생체신 공여자의 수술전후 관리에 대한 연구 -윤리적인 측면을 중심으로- (Pre and Post Operative management of Living Kidney donor - Focus on bioethical aspect -)

  • The purpose of this study was presented basic data for management of living kidney donor to make bioethical decision. The research subjects were the documents and progress notes of Doctor's and Nurses in medical records related to kidney donation and nephrectomy of 20 kidney donors who received nephretomy at 4 general hospitals in Pusan. The result of this study, a desirable documents to help the living kidney donor might include following : 1st, identification of the donor and the guardian. 2nd, confirmation of the intension of kidney donor which is based on humanity or not and 3rd, a written oath about Nephrectomy. Especially it is more desirable to participate of paramedical personnels such as the religious, the social workers, the counsellors, and etc when assess the motivation of kidney donor and to use of a formulated visual educational materials about renal angiography and nephrectomy which required written consent of kidney donor. Further more, the donor should be educated sufficiently about the kidney itself and procedure for nephrectomy-the anatomical position of kidney, the function of the kidney, the operative maneaver, pre and post operative complication, the prevention of the complication, the possiblity of rejection phenomenon and loss of the transplanted kidney etc. In conclusion, medical team members for kidney transplantation must suggest not only physical problem but also psychological problem And the educated donor ought to have enough time before a making decision and all these process should be recorded in medical records

간호사 행복에 대한 국내 연구 동향 (Domestic Research Trends on Nurse Happiness)

  • 본 연구에서는 국내에서 연구된 간호사의 행복에 관한 문헌을 고찰함으로써 간호사 행복의 주요 특성과 추후 연구의 방향을 모색하고자 실시하였다. 연구대상은 국내 검색데이터 베이스(NANET, RISS, KSI-KISS, DBpia)에서 '간호사'와 '행복' 또는 'nurses' and 'happiness'로 각각 조합하여 발표된 학술논문 중 온라인상에서 검색된 총 8편을 선정하였다. 2013년 이후부터 간호사 행복 논문이 출판되었다. 주요 연구결과, 연구출처는 '간호학 저널', '기타 간호학 저널'이, 연구도구는 Lyuvomirsky & Lepper(1999)와 OECD Better Life Index (2012), 생명윤리위원회의 '승인'과 '미승인'이 각각 50% 차지하였다. 추후 연구에서는 간호사의 조직문화를 고려한 행복측정도구개발과 간호사의 행복감을 향상시킬 수 있는 중재방안 모색 등이 요구된다.

요양병원 간호사의 신체적 억제대 사용과 관련된 간호실무에 영향을 미치는 요인 (Factors Influencing Nursing Practices of Physical Restraint Use among Nurses working in Long-term Care Hospitals)

  • 환자에게 사용되는 신체적 억제대가 신체적, 정신적 윤리적으로 많은 문제점을 일으키지만, 여전히 임상에서는 무분별하게 사용되고 있다. 따라서 본 연구에서는 요양병원 간호사의 신체적 억제대 사용과 관련된 간호실무에 대한 관련 요인을 확인하기 위한 서술적 상관관계 조사연구를 시행하였다. 자료 수집은 2017년 1월 2일부터 6월 30일까지 11개의 요양병원 간호사 128명을 대상으로 자가 보고식 설문지를 통해 시행되었다. 자료 분석은 기술적 통계, t-test, one-way ANOVA, Welch's test, Pearson correlation coefficients 및 다중 회귀분석을 사용하였다. 연구결과 요양병원 간호사들의 신체적 억제대에 대한 지식, 태도, 인식 및 간호실무 정도는 보통 정도로 나타났으며, 간호실무와 지식 사이에는 유의한 양의 상관관계가 있었다. 다중 회귀분석을 통해 신체적 억제대 사용과 관련된 간호실무에 영향을 미치는 요인으로는 지식(${\beta}=0.27$, p=.002), 태도(${\beta}=-0.17$, p=.044)순으로, 이들 전체 변수의 설명력은 9%였다. 본 연구에서 신체적 억제대 사용에 대한 간호실무에 가장 중요한 예측 인자는 신체적 억제대 사용에 대한 지식임을 확인하였다. 따라서 요양병원에서 신체적 억제대 사용을 줄이기 위한 근거기반의 교육 프로그램의 개발과 적용이 필요하겠다.

간호대학생의 인문학적 소양과 공감능력이 윤리적 가치관에 미치는 영향 (Influence of Humanities and Empathy on Ethical Values of Nursing Students)

  • This study was to identify the factors of ethical values in nursing students. The survey was conducted in two nursing colleges (n=180) using self-reported questionnaire. The data were analyzed with descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficient and multiple regression analyses using SPSS/WIN 21 software. Influencing factors of ethical values were empathy(t=3.436, p=.001), teaching method except lecture(t=3.037, p=.003), and curriculum track at high school(t=2.309, p=.022). The model was statistically significant explaining 16.2% of the variance (F=11.185, p<.001). This study suggests that strategies for increasing nursing students' ethical values include reinforcing empathy, utilizing various type of teaching method except lecture in nursing ethics class, and allowing high school students applying to college of nursing regardless of their curriculum track.

