갑상선기능저하증(甲狀腺機能低下症)의 임상적(臨床的) 연구(硏究) (A Clinical Study on Hypothyroidism)

  • A clinical study was made on 263 patients of hypothyroidism among the 5,970 patients of various thyroid diseases diagnosed and treated at the Radioisotope Clinic and Laboratory, Seoul National University Hospital from May, 1960 to Aug., 1975. The results obtained with this study are as follows: 1) The etiological classification of hypothyroidism revealed 244 cases (93%) of primary hypothyroidism and 19 cases (7%) of secondary hypothyroidism. 2) The most frequent cause of the thyroprivic primary hypothyroidism was post radioiodine therapy with 109 cases (41.4%). 3) There were 37 cases (14%) of male and 226 cases (86%) of female, showing a ratio of 1:6. 4) The majority of patients were between the ages of 30 and 60 with the peak incidence (87 cases, 33%) in their fourth decades of lives. 5) The major symtoms and signs were weakness (97%), edema of face and extremities (92%); Decreased Achilles tendon reflex (87%), cold intolerance (82%), gain in weight (76%), constipation (58%) and cold skin (51%). 6) The cumulative incidence of hypothyroidism in patient treated with $^{131}I(3{\sim}8mci)$ was 7.2% at first year, 33.3% at tenth year and the 50% at fourteenth year and the annual increment was 2.9%. 7) The incidence of hypothyroidism related to thenumbers of $^{131}I$ the rapy was not linear. 8) The diagnostic compatibilities of the various tests to hypothyroidism were TSH (100%), $T_4$ (93.8%), 24-hour-RNIU (91.5%), ATR (86.7%), $T_3RU$ (66.1%) and BMR (64.9%).

인삼양영탕(人蔘養榮湯)이 PTU로 유발된 Rat의 갑상선기능저하증에 미치는 영향 (Effects of Insamyangyoung-tang Aqueous Extracts on the Hypothyroidism Induced by Propylthiouracil in Rats)

  • Objectives : The object of this study was to evaluate the effect of Insamyangyoung-tang aqueous extracts (ISYYT) on the propylthiouracil (PTU) induced rat hypothyroidism. Methods : The rats were divided into 6 groups : intact control, PTU control, Levothyroxine (LT 4 ), 500, 250 and 125 mg/kg ISYYT treated groups. In ISYYT treated groups, PTU and ISYYT were administered for 4 weeks after 500, 250 and 125 mg/kg ISYYT were administered for 2 weeks. In LT 4 group, PTU and LT 4 were administered for 4 weeks. The changes were observed : the weigh, serum thyroid hormone levels, serum sex hormone levels, serum lipid profiles, serum liver enzyme levels, liver and testis antioxidant defense system, histopathology of thyroid gland, liver, epididymis, prostate and testis. Results were compared with PTU control group in this experiment. Results : In comparison with PTU control group, 500 and 250 mg/kg ISYYT treated groups showed significant increase of body, thyroid, liver, testis, epididymis and prostate weights, decrease of serum TSH levels with increase of serum T3 and T4 level, increase of serum testosterone and DHT levels with decrease of serum FSH levels, decrease of serum HDL with increase of TG and increase of serum AST levels. Histopathological inspections of hepatic and male reproductive organ damage induced by PTU were improved. And the changes of antioxidant defense system of Liver and testis induced by PTU were improve. There was no significant difference between 125 mg/kg ISYYT treated group and PTU control group in this experiment. Conclusions : The results obtained in this study considered that Insamyangyoung-tang may be effective in hypothyroidism and related organ damages.

