The control of the hypertension in rural primary health care settings in Korea (농촌지역 일차건강관리 기관의 고혈압환자 관리실태 -전남지역을 중심으로-)

  • The control of the chronic degenerative diseases becomes a challenge in Korea as the aging of the population progresses. Accordingly, the importance of the control of the hypertension, one of the major chronic degenerative diseases, in the primary health care settings increases. However, such control activities are still minimal. This study gives brief description of how the rural residents who are currently registered for the control of the hypertension feel about the activities of the health centers, subcenters and the CHP posts, in relation to the control of the hypertension. We also describe the knowledge and practice of the registered hypertensive. In general, the knowledge, the acceptance and the satisfaction of the respondents about the hypertension control activities were not very high. Respondents being managed by the health center showed the highest knowledge, acceptance and satisfaction, and those being managed by the subcenters ranked lowest. The knowledge about the hypertension was not satisfactory. There were great variations among the items in terms of the proportion answered correctly. As many as 60% of the respondents take medication regularly but only 4.5% were practicing weight control regularly. Despite some limitations of the study, the results can be very useful for those planning hypertension control programs. The educational materials and the dietary protocols need to be developed in accordance with the regional social and food pattern, so that they can be utilized with minimal modification for each beneficiary. We also suggest to minimize the area a health personnel covers. In other word, running several small health posts at village level would be more effective than running a subcenter at the township level, in terms of the hypertension control.

A Study on Self-care Behavior Types of Hypertensives : Q-methodological Approach (고혈압환자의 자가간호행위 유형에 관한 연구 : Q 방법론 적용)

  • Essential hypertension is a typical chronic disease requiring adequate and continuous management. And many studies supported that self-care was the essential factor to promote the wellbeing. The purpose of this study is to identify and understand the behavior patterns of self-care in hypertensives. As a research method, 35 Q-statements were collected through Individual interviews and review of the related literatures. 21 subjects were interviewed and the data were analyzed by the PC QUANL program with principal component analysis. There were 6 different self-care types classified as follows 1) Type 1 was the self-oriented control type, monitoring the blood pressure and taking the low salt diet. But they didn't take the anti -hypertensive drug and visit the health agency regularly. 2) Type 2 was the stress-control type. Their main activities were meditation to relieve the stress and communication with family. 3) Type 3 was called daily-life control type. This type tried to make their mind comfort and think positively. They also preferred walking and exercise regularly. 4) Type 4 was the medical-oriented control type, taking the anti-hypertensive drug, visiting the medical personnel and following the medical regimens. 5) Type 5 was the medication-oriented type. They only took the anti -hypertensive drug regularly and didn't any other self-care like as monitoring the blood pressure, taking the low salt diet and exercise. 6) Type 6 was called non-medication control type. This type had no medication, but tried to visit the health agency and health personnel. From the above results, it can be concluded that the self-care types were very various and self-care education have to provide individually according to the characteristics of self-care type. Another repeated study can be recommended to improve the nursing intervention the self-care behavior in chronic patient like as diabetics or rheumatoid arthritis.

The Relationship between Anxiety, Depression and 24-hour Ambulatory Blood Pressure in Hotel Employees (호텔 종사자들에서 불안 및 우울과 24시간 활동혈압 관련성)

  • Objectives: Anxiety and depression are known to be associated with hypertension, and blood pressure can vary spontaneously throughout the day. The aim of this study was to evaluate anxiety, depression and 24-hour ambulatory blood pressure (24-h ambulatory BP) in employees at their worksite. Methods: A total of 107 volunteers among 136 employees at a hotel in Gyeongju, Korea were enrolled in this study between December 2009 and March 2010. The Beck Anxiety Inventory (BAI) and the Beck Depression Inventory (BDI) were used to assess anxiety and depression, respectively. Blood pressure was measured using a 24-h ambulatory BP monitoring system. Results: No significant differences in either BAI or BDI scores were found when hypertensive individuals were compared to normotensive individuals. The frequency of diastolic non-dipper was significantly higher in individuals with depressive symptoms compared to those without (p<0.05). Depression was significantly associated with diastolic non-dipper (OR: 6.85, 95% CI: 1.50-30.01). Conclusions: The results of this study indicate that depression should be considered when deciding upon blood pressure control regimens, and appropriate additive psychotherapy may be beneficial in the treatment of hypertensive patients.

