Effectiveness of Radionuclide Cisternography to Detect the Leakage Site of CSF in Spontaneous Intracranial Hypotension; Preliminary Report (자발성 두개강내 저뇌압증 환자의 뇌척수액 누출부위 진단에 방사성동위원소 뇌조조영술의 유효성: 예비결과 보고)

  • Purpose: Although radionuclide cisternography (RNC) is an useful study to detect cerebrospinal fluid (CSF) leakage in the patient with spontaneous intracranial hypotension (SIH), it sometimes fails to demonstrate the site of CSF leakage. The aim of the study is to improve the detection of leakage site of CSF and to reduce time for the study in RNC using modified protocol (m-RNC). Materials & methods : The study consists of 8 studies of 7 patients ($38{\pm}8$ years, M:F=2:5) with SIH, who underwent m-RNC following administration of 185-222 MBq of $^{99m}Tc$-DTPA into the lumbar subarachnoid space. Sequential images were obtained the whole spine with the head including urinary bladder at 10 minute, 30 minute, 1 hour, 2 hour, 4 hour and 6 hour. Radioactivity of extradural space and urinary bladder was evaluated. Results: Leakage site of CSF was identified in all 8 cases by m-RNC. Leakage site was cervicothoracic junction (CTJ, n=3), CTJ with C1-2 (n=2), CTJ with thoracic spine, thoracolumbar spine and lumbar spine (each n=1). All cases presented leakage sites within 1 hour and multiple sites, where CTJ was included in 6 cases. Only one case presented additional site in 6 hour image. Early radioactivity within the urinary bladder was noted in 6 cases, but that was fellowing after identification of the leakage site. Conclusion: Radionuclide cisternography is sensitive to detect the leakage site of CSF and is expected to improve the detection of CSF leakage site and reduce time for the study using modified protocol.

  • Purpose: The authors investigated whether 35 flexible nail-rotating hinge composite reconstructions around knee joint minimize junctional osteolysis of host bone. Material and Method: The reconstructive technique was as follows: 1) en bloc tumor resection, 2) filling of the host bone marrow cavity with multiple Ender nails, 3) assembling the Ender nails and an Endo-Link type total knee component with wire and bone cement. Result: Mean follow-up was 53 months (ranged 30~79). At final follow-up, 29 patients retained a mobile joint. Resection of more than 40% of bone showed a positive relationship with junctional hypertrophy (p=0.028). Eight patients showed nail breakage and eight prostheses were removed due to early or late infection. The cumulative prosthetic survival rate was 33% at 6 year. Average functional score according to the MSTS criteria was 26.8. Conclusion: Mid-term evaluations showed that results were fair. The revision process was straightforward. Junctional hypertrophy observed appears to give some clues as to how to minimize osteolysis at the prosthesis-host junction after modular prosthesis fixation.

A Study on the Development of Non-PC High-Early-Strength Concrete Without Steam Curing (증기양생이 불필요한 PC부재용 조강형 콘크리트 개발에 관한 연구)

  • This study aims to develop a rapidly hardening type of concrete to achieve the removal of form intensity (more than 10MPa) using the method of curing at room temperature in order to solve some economic environmental problems by omitting the steam curing process involved in producing PC (Precast Concrete). Therefore, this study evaluated a rapidly hardening cement containing a high amunt of C3S, which is very responsive in expressing early intensity, and a rapidly hardening type of concrete which uses some hardening accelerator to increase thehydration reaction of $C_3S$. The results of the experiment on concrete using some hardening accelerator are asfollows. In the slump flow experiment for identifying the liquidity and the air test, the desired values were met. The compression strength showed rapid expression response by 12 hours, and met the desired value within 6~9 hours. Its drying shrinkage value and Autogenous shrinkage value were measured as below ($-754.5{\times}10^{-6}$),and satisfied the requirements. In addition, in the Semi-Adiabatic Temperature Test, it was found that the concrete rose to its peak temperature within 24 hours and then its temperature dropped.

Mechanical Properties of Lean-mixed Cement-treated Soil for Effective Reuse of Dredged Clay (준설점토의 친환경 재활용을 위한 시멘트계 처리토의 장단기 역학거동)

  • Cement treating technique, such as deep mixing method, has been used widely to stabilize the dredged clayey soil for many years. Despite of its effectiveness in treating soil by cement, several efforts have also been made to try to reduce the side effect of the cement that used to stabilize the dredged clay. However, authors considered that more detailed study on the physical and mechanical properties of lean-mixed soil-cement has been required to establish the design procedure to apply the practical problems. Therefore, in this study, the curing time and mixing ratio was used as key parameters to estimate the physical and mechanical properties including long-term behavior. The unconfined strength of lean-mixed soil-cement increase continuously during curing period, 270 days, while increasing rate becomes low in ordinary cement-treated dredged clay. We also concluded that cement-treated dredging clay shows apparent quasi overconsolidation behavior even in low cement proportion. By this study, fundamental approach was carried out for effective reuse of very soft dredged clayey soil both in mechanical and environmental aspect. It can be also expected that this study can propose a basic design data to use the lean-mixed soil cement.

