recurrence in Hepatocellular Carcinoma Patients after Radiofrequency Ablation: Portal Hypertension as an Indicator of Recurrence of Hepatocellular Carcinoma (간세포암 환자의 고주파열치료 후 종양 재발: 예후인자로서 문맥고혈압)

  • Purpose: To evaluate the effect of portal hypertension on the tumor recurrence in patients with hepatocellular carcinoma (HCC) and without hepatic decompression following radiofrequency ablation (RFA). Materials and Methods: Treatment-naïve HCC patients within the Milan criteria and with Child-Pugh class A were included in this study, who had performed RFA in our hospital between January 2010 and March 2017. Univariate and multivariate analyses using the Cox proportional hazard model were performed to find the predictors of local or distant tumor recurrence. Results: Overall, 178 patients were included in this study. Median follow-up period was 40.2 months. The difference in the local tumor progression rates depending on the absence or presence of portal hypertension was not statistically significant (p = 0.195). The 1-, 3-, and 5-year distant intrahepatic tumor spread rates were 6.6%, 29.5%, and 537% in patients without portal hypertension, and 23.4%, 51.9%, and 63.6% in patients with portal hypertension, respectively. The difference was statistically significant (p = 0.011). Univariate and multivariate analysis showed that portal hypertension was an independent predictor for distant intrahepatic tumor spread (p = 0.008). Conclusion: For HCC patients with Child-Pugh class A, portal hypertension adversely affected distant intrahepatic tumor progression.

The Effects of Health Education on Treatment Compliance of Patients with High Blood Pressure and Diabetes in Private Clinics (개인 의료기관에 내원한 고혈압 및 당뇨병환자에게 실시한 건강교육이 치료 순응도에 미치는 영향)

  • To provide fundamental educational materials for the effective management of patients with high blood pressure and diabetes in korea, and to expand the scope of such educational businesses the survey was administered between September 2007 and February 2008 to 1,213 consenting patients suffering from high blood pressure and diabetes in private medical centers within Daejeon in Korea. The patients were provided with standardized education materials and were monitored whether they were conducting a controlled management of their blood pressure and diabetes level as well as monitoring changes in their daily routine. Out of the 1,213 patients, 51.3% of the patients suffered only from high blood pressure, 26.3% suffered only from diabetes and 22.4% suffered from both high blood pressure and diabetes (Condor difference was not considered in this research). Physical education was the most commonly used education upon visitation to these medical centers, which was then followed by eating habits and weight management, respectively. The education demanded and preferred from the patients was similar to the education provided during the visitations. 56.7% of the medical doctors responded that the majority of the patients were passive in controlling and managing their illnesses. 63.7% of the participants believed that the education provided through this research was effective.

  • This study attempted to explore the experience of patients' having hypertension and diabetes participation in health care. 11 chronic patients who regularly visited community health centers for more than a year participated in this study. The data was collected through individual interviews and analyzed by thematic analysis method. As a result, 6 sub-themes and 3 themes derived from the data. The themes were 'Trying to utilize short consultation time efficiently', 'Answering within narrow limits to healthcare provider', 'Being lack of continuous motivation for health care'. The result showed the need for nurses to provide accurate and sufficient health information and motivate patients in supportive manner to encourage chronic patients' participation in health care.
