Predictors of Drug-resistance in Patients with Pulmonary Tuberculosis (폐결핵환자에서 약제내성의 예측인자)

  • Background: The drug-resistant tuberculosis has recently decreased in Korea, but it is still one of the major obstacles in the treatment of pulmonary tuberculosis. Unfortunately there are no reliable ways to figure out the drug sensitivity pattern of the M. tuberculosis in the starting point of treatment. At least several months which is critical for the success of treatment have to be passed away before getting the report of drug-sensitivity test. The aim of this study was to find out the clinical and radiological parameters that make it possible to predict the drug-resistant pulmonary tuberculosis and to make a correct decision on the antituberculosis drug regimens. Method: We studied 253 pulmonary TB patients with sputum and/or bronchial washing fluid culture-positive diagnosed at the Chung-Ang University Young-San Hospital in the period of 1989-1994. The differences in the clinical and raiological variables between the drug-sensitive and the drug-resistant tuberculosis patients were evaluated. Results: In 66 out of 253 patients(26.1%), drug resistant tuberculosis to at least one antituberculosis drug were found. Patients with retreatment showed higher resistance rate than those with initial treatment(30/69, 43.5% vs 36/184, 19.5%, p<0.01). Patients with cavitary TB showed higher resistance rate than those with non-cavitary TB(24/54, 44.4% vs 42/199, 21.1%, p<0.05). Among patients with initial treatment, those with far-advanced TB showed a higher drug resistance rate than those with minimal lesion(9/23, 36.9% vs 10/82, 12.5%, p<0.05). Patients with culture positive only in the bronchial washing fluid showed lower resistance rate than those with sputum culture positive(7/63, 11.1 % vs 59/190, 31.1%, p<0.05). Conclusion: Prior treatment history for pulmonary tuberculosis, the presence of cavity & far advanced tuberculosis in the radiologic exam, sputum rather than solely bronchial washing culture positivity would be the related factors to the drug resistance. So in the patients with such characteristics, it is needed to try to find out the drug sensitivity pattern of the infecting tuberculosis organism as soon as possible.

Development of statistical forecast model for PM10 concentration over Seoul (서울지역 PM10 농도 예측모형 개발)

  • The objective of the present study is to develop statistical quantitative forecast model for PM10 concentration over Seoul. We used three types of data (weather observation data in Korea, the China's weather observation data collected by GTS, and air quality numerical model forecasts). To apply the daily forecast system, hourly data are converted to daily data and then lagging was performed. The potential predictors were selected based on correlation analysis and multicollinearity check. Model validation has been performed for checking model stability. We applied two models (multiple regression model and threshold regression model) separately. The two models were compared based on the scatter plot of forecasts and observations, time series plots, RMSE, skill scores. As a result, a threshold regression model performs better than multiple regression model in high PM10 concentration cases.

DNA Ploidy as a Predictive Index of Therapeutic Response in Lung Cancer (폐암환자에서 치료에 대한 반응 예측지표로서의 DNA Ploidy)

  • Background: Although many authors have reported that the median survival time of surgically resected non-small cell lung cancer (NSCLC) was shorter in aneuploid than in diploid determined by flow cytometry, there are few reports about DNA ploidy using bronchial brushing material in all types of lung cancer. Method: The DNA ploidy test results of 109 consecutive patients with lung cancer were analyzed to find the relationship of DNA ploidy and anatomic or physiologic stage. And the differences of the response to various therapeutic modalities according to DNA ploidy were evaluated at least 8 weeks after the begining of the therapy. Results: Numbers of patients with DNA aneuploid pattern or high proliferative activity (S+G2M>22%) were not different among the various cell types of lung cancer. The relationship of DNA ploidy and anatomic or physiologic stage was not significant. However, NSCLC patients with high proliferative activity showed more advanced anatomic stage than those without that (p<0.05). The short-term response rate to therapy depended on the anatomic (p<0.005) or physiologic stages (p<0.05) in patients with NSCLC, and not on DNA ploidy or proliferative activity. Conclusion: DNA ploidy test using bronchial brushing material revealed that high proliferative activity means advanced anatomic stage, but it was not useful to predict the therapeutic response.

