  • 당뇨가 오래되면 삶의 질을 떨어뜨리는 여러 가지 합병증들이 오게 되는데 그 중에서 시력을 위협하는 제일 심각한 합병증은 당뇨망막병증으로 이는 망막에 영양을 공급하는 혈관이상에 의해 발생하는 진행성 질환이다. 당뇨망막병증의 발생빈도는 당뇨의 유병기간과 밀접한 관계가 있다. 당뇨로 진단받고 5$\sim$10년이 지나면 25$\sim$50%가 망막병증이 생기고 15년이 지나면 거의 90%환자가 망막병증이 생긴다. 이밖에도 고혈압, 임신, 신장질환, 고지혈증과 같은 위험인자들이 있으며 어린이나 사춘기에 당뇨병이 있는 경우에는 조기에 망막병증의 빈도가 매우 높게 나타난다.

  • 고혈압과 당뇨와 같은 만성질환은 장기적인 관리가 필요하다. 최근 주목받고 있는 폐쇄형 SNS 시스템 설계를 통해서 동일한 질병을 가지고 있는 그룹간의 커뮤니케이션을 활성화하여 자유로운 게시글을 통해서 환자 맞춤형 건강정보를 발굴하고 공유하는 시스템을 안드로이드 어플리케이션을 통해서 구현하였다. 본 서비스를 통해서 건강정보 공유 서비스 연구가 활성화될 것으로 기대한다.

  • Objective: A case report on the improvement of hypertension patients by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 59-year-old Korean male with hypertension having bad habits as alcohol, smoking and overweight. Results: Weight loss and blood pressure improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of patients with chronic symptoms to hypertension.

The Necessity for Coronary Angiography in Atherosclerotic Arterial Obstruction in the Lower Extremities and the Clinical Features of Accompanied Coronary Arterial Diseases (죽상동맥경화성 하지동맥폐쇄증에서 관상동맥조영술의 필요성 및 동반되는 관상동맥 질환의 양상)

  • Background: Peripheral arterial disease is frequently accompanied with systemic arteriosclerosis and more than half of the cause of deaths is due to the development of coronary arterial disease. Moreover, it is known that the most frequent cause of death after a bypass surgery of chronic arterial obstruction is heart related complications. Especially in patients with atherosclerotic arterial obstruction in the lower extremities who had no history of heart disease or had no presenting symptoms of ischemic heart disease showed a high rate of postoperative mortality and for this reason we suggest preoperative evaluation in these patients to evaluate whether or not coronary arterial disease is accompanied. Material and Method: Since Feb. 2001 to Oct. 2004, we analyzed 52 patients who were operated on for atherosclerotic arterial obstruction in the lower extremities, with the exception of patients with a past history of heart disease or symptoms of ischemic heart disease. They underwent on the same day a coronary and femoral angiography for evaluation of accompanying coronary arterial disease. Of among these patients, we compared those who received bypass surgery of the arteries of the peripheral extremities alone to those who underwent combined coronary artery bypass surgery. Result: 63% of the reported cases of atherosclerotic arterial obstruction in the lower extremities were accompanied with coronary arterial disease. Old age, hypertension, diabetes mellitus, smoking, and hypercholesterolemia are known risk factors for arteriosclerosis and of these, only old age and hypertension had statistically significance in patients with severe atherosclerotic arterial obstruction in the lower extremities accompanied with coronary arterial disease. Diabetes, smoking, and hypercholesterolemia showed no statistical significance in this group. With the increase in severity of the range and the degree of atherosclerotic arterial obstruction, coronary arterial disease is frequently accompanied and its severity also increased. Patients who received both peripheral artery and coronary artery bypass surgery showed no difference in the period of hospitalization and ICU stay period compared with patients who received bypass surgery of the arteries of the lower extremities alone. Conclusion: Patients with atherosclerotic arterial obstruction in the lower extremities without symptoms of ischem to evaluate coronary arterial disease for active treatment, especially in the patients with old age, hypertension and high AVD scores.

  • Moyamoya disease is a progressive cerebrovascular disorder with stenosis or occlusion of the bilateral internal carotid arteries with abnormal vascular networks at the base of the brain. Previous reports have shown that there are extracranial vascular involvements in Moyamoya disease, especially in the renal artery. We report a 7-year-old patient with Moyamoya disease associated with renovascular hyper tension, who presented in infancy with seizures and hemiparesis. Renal angiography showed multiple stenoses of the right renal artery. Although renal artery stenosis in Moyamoya disease has been effectively treated with balloon angioplasty, stent implantation, or surgery, bat-loon angioplasty could not be done in this patient due to multiple stenoses. His blood pressure was successfully controlled with medical treatment, and remained normotensive during the follow up period of 6 months.

Factors Influencing on Early Patency Rate of Autogenous Arteriovenous Fistula for Hemodialysis (혈액투석을 위한 자가 동정맥루 수술에 있어서 조기개존율에 영향을 미치는 요인)

  • It is very important for hemodialysis in patients with end stage renal disease to obtain vascular access that resists repeated punctures and maintains adequate blood flow. This study was designed to indentify factors that may influence early patency rate of autogenous arteriovenous fistula. Material and Method: 49 cases in 47 patients who underwent radiocephalic fistula formation in our hospital from June 2002 through May 2003 were reviewed and analyzed. Result: The early patency rate was 79.6%. Age, sex, hypertension, and diabetes mellitus were not significant factors for patency. Body mass index and duration of hypertension and diabetes did not influence the early results either. Cephalic vein diameter measured preoperatively and blood flow at radio-cephalic fistula were significantly positive correlative factors. Groups with the vein diameter less than 2.7mm, or with the blood flow less than 100 mL/min had significantly lower early patency rate than the other groups. Conclusion: To improve early patency rate of radiocephalic fistula, large sized cephalic vein should be selected and if the intraoperative flow at radiocephalic fistula is less than 100 mL/min, another arteriovenous fistula formation should be considered.

Effects of 6-week Foot Reflexology on the Blood Pressure and Fatigue in Elderly Patients with Hypertension (6주간의 발반사마사지가 노인 고혈압환자의 혈압과 피로에 미치는 효과)

  • Purpose: This study was done to evaluate the effects of foot reflexology on blood pressure, serum lipids, fatigue and self-efficacy in patients with hypertension. Methods: The research was done between June 23 and July 31, 2003 and the design was an experimental treatment design. The thirty-four participants were assigned to either an experimental group (18) or a control group (16), Foot reflexology was administered twice a week for 6 weeks to participants in the experimental group. Results: There was a significant decrease in systolic blood pressure and diastolic pressure in the experimental group compared to the control group. After the foot reflexology, total cholesterol and triglyceride levels for the experimental group did not decrease significantly compared to the control group. High density lipoprotein find low density lipoprotein levels also did not decrease significantly after foot reflexology Fatigue in the experimental group decreased significantly after foot reflexology. Self-efficacy in the experimental group did not decrease significantly after foot reflexology. Conclusion: The results show that foot reflexology is an effective nursing: intervention to decrease systolic blood pressure, diastolic blood pressure and to treat fatigue but not serum lipids. Therefore, blood cholesterol should be further evaluated with a larger group of participants and for a longer period. Further research is necessary to evaluate and to compare effects of self-foot reflexology and foot reflexology.
