Nursing outcomes of inpatient on level of nursing staffing in long term care hospitals (요양병원 간호인력 확보수준에 따른 입원환자의 간호결과)

  • This study was conducted to explore the impact of nursing staffing on inpatient nursing outcomes in long term care hospitals. A secondary analysis was done of national data from the Health Insurance Review and Assessment Services including evaluation of long term care hospitals. Patients per RN was a significant indicator of foley catheter ratio in high risk group and low risk group. Patients per RN&NA was a significant indicator of decline in ADL for patients with dementia, non dementia, urinary incontinence and new pressure ulcer development in the high risk group. The average nursing outcome of inpatient in high grade was higher than that low grade in long care hospital. This higher level of nursing staffing and the higher the grade shown a positive effect on the nursing outcomes of the inpatient. We therefore recommend modifying the above nurse staffing policy so as to make it more effective in improving nursing outcomes.

Determinants of Registered Nurse Skill Mix & Staffing Level in Korea (간호인력 구성 및 확보수준 결정 요인)

  • Purpose: This study was done to identify determinants of registered nurse (RN) skill mix and staffing level focused on hospital characteristics. Methods: Data were obtained from health insurance claims data and hospital reporting system in the Health Insurance Review and Assessment Service (HIRA) for the year 2010. Data from 2,998 hospitals were analysed using t-test, ANOVA, Pearson correlation, and regression analysis. Results: The RN skill mix and staffing level were positively related to hospital size and the percentage of inpatients to total patients. RN skill mix and staffing level were statistically different across regions. Including nursing aides (NA), however, there was no difference in staffing levels across regions. Medically vulnerable regions, bed operation rate, and the number of patients per doctor were also related to RN skill mix and staffing level. Conclusion: The statically significant determinants of RN skill mix and staffing level included hospital size, region, bed operation rate, percentage of inpatients, doctor-patient ratio. Further study needs to be done to investigate factors including RN supply and wages.

Effects on Long-Term Care Hospital Staff Mixing Level after Implementing Differentiated Inpatient Nursing Fees by Staffing Grades (간호등급제가 요양병원의 간호인력 확보수준에 미치는 영향)

  • Purpose: The purpose of this study was to examine trends in number of nursing staff and skill mix. Methods: Nursing staff and skill mix were measured using the number of nursing staff including nurse aids and registered nurses per bed. Descriptive and panel data regression analyses were conducted using data on long-term care hospitals which included yearly series data from 2006 to 2010 for 119 hospitals. Results: The number of nursing staff per bed increased significantly but percentage of registered nurses decreased significantly from 2007 to 2010. The regression model explained this variation as much as 34.9% and 43.8%. Conclusion: The results showed that in long-term care hospitals there were more nurse aids employed instead of registered nurses after the implemention of differentiated inpatient nursing fees. Thus clarifying the job descriptions for nurses and nurse aids is needed and appropriate hospital incentive policies should be implemented.

Impact of Nurse Staffing Level and Oral Care on Hospital Acquired Pneumonia in Long-term Care Hospitals (요양병원의 간호인력 확보수준과 구강간호 실시여부가 노인 환자의 폐렴발생에 미치는 영향)

  • Purpose: This study was conducted to explore the impact of nurse staffing level and oral care on pneumonia in elderly inpatients in long-term care hospitals (LTCHs). Methods: Data were obtained from the Health Insurance Review and Assessment Services (HIRA) including the profiles of LTCHs, monthly patient assessment reports and medical report survey data of pneumonia patients by HIRA in the fourth quarter of 2010. The sample consisted of 37 LTCHs and 6,593 patients. Results: Patient per nurse staff (OR=1.43, CI=1.22~1.68) and no oral care (OR=1.29, CI=1.01~1.64) were significantly related with hospital acquired pneumonia. The difference in percent of oral care by hospital was not significant between high and low group in nurse staffing level. Conclusion: In order to reduce the occurrence of pneumonia in eldery patients, effective nursing interventions are not only required but also nurse staffing levels that enable nurses to provide the intervention.