진무탕(眞武湯)이 백서의 갑상선기능저하증에 미치는 영향 (Effects of Jinmutang (JMT) on Hypothyroidism in Rats)

  • Objectives : This study was done to investigate the effects of JMT on hypothyroidism in rats. Materials and Methods : After pretreatment with 50mg/kg thiouracil for 30 days, the experimental group was treated with 0.56g/kg JMT orally and the control group was treated with 0.56g/kg of normal saline instead for 20 days. The various indicators related to hypothyroidism were measured, such as T3-uptake content, T3 content, T4 content, TSH content, hematological values, enzyme activity, total cholesterol content, Na+ content change in the serum and body weight of rats induced by thiouracil. Results : The JMT-treated group showed significant increase in comparison with the control group in serum T3-uptake and T4 content. The JMT-treated group also showed significant decrease in comparison with the control group in serum TSH content. However, the JMT-treated group did not show significant increase in comparison with the control group in serum T3 content. The JMT-treated group showed significant increase in comparison with the control group in serum RBC. The JMT-treated group showed significant decrease in comparison with the control group in serum CPK, ALP, LDH content and serum total cholesterol content. The JMT-treated group did not show significant increase in comparison with the control group in serum Na+ content. The JMT-treated group showed significant decrease in comparison with the control group in body weight. Conclusions : These results suggest that JMT is useful in treatment of hypothyroidism.

부자(附子)가 6-propyl-2-thiouracil(PTU)로 유발된 rat의 갑상선 기능저하증에 미치는 영향 (The Effects of Aconiti Radix on Thyroid Function in Hypothyroidism Rat Model Induced by 6-propyl-2-thiouracil(PTU).)

  • Objective : Although hypothyroidism is a common disease in the endocrinology system, it is reported that there are a lot of difficulties in treating it effectively. Aconiti Ra얕 traditionally has been used in treatment of coldness, fatigue, and bradycardia. In this study, we investigated the therapeutic effects of Aconiti Radix on hypothyroidism rat model induced by 6-propyl-2-thiouracil (PTU). Methods : Two-month-old rats were used with administration of PTU which induced hypothyroidism in the rats. After 2 weeks, Aconiti Radix and thyroxine were administered, respectively. The body weights were measured every week. After 4 weeks, the blood samples of all rats were taken from their hearts. They were analyzed biochemically and $T_4$ (thyroid hormone) & TSH (thyroid stimulating hormone) was measured by ELISA kits. Results : In comparison with normals, controls showed hypothyroidism with significantly low $T_4$ and high TSH the statistics. In Aconiti Radix administration groups significantly increased $T_4$ was observed in the statistics and its effects were dose-dependent. There was no difference statistically in TSH of Aconiti Radix treatment groups from controls, nor were statistical differences observed significantly in biochemical labs and weight of each group. Conclusions : These findings suggest that Aconiti Radix protects thyroid cells and makes thyroid cells produce thyroid hormones. It is also very safe in the view of liver, kidney function, and other metabolism. It may be a useful agent for treating hypothyroidism.

불현성 갑상선기능저하증이 병발한 고령의 뇌졸중 환자의 호전 1례 (A Case Report of Cerebral Infarction in an Elderly Patient with Subclinical Hypothyroidism)

  • Hypothyroidism is a common disease of the endocrinal system, characterized by fatigue, cold intolerance, bradycardia, and so on. Subclinical hypothyroidism is a common biochemical abnormality which can be found in routine screening tests of thyroid function. It is defined as an asymptomatic state which characterized by normal free thyroxine(FT4) and elevated thyroid stimulating hormone(TSH) levels. The purpose of this study was to evaluate the effect of treatment with palmijihwang-tang and to observe the changes in triiodothyronine(T3), free thyroxine(FT4), thyroid stimulating hormone(TSH) level and VAS of cold intolerance, hoarseness, dry skin. After the treatment, triiodothyronine(T3) increased from 57.12ng/dl to 120.53ng/dl. Free thyroxine(FT4) increased from 12.59pg/ml to 14.21pg/ml. Thyroid stimulating hormone(TSH) decreased from 10.61mU/L to 1.57mU/L. Cold intolerance, hoarseness and dry skin changed for the better. These results support a role for oriental medical therapy in treating subclinical hypothyroidism. Further case studies of herbal treatment of this ailment are needed.

