Probability Model-Based Data Mining Approach for Real-Time Processing of Large Data: High-Risk Group Detection and Rule Management System for Patients with High Blood Pressure (대용량 데이터의 실시간 처리를 위한 확률모형 기반 마이닝 기법: 고혈압환자 관리를 위한 고위험군 탐지 및 룰 관리 시스템)

  • 본 연구에서는 보건기관이 효율적으로 고혈압 관리 대상자를 탐색하고, 고혈압 관련 요인에 대한 지식을 효과적으로 관리할 수 있도록 하는 고혈압 고위험군 추정 모형 및 우선 사업 대상자 탐색 모형을 제안한다. 특히, 대용량 데이터 처리 및 실시간 시스템 운영, 외부 환경 변화를 고려한 자동 학습과 같은 현실적인 제약 조건을 해결하는 모형을 개발하는 것을 주 목표로 한다. 지역 보건소에서 수집된 의료 데이터를 이용하여 최적의 파라미터 값을 설정한 고혈압 고위험군 탐색 모형을 도출하였으며, 모형의 검증을 위하여 고혈압 환자정보로 구성된 평가용 데이터를 사용하여 고혈압 자연 발병률 대비 약 2배 수준으로 향상된 고혈압 환자 예측 정확도가 얻어지는 것을 확인하였다. 시스템 운영과 유비보수 측면에서 현실적으로 중요한 문제인 대용량 데이터 처리 및 외부 환경 변화에 강인한 자동학습 이슈를 해결하기 위한 방안에 대해서도 설명하였다.

A Development of a Tailored Follow up Management Model Using the Data Mining Technique on Hypertension (데이터마이닝 기법을 활용한 맞춤형 고혈압 사후관리 모형 개발)

  • This study used the characteristics of the knowledge discovery and data mining algorithms to develop tailored hypertension follow up management model - hypertension care predictive model and hypertension care compliance segmentation model - for hypertension management using the Korea National Health Insurance Corporation database(the insureds’ screening and health care benefit data). This study validated the predictive power of data mining algorithms by comparing the performance of logistic regression, decision tree, and ensemble technique. On the basis of internal and external validation, it was found that the model performance of logistic regression method was the best among the above three techniques on hypertension care predictive model and hypertension care compliance segmentation model was developed by Decision tree analysis. This study produced several factors affecting the outbreak of hypertension using screening. It is considered to be a contributing factor towards the nation’s building of a Hypertension follow up Management System in the near future by bringing forth representative results on the rise and care of hypertension.

  • 고혈압은 크게 본태성 고혈압과 이차성 고혈압으로 나뉜다. 우리가 일반적으로 알고 있는 고혈압은 본태성 고혈압으로 수많은 원인이 복합적으로 작용해 발생하는 고혈압을 말하며, 신장혈관질환이나 호르몬 질환 등에 의해 발생하는 고혈압을 이차성 고혈압이라고 하는데, 혈압의 정도에 따라 음식조절이 중요하다.

  • 최근 의료 패러다임은 치료에서 예방과 관리의 개념으로 변화하고 있다. 특히 우리나라 성인 인구 중 20~30%에서 발견되는 만성질환은 예방과 관리가 더욱 필요한 성인병이다. 본 논문에서는 만성 질환 중 지속적인 치료와 예방 그리고 관리가 필요한 고혈압 환자들을 관리하기 위하여 생체 데이터, 검진 데이터, 문진 데이터, 생활 습관 등을 이용하여 순차패턴 방법을 적용한 고혈압 환자에 대한 약제 처방 시스템과 연관규칙을 적용하여 고혈압 환자들에게 24시간 관리가 가능하도록 하는 고혈압 환자 관리 시스템을 개발한다.