  • 한국경제 수준의 향상과 보험제도의 도입으로 병원을 찾는 대상자의 수가 증가하게 되었다. 아울러 저렴한 보험숫자가 병원슷가가 병원의 경영난을 초래하게 되므로 병원주는 이를 극복하기 위한 과학적이고 체계적인 병원관리에 관심을 갖게 되었다. 특히 간호부(과)에서도 목적 지향적인 계획과 업무수행에 관심을 갖게 되어 그 관리체계나 간호의 질이 향상하기 시작하였다. 간호의 질을 향상시키기 위한 간호관리란 그 내용이 방대하므로 본 논문에서는 수간호원의 역할에 중점을 두었다. 즉 간호의 질 향상을 위한 평가 중 소급평가로써 퇴원환자 기록지 감사, 퇴원예정환자면접 및 병실 집담회에 관한 내용과 동시 평가로써 환자 기록지 감사 및 환자 면접 내용에 관해서 논하였다. 또한 인력관리를 위해서는 간호원-환자간의 관계, 간호원-의사간의 관계, 직무평가, 업무평가 및 간호인력 활용에 관해서 미국과 한국의 차이점을 제시하였다. 결론적으로 한국의 간호관리는 병원표준화 심사실시 및 간호원의 계속적인 노력으로 많은 향상을 보았으나 아직까지는 간호업무 수행절차에 관한 평가에 불가하므로 목적 지향적인 업무체계화와 한국병원상황에 적합한 평가도구의 틀개발과 질적간호를 위한 적정간호인력의 확보에 관한 노력이 계속되져야 한다고 본다.

  • 오늘날 건강은 생존하는 인간이 지녀야 할 기본권리로 인정되고 있으며 국민의 복지향상이 없는 경제발전은 국제사회에서도 신망을 얻지 못하는 시대에 이르렀다. 국민의 건강수준은 그 사회전체의 문명척도를 의미하는 것이기 때문에 세계도처에서 인간의 복지향상과 인권보존을 위해 여러가지 형태로 건강사업을 추진하고 있으며 건강관리를 수행하는데 있어서 중추적 역할을 담당하는 간호의 교육은 매우 중요하게 다루어지고 있다. 1903년, 외국 선교사에 의해 간호교육기관이 설립된 이후 사회 정치적 배경에 따라 교육제도상 많은 변화를 가져왔다. 대학과정은 1950년에 개설했으나 6.25동란으로 첫번 학사 간호원은 1959년에 배출되었으며 현재 14개 대학이 있어 대학원과정도 운영하고 있다. 3년제 간호교육은 간호교육이념에 따라 1962년부터 초급대학 수준으로 승격되었고 1970년에는 교육법령이 개정되면서 전문학교로 개칭하게 되었으며 1979년 부터는 전문대학으로 개편하여 입학수준을 대학입학예비고사 합격자로 수준을 높였다. 계속 교육제도에 있어서는 석사 및 박사과정 이외에 여러가지 전문분야의 연구과정 및 실무 수련과정이 마련되어 있다. 간호교육 이념정립과 제도 개선을 위해 학계에서는 꾸준한 연구를 지속하여 정책수립에 반영했다. 지금까지의 진료중심의 간호에서 지역사회 건강중심의 간호역할로의 전환기에 있음을 인식하여 바람직한 변화가 올 수 있도록 교육목표를 설정하는 일은 매우 중요하다고 본다. 전문기관에서는 양적, 질적 간호인력 수급계획을 마련해야 할 것이며 간호원의 사회 경제적 지위향상은 물론 간호원의 역할을 법적으로 규정하여 효율적인 인력활용을 위해 계속 연구하는 일은 매우 중요한 과제라고 생각한다. 앞으로의 간호교육은 첫째, 현직 간호원의 사회 경제적 지위를 향상하고 둘째, 적성에 맞는 학생을 선발하고 셋째, 유능한 교수를 확보하는 일이라고 본다.

