A Retrospective Study of Survival Rate in single Brnemark TiUniteTM Implant (단일 치아 결손시 TiUniteTM 표면 처리한 임플란트의 생존율에 대한 후향적 연구)

  • Recently implant supported single crown is the popular treatment option to replace a single missing tooth. The purpose of this retrospective study was to analyze and evaluate the survival of implants with the $TiUnite^{TM}$ surface for single tooth replacement. From September 2002 to December 2006, 269 TiUniteTM surfaced implants were used in single tooth replacements at the Institute of Oral Health Science, Samsung Medical Center. Twenty one cases were excluded because of neighbor implants, missing records & short follow up period. Among 248 implants, the 129 implants (52.0%) were inserted in the maxilla and 119 (48.0%) in the mandible. One hundred implants placement (40.3%) were combined with guided bone regeneration, and 36 implants placement (14.5%) were combined with sinus bone augmentation. Mean observation period was $26.0{\pm}11.8$ months after implant placement. Twelve implants were recorded as failures, rendering a single implant survival rate of 95.2% over the observation period. Among failed 12 implants, 10 implants placed in the maxilla. The survival rate in the maxilla was 92.2% and in the mandible was 98.3%. The use of $TiUnite^{TM}$ surfaced single implant placement showed high survival rate for short time period.

Re-establishment of occlusal plane in a patient with a failed implant prosthesis (실패한 임플란트 보철수복물을 가진 환자의 교합평면 재설정)

  • A non-physiological occlusal plane caused by continuous tooth loss, occlusal wear, and failure of a prosthesis may result in an unattractive appearance and functional problems, such as reduced masticatory efficiency and occlusal interference. Therefore, when undertaking prosthetic treatment for edentulous patients or patients with a collapsed occlusal plane, it is important to establish an occlusal plane that is compatible with masticatory function. The patient in this case report had undergone restoration of a completely edentulous maxilla using an implant-supported fixed prosthesis. On follow-up examination in the following 6 years, mechanical complications were observed in the existing implant prosthesis, including porcelain chipping, occlusal wear, and screw loosening. Moreover, due to occlusal wear and supraeruption of the opposing anterior teeth, as well as loss of some posterior teeth, the occlusal plane had collapsed. Following diagnosis, the patient underwent full mouth rehabilitation, involving additional implant installation in edentulous sites, recreation of the existing prosthesis, and prosthetic restoration of all remaining teeth.

The Role of Angled Blade Plate in Treatment of Bone Tumor Occurred in Femur (대퇴골 골종양의 치료에 있어서의 Angled Blade Plate의 역할)

  • Purpose: Bone tumor occurred in femur frequently involve proximal intertrochanteric region or distal metaphyseal region. Sometimes, the pathologic fracture can happen according to the size of tumor due to the substantial biomechanical stresses. Therefore, the prognosis can be improved biomechanically by the angled blade plate considering the anatomic configuration after the excision of tumor. Materials and Methods: Between October 1991 and April 2005, there were a total of 16 patients(17 cases) who were treated by the excision of tumor and internal fixation with the angled blade plate for bone tumor occurred in femur. After the excision of tumor, we filled the cavity by bone graft in 11 cases and bone cement in 6 cases. The internal fixation was used by angled blade plate in all cases. Result: The average follow-up time was 55.5 months(6-144 months) in 16 patients(17 cases). No metal failure occurred after the operation. Reoperation was performed in 4 cases due to tumor recurrence, and the internal fixation was firm until that. Conclusion: The angled blade plate can prevent the fracture until grafted bone incorporated to host bone, and protect fragile connection between cement and host bone.

A Real-time Multicasting Protocol using Time Deadline in Wireless Sensor Networks (무선 센서 망에서 제한시간을 이용한 실시간 멀티캐스팅 프로토콜)

  • Real-time multicasting is a packet transmission scheme ensuring that multiple destinations receive a packet within the desired time line. In wireless sensor networks, a packet can be delivered to a limited distance under a given deadline, since the end-to-end delay tends to be proportional to the end-to-end physical distance. Existing real-time multicasting protocols select the distance between the source and the furthest destination as the distance limitation and construct a multicasting tree guaranteeing delivery paths to each destination within the distance limitation. However, the protocols might lead to real-time delivery failures and energy efficiency degradation due to the fixed distance limitation. In this study, we proposed a real-time multicasting protocol using time deadline. The proposed protocol obtains the maximum transmittable distance with a given time deadline and subsequently constructs a multicasting tree using the maximum transmittable distance. The form of the multicasting tree varies according to the given time deadline to trade off the energy efficiency against the real-time delivery success ratio. The simulation results showed that the proposed scheme is superior to the existing protocols in terms of energy efficiency and real-time delivery success ratio under various time deadlines.

  • This study analysed the narrative of and , two history documentary broadcasted on KBS, in terms of story-telling and discourse. And it also examined whether TV as mass media could provide an alternative interpretation against the dominant historical awareness. As a result, both programmes showed limitations on representing subversive point of view to the dominant ideology. At the story-telling level, firstly, they represented in a way of male-hero narrative though they were describing the history of woman, and while representing woman as a public figure they eliminated her feminity and individuality. Secondly, before evaluating woman as a historic figure they previously appreciated her appearance in a male-point of view. Thirdly, although they were telling the story of woman in a political view, they focused on love triangle, therefore failed to make her as a public figure. The discourses of both programmes were anchoring the existing historical interpretation instead of offering an alternative historical imagination. The narrator who were telling history at the studio in a omniscient viewpoint took a role as a meaning definer, placed at the highest rank in the hierarchy of discourse structure. Especially in , the dramatized images to cover lack of visual data helped anchor the patriarchal narrative and reduced the possibility of subversive interpretation on historic figure.