전현성 갑상선 기능저하증의 임상적 고찰 (The Clinical Study on 39 Cases of Subclinical Hypothyroidism)

  • Subclinical hypothyroidism can be defined as an asymptomatic state in which a reduction in thyroid activity has been compensated by an increased TSH output to maintain a euthyroid state. We analysed clinical features, laboratory data, and pathologic findings in 3g cases of subclinical hypothyroidism who were diagnosed at the Dept. of Internal Medicine, Chungnam National University Hospital from Aug. 1984 to June, 1985. 1) The age distribution was from sixteen to sixty-nine and mean a9e was 34.8. Peak incidence was in the 4th decade and 3rd, 5th, 6th decade in order. 2) The sex distribution showed female preponderance with a ratio of 18.5 to 1. 3) The major presenting manifestations were nonspecific ones such as fatigue, indigestion, and anorexia. 4) Physical examination revealed diffuse goiter in 47.6%. Major abnormalities were no gross abnormality (30.9%), nodular goiter and facial edema. 5) There was no significant difference of the basal serum $T_3\;and\;T_4$ concentrations between subclinical hypothyroidism and normal controls (p>0.05). 6) The basal serum TSH concentration of subclinical hypothyroidism $(32.61{\pm}14.95{\mu}U/ml)$ was significantly higher than that of normal controls $(3.92{\pm}1.05{\mu}U/ml)$ (p<0.005). 7) Microsomal antibody was detected in 80.6% and thyroglobulin antibody was detected in 30%. 8) The pathologic findings in 26 cases revealed Hashimoto's thyroiditis in 76.9% (lymphocytic type, 34.6%; oxyphilic type, 26.9%; fibrotic type, 15.4%). The others were adenomatous goiter(15.4%), adenomatous carcinoma (3.8%) and subacute thyroiditis(3.8%).

PTU로 유발된 흰쥐의 갑상선 기능저하증에 미치는 십전대보탕(十全大補湯)의 효과 (Effects of Sipjeondaebotang on the white rat Hypothyroidism Induced by PTU(6-n-propyl-2-thiouracil))

  • Objectives : This study was to evaluate Sibjeondaebo-tang’s(Shiquan dabutang) effect to rat hypothyroidism which induced by 6-n-propyl-2-thiouracil(PTU)Methods : Aqueous extracts of Sibjeondaebo-tang(Shiquan dabutang)(21.29%) were administered by oral dose of 300 and 150 mg/kg(rat’s body weight) for 42 days from 2 weeks before start of PTU treatment. During this moment the body weight, thyroid gland weights, liver weight, serum thyroid hormone, lipid content and liver antioxidant defense system were measured with histopathological changes of thyroid glands, Liver and these result were compared with LevoT4 0.5mg/kg treated ratsResults : As a result of PTU treatment, marked decreases of body weight, liver weight, serum thyroid hormone levels of T3, T4, triglyceride content and liver CAT activation were observed with marked increases of thyroid gland weight, thyroid gland hormone TSH, activate liver H2O2 and SOD, serum Total cholesterol, HDL and AST amount were admitted. In addition, marked hyperplasia of follicular cells with reduce of follicular colloid contents and sizes were showed at histopathological inspections. These PTU-induces histopathological changes related to hypothyroidism were significantly reduced by treatment of both different dosages of Sibjeondaebo-tang(Shiquan dabutang).Conclusions: This study suggest that Sibjeondaebo-tang(Shiquan dabutang) extracts have favorable effect on the thyroid hormone productions with beneficial effect on hypothyroidism medicated by modulatory effects on antioxidant defense system.

