Customized Model Manufacturing for Patients with Pelvic Fracture using FDM 3D Printer (FDM 방식의 3D 프린터를 이용한 골반 골절 환자의 맞춤형 모델제작)

  • At present trend 3D Printing technology has been using more efficiently than conventional subtractive manufacturing method in various medical fields, in particular this technology superior in saving production time, cost and process than conventional. Especially in orthopedics, an attractive attention has been paid by adopting this technology because of improving operation, operation accuracy, and reducing the patient's pain. Though 3D printing technology has enormous applications still in some hospitals have not been using due to having the problem of technical utilization of hardware, software & chiefly financial availability and etc. In order to solve these problems by reducing the cost and time, we have used CT images in pre-operative planning by directly making the pelvic fracture model with open source DICOM viewer and STL file conversion program, assembly 3D printer of FDM wire additive manufacturing. After having the customized bone model of six patients who underwent unstable pelvic fracture surgery, we have operated our system in orthopedic section of University Hospital through the clinician. Later, we have received better reviews and comments on utilization availability, results, and precision and now our system considered to be useful in surgical planning.

Review of Rehabilitation Programs for Cancer survivors (암생존자의 재활을 위한 국내 프로그램 고찰)

  • With advances in health care, the cancer survival rate has significantly increased. Even though cancer treatment has finished, cancer survivors may continuously suffer from physical and psychosocial problems. In order to investigate the problems cancer survivors have experienced, a comprehensive rehabilitation program must be developed. This study sought to determine the content and structure of rehabilitation programs for cancer survivors, analyze the effectiveness of such programs, and develop a basis for future comprehensive rehabilitation program development. Through this review, thirteen programs were selected and classified according to exercise, psychosocial, and integrated or comprehensive aspects. Exercise programs improve physical function, fatigue, and hormone-related cancer effects. Psychosocial programs improve quality of life, mood changes such as depression, anxiety, and post traumatic factors. Comprehensive programs improve both physical and psychosocial aspects.

  • Dardenne brothers' films deals with minorities' issues who are shoved out to periphery of society, and especially takes interest in value-deprived human life in labor market of today. This study examines such social awareness of Dardenne brothers targeting their latest works awarded in Cannes Film Festival, , , , , . And analysis is attempted about Dardenne brothers' unique formal aesthetical style that exactly captures such social awareness. In Dardenne brothers' films a person shows up who faces abject matter of living. Their agony is closely connected to unemployment, and shows images of irresistible anxiety and alienation. Dardenne brothers' images are not only simple platform with contents embodied, but designed form showing essential thematic consciousness. Location and gaze, motion of camera capturing a character makes us to reflect the situation a character is facing in the world. There are almost no regard about plots, no dramatic devices, superficial and even straightforward, but Dardenne brothers' films have such independent formal aesthetic, which makes able to give endless tension.

  • Childhood depression was not officially accepted as a separate diagnosis until 20-25years ago. Although DSM-III R criteria gave us a starting point and are gaining acceptances as a useful tool for diagnosing childhood depression recently, many professionals feel that additional modifications are needed to better reflect the developmental aspects of children. And because children are relatively poor in expressing their moods or feelings due to weak reality sence and/or cognitive and linguistic limitations, the author too carefully suggest that developmental perspectives of children should be considered in diagnosing childhood depression. Little is known about the extent to which childhood depression precedes adult depression and persists into adult life and about the impact of it on the personality formation. Some evidences that childhood depressive symtoms can be quite enduring make it important to make an early diagnosis and treatment.

A concept analysis of FoMO (Fear of Missing Out) in cancer patients (암 환자의 FoMO(Fear of Missing Out) 개념분석)

  • This study aimed to identify and analyze the attributes of FoMO, and to provide basic data for the intervention of FoMO in cancer patients. In order to confirm conceptual attributes, antecedents, and consequences by applying the conceptual analysis method of Walker & Avant (2010), 82 previous studies published from 2004 to 2021 were analyzed. The conceptual attributes were relative deprivation, social exclusion, lack of sense of belonging, lack of interaction with others, negative emotions, and the antecedents were a sense of alienation when you miss the flow or when excluded from activities and the desire to connect with others. The consequences of this were increased emotional distress, triggering negative emotions such as depression and anxiety, and decreased life satisfaction and self-esteem. The attributes of these FoMO were also confirmed in cancer patients. Based on this, it is necessary to develop an effective intervention program to improve FoMO in cancer patients.

The Effect of Psychological Factors on Postoperative Pain in Gastric Tumor Patients after Endoscopic Submucosal Dissection (내시경 점막하 박리법을 시술 받은 위종양 환자에서 심리적 요인이 시술 후 통증에 미치는 영향)

  • Objectives : Since pain in cancer patients is an important factor that greatly affects the quality of life and prognosis, many attempts have been actively made to reduce the pain. Despite continuous effort on reducing pain after Endoscopic submucosal dissection (ESD), research has not been done on psychological factors as much as on biological factors affecting pain. The objective of this study is to investigate the psychological factors affecting postoperative pain in gastric tumor patients who underwent gastric ESD. Methods : 91 gastric tumor patients who visited National Health Service Ilsan Hospital in Korean between May 2015 and June 2016, and received ESD were evaluated. Baseline characteristics including sociodemographic factors, anxiety, depression, and resilience were evaluated before the procedure. Multivariate logistic regression was done to analyze factors affecting postoperative pain. Results : The group with high postoperative pain showed lower alcohol consumption and higher depressive symptom scores than the group with low postoperative pain. Also, the group with high postoperative pain showed lower total resilience score with lower subtotal scores in self-control and positive item. Multivariate logistic regression analysis of the postoperative pain showed that patients with lower score in self-control of resilience [odd ratio (OR), 0.911 ; 95% CI, 0.854-0.971, p=0.004) reported more pain after ESD. Conclusions : This study showed that patients with lower self-control ability of resilience felt more pain after ESD. Among the psychological factors evaluated in this study, resilience of an individual seems to have effect on pain.