Evaluation of Hypertension Prevention and Management Programs Conducted by Community Health Nurse Practitioners in Rural Areas of Korea (농촌지역 보건진료소의 고혈압 예방 및 관리사업 평가)

  • Objectives: This study aims to evaluate effectiveness of hypertension prevention and management programs (HPMPs) in rural Korea. Methods: A questionnaire survey was conducted with 700 community health practitioners working at primary health care posts (PHCPs). The questionnaire had four domains, which were hypertension management, hypertension prevention, the PHCP environment, and evaluation system, each with different maximum weighted values (WVs). Weighted values of 100 indicate the best effectiveness for the HPMP in all four domains. Results: The average WVs and percent effectiveness of all four domains combined was 61.65 (62%); the hypertension management domain was 28.81 (72%); the hypertension prevention domain was 23.44 (67%); the PHCP environment was4.29(43%); and the evaluation system was5.10(34%). Conclusion: The HPMPs were generally effective, and hypertension management was the most effective. The environment of PHCPs and the evaluation system of HPMPs should be improved to increase the effectiveness of the HPMPs.

Effects of Self-management Education Program on Hypertension Specific Self-efficacy in the Aged (고혈압 자기관리 교육프로그램이 노인의 고혈압 특이형 자기효능감에 미치는 효과)

  • This study was to examine the effect of the hypertension self-management education program on hypertension specific self-efficacy in the aged. This study was conducted with one group pre-test and post-test design. A total 31 the aged participated in 4 weeks of intervention. Data were analyzed as SPSS ver. 21.0 using descriptive statistics and paired t-test. We have identified that hypertension specific self-efficacy(t= -3.46, p< 0.000***) and disease awareness(t= -5.69, p<0.000***) significantly increased. We found that 4-weeks is appropriate for hypertension self-management education program. We suggested that for over 80's, customized program should be provided.

An Efficient Management by Analyzing Control Level of Hypertension (고혈압 관리수준에 따른 효율적인 관리 방안)

  • The purpose of this study is to contribute basic data toward the efficient management of Hypertension by comprehensive analysis of several factors which make influences on indicators of Hypertension management. To achieve this, using the data of 2007, 2008 Korea National Health and Nutrition Examination Survey, 2,580 adults aged over 19 who suffered from hypertension have been analyzed comprehensively. The major results can be summarized as follows: First, in regard to the influential factors for awareness and treatment of hypertension, the older the patients were, the poorer their self-rated health status were. And when they had experienced a health examination, the more likely the hypertensive patients were aware of their hypertension and took the treatment for it. Second, as for the influential factors for control of hypertension, those who were older and those who had a health examination were significantly more likely to control their hypertension irrespective of gender. However, regular drinkers fell significantly behind in that aspect. The findings of this study show that systematic hypertension management, focusing on health behavior factors that affect the level of hypertension management, in accordance with the patients' sex and age will provide a more efficient way.

A Study on Hypertension Management of Community Health Practitioner Posts (보건진료소 고혈압 관리사업의 실태)

  • Objectives: The purpose of this study is to provide data for the improvement of hypertension management of community health practitioner posts through the study on hypertension management in community health practitioner posts. Methods: A questionnaire was mailed to 700 community health practitioners and 205 of them responded during the period from March 13, 2003 to May 13. The survey results were analyzed using SPSS program, version 11. Results: The results are as follows; 1. There are two major activities in a hypertension prevention project for community: health education and early detection. About 57% of community health care practitioners perform a health education for community people four times a year. The 64.5% of them used the materials for health education provided from a community health center and 22.1% of them performed a post-evaluation. The main method of early detection of hypertension was measurement of blood pressure of person to visit, which was 96.1%. Other methods included home visiting(89.3%), a referral from community hospitals and other resources(49.1%), health promotion events(39.5%), and a review of medical records(35.7%). 2. For the registration and management of patients with hypertension, about 36% of community health centers used a special form and more than 50% of them have registered patients who were managed by other health care institutions in the community. A computerized program was used for the management of patients with hypertension in 68.5% of them. More than 60% of them responded that it was used for report, treatment, and follow-up of patients with hypertension.