간호사와 간호대학생의 문화적 역량 구성요소에 관한 융합적 연구 (A Convergence Study of Cultural Competence Dimensions of Nurses and Nursing Students)

  • 본 연구는 국내·외에서 발표된 간호사와 간호대학생의 문화적 역량 구성요소에 대한 양적 연구를 체계적으로 고찰하여 문화적 역량에 대한 이론적 이해를 돕기 위해 시도되었다. 문헌검색은 2020년 6월부터 8월까지 약 3개월 동안 수행하였으며, 2010년 1월 1일 부터 2019년 12월 31일까지 발표된 학술지 논문 및 학위 논문을 대상으로 검색하였다. 문헌추출은 PRISMA 가이드 라인에 따라 진행하였으며 총 1,175편의 논문 중 67편이 체계적 문헌고찰 논문으로 선정되었다. 연구 결과 가장 많이 차지하는 구성요소로는 문화적 인식이었으며 문화적 지식, 문화적 기술, 문화적 민감성, 문화적 만남, 문화적 태도, 문화적 행위, 문화적 갈망 그리고 문화적 수용성, 문화적 경험, 문화적 안위, 문화적 윤리, 문화적 신념, 문화적 실천, 동기부여와 호기심, 문화적 감정과 공감 등이었다. 따라서 간호사와 간호대학생의 문화적 역량을 향상시키기 위하여 다양한 구성요소를 바탕으로 하여, 각 사회에서 필수적인 합의와 다각적인 접근 방법이 모색되어야 할 필요가 있다.

인간생명에 대한 간호사의 윤리의식 (A Study on the Clinical Nurse's Ethical Value)

  • Purpose: This descriptive study was designed to explore the clinical nurse's ethical value regarding human life. Method: Data were collected from September to October, 2002. Study subjects were 527 clinical nurses working in General Hospital as tertiary located in Seoul. Ethical value was measured with questionnaire developed by researchers and consisted on items regarding ethical value on human life. Result: Among the items, most nurses highly agree with the item, "When a patient requests his/her health care provider to keep his/her personal secret, the health care provider is obliged to do so." and "When a patient asks for information on his/her medicinal and dietary contents, his/her wish must be granted." Most clinical nurses mainly agree with the item. "Health care providers must always be honest to the patient and/or his/her family". However, most nurses disagree with the item, "When a patient is on the verge of death after an accident, it is justifiable to soothe his/her family by saying 'he/she is OK' instead of telling them the truth, in order to avoid a sudden shock befalling on them". There were significant differences in some items of ethical value according by clinical nurse's age and professional experience, current position, religion, education, marital status, continued education on ethics, and the experience of holing on life saving treatment. Conclusion: It is intensifying the notion of ethical underpinning for human rights, truthfulness is essential to a trust relationship under what circumstances. Also most clinical nurses agree with that It is essential to trust in the nurse-patient relationship, patients have the right to know and it is the ethical thing to do as health care provider.

인공임신중절과 관련된 윤리학적 논쟁과 문제 분석 (Ethical Argumants and Problems Analysis Related to Induced Abortion)

  • Over one million cases of the induced abortion have been conducted annually in Korea. Among those cases, most of all were illegally done, but this has not been addressed in the literature. While Korean Nurse Association Code of Ethics presents the respect for life as one of the basic ideology, it was not dealt enough in nursing education. The purposes of the study were to activate the debate on the issues related to an induced abortion ; to introduce the related ethical theories ; and to seek the solution of the ethical problems, which will eventually result in establishing the morality of nursing practice. The ethical theories of an induced abortion have traditionally addressed two extreme perspectives ; the conservatives who emphasize the sanctity of human life and the right of life that will never ethically allow the killing fetus ; and the liberalists who insist the right to choice for women to control their body. Since these extreme theories has not been helpful to solve the ethical problems, the recent trend is leading to the modified theories both from conservative and liberal perspectives. The examples of the theories are the potentiality of fetus(Singer, 1993), the obligation of women to serve their body to fetus(Held, 1987), the Replacement theory (Callahan, 1987), and the Principle of Caring (Gilligan, 1982). The study conducted the indepth interviews with 17 women who experienced the induced abortion and the 6 cases were selected to be analyzed. The cases were analyzed and interpreted by using an integrated case method which was combined of the New Casuisty(Jonsen & Toulmin, 1988 ; Jonsen, 1991) and the Specified Principlism (Richardson, 1990 ; Degrazia, 1992). The result of analysis revealed three types of ethical problems ; (1) the responsibility of taking care of the baby to be born (2) the fear for the condition of the fetus, and (3) the choice of induced abortion as the method of birth control. The findings also revealed the related ethical principles for various situations ; the principle of caring was used for choosing an induced abortion by the subjects ; the principles of the potentiality of fetus and the obligation of women to serve their body were for the consideration for the life of fetus ; and the principle of replacement was utilized for the right to choice for women. The ethical principles related to an induced abortion introduced in the study provided the way to solve the moral problems by applying to the clinical situations for nurses. The study also revealed the possibility of modifying the current ethical theories from the method of applying the principles to the various situation in the study. The modified theories would be more useful to guide the clinical practice with similar ethical problems.

