고체 팬톰을 이용한 방사선치료계획시스템의 정도관리

  • 목적 : 방사선치료기술이 날로 발전함에 따라 방사선치료계획시스템에 대한 주기적인 정도관리의 필요성은 증대하고 있으나, 국내 실정에 적합한 표준화된 정도관리절차서가 없는 실정이다. 따라서 본 연구에서는 방사선치료계획용 시스템에 대한 정도관리용 고체팬톰을 제작하여 주기적인 정도관리 활용 및 절차서를 제시하고자 한다. 대상 및 방법 : 체윤곽 보정을 위한 삼각기둥 모형 (30cm$\times$30cm$\times$5cm, 30cm$\times$15cm$\times$5$\times$) 및 정형ㆍ부정형, 불균질 측정이 가능한 물등가고체팬톰을 제작하였고, 컴퓨터단층촬영(AcQsim)을 통해 영상을 얻었으며, RTPS(AcQplan)에 입력하여 영상 내 기준점에서의 선량값을 계산하였다. RTPS를 통해 계산된 값의 평가를 위해 동일한 조건하에서 각 기준점에 대한 실제 측정을 이온함을 이용하여 측정하였다. 평가 항목으로는 정방형 조사면, 부정형 조사면, 쐐기 조사면, 불균질 물질 보정, 사방향 조사 등에 대해서 알고리즘별로 수행하였다. 결과 : RTPS를 이용하여 계산된 값과 실제 측정한 값을 비교하여 RTPS의 정확성을 평가한 결과로 합성의 불확도 허용 기준 (3%), 선속 중심축 상에서의 허용 기준 (2%) 등, 선진 각국 및 각 학회에서 권고하고 있는 허용 범위 내에서 잘 일치하였다. 결론 : RTPS는 측정된 심부선량과 선량분포 등 물리적인 인자에 의존하는 제한성이 있고, 실제로 선량계산 알고리즘과 기하학적 변화에 따라 계산값과 측정값 간에 차이가 발생할 수 있었다. 실제 인체의 체윤곽 불균일성과 불균질성을 모사한 팬톰을 제작하여 이용함으로써 다양한 RTPS간의 비교를 통한 치료 선량의 정확성을 평가하고, 방사선 치료의 원활하고 정확한 수행을 위해 실용적이고, 보편적인 치료계획 시스템의 정도관리 방법과 절차서를 수립하는데에 유용할 것으로 사료된다.

  • In the International Code of Practice for dosimetry TRS-398 published by International Atomic Energy Agency(IAEA), water equivalency plastic phantom may be used under certain circumstances for electron beam dosimetry for beam quality E0${\leq}$ 10 MeV. In this study, Palstic Water$^{TM}$ and Virtual Water$^{TM}$ were evaluated in order to determine fluence scaling factor hpl. Plastic phantom was evaluated for five electron energy from 6 MeV to 20 MeV. From the measured data of Palstic Water$^{TM}$, the fluence scaling factor hpl was found to be average 0.9964 and Virtual Water$^{TM}$ fluence scaling factor was 1.0156.

  • The purpose of this paper is to develop a simple system to measure dose distribution in small fields of NEC LINAC 6 MVX using film and solid water instead of ion chamber and water phantom. Specific quantities measured include percent depth dose (PDD), off-axis ratio (OAR). We produced square fields of 1 to 3cm in perimeter in 1cm steps measured at SAD of 80cm. The PDD and OAR measured by film was compared with measurement made with ion chamber. We calculated the TMR from the basic PDD data using the conversion formula. The trends of our measured beam data and philips LINAC are similar each other. The measurement for the small field using film and solid water was simple. Hand-made film phantom was especially useful to measure OARs for the stereotactic radiosurgery.

Feasibility of MatriXX for Intensity Modulated Radiation Therapy Quality Assurance (세기변조방사선치료의 품질관리를 위한 이온전리함 매트릭스의 유용성 고찰)

  • Purpose: To evaluate the feasibility of a commercial ion chamber array for intensity modulated radiation therapy (IMRT) quality assurance (QA) was performed IMRT patient-specific QA Materials and Methods: A use of IMRT patient-specific QA was examined for nasopharyngeal patient by using 6MV photon beams. The MatriXX (Wellhofer Dosimetrie, Germany) was used for IMRT QA. The case of nasopharyngeal cancer was performed inverse treatment planning. A hybrid dose distribution made on the CT data of MatriXX and solid phantom all of the same gantry angle (0$^\circ$). The measurement was acquired with geometrical condition that equal to hybrid treatment planning. The $\gamma$-index (dose difference 3%, DTA 3 mm) histogram was used for quantitative analysis of dose discrepancies. An absolute dose was compared at the high dose low gradient region. Results: The dose distribution was shown a good agreement by gamma evaluation. A proportion of acceptance criteria was 95.8%, 97.52%, 96.28%, 98.20%, 97.78%, 96.64% and 92.70% for gantry angles were 0$^\circ$, 55$^\circ$, 110$^\circ$, 140$^\circ$, 220$^\circ$, 250$^\circ$ and 305$^\circ$, respectively. The absolute dose in high dose low gradient region was shown reasonable agreement with the RTP calculation within $\pm$3%. Conclusion: The MatriXX offers the dosimetric characteristics required for performing both relative and absolute measurements. If MatriXX use in the clinic, it could be simplified and reduced the IMRT patient-specific QA workload. Therefore, the MatriXX is evaluated as a reliable and convenient dosimeter for IMRT patient-specific QA.