An Analysis of Nurse Staffing Level and Nursing Performance in the General Unit (일반병동 간호인력 확보수준과 간호업무성과 분석)

  • Purpose: To examine the relationship between nurse staffing level and nurse perception of nursing performance. Method: The subjects of this study were 724 nurses from 70 medical or surgical wards in 23 hospitals from February to March 2006. A self-reporting questionnaire, which was developed by the researcher through a preceding study, was used. In data analysis, SAS program was utilized for descriptive statistics, t-test, ANOVA, and Tukey test. Results: The mean score of the nursing performance was 53.3. The mean of bed-to nurse ratio was 3.39:1. The study revealed a negative correlation between bed-to nurse ratio and the nurses' perception of nursing performance. There was a significant relationship between above 3.5:1 and below 2.5:1(F=4.59, p=0.010). Conclusion: We found that the higher the nurse staffing, the better the nursing performance. It is recommended that future research measures nurses' performance and nursing organizational performance using objective measurement tools including clinical indicators.

  • Purpose: In this paper issues and challenges of the workforce policy for nursing were explored and appropriate policy responses identified. Results: Many countries, including South Korea, are facing a shortage of nurses. In South Korea, the number of practicing registered nurses is about half the average for Organization for Economic Cooperation and Development (OECD) member countries. The shortage of nurses is not necessarily a shortage of individuals with nursing qualifications but also includes complex issues, such as lack of well-educated nurses, shortage of nurses willing to work, and geographical imbalances of nurses. The present nurse workforce policies are to increase number of nursing schools, to reduce the length of training, and to replace nurses with nursing assistants. However, the findings of many studies have shown that these attempts resulted in a worsening of patient outcomes and increasingly low quality of care. Conclusion: The findings in this study indicate that nurse workforce policy should have a multi-faceted approach in order to address the many factors affecting nurse shortages.

Changes in Hospital Nurse Staffing after Implementing Differentiated Inpatient Nursing Fees by Staffing Grades (입원환자 간호관리료 차등제 도입이후 간호사 확보수준의 변화)

  • Purpose: To examine the changes in nurse staffing in hospitals after implementing the policy of differentiating inpatient nursing fees by staffing grades. Method: The study sample included 43 tertiary hospitals, 185 general hospitals, and 282 non-general hospitals that were operating in both 1999 and 2008. Nurse staffing grade was categorized from Grade 1 (highest) to 6 (lowest) in 1999 or Grade 7 in 2008, based on the nurse-to-bed ratio. Results: Tertiary hospitals at Grade 3 and Grade 4 accounted for 49% and 35%, respectively, in 2008, whereas 63% were Grade 6 in 1999. General hospitals at Grade 6 decreased from 87% to 48%. In non-general hospitals, little change was found in the staffing distribution, in that 92% still remained in Grade 6 or 7 in 2008. Forty tertiary hospitals (93%) and 45% of general hospitals improved their staffing grades, while only 7% of non-general hospitals did. Greater likelihood of improvement in staffing grades was found in general hospitals located in metropolitan areas or having 250 or more beds. Conclusion: Elaboration of the financial incentive system is needed to increase the policy impact on staffing improvement.

  • Purpose: The purpose of this study was to figure out the effect of nurse staffing in nursing home on the quality of care and quality of life of the residents, using Castle & Engberg's conceptual framework. Methods: A total of 31 studies published between 1996 and 2021 were selected from 6 databases, searched for keyword such as "Nurse staffing", "Nursing staff", "Outcome", "Quality of care", "Quality of life", "residents outcomes", "nursing home", "long-term care". By using Castle & Engberg's conceptual framework, systematic review was conducted. Results: More time provided by nursing staff, high level of licensed nurse staffing, good relationship between nursing staff and residents, and generous supervision system have a positive effect on resident outcomes. Conclusion: Establishing regulatory strategies of having mandatory Registered Nurse is needed because nurse staffing in nursing homes was related to resident outcomes. In addition, it is necessary to conduct further research about quality of life beyond quality of care for residents.