CARIES PREDICTION MODEL USING LASER FLUORESCENCE (레이저 형광법을 이용한 우식유발 예측모형)

  • The purpose of this study was to evaluate the specificity, sensitivity, and diagnostic power of caries activity test using laser fluorescence. The subjects of this study were 50 children of $7\sim9$ years old. Fluorescence from initial carious lesion of teeth illuminated by an argon laser(480nm) was observed and photographed with barrier filter. Visual examination for the dDfFtT rate and Streptococcus mutans colony counting was done to evaluate correlation with caries activity test using laser fluorescence. Data analysis was accomplished by Axelsson's method. The results from the present study can be summarized as follows: 1. There was positive correlation $(\gamma=0.48)$ between laser fluorescence test and Streptococcus mutans count. And also positive correlation $(\gamma=0.39)$ exists between laser fluorescence test and dDfFtT rate (P<0.01). 2. Positive correlation $(\gamma=0.27)$ between Streptococcus mutans colony count and dDfFtT rate was found(P<0.05). 3. When dDfFtT rate was defined to standard testing method, the specificity, senstivity, and diagnostic power of laser fluorescence test were 44.4%, 85.7%, and 87.8%. 4. When dDfFtT rate was defined to standard testing method, the specificity, senstivity, and diagnostic power of S. mutans colony counting were 77.8%, 92.9%, 84.8%. 5. When S. mutans colony counting was defined to standard testing method, sensitivity, specificity and diagnostic power of laser fluorescence test were 40.0%, 84.8%, 95.1%. In regard to above results, laser fluorescence test considered to be accurate and reliable method for determining caries activity because of it's close relationship with caries susceptibility test and caries experience measurements. And it was also considered to be practical because it would be simple, inexpensive, and time saving method.

Relationships between Physical Activity Participation, Depression and Body Function of the Elderly in Community (지역사회 노인의 신체활동 참여와 우울 및 신체 기능과의 관계)

  • The purpose of this study was to determine the relationships with physical activity and depression and body function in elderly. Methods: Total 79 elderly(over 65years old) were participated in this study. A Korean Form of Geriatric Depression Scale(KGDS), Mini-Mental State Examination(MMSE-K), Jebsen hand function test, dynamometer, five time sit-up test were used to measure the depression and cognition, body function(hand function, grasp power, lower extremity strength). Results: Physical activity participated group shows higher in the body function and lower in the depression than non participated group. Conclusion: The findings of this study indicate that the physical activity is helpful to indicate and manage the depression and body function in elderly.

  • Assessment of myocardial ischemia in patients with stable angina is important in deciding whether to treat coronary artery disease and in predicting clinical outcome. The fractional flow reserve is a standard reference for the diagnosis of myocardial ischemia, but this procedure has limitations because of its invasiveness. Coronary computed tomography angiography (CCTA) is now an established tool in the anatomic diagnosis of coronary artery disease; however, there are limits to the diagnosis of hemodynamically important stenosis that causes myocardial ischemia. In order to address this problem, studies using quantification of coronary atherosclerotic plaques, myocardial perfusion, and noninvasive calculation of fractional flow reserve based on CCTA have been actively conducted and recognized for their diagnostic value. In this review, several imaging techniques of CCTA used to assess myocardial ischemia are described.

Remission rate and remission predictors of Graves disease in children and adolescents (소아 및 청소년 그레이브스병 환자에서의 관해 예측 인자와 관해율)

