Issues in clinical evaluation tools to measure and evaluate nursing student clinical performance (간호학생의 임상실습평가와 측정에 관한 도구의 신뢰도와 타당도)

  • 이 연구는 현존해 있는 다양한 임상평가도구의 신뢰도와 타당도에 관한 분석 서술 연구이다. Rines(1974)에 의하면 평가는 학습경험을 통하여 목적을 달성하는 지의 여부를 측정하는 것이라고 하였고 평가의 목적은 교육증진을 도모하며, 목표를 설정하고, 학생의 학습진행과정을 사정하여 학생들을 위한 프로그램을 사정, 개발시키는데 있다고 피력 하였다. 간호교육에는 실무교육이 많은 비중을 차지하고 있다. 또한 많은 간호교육자들은 실습평가에 있어 많은 어려움을 경첩하고 있다. 이러한 난해한 실습평가도구를 개발하기 위하여 선행된 우선순위로써 현존해 있는 다양한 평가도구의 신뢰도와 타당도를 평가하는 것이 의의 있는 일이라 사료된다. 이 논문은 실습평가도구의 신뢰도와 타당도에 관심을 가진 임상지도교수나 간호교육자들을 대상으로 한 서술적 연구이다. 우선 간호교육평가에 있어서의 문제점을 분석하고 평가도구에 대한 전문가(Authority)의 의견을 피력한 다음 저자의 의견을 서술하였다. 결론적으로 현재까지 개발된 완전한 임상실습평가의 방법은 없다. 그러므로 현존해 있는 평가방법을 서술평가하고 문제점을 지적하므로써 질적인 평가도구개발에 도움을 줄 수 있다. 앞으로 간호교육자들이 각 문제점들을 보완하여 교육목적을 달성시킬 수 있는 효율적인 평가도구를 개발하는 것이 급선무이다.

Establishing Standards for Nursing Practice (간호업무를 위한 기준확립)

  • 지난 이태 동안, 미국간호협회에서는 5개 분과위원회의 간호원들이 모여 ''간호란 무엇인가?''라는 문제에서 그 기준을 모색하기 위해 고심해 왔다. 공중 보건간호나 노인병간호, 모자보건간호, 일반 내$\cdot$외과간호, 정신과 간호등 다섯 분과위원회는 각분야의 업무평가 기준을 확립하기 위하여 수차 회으를 통하여 각분야의 간호업무 본질을 조사하였다. 이들의 조사결과는 ''간호란 무엇인가''를 확실히 하여 다른 사람을 이해시키고 업무수행 능력의 측정의 기준이 될 것으로 기대한다. 최근에 다섯개분과의 간호원들은 이 결과를 보고하기 위해서 저널지에서 토론하였는데 이 토론 가운데는 각 분과 위원회의 유사점을 비교했고 상반되는 점들을 서로 알게 되었다. 간호원들의 능력을 측정하는 수단 이외에는 어떤 분과도 간호업무의 실행능력 측정의 방법에 대해서 아무 전례도 가지고 있지 못했다. 각 분과회의는 장시간에 걸쳐 맡겨진 분야를 연구하여 초고(草稿)를 만들고 다시 수정하고 난 후에 실무간호원 및 고문으로부터 여러가지 의견을 받아 들였다. 이렇게 하여 간호기준은 여러가지 단계를 거쳐 완성되어 가고 있으며 각 부분이 끝나는 대로 발행될 것이다. 제일 먼저 ''노인병간호'' ''정신과 간호''의 업무기준이 곧 발행될 것이다. 모자보건위원회는 마지막 평가를 받기 위해 각 주에 있는 위원회에 그 간호기준을 보낼 계획이다. 일반 내$\cdot$외과간호 위원회에서는 이미 만들어 놓은 간호 기준을 50명의 전문 간호원들로 페널 디스커?을 가질 계획이며, 공중보건간호위원회는 9월에 열릴 회의에서 마지막 단계로 들어 갈 것으로 보고 있다.

