장기요양시설 요양보호사의 노인에 대한 태도, 공감, 직무 스트레스가 서비스 질에 미치는 영향 (The Influence of Attitude toward the Elderly, Empathy and Job Stress on Quality of Service among Caregivers in Long-term Care Facilities)

  • 본 연구의 목적은 장기요양시설 요양보호사의 노인에 대한 태도, 공감, 직무 스트레스가 서비스 질에 미치는 영향을 파악하기 위함이다. 자료수집은 C도 소재 장기요양시설에 근무 중인 요양보호사 133명을 대상으로, 2019년 5월 1일부터 8월 31일까지 자가 보고식 설문조사로 시행되었다. 수집된 자료는 기술통계, t-test, ANOVA, Pearson correlation coefficient, stepwise regression으로 분석하였다. 연구 결과, 요양보호사의 공감은 직무 스트레스와 음의 상관관계, 서비스 질과 양의 상관관계를 보였으며, 노인에 대한 태도, 직무 스트레스는 서비스 질과 음의 상관관계가 나타났다. 대상자의 직무 스트레스, 공감, 노인에 대한 태도가 서비스 질에 영향을 주는 요인으로 확인되었으며, 요양보호사의 서비스 질에 대해 약 31.6%의 설명력을 나타냈다. 본 연구결과는 요양보호사의 직무 스트레스, 공감, 노인에 대한 태도가 서비스 질 향상을 위해 고려해야 할 주요한 요인임을 제시하였다.

노인 간호 요양시설에서의 치매환자 통증관리 프로토콜 개발 및 효과 (Development and Effect of Pain Management Protocol for Nursing Home Patients with Dementia)

  • Purpose: This study was done to develop a pain management protocol for nursing home patients with dementia and to examine effects of the protocol on pain assessments and interventions by the nurses and on pain relief signs in the patients. Method: The six steps in the protocol development and the examination of effect are outlined. Three rounds using the Delphi technique and one group pretest-posttest design experiment were developed. Design issues, such as sample selection and sample size, are addressed in relation to the study protocol. Results: After implementation of the pain management protocol, there were significant changes nursing actions including frequency of number of physical examinations, utilization of pain assessment tools, and request to doctors for discomfort management and there were significant changes in frequency in the number of verbal and physical expressions of pain, and emotional patterns. Conclusion: This is the first pain management protocol for patients with dementia in Korea. However, more study will be needed to determine the methodological strength and necessary revisions for the protocol.

시설입소 노인환자 부양가족의 만족도 영향요인 (Factors related to Family Caregiver Satisfaction with Elderly in Nursing Facilities)

  • 본 연구는 노인요양시설에 입소한 노인 환자들의 부양가족을 대상으로 만족도, 가족지지, 성과지표의 관계를 파악하고, 만족도에 영향을 미치는 요인을 파악하는 것이다. 본 연구의 대상은 서울, 경기지역에 위치한 노인요양 시설 5개소에 입소한 노인환자의 부양가족 126명을 대상으로 하였으며 만족도, 가족지지, 성과지표 측정도구로 자료수집 하였다. 만족도는 가족지지와 성과지표와 유의한 순상관 관계가 있었으며(r=.250, p=.008; r=.356, p=.000), 관련변인들의 만족도에 대한 설명력은 23%였고, 만족도에 유의한 영향을 미치는 요인으로는 경제수준, 가족지지, 성과지표로 나타났다. 본 연구결과는 노인요양시설에 입소한 노인환자 및 부양가족의 만족도 향상 향상시키는데 필요한 기초자료로 활용될 수 있을 것이다. 사회복지 정책적 함의 및 향후 연구 방향을 제안하고 있다.

코로나19 대유행 시 일 노인요양시설에 종사하는 요양보호사의 직무경험에 대한 연구 (A Phenomenological Study on the Experiences of Long Term Care Facility Workers during the COVID-19 Pandemic)

  • Purpose: The purpose of this study was to understand the experiences of those who take care of the elderly at a long-term care facility during the COVID-19 pandemic. Methods: Colaizzi's phenomenological research method was applied to describe the meaning and nature of the experiences of eight workers in an elderly care facility during the COVID-19 crisis. Results: Studies have shown that these workers experienced 'fear from infectious disease' and had the opportunity to 'reflect upon life changed by infectious disease.' They also worked together to overcome the crisis by 'protect nursing home with the nurses at COVID-19 sites' and felt 'safety from the nursing home,' and experienced 'a strong sense of calling for the job' through the crisis. Conclusion: This study is the first of its kind conducted in Korea on workers in an elderly care facility. The results indicate that it is necessary to supports these workers with practical useful education, counseling, and emotional support during the epidemic.