Three-dimensional finite element analysis according to the insertion depth of an immediately loaded implant in the anterior maxilla (상악 전치부 즉시하중 임플란트의 식립 깊이에 따른 삼차원 유한요소 분석)

  • Purpose: The purpose of this study was to investigate the effects of the insertion depth of an immediately loaded implant on the stress distribution of the surrounding bone and the micromovement of the implant using the three-dimensional finite element analysis. Materials and methods: A total of five bone models were constructed such that the implant platform was positioned at the levels of 0.00 mm, 0.25 mm, 0.50 mm, 0.75 mm, and 1.00 mm depth from the crest of the cortical bone. A frictional coefficient of 0.3 and the insertion torque of 35 Ncm were simulated on the interface between the implant and surrounding bone. A static load of 178 N was applied to the provisional prosthesis with a vertical load in the axial direction and an oblique load at $30^{\circ}$ with respect to the central axis of the implant, then a finite element analysis was performed. Results: The implant insertion depth significantly affected the stress distribution on the surrounding bone. The largest micromovement value of the implant was $39.34{\mu}m$. The oblique load contributed significantly to the stress distribution and micromovement in comparison to the vertical load. Conclusion: Increasing the implant insertion depth was advantageous in dispersing the concentrated stress in the cortical bone and did not significantly affect the micromovement associated with early osseointegration failure.

Full mouth rehabilitation with a few remaining teeth and implants for a patient with chronic periodontitis: a case report (만성 치주염 환자에서 소수 잔존치와 임플란트를 이용한 전악 수복: 증례 보고)

  • Chronic periodontitis involves subsequent loss of teeth, and if left untreated, can lead to adjacent teeth drifting and supraeruption of the rest dentition. Careful consideration has to be given when deciding extraction of remaining teeth in treatment of periodontally compromised dentitions. For tooth-supported fixed partial dentures or removable partial dentures, periodontally compromised teeth are extracted due to possible early failure from functional overload, but for implant restoration, the teeth could be used as supports for fixed partial dentures because implants can reduce overload on teeth. The remaining natural teeth can help clinicians restoring vertical dimension and normal occlusal plane in full mouth rehabilitation because it conserves patients' proprioceptive response. This clinical report describes treatment of a patient who has a few remaining teeth and supraeruption of the rest dentition from severe chronic periodontitis. Satisfactory clinical result was achieved with full mouth rehabilitation using a few teeth and implants.

A 3 year prospective study of survival rate of narrow diameter implants (좁은 직경 임플란트 3년간의 생존율에 관한 후향적 연구)

  • Purpose: The purpose of present study was to retrospectively analyze the survival rate of narrow diameter implant less than 3.6 mm by initial stability and radiographic measurements. Materials and Methods: In total, 24 patients who received 38 narrow diameter implants (${\leq}3.6mm$ in diameter, ${\geq}7mm$ in length) were enrolled in this retrospective study. The cumulative survival rate was calculated and various factors were investigated according to the implant platform diameter, body diameter, length, position, concomitant use of guided bone regeneration in implant placement and final prosthesis type. Initial stability was investigated with implant stability quotient (ISQ) value. The mesial and distal marginal bone level (MBL) change was calculated with radiography. Results: The overall survival rate was 92.11%. Mean ISQ value and MBL change of survival implants was 66.26 and $0.14{\pm}0.31mm$, respectively. None of the implants with platform diameters larger than the body diameter failed. Conclusion: In conclusion, the findings of present study suggest that narrow diameter implant could be predictable treatment in narrow alveolar ridge.

Effects of implant alignment and load direction on mandibular bone and implant: finite element analysis (임플란트 배열과 하중 방향이 임플란트와 치조골에 미치는 유한요소 응력분석)

  • Purpose: To evaluate the effects of load direction, number of implants, and alignment of implant position on stress distribution in implant, prosthesis, and bone tissue. Materials and Methods: Four 3D models were made to simulate posterior mandible bone block: two implants and 3-unit fixed dental prosthesis (FDP) with a pontic in the center (model M1), two implants and 3-unit FDP with a cantilever pontic at one end (model M2), FDP supported by three implants with straight line placement (model M3) and FDP supported by three implants with staggered implant configuration (model M4). The applied force was 120 N axially or 120 N obliquely. Results: Peak von Mises stresses caused by oblique occlusal force were 3.4 to 5.1 times higher in the implant and 3.5 to 8.3 times higher in the alveolar bone than those stresses caused by axial occlusal force. In model M2, the connector area of the distal cantilever in the prosthesis generated the highest von Mises stresses among all models. With the design of a large number of implants, low stresses were generated. When three implants were placed, there were no significant differences in the magnitude of stress between staggered arrangement and straight arrangement. Conclusion: The effect of staggering alignment on implant stress was negligible. However, the number of implants had a significant effect on stress magnitude.

Mini-T Plate Fixation for Neer Type II Distal Clavicle Fracture (Neer 분류 제 2형 원위부 쇄골 골절의 mini-T형 금속판 고정)

  • Purpose: To evaluate and report the clinical and radiological outcomes of open reduction and internal fixation with a mini-T plate for unstable distal clavicle fractures. Materials and Methods: From December 2004 to July 2007, fifteen patients who had a fracture of the distal clavicle (Neer type II fracture) were treated with an open reduction and internal fixation using a mini-T plate. They were followed up for a minimum of one year and the clinical and radiological results were analyzed. Results: The average time to fracture union was 3.1(3~4) months. There were no complications, such as deep infection or fixation loss. The mean ASES score was 97 points (85~100points) at the last follow up period, and 14 patients had a full range of motion of the shoulder. Conclusion: Open reduction and internal fixation with a mini-T plate for unstable distal clavicle fracture is a good surgical method with good clinical and radiological results.