Effects of Self Concept on Clinical Competency in Hospital Nurses (병원 간호사의 자아개념이 실무능력에 미치는 영향)

  • Purpose: This study was designed to identify the self concept and clinical competency of nurses in hospitals and to examine the relationship between two concepts. Methods: A questionnaire was used for data collection which was done from July 1 to July 30 in 2005. The respondents were 267 staff nurses in a university hospital and general hospitals in two cities. The instruments were Son's(2002) self concept scale and Lee's(2002) clinical competency scale. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression using SPSS-WIN. Results: The results of this study were summarized as follows: First, the total mean score of self concept was 2.56, and clinical competency was 2.59 on a 4 point scale. Second, the self concept and clinical competency were significantly correlated. Third, the results of multiple regression showed that self concept explained 45.2% of clinical competency. Conclusion: Self concept has a strong influence on clinical competency. Therefore, individualized nursing management strategies and education programs must be developed in order to increase a nurses self concept.

Nurses' Experience of Practice in Nursing Homes (노인요양시설 간호사의 실무 경험)

  • Purpose: This article aimed to identify nurses' experience of practice in nursing homes. Method: Using phenomenological approach for qualitative research, the participants were 8 nurses who were working at nursing homse in C city. The data was analyzed by using phenomenological methodology by Colaizzi. Results: Five theme clusters were derived from the data. They were: burden (independent decision making in clinical situations, tension of as a multifunctional member); lowered self-esteem (exhaustion, relative self-evaluation is low, comparing with others); conflict (negative image of elderly, communication break with elderly family, monitoring of nursing home staff); worth (slight improvement of elderly health state, trust and recognition, intimacy); and self-reflection (need for knowledge extension, effort towards enhancing virtue, perception of importance in their health management). Conclusion: The results showed that nursing home nurses experienced negative aspects of various nursing practice, suggesting the ways to solve their negative experience.

Feminism in Nursing Science in Korea (한국 간호학문에서의 페미니즘)

  • Purpose: Although feminism has been actively discussed and applied to nursing in Western societies since the 1980s, it is little known among Korean scholars as well as Korean nurses. This article explores the use of feminist perspectives in nursing science in other developed countries and suggests how feminism could be applied to nursing science in Korea. Method: The literature related to nursing and feminism were reviewed in terms of nursing practice, education, and research. Result: This article describes what feminism is and how feminism and nursing have evolved historically over time in other countries, especially in Western societies. In addition, it discusses how it can be applied to nursing practice, education, and research in Korea. Conclusion: Accepting feminist perspective in Korean nursing could benefit in empowering nurses by valuing nursing, by raising self-esteem of nurses, and by raising the consciousness of socio-political realities. Eventually it could benefit in changing and developing nursing science in Korea.

Core nursing practice and educational requirements according to nursing unit and clinical career in tertiary hospital (국내 상급종합병원의 간호단위 및 임상경력별 핵심간호실무와 교육요구도)

  • Purpose: The purpose of this study was to provide fundamental data to develop an educational program for the development of nurses' core competences, and to improve the curriculum in nursing education institutions. Methods: This study, based on a descriptive survey, was conducted with 1,019 nurses working in a tertiary hospital in Seoul. Among 47 core nursing practice items, core nursing practices as perceived by the participants and relevant educational requirements were measured on a four-point scale. The collected data were analyzed using descriptive statistics for the mean and standard deviation, one way ANOVA, and Duncan test as post-hoc test. Results: Regarding the core nursing practice perceived as by the nurses, there were statistically significant differences in 43 out of 47 items according to nursing unit, and in 15 items according to clinical career. Regarding the educational requirements for core nursing practice, there were also statistically significant difference for the same items as perception differences according to nursing unit and clinical career. Conclusion: The results of this study suggest further study on the development of educational programs to advance the clinical expertise of nurses and for the development of curriculum reflecting the actual conditions and requirements in clinical nursing practice fields.

Development of Nursing Practice Guidelines for Non-humidified Low Flow Oxygen Therapy by Nasal Cannula (비강 캐뉼라를 이용한 저유량 산소 요법에서 비가습 적용의 간호실무 가이드라인 개발)

  • Purpose: The purpose of this study was to provide a basis for non-humidified low flow oxygen by nasal cannula and to provide a guide for consistent care in nursing practice. Methods: A methodological study on the development of guidelines with experts' opinions on collected items, framing PICO questions, evaluating and synthesizing texts which were searched with the key words (low flow oxygen, nasal cannula, humidification of oxygen, guideline) from web search engines. Results: Of the 45 researched texts on the web, 9 texts relevant to the theme were synthesized and evaluated. All patients with humidified or non-humidified oxygen therapy reported that they had no discomfort. Conclusion: The results indicate that there are no tangible grounds for patients' perceived differences between the humidified and non-humidified oxygen under 4L/min supplied by nasal cannula. with oxygen. Therefore, non-humidification oxygen therapy is strongly advised when suppling under 4L/min oxygen by nasal cannula (recommended grade A).