요양시설 종사자의 코로나 19 대응 전·후 직종별 직무스트레스 차이 (Differences in Job Stress by Occupation Before and After the Reaction to COVID-19 among Care Facility for the Elderly)

  • 본 연구는 요양시설 종사자의 코로나 19 대응 전·후 직종별 스트레스 차이를 알아보고자 시도된 서술적 조사연구이다. 본 연구에서는 2021년 3월 22일에서 4월 25일까지 충청남도와 경기도 지역의 요양시설 종사자들에게 설문조사를 시행하였다. 총 220부의 설문지 중 206부의 설문지가 수거되었으며 자료는 t-test, One way ANOVA, chi-square test 등을 통해 분석되었다. 연구결과에서 요양시설 종사자의 코로나 19 대응 전·후 직무 스트레스 하부영역 차이를 보면 간호사는 직무요구(t=-3.90, p<.001), 직무불안정(t=-3.30, p=.002)에서 간호조무사는 직무요구(t=-2.45, p=.018), 요양보호사는 직무자율(t=-3.34, p=.001)에서 유의한 차이를 나타냈다. 따라서 코로나 19 시기에 요양시설 종사자의 직종별 직무 스트레스를 효과적으로 해결하기 위해서는 직종에 따른 스트레스 완화 프로그램을 알맞게 제공해야 한다.

노인요양시설 거주자의 장기요양등급에 따른 요양서비스 및 자원이용량 분석 (Relationship between Resource Utilization and Long-term Care Classification Level for Residents in Nursing Homes)

  • Purpose: This study was conducted to examine whether the level of classification for long-term care service under longterm care insurance reflects resource utilization level for residents in nursing homes. Methods: From 2 long-term care facilities, the researchers selected 95 participants and identified description and time of care services provided by nurses, certified caregivers, physical therapists and social workers during a 24-hr-period. Results: Resource utilization level was: 281.04 for level 1, 301.05 for level 2 and 270.87 for level 3. Resource utilization was not correlated with level. Differences in resource utilization within the same level were similar with the coefficient of variance, 22.7-27.1%. Physical function was the most influential factor on long-term care scores (r=.88, p<.001). The level for long-term care service did not reflect differences in resource utilization level of residents on long-term care insurance. Conclusion: The results of this study indicate that present grading for long-term care service needs to be reconsidered. Further study is needed to adjust the long-term care classification system to reflect the level of resource utilization for care recipients on the long-term care insurance.

노인요양시설과 노인요양공동생활가정 요양보호사의 응급상황 경험 및 응급지식, 응급상황대처능력 비교 (Comparison of Emergency Experience and First Aid Knowledge, Emergency Coping Ability of Elderly Care Facilities and Group Home Caregivers)

  • 본 연구는 노인요양시설과 노인요양공동생활가정 요양보호사의 업무특성에 맞는 응급상황대처능력을 강화시키기 위한 응급교육프로그램을 개발하는데 기초자료로 활용하고자 실시한 조사연구이다. 자료수집은 S 및 G지역 노인요양시설과 노인요양공동생활가정을 대상으로 2020년 7.10~8.15 실시하였으며, 최종 236부를 분석에 사용하였다. 자료는 χ2 검정, t-test, ANOVA, Pearson's correlation coefficient로 분석하였다. 연구결과 응급상황 경험은 시설군 68.9%, 공생군 50.7%로 두군간에 차이가 있고(χ2=8.42, p=.004), 응급처치 담당자는 시설군 간호(조무)사 55.3%, 공생군 시설장 42.7%였으며, 두군간에 차이를 보였고(χ2=27.84, p<.001), 응급처치 경험은 시설군 56.5%, 공생군 68.0%, 응급처치 항목은 시설군 해열처치, 복약도움, 하임리히법, 공생군 119 및 보호자 연락, 해열처치, 하임리히법순으로 실시하였다. 응급처치 지식은 시설군 11.60±2.09점, 공생군 9.08±2.28점으로 집단간 차이가 있었고(t=8.39, p<.001), 응급상황대처능력은 시설군 52.94±5.27점, 공생군 47.33±4.39점으로 두집단 간 차이를 보였다(t=8.00, p<.001). 또 시설군에서 응급상황 경험과 응급상황대처능력 간에 양의 상관관계가 있었다. 본 연구결과 시설과 공생 요양보호사가 경험한 응급상황 경험 및 응급지식, 응급상황대처능력에 유의한 차이가 있음이 확인되었으므로 규모별 기관특성에 맞는 응급교육을 개발하여 적용해보는 것이 중요하다고 생각된다.