  • Purpose:Medical therapy is the initial treatment for children with Graves disease to avoid complications of other treatments. However, optimal treatment for childhood Graves disease is controversial because most patients require relatively long periods of medical therapy and relapse is common after medication discontinuation. Therefore, this study aimed to search clinical or biochemical characteristics that could be used as remission predictors in Graves disease. Methods:We retrospectively studied children diagnosed with Graves disease, treated with anti-thyroid agents, and observed for at least 3 years. Patients were categorized into remission and non-remission groups, and the groups were compared to determine the variables that were predictive of achieving remission. Results:Sixty-four patients were enrolled, of which 37 (57.8%) achieved remission and 27 (42.2%) could not achieve remission until the last visit. Normalization of thyroid-stimulating hormone-binding inhibitory immunoglobulin (TBII) after treatment was faster in the remission group than in the non-remission group (remission group, $15.5{\pm}12.07$ vs. non-remission group, $41.69{\pm}35.70$ months). Thyrotropin-releasing hormone (TRH) stimulation tests were performed in 28 patients. Only 2 (8.3%) of 26 patients who showed normal or hyper-response in TRH stimulation test relapsed. Binary logistic regression analysis identified rapid achievement of TBII normalization after treatment as a significant predictor of remission. Six percent of patients achieved remission within 3 years and 55.8% achieved it within 6 years. Conclusion:Rapid achievement of TBII normalization can be a predictor of remission in childhood Graves disease. The TRH stimulation test can be a predictor of maintenance of remission.

Numerical Modeling of the Hall Sensor Signal Used in Pulsed Eddy Current Testing and Comparison of Its Characteristics with a Coil Sensor Signal (홀센서를 사용한 펄스와전류탐상 신호의 수치모델링 및 코일센서 신호와의 특성 비교)

  • Pulsed eddy current (PEC) testing signals have typically been obtained from the electromotive force induced in a sensor coil. However, an increasing number of studies have elected to incorporate the Hall plate as a sensor. Thus, accurate numerical modeling of the Hall sensor signal is necessary. In this study, a PEC probe is designed and a numerical modeling program is written so that Hall sensor signals and coil sensor signals can be calculated simultaneously. First, a step current is used as the input current. The predicted Hall sensor signals show similar characteristics to those of the experimental signals reported by other researchers. The characteristics of the two types of signals are then analyzed and compared as the thickness of test object changes. The results show that the Hall sensor signal provides less information for evaluating the thickness of the test object than the coil sensor signal. The response signals from a pulsed input current are also calculated, and it is confirmed that an equivalent reversed signal pattern appeared after the pulse width at both signals.

Social Competence, Language and Literacy Ability of Kindergartners: The Affects of Parent-Child Interaction, Peer Interaction and Teacher-Child Interaction (부모-유아 상호작용, 또래상호작용, 교사-유아 상호작용이 유아의 사회적 유능감과 언어 및 문해 능력에 미치는 영향)

  • This study investigated the affects of Parent-child interaction, peer interaction and teacher-child interaction on kindergartners' social competence, language and literacy Ability. Participants were 1203 children attending panel study on Korean children. It used the 'Social Competence Inventory', 'SECCYD', 'HEQ', 'PIPPS' and the 'Teacher-Child Relation Scale' in collection of data. Collected data were analyzed through the SPSS 18.0 program and pearson's correlation and step-wise multi regression analysis. Findings are as follows. First, there were positive correlations between children's social competence and parent-child interaction, peer interaction and teacher-child interaction. Second, there were positive correlations between children's language and literacy ability and peer interaction and teacher-child interaction. Third, Parent-child interaction, peer interaction were predictable variable affecting the young children's social competence. Fourth, peer interaction and teacher-child interaction were found to be predictable variables affecting the young children's language and literacy ability.

Cognitive Ability and Personality as Predictors of Academic Performance: Science Gifted Students (과학영재의 인지특성 및 성격변인이 학업성취도에 미치는 영향)

  • The purpose of the present study is to investigate the factors which affect academic achievements among gifted students' intellectual and personality characteristics. For this purposes, 125 science gifted students' learning ability test and NEO personality test data were collected and analyzed along with the school achievement scores. According to the results of this study, there are significant relationships between academic achievements and conscientiousness, executive function, and learning motivation. Based on the achievement level, there are positive correlation between academic achievement and conscientiousness in a high academic achievement group. For the middle academic achievement group, there are positive relationship with executive function and learning motivation attention but negative relationship with extraversion. For the low achievement group, concentration was the only factor highly correlated with academic achievement. For the high academic achievement group, conscientiousness is a significant predictors of academic achievement; for middle academic achievement group, executive function and learning motivation were the significant predictors; for low academic achievement group, concentration was the significant predictors of academic achievement. Results were discussed in terms of practical value of enhancing gifted students' academic achievement.