  • Objective : Hypothyroidism is a common disease of the endocrinal system. characterized by fatigue, cold intolerance, bradycardia, and so on. Clinically, Levothyroxine(L-T4) has been usually used for replacement therapy, but it often has side effects. so many hypothyroidism patients wants oriental medical therapy. Zingiberis rhizoma, traditionally has been used in treatment of coldness, fatigue, and bradycardia. In this study. I investigated the therapeutic effects of Zingiberis rhizoma on PTU induced hypothyroidism in rats. Methods : I used two-month-old rats administered PTU and induced with hypothyroidism. After 2 weeks. Zingiberis rhizoma and thyroxine were daily administered, respectively. Body weights was measured every weeks. After 4 weeks, blood samples were taken and analyzed biochemically and T4 and TSH were measured by ELISA kits. Results : In comparison with normal groups, control groups showed hypothyroidism with low T4 and high TSH level. In Zingibris rhizoma administration groups were observed T4 level elevation, this elevation was dependent on the dose of Zingibris rhizoma. Between experimental groups and control groups, there was no difference in TSH level, statistically. Changes of biochemistry were not observed in any experimental groups. Conclusions : These findings suggest that Zingiberis rhizoma makes thyroid cells producing thyroid hormones. There is also a non-toxic effect on the cardiovascular system, liver and kidney function. So, Zingiberis rhizoma should be an effective agents for treating hypothyroidism.

포부자추출물의 갑상선기능저하증 흰쥐모델에서의 한열조절작용에 의한 개선효능 연구 (Effects study of Aconiti Lateralis Radix Preparata extract on the regulation of heat and cold in PTU-induced hypothyroidism rats)

  • Objective : To suggest a scientific evidence of Aconitum carmichaeli Debx. (Aconiti Lateralis Radix Preparata: ALRP) as one of cooling and heating medicines on the regulation of body temperature, we investigated the effects of ALRP water extract on hypothyroidism. Methods : Hypothyroidism was induced by intradermal injection with PTU for 4 weeks in SD rats. ALRP extract or L-thyroxine as a control drug was orally administrated for 2 weeks with PTU injection in rats. The physiological and serological parameters were measured in rats. The histological change of thyroid tissues was observed by H&E staining, and also the expression of thermo-regulating proteins was determined by Western blot in dorsal root ganglia and brain tissues of rats. Results : The administration of ALRP extract in PTU-induced hypothyroidism rats was significantly increased body temperature, but did not changes on body weight, food and water intake. ALRP extract did not effect on the levels of TSH and T4 in the hypothyroidism rats. ALRP extract significantly decreased the levels of GPT, glucose, HDL-cholesterol, LDL-cholesterol, and total cholesterol in the hypothyroidism rats. In histological observation, the enlarged epithelium and atrophic follicles with higher concentration of follicular cells on hypothyroidism were improved by ALRP extract. In addition, ALRP extract increased the expression of TRPV1 and TRPM8 ion channel proteins in hypothyroidism rats. Conclusion : These results indicate that ALRP extract can improve PTU-induced hypothyroidism through regulation of body temperature and lipid accumulation. The action mechanism of ALRP extract is related with body temperature control by thermoregulation with TRP ion channels.

GNAS 메틸화 이상으로 인한 거짓부갑상선기능저하증 Ib 1예 (A Case of Pseudohypoparathyroidism Type Ib Caused by Aberrant Methylation in the GNAS Complex Locus)

  • Pseudohypoparathyroidism (PHP) is a rare disorder caused by genetic and epigenetic aberrations in the GNAS complex locus resulting in impaired expression of stimulatory G protein ($Gs{\alpha}$). PHP type Ib (PHP-Ib) is characterized by hypocalcemia and hyperphosphatemia due to renal resistance to the parathyroid hormone, and is distinguished from PHP-Ia by the absence of osteodystrophic features. An 11-yr-old boy presented with poor oral intake and cramping lower limb pain after physical activity. Laboratory studies revealed hypocalcemia, hyperphosphatemia, and increased parathyroid hormone levels. The GNAS complex locus was evaluated using the methylation-specific multiplex ligation-dependent probe amplification (MS-MLPA) assay. Gain of methylation in the NESP55 domain and loss of methylation in the antisense (AS) transcript, XL, and A/B domains in the maternal allele were observed. Consequently, we present a case of PHP-Ib diagnosed using MS-MLPA.