노인요양시설의 간호인력과 비간호인력간 상처관리에 대한 병원중심 가정간호 요구도 비교 (Wound Care Needs of Hospital-based Home Care Nursing at Elderly Care Facilities)

  • Background: The purpose of this study is to investigate the needs for wound care services in the elderly care institutions by analyzing hospital-based home care nursing in select facilities in Korea. Methods: A total of 92 staffs at the elderly care institutions, located throughout the country, completed self-report questionnaires between February 1 and November 31, 2009. SPSS ver. 17.0 was used for data analysis, regarding frequency and chi square. The 5% level of significance was the critical level for acceptance of the study's hypotheses. Results: Results were as follows; group A and group B were rated as 'high' and 'very high', respectively, in care of simple pressure ulcer, care of complicated pressure ulcer, stitch out, care of simple surgical wound, ostomy care, complicated ostomy care, simple dressing and complicated dressing. Conclusion: Political decisions intended to promote visits by hospital- based home care nurses to the elderly care institutions would be a prudent course.

장기요양시설 노인에 대한 간호서비스 제공시간 분석 (Care Time of Elderly in Long-Term Care Facilities)

  • Purpose: This study was to define the care time of elderly in long-term care facilities and to compare and analysis the care time by staff and facility types. Method: The data were collected from 530 elderly over sixty, residing in two long-term care hospitals for dementia, three long-term care hospitals for the elderly and two skilled nursing facilities. Care time for individual residents was measured the total time spent by nurses, aides. Result: The average care time measured by nursing staff was 158.6 minutes a day. The average care time for one resident by nurse was 40.4 minutes, and by aides, 118.2 minutes. The difference of the care time showed statistical significance between long-term care facility types(p<0.001) : average care time for dementia hospital(199.1min) was twice as long as that of skilled nursing facility(94.1min). Conclusion: The results of the study showed that the care time differentiates care time provided to elderly in long-term care facilities in Korea. The study suggests the need to emphasize the importance of standardization of level of staff and service programs by the long-term facilities.

노인요양시설 관리자의 역량 및 행동지표 개발 (Development of Competency and Behavioral Indicator of an Administrator in Long-term Care Facility)

  • Purpose: The purpose of this study is to develop the competencies of the administrator and the behavioral indicators, and with those as the foundation it is to measure the importance and the performance. Method: On the basis of literature review, the researcher made a preliminary competency and the behavioral indicator for the administrator. Developed competency and behavioral indicator went through the content validity using CVI. Finally, 20 different competencies and 87 behavioral indicators of the administrators in long-term care facility helped measuring the importance of the present administrator's competency and the performance. Results: The competencies are Achievement Orientation, Initiative, Information Seeking, Interpersonal Understanding, Customer Service Orientation, Marketing, Influence, Organizational Awareness, Relationship Building, Staff Development, Assertiveness, Teamwork and Cooperation, Leadership, Analytical Thinking, Conceptual Thinking, Managerial Expertise, Self Control, Self Confidence, Flexibility and Organizational Commitment. For the contents, 87 behavioral indicators are included. The importance and the performance were confirmed by Importance Performance Analysis. The importance of each behavioral indicator in each field and the performance were confirmed with the mean and the standard deviation. Conclusion: The competencies and behavioral indicators of an administrator which were developed by this study are considered that they can be used as the effective guides in the practical field and can also be used as basic materials for developing educational program.