Development of the Nursing Practice Capacity Scale for Evaluating Achievement of Nursing Education Objectives (일 대학의 간호교육목표 달성도 평가를 위한 간호실무능력 평가도구 개발)

  • Purpose: The study was conducted to develop the Nursing Practice Capacity scale for evaluating achievement of education objectives among students in nursing school. Methods: This was a methodological study. The scale development process included generation of initial items, verification of content validity, selections of secondary items, preliminary study, and extraction of final items. The participants were five graduates 1 to 4 years after graduation (for the preliminary study) and 125 graduates 1 to 4 years after graduation from the nursing school at A University, South Korea. Item analysis, criterion validity, and internal consistency were used to analyze the data. Results: Twenty-six items were selected for the final scale. The scores on the scale were significantly correlated with the Nursing Competency Scale(r=.62, p<.001) and Nursing Performance Scale(r=.63, p<.001). Chronbach's alpha coefficient for the 26 items was .87. Conclusion: The Nursing Practice Capacity scale for evaluating achievement of nursing education objectives has good validity and reliability. The measurement scale can be useful for evaluating the nursing practice capacity of nurses as well as measuring the extent to which graduates have achieved the established education objectives in the education fields.

Analysis of RN-BSN Students' Clinical Nursing Competency (RN-BSN 과정 학생의 간호실무 수행능력 분석)

  • Purpose: The purpose of this study was to investigate RN-BSN students' clinical nursing competency in order to establish baseline data for developing nursing competency based clinical education for RN-BSN students. Method: A survey of 1,453 RN-BSN students from 21 nursing schools was conducted using a self administered questionnaire. Result: The mean score of the clinical nursing competency was 2.93. The scores for competency were shown as 2.91 for nursing management, 2.94 for developing professionalism & legal implementation, 2.95 for critical thinking, 2.96 for teaching & leadership, and data collection, basic nursing care, and communication were above 3.00. The items perceived as insufficient competency were physical examination and observation & monitoring in data collection, cardiopulmonary resuscitation, psycho-social care, spiritual care, hospice in basic nursing care, application of knowledge and theory, formulating nursing diagnosis, nursing care planning in critical thinking, education material development, leadership, delegation in teaching and leadership, analysis of organization, planning, infection control, role & job description, evaluation of nursing activities in nursing management, quality improvement, and research in developing professionalism and legal implementation. Conclusion: This study will contribute to developing a nursing competency based on clinical education for RN-BSN students who have various education needs and clinical backgrounds.

The Effect of Hand Reflexology on Pain, Skin Temperature and Nursing Practice (입원 환자에서 손 반사요법의 통증, 피부체온 및 간호실무 효과)

  • Purpose: This study is aimed to examine the effectiveness on pain, feeling and nursing practice of hand reflexology applied to in-patient in the clinical setting. Method: The pre/post test methods using VAS for pain and feeling, physiologic measuring, and questionnaire of nursing care were conducted. Subjects were selected 45 in-patients from a surgical ward in hospitals during Nov.-Dec. 2005. 5 minute hand reflexology therapy was applied to both hands of participant by two research assistants at the same time. Data were analyzed based on SAS program using frequency, $x^2$-test, paired t-test, ANOVA and Pearson's correlation coefficients. Result: 1. After receiving hand reflexology therapy, the subjects showed significant pain relief(t=-4.94, p=.0001), improvement in feeling(t=19.44, p=0001) and an increase in skin temperature(t=3.54, p=.001). 2. The applied skills that the participants preferred were press-rotate(80.0%), press-walk(35.56%) and press-rolling(31.11%). The effectiveness of nursing practice scored $3.99{\pm}1.97$ out of 5. Nurse-Patient Relationship and effective response of nursing intervention ranked the highest 4.31. Conclusion: Hand reflexology was considered as an effective nursing intervention in the clinical settings, but in order to evaluate re-verifies and reliances for this effects, it is necessary that comparative studies should be conducted.