  • Asphalt pavements have to perform under the conditions of heavily-loaded vehicles due to the industrialization and large temperature variance between the summer and the winter. Due to these factors, a characteristics change of early permanent deformation becomes a big issue, and to remedy this problem many research to use modified asphalt are being widely conducted. However, most of the modified asphalt is being paved after milling the surface course and applying tackcoating, and it is being used mostly for the repair and maintenance purpose rather than pavement of new national road. The purpose of this investigation is to obtain some fundamental data for the evaluation of the performance and long-term performance of the construction material mixtures by the laboratory test and field experiments. For the field experiment, 200m of two-lanes national road, that is being paved for the new national road under the direction of Pusan Regional Construction Management Office, was paved with SBS PMA and PSMA asphalt mixtures, which are an modified asphalt mixtures used for the surface course, on top of the base course paved with other modified asphalt mixtures. The remaining section of the new national road was paved with dense grade mixture. The laboratory tests assessed and analyzed the mixture characteristics by Marshall's stability test, strength tests and wheel-tracking test. On the basis of the evaluation result of the temperature control and roughness of the newly constructed road at the field experiment site, it is desired to evaluate and identify the most economic modified asphalt mixtures by long-term performance evaluation and LCC(Life Cycle Cost) analysis in order to apply the test result to the design of new road construction in the future.

Material Property Evaluation for UFFA Rapid Setting Concrete including Calcium Hydroxide (수산화칼슘을 첨가한 UFFA 초속경 콘크리트의 물성특성 평가)

  • Generally, UFF A(Ultra Fine Fly Ash) has merit that advances a greater concrete workability and activates a greater pozzolanic reaction than common fly ash due to its ultra fine particle size. These properties enhance concrete durability by reducing permeability and increasing resistance of alkali silica reaction(ASR) and sulfate attack, etc. Due to these reasons, UFFA can be used in a rapid setting concrete. The purpose of this study is to develop and evaluate the rapid setting concrete with UFF A as a repair material for early-opening-to-traffic. In previous studies, if only UFFA is added to the rapid setting concrete mixture, pozzolanic reaction doesn't happen actively. Therefore, in this study, the chemical and physical tests were performed for rapid setting concrete with UFFA including calcium hydroxide and the activity of pozzolanic reaction was evaluated. Finally, the effectiveness of this mixture on enhancing concrete durability was investigated. As results, adding UFF A decreased the water/cement ratio of concrete, and compensated the reduced portion of the early strength of concrete. Also, rapid setting concrete with UFFA including calcium hydroxide activated a greater pozzolanic reaction than normal-UFF A concrete. As calcium hydroxide increases, electrical indication of concrete's ability to resist chloride ion penetration is promoted significantly.

Quantitative Analysis of Voice Quality after Radiation Therapy for Stage T1a Glottic Carcinoma (T1a 병기 성문암의 방사선 치료 후 음성에 관한 연구)

  • Purpose : To evaluate the voices of irradiated patients with early glottic carcinoma and to compare these with the voices of healthy volunteers. Materials and Methods : The voice samples (sustained vowel) of seventeen male patients who had been irradiated for T1a glottic squamous carcinoma at least 1 year prior to the study were analyzed with objective voice analyzer (acoustic voice analysis, aerodynamic test, and videostroboscopic analysis) and compared with those of a normal group of twenty age- and sex-matched volunteers. Average fundamental frequency, jitter, shimmer, and noise-to-harmonic ratio were obtained for acoustic voice analysis. Maximal phonation time, mean flow rate, intensity, subglottic pressure, glottal resistance, glottal efficiency, and glottal power were obtained for aerodynamic test. Results : The irradiated group presented higher values of shimmer in acoustic voice analysis. There was no significant difference between two groups in other parameters. Conclusion : In this study all the objective voice parameters except shimmer were no4 significantly different between the irradiated group and the control group. These results suggest that the voice quality is minimally affected by radiation therapy for 71 a glottic carcinoma.