  • Purpose : This study has already started commercial Gated RapidArc automation equipment which was not previously in the Gated radiation therapy can be performed simultaneously with the VMAT Gated RapidArc radiation therapy to the accuracy of the analysis to evaluate the usability, Amplitude mode applied to the patient. Materials and Methods : The analysis of the distribution of radiation dose equivalent quality solid water phantom and GafChromic film was used Film QA film analysis program using the Gamma factor (3%, 3 mm). Three-dimensional dose distribution in order to check the accuracy of Matrixx dosimetry equipment and Compass was used for dose analysis program. Periodic breathing synchronized with solid phantom signals Phantom 4D Phantom and Varian RPM was created by breathing synchronized system, free breathing and breath holding at each of the dose distribution was analyzed. Evaluation of the Efficiency of the Foxtail Millet Vacuum Cushion in Skin Cancer Radiation Treatment (자체 제작한 Foxtail Millet Vacuum Cushion의 광자선 피부암 치료 시 유용성 검증)

  • Purpose: The sufficiency of skin dose and the reemergence of patient set-up position to the success of skin cancer radiation treatment is a very important element. But the conventional methods to increase the skin dose were used to vacuum cushion, bolus and water tank have several weak points. For this reason, we producted Foxtail Millet Vacuum Cushion and evaluated the efficiency of the Foxtail Millet Vacuum Cushion in skin cancer Radiation treatment. Materials and Methods: We measured absolute dose for 3 materials (Foxtail Millet Vacuum Cushion, bolus and solid water phantom) and compared each dose distribution. We irradiated 6 MV 100 MU photon radiation to every material of 1 cm, 2 cm, 3 cm thickness at three times. We measured absolute dose and compared dose distribution. Finally we inspected the CT simulation and radiation therapy planing using the Foxtail Millet Vacuum Cushion. Results: Absolute dose of Foxtail Millet Vacuum Cushion was similar to absolute dose of bolus and solid water phantom's result in each thickness. it Showed only the difference of 0.1~0.2% between each material. Also the same result in dose distribution comparison. About 97% of the dose distribution was within the margin of error in the prescribed ranges ($100{\pm}3%$), and achieved the enough skin dose (Gross Tumor Volume dose : $100{\pm}5%$) in radiation therapy planing. Conclusion: We evaluated important fact that Foxtail Millet Vacuum Cushion is no shortage of time to replace the soft tissue equivalent material and normal vacuum cushion at the low energy radiation transmittance. Foxtail Millet Vacuum Cushion can simultaneously achieve the enough skin dose in radiation therapy planing with maintaining normal vacuum cushion' function. Therefore as above We think that Foxtail Millet Vacuum Cushion is very useful in skin cancer radiation treatment.

  • It is ideal thing to compensate tissue deficit without skin contamination in curvatured irradiation field of high energy photon beam. The 3-dimensional compensating technique utilizing tissue equivalent materials to ensure an adequate dose distribution and skin sparing effect was described. This compensator was made of paraffin ($70\%$) and stearin wax ($30\%$) compound. The parameters for evaluation of the effect on skin dose in application of compensator were considered in the size of the field, the thickness of the compensator and the source-to-axis distance. The results are as follows; the skin doses were not changed even though application of the compensator, but depended on the field size and the source-to-axis distance, and the skin doses were only slightly changed within $1\%$ relative errors as increasing the thickness of the compensator in these experiments.

  • The periodic Quality Assurance (QA) of each radiation treatment related equipments is important one, but quality assurance of the radiation treatment planning system (RTPS) is still not sufficient rather than other related equipments in clinics. Therefore, this study will present and test the periodic QA program to compare, evaluation the efficiency of the treatment planning systems. This QA program is divided to terms for the input, output devices and dosimetric data and categorized to the weekly, monthly, yearly and non-periodically with respect to the job time, frequency of error, priority of importance. CT images of the water equivalent solid phantom with a heterogeneity condition are input into the RTPS to proceed the test. The actual measurement data are obtained by using the ion chamber for the 6 MV, 10 MV photon beam, then compared a calculation data with a measurement data to evaluate the accuracy of the RTPS. Most of results for the accuracy of geometry and beam data are agreed within the error criteria which is recommended from the various advanced country and related societies. This result can be applied to the periodic QA program to improve the treatment outcome as a proper model in Korea and used to evaluate the accuracy of the RTPS.

An Experimental Method for the Scatter Correction of MV Images Using Scatter to Primary Ratios (SPRs) (산란선 대 일차선비(SPR)를 이용한 MV 영상의 산란 보정을 위한 실험적 방법)

  • In general radiotherapy, mega-voltage (MV) x-ray images are widely used as the unique method to verify radio-therapeutic fields. But, the image quality of MV images is much lower than that of kilo-voltage x-ray images due to scatter interactions. Since 1990s, studies for the scatter correction have performed with digital-based MV imaging systems. In this study, a novel method for the scatter correction is suggested using scatter to primary ratio (SPR), instead of conventional methods such as digital image processing or scatter kernel calculations. We measured two MV images with and without a solid water phantom describing a patient body with given imaging conditions, and calculated un-attenuated ratios. Then, we obtained SPR distributions for the scatter correction. For experimental validation, a line-pair (LP) phantom using several Al bars and a clinical pelvis MV image was used. As the result, scatter signals of the LP phantom image were successfully reduced so that original density distribution of the phantom was restored. Moreover, image contrast values increased after SPR correction at all ROIs of the clinical image. The mean value of increases was 48%. The SPR correction method suggested in this study has high reliability because it is based on actually measured data. Also, this method can be easily adopted in clinics without additional cost. We expected that the SPR correction can be an effective method to improve the quality of MV image guided radiotherapy.