Cognitive and Psychological Characteristics of Psychiatric Patients with Military Service Eligibility Issues (군 복무 적합성 판정을 위해 내원한 환자들의 인지적 및 심리적 특성)

  • Objectives : The purpose of this study was to examine cognitive and psychological characteristics of patients who visited psychiatric outpatient clinic with military service suitability issues. Methods : Since January, 2017 to September, 2018, patients were recruited from the department of psychiatry of university hospital. Participants were diagnosed using DSM-5 criteria by board certified psychiatrists. And each of participants tested with K-WAIS-IV, K-SCL-95 by clinical psychologist. The data were analyzed using SPSS 18.0. Results : The results indicated 24.6% of the patients have significantly low level of intellectual functioning. and 10.8% of the patients have borderline level of intellectual functioning, and 26.3% of them were below average. The results of psychological characteristics showed patients have cautious and potentially dangerous level of depression, anxiety, agoraphobia, and suicidal idea. And PTSD subscale's mean score was above problematic level. Next, we divided patients into two groups based on question of whether the patient is eligible to serve in the military service or not arises from recruitment process or army training center. But two group didn't differ significantly. And for further analyses, we also divided patients into two groups based on if they got single psychiatric diagnosis or more than one psychiatric diagnosis. Conclusions : The results indicated that group which has coexisting psychiatric disorders were more depressed, had higher level of anger and aggression, and had more serious levels of suicidal ideas than single psychiatric diagnosis group. The implications and limitations were discussed along with some suggestions for the future studies.

USE OF ENFLURANE FOR CHILDREN WHO FAIL TO RESPOND PROPERLY TO ORAL CHLORAL HYDRATE (Chloral Hydrate 경구투여 후 진정 효과가 나타나지 않은 소아환자에 대한 Enflurane의 사용)

  • Chloral hydrate is one of the most widely used sedative agents to control the difficult to treat young age group in the dental clinic. We are often frustrated to see the patient still awake and cry with agitation even after far more than the normal onset time. In such a case, the patient has to be rescheduled for another sedation visit with different agents and/or routes which greatly disappoints the guardians. This case report presents a sedative regimen that can possibly help the clinician complete scheduled treatment without postponement. We have tried sleep induction with Enflurane (1-1.5vol%) for 60 seconds to 37 patients of those who failed to respond properly to the dose(70mg/kg)of oral Chloral hydrate. The average age and weight of the patients was 34.3 months(22-43mo.) and 14.9kg(11-21kg) respectively. It is suggested that sleep induction with low dose Eflurane produced a stable state during dental treatment with respect to vital sign and behavior. Evidence of adverse effect was not detected or reported during and/or after the procedures.

The Understanding of Depression Subtypes (우울증 아형들의 이해)

  • The debate about whether depressive disorders should be divided into categories or arrayed along a continuum has gone for decade, without resolution. In our review, there is more evidence consistent with the spectrum concept than there is with the idea that depressive disorders constitute discrete clusters marked by relatively discontinuous boundaries. First, "depression spectrum", "is there a common genetic factors in bipolar and unipolar affective disorder", "threshold model of depression" and "bipolar spectrum disorder" are reviewed. And, a new subtype of depression is so called SeCA depression that is a stressor-precipitated, cortisol-induced, serotonin-related, anxiety/aggression-driven depression. SeCA depression is discussed. But, there is with the idea that depressive disorders constitute discrete subtypes marked by relatively discontinuous boundaries. This subtypes of depressive disorder were reviewed from a variety of theoretical frames of reference. The following issues are discussed ; Dexamethasone suppression test(DST), TRH stimulation test, MHPG, Temperament Character Inventory(TCI), and heart rate variability(HRV).

A Comparison of Psychological Characteristics between Sexually Assaulted and Molested Victims Using the Minnesota Multiphasic Personality Inventory-2 (다면적 인성검사 II를 이용한 성폭행과 성추행 피해자의 심리적 특성 비교)

  • Objectives : This study analyzed psychological aftereffects and made comparisons between victims who were either sexually assaulted or molested using the Minnesota Multiphasic Personality Inventory-2(MMPI-2). Methods : The participants were adult women who visited the Busan Smile Center within 6 months following sexual assault. This study categorized the participants according to types of sexual violence and then identified their social demographic features and characteristics. Student's t-tests were carried out to compare the MMPI-2 results between the sexually assaulted group and sexually molested group. Chi-square tests were also conducted to compare between participants who scored above 65T. Results : Among the 55 participants, 32 were sexually assaulted; 23 were sexually molested. Significant differences between the two groups were found in the following scales: F, F(B), F(P), Pt, Sc, Ma, RC6 and PSYC. Moreover, the number of participants with higher scores in the Pa, Pt, Sc, RC6, and PSYC scales in the sexually assaulted group was significantly higher than in the sexually molested group. Conclusions : Both groups of victims may commonly suffer beyond the moderate level of depression and anxiety in the post-case term. In addition, the sexually assaulted group seems to suffer relatively more from panic and psychological pain than the sexually molested group does.