권역 외상 중환자실 간호사의 교육 현황, 교육 요구 및 간호역량 관련 융합 연구 (A Convergence study on Education Status, Educational Needs, and Nursing Competence of Regional Trauma Intensive Care Unit Nurses)

  • 본 연구의 목적은 권역 외상 중환자실 간호사의 교육 현황, 교육 요구 및 간호사 역량을 파악 하는데 있다. 자료 수집은 5개 권역 외상 중환자 치료센터에 근무하는 123명 간호사로부터 설문지를 수집하였다. 조사 설문지는 외상 중환자실 간호사의 교육 현황, 교육 요구 및 간호사의 역량으로 구성되었다. 연구 분석은 SPSS 21.0 프로그램을 이용하여 서술통계, t-test, One-way ANOVA, Pearson Correlation으로 분석하였다. 연구결과 국내 외상 중환자 간호 교육 참여는 78.9%이었다. 외상 중환자 간호 교육 중요도 결과 '태도'는 3.64, '지식'은 3.52, '기술'은 3.47로 나타났다. 교육 요구도 결과 '지식'은 3.18, '태도'는 3.05, '기술'은 2.97로 나타났다. 간호사의 역량 조사 결과 '윤리적 역량'은 3.62, '심미적 역량'은 3.53, '인격적 역량'은 3.39, '과학적 역량'은 3.37로 나타났다. 결론적으로 현시점에 맞고 적용 가능한 외상 간호교육의 표준화된 교육과정 및 실습위주의 교육개발에 초점을 두는 것이 필요하다. 또한 외상 중환자실 간호사의 전문성과 간호역량증진을 위한 전략모색이 필요하다.

심폐소생술 금지와 관련된 병원간호사들의 경험 (Hospital Nurses' Experience of Do-Not-Resuscitate in Korea)

  • Purpose: The purpose of the study was to describe the experiences of do-not-resuscitate (DNR) among nurses. Methods: Data were collected by in-depth interviews with 8 nurses in 8 different hospitals. Conventional qualitative content analysis was used to analyze the data. Results: Eight major themes emerged from the analysis: DNR decision-making bypassing the patient, inefficiency in the decision-making process of DNR, negative connotation of DNR, predominance of verbal DNR over written DNR, doubts and confusion about DNR, least amount of intervention in the decision for DNR change of focus in the care of the patient after a DNR order, and care burden of patients with DNR. Decision-making of DNR occurred between physicians and family members, not the patients themselves. Often high medical expenses were involved in choosing DNR, thus if choosing DNR it was implied the family members and health professionals as well did not try their best to help the patient. Verbal DNR permission was more popular in clinical settings. Most nurses felt guilty and depressed about the dying/death of patients with DNR. Conclusion: Clearer guidelines on DNR, which reflect a family-oriented culture, need to be established to reduce confusion and to promote involvement in the decision-making process of DNR among nurses.

임상 간호사의 도덕성 발달과 윤리적 가치관에 대한 연구 (A Study of Moral Development and Ethical Values of the Nurse)

  • This study is to provide the basic sources to develop educational data that can establish ethical values with research on the interrelation between moral development and ethical values of nurses. The subjects of this study are the convenience sample from 111 nurses working in clinical settings from 5 general hospitals located in Pusan area. The questionnairs for this study are comprised two parts: Part 1 is SRM-SF(1995) guestionnaire, and translated by Kil, 1m Ju(1997): Part 2 is the guestion papers about the ethical values, and disigned by Jung, Hee Ja. The data were collected from June lth to 31th, 1997 and analyzed by S.A.S(Statistical Analysis System) Package for frequency, percent, and Pearson's correlation coefficient. The results of the study is as the followings: 1) According to the moral development. the nurses are in the levels of Stage3 or Transition3/4, which are generally focused on the personal relations or personality. Thus, nurses are considered to need to get the stage4 of education that helps them recognize a person as a member of society. a family as an unit of society, and human life as the values of human dignity. 2) According to the ethical values, nurses have the positive ethical values of respecting the right of the patients, providing information, keeping the secret, truth. and credit of patients. But they also experience some troubles by the patients to care and the clinical situation. Thus, to overcome the problems, nurses are considered to need to get the education that establishs the right ethical values. 3) The analysis result on the interrelation between moral development level and ethical values shows as r=0.28 and p=0.003. This indicates that the interrelation between them is low. In conclusion, it is considered that the moral development level is viewed broadly from a person and family to a society, and then it can affect to the ethical values positively.