The Effects of Hypertension Health School Program on Hypertension-related Knowledge, Self-efficacy, Self-care Behavior and Physiological Parameters in Hypertensive Patients (고혈압 건강교실 프로그램이 고혈압 환자의 질병 관련 지식, 자기효능감, 자가간호행위 및 생리적 지수에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of hypertension health school program performed in a public health center located in Y-si. Most interesting were the effects on hypertension-related knowledge, self-efficacy, self-care behavior and physiological parameters for hypertensive patients. Methods: Nonequivalent control group pretest-posttest design was employed. 45 patients with hypertension living in Y-si were assigned into an experimental group (n=23) or a control group (n=22). Experimental group was provided with the 8-weeks hypertension health school program from April 7 to May 20 in 2014. Data were analyzed with SPSS/WIN 21.0 using descriptive statistics, $x^2$-test, Fisher's exact test and t-test. Results: The result indicated a significant difference between the experimental group and control group in the scores of hypertension-related knowledge (t=-10.97, p<.001), self-efficacy (t=-4.56, p<.001), self-care behavior (t=-407, p<.001), physiological parameters including systolic blood pressure (t=2.18, p=.032) and diastolic blood pressure (t=2.74, p=.008) and cholesterol levels (t=5.04, p<.001). Conclusion: The conclusion of this study is that the hypertension self-help group program has a significant effect on the change of hypertension-related knowledge, self-efficacy and self-care behavior for hypertensive patients.

  • With the increasing number of chronic disease patients, the importance of everyday health care has grown more significant. The study researchers expected it would help for patients' health improvement and healthcare service expansion if users check their vital signs in their daily lives and send the results to a medical center's servers through a specific device automatically. This thesis, in line with this idea, seeks to develop a portable healthcare gateway. The gateway is designed in a USB type and can transmit standardized data, operating regardless of a user's location and Personal Healthcare Devices (PHDs). The developed portable healthcare gateway provides effective services in ubiquitous environments to customers, which will improve the health of chronic patients.

An Effect of Qi Gong Gymnastics Program on the Physiopsychological Parameter in Essential Hypertension (기공체조프로그램이 고혈압 환자의 생리.심리적 지수에 미치는 영향)

  • The purpose of this study is to evaluate the effects of Qigong gymnastics exercise program on the physiopsychological parameter in essential hypertension. The design of the research was a one group pretest-posttest design. A total of 20 pateints with hypertension who were from forty to sixty-five years old participated in the study. The Qigong gymnastics program was carried out twice a week for thirty minutes or forty minutes for six weeks from 22, Sep. to 31, Oct. in 1997. In order to evaluate the effect of the Qigong gymnastics program physiological parameter (blood pressure, pulse rate, FVC, FEV1, Epinephrine, Norepinephrine, Cortisol) and psychological parameter(self-efficacy, Life satisfaction) were measured before and after the exercise program. Collected data was analyzed by the paired t-test with SAS package. The result are as follows : 1) There were significant decrease in systolic blood pressure and diastolic blood pressure, but there was no significant change in pulse rate. 2) There were no significant difference in FVC and FEV1 3) There were significant decrease in epinephrine, norepinephrine and cortisol. 4) There were significant improved in general self-efficacy and life satisfaction. 5) The effect of Qigong gymnastics program by measurement time on blood pressure and pulse rate were as follows : In a systolic blood pressure was gradually significant decreased from 1 week to 6 weeks and diastolic blood pressure was significantly decreased after 4weeks. But there was no significant change in pulse rate. This results suggest that Qigong gymnastics are an appropriate nursing intervention for clients with hypertension. For further research is necessary to reevaluate the effect with a letter experimental design and longer period than 8 weeks for the Qigong gymnastics program.

  • In recent years, health management services have been increased according to the increase in interests on health and the development of information technology (IT) based on increases in advanced ages and chronic disease patients. Thus, it requires the monitoring of health conditions and the specialized healthcare services not only in a hospital but also their own home. This study provides the specific notification services related to the context information based on users' bio signal data and the notification services of specific patients and attempts to develop a hypertension monitoring system and a notification service system according to indexes. Because this system considers the context of users by differing it from the conventional monitoring services, it makes possible to obtain more exact measurement values. In addition, it is able to reduce certain health risks through managing specific patients and based on living indexes. Also, it can provide more customized services to users due to the exact and finely classified services.