Expression of VEGF, p53, Apaf-1 and Caspase-9 in Head and Neck Squamous Cell Carcinoma (두경부 편평세포암에서 VEGF, p53, Apaf-1 and Caspase-9의 발현)

  • 배 경 편평상피암은 두경부의 악성종양 중 가장 흔하며, 임상적인 경과가 불량하다. 따라서 나쁜 예후를 가지는 환자군을 조기에 선별하여 더 적극적인 치료의 시행을 결정짓는 표지자의 필요성이 대두된다. 우리는 일련의 두경부 편평세포암 검체에서 몇몇 분자 표지자의 예후적 유용성을 평가하고자 하였다. 방 법 23예의 두경부 편평세포암 검체를 대상으로 VEGF, p53, Apaf-1, caspase-9의 발현과 몇몇 임상병리학적 지표들간의 연관성을 면역조직화학염색을 통해 조사하였다. 결 과 환자군은 남성이 더 많았으며 평균연령은 63.7세였다. 1기가 5예, 2기가 2예, 3기가 8예, 4기가 8예였다. 평균생존기간은 37.3개월이었다. VEGF단백 발현은 종양의 크기와 통계적으로 유의한 연관성을 보였다. 이와 더불어 VEGF단백 발현은 병기, 그리고 림프관 침습과 연관적인 경향성을 보였다. 그러나 VEGF단백 발현과 생존기간과는 관련성이 없었다. 또한, Apaf-1과 caspase-9의 단백발현은 다른 임상지표, 환자의 생존기간과는 관련이 없었다. 결 론 VEGF단백 발현은 두경부 편평세포암 환자에서 나쁜 임상경과를 예측할 수 있게 하는 표지자로서의 역할을 할 수 있다. 또한 본 연구는 두경부 편평세포암에서 별로 연구되지 않은 Apaf-1과 caspase-9의 발현상태를 밝힌 점에서 의의가 있다.

  • 오늘날 도시지역의 무분별한 개발로 인해 불투수면적이 증가하여 첨두유량 및 유출용적의 증가와 홍수도달시간을 단축시키고 있으며, 특히 도시유역에서는 하천 홍수위 상승에 의한 외수피해와 순간적인 집중호우에 의해 도로 노면수의 배수불량에서 기인하는 내수피해가 결합되어 홍수피해가 더욱 가중되고 있는 실정이다. 이에 대한 효율적인 조기 대응책으로 도시 수문기상 현상의 변화 및 현황을 파악하여 홍수로 인한 인명 및 재산피해를 최소화할 수 있는 적절한 홍수 예 경보 시스템의 구축을 들 수 있다. 이를 위하여 선진 외국의 경우 기상전문가에 의한 집중호우 현상규명 및 사전예보 기법 확립과 수자원 전문가에 의한 이들 예보자료를 활용한 특정지역의 홍수피해 유무를 사전에 예측하는 기상과 수자원의 학제간 연구가 활발히 진행되고 있다. 국내의 경우 기상 현업에서는 국지성 집중호우 예측과 단시간 강수예보를 위해 수치예보모형, 레이더 및 기상위성을 활용하고 있으나 수자원 분야에서는 예측강우를 활용한 홍수예보에 관한 연구는 매우 미진한 상태이다. 특히 도시홍수의 경우 도달시간이 매우 짧으므로 강수의 초단시간 예보기법을 통한 강수예보의 선행시간 확보는 매우 중요하다. 이를 위해 본 연구에서는 기상레이더 정보와 이류모델을 활용한 초단시간 강수예보의 적용성을 검토하였다. 이류모델은 강우강도 분포를 이류벡터에 따라 이동시키면서 강우의 발달쇠약 회전 등을 고려하여 강수를 예측하는 모형이다. 본 연구에서는 초단시간 강수예보의 적용성 검토를 위해 Least-square fitting 기법으로 레이더 강수를 추정하고, 추정된 강수를 이류모델의 입력장으로 활용하였다. 또한, 도시홍수예보의 활용을 위해 중랑천 유역을 대상으로 초단시간 예측강수의 유역면적평균강우량을 산정하여 적용성을 평가하였다.

The Treatment of Tibial Shaft Fractures by Interlocking Nailing (Interlocking Nail을 사용한 경골간부 골절의 치료)

  • The treatment of tibial shaft fracture has become one of the most controversial subjects in orthopedic surgery. Comminuted, segmental and rotationally unstable fractures or bone defect at fracture site have problems of the fixation. The interlocking nail solve these problems. We have experienced 8 cases of the tibial shaft fractures treated with interlocking nail from 1986 to 1988. Authors analysed these cases and our own clinical study. The results were as follows. 1. The average bone union rate was about 15 weeks. 2. The interval between operation and crutch walking was 4:3 weeks. 3. The merits of this operation were the short hospitalization and early adaptation of social activity. 4. The advantage is be able to do early ambulation without following muscular atrophy or joint stiffness. 5. The results were assessed on clinical examination and radiographic appearance by Hamza et al. An excellent results were 7 cases and good result was 1 case.