Approximate Entropy of hypertension: Effect of Anesthesia (정상혈압환자와 고혈압환자의 마취전후의 근사엔트로피의 비교)

  • Background: Recently, measure of heart rate variability and the nonlinear "complexity" of heart rate dynamics have been used as indicators of cardiovascular health. Several investigators have demonstrated that heart rate variability decreased in aging, congestive heart failure and coronary heart disease. Because hypertensive patients showed alternation of cardiovascular homeostasis, we designed this study to evaluate the effect of anesthesia in hypertensive patients with approximate entropy (ApEn). Methods: With informed consent, eighteen normotensive patients and eighteen hypertensive patients were given no premedication. ECG data were collected from 10 minutes before induction to 15 minutes after induction. Collected ECG data were stored into computer binary files. We calculated ApEn from the collected ECG data. Results: Before induction, ApEn of hypertensive patients was significantly lower than normotensive patients(p<0.05). During induction and maintain of anesthesia, there was no difference of ApEn between two groups. During induction and maintain of anesthesia, in normotensive group, ApEn was significantly lower than that of preinduction(p<0.05). And ApEn during maintain of anesthesia was lower than that of induction(p<0.05). During maintain of anesthesia, in hypertensive group, ApEn was significantly lower than that of preinduction(p<0.05). Conclusions: Before induction, ApTn of hypertensive patients is significantly lower than normotensive patients. As anesthesia was deepened, ApEn of two groups were decreased. Because the baroreflex of hypertensive patients is already decreased, decreasing of ApEn of hypertensive patients during anesthesia is less than that of normotnesive patients.

Surgical Treatment of Main Pulmonary Artery Aneurysm with Patent Ductus Arteriosus -A Case Report- (동맥관개존증에 동반된 주폐동맥류의 수술치험 - 수술치험 1례)

  • Pulmonary artery aneurysms are extremely rare conditions usually associated with congenital cardiac defects that cause increased pulmonary blood flow and pulmonary hypertension. The prognosis of pulmonary artery aneurysms is fatal due to the potential for rupture of the aneurysm and the underlying severe pulmonary hypertension. A 40-year old woman was admitted to our hospital with headache following traffic accident. On admission a continuous murmur was heard over the 2nd to 3rd intercostal space along left sternal border and a calcified cystic mass at left hilar portion was incidentally discoverd on chest reontgenogram. Cardiac catherization was diagnostic of a left to right shunt at main pulmoanry artery level and pulmonary hypertension. Pulmonary angiogram demonstrated a large aneurysm of main pulmonary artery extending into proximal left pulmonary artery. The pulmonary artery aneurysm associated with patent ductus arteriosus was diagnosed definitively and the patient was underwent resection of pulmonary artery aneurysm, closure of PDA and Dacron prosthetic graft replacement under cardiopulmonary bypass. The postoperative course was uneventful and the patient was discharged without any problem.

Decision-Tree Analysis to Predict Blood Pressure Control Status Among Hypertension Patients Taking Antihypertensive Medications (약물복용 중인 고혈압 환자의 혈압관리양상 예측을 위한 의사결정나무분석)

  • Purpose: This study was performed to analyze the levels of blood pressure and to identify good or poor blood pressure control (BPC) groups among hypertension patients. The study was based on the Korea National Health and Nutrition Examination Survey (KNHANES VI and VII) conducted from 2013 to 2016. Methods: The sociodemographic and clinical data of 4,151 Korean hypertension patients aged 20-79 years and who were taking antihypertensive medications was extracted from the KNHANES VI and VII database. Descriptive statistics for complex samples and a decision-tree analysis were performed using the SPSS WIN 24.0 program. Results: The mean age was $62.46{\pm}0.21years$. The mean systolic blood pressure (SBP) was $128.07{\pm}0.28mmHg$, and the diastolic blood pressure (DBP) was $76.99{\pm}0.21mmHg$. 71.9% of participants showed normal blood pressure (SBP < 140mmHg and DBP < 90mmHg). From the decisiontrees analysis, the characteristics of participants related to good BPC group were presented with 9 different pathways same as those from the poor BPC group. Good or poor BPC groups were classified according to the patients' characteristics such as age, living status, occupation, education, hypertension diagnosis period, numbers of comorbidity, perceived health status, total cholesterol, high density lipoprotein-cholesterol, alcohol drinking per month, and depressive mood. Total cholesterol level (< 201mg/dL or ${\geq}201mg/dL$ cutoff point) was the most significant predictor of the participants' BPC group. Conclusion: This decision-tree model with the 18 different pathways can form a basis for the screening of hypertension patients with good or poor BPC in either clinical or community settings.
