Common Scope of Practice for Advanced Practice Nurses in Korea Derived from Expert Agreement (전문가 합의로 도출된 한국 전문간호사의 공통 업무범위)

  • Purpose : This study aimed to propose a common scope of practice (SOP) for 13 specialties of Advanced Practice Nurses (APNs) in Korea. Methods : The first draft of a common SOP was extracted from domestic and international laws with a literature review by 17 experts from the Korean Association of Advanced Practice Nurses (KAAPN). Then, the common SOP was finalized after comparing the activities of APNs in clinical settings. Results : A total of 70 duties were identified and six categories were suggested for the common SOP. The SOP proposed by the KAAPN featured the following: 1) identification of and discrimination between health problems; 2) prescription and implementation of diagnostic tests; 3) treatment of injuries and diseases while implementing measures to prevent exacerbation; 4) prescription of medicinal products in line with 1) to 3); 5) referral and consultation; and 6) education and counseling. It was then confirmed that the proposed six categories in the common SOP reflected all the duties performed by APNs in clinical practice, including all 40 activities. Conclusion : The results of this study can be used as evidence for the legalization of a common SOP for APNs. Given the increasing multidisciplinary team approach adopted in Korean hospitals, it may be desirable to establish a broader SOP to reflect the diverse duties of APNs.

A Study on the Experience of Nursing Student's Clinical Education in School Practice: Focused on Psychiatric Nursing Practice (간호대학생의 임상실습 교과의 교내실습 경험연구: 정신간호학 실습을 중심으로)

  • This study is a qualitative study to explore the experiences of nursing students' psychological nursing practice in-school. The subjects of this study were 62 students who were the 3rd grade nursing students at a University in Gangwon-do. The data were collected from August 3rd to August 20th, 2020. For data analysis, Colaizzi's qualitative analysis method was applied. Six categories were derived from data analysis: 1) Lingering affection for clinical practice in field. 2) Achievement through learning 3) Fear of nurse's role in future 4) Burden for many tasks 5) Feeling free of stress from clinical practice in field 6) Importance of cooperation with other departments. The positive aspects of in-school practice education, which from nursing students' evaluation, were immediate feedback, detailed explanations, and indirect experiences. Based on this study result, it is recommended to develop in-school nursing practice education program for gap-reducing with clinical practice. It would be consisted of various contents: simulation, online/non-face-to-face practice, face-to-face practice for indirect experience. In addition, these multi-aspect effort is needed more in psychological nursing practice education to reduce the gap with clinical practice such as therapeutic communication and hallucination interventions.

A Comparative Study on the Perceptions of Nursing Students and Clinical Instructors on the Importance of Learning Contents of Nursing Management and Practical Experience (간호관리학실습 교과내용에 대한 중요도와 실습 경험에 대한 간호대학생과 임상현장지도자의 인식 비교 연구)

  • This study was conducted to provide basic data for improving nursing management practice by comparing and analyzing the perceptions of nursing students and clinical instructors about the importance of the learning contents of nursing management and their practical experience. The final 343 copies were analyzed using the Mann-Whitney test and cross-analysis, targeting 4th-grade students and clinical instructors at three universities. Both nursing students and clinical instructors recognized 'communication', 'interpersonal relationship', 'responsibility', 'trust', and 'morality' as important factors in the evaluation items of practice. Among the top 10 most importantly recognized items among nursing-management practice learning content, six items ('infection control', 'nursing record management', 'patient nursing management', 'drug management', 'patient safety', and 'nursing malpractice') were matched in the two groups. Moreover, clinical instructors recognized that interpersonal relationships and relationship ethics were important, while nursing students chose legal responsibility. As a result of the practical experience analysis between the two groups, nursing students had high experience of indirect participation, but the clinical instructors had a high experience of direct instruction. A plan is needed to reduce the difference by establishing an organic partnership relationship between the university and practice institutions and increase the experience of direct practice of nursing students.

Association between Sleep and Loneliness in the Elderly Living Alone (독거노인의 외로움과 수면의 관계)

  • Objectives : Loneliness and poor sleep quality are common phenomena in old age and are associated with negative physical and mental health. However, little is known regarding the relation between loneliness and sleep quality. The aim of this study was to examine the association of loneliness and sleep quality among the elderly living alone. Methods : This is a cross-sectional study that enrolled 1,090 subjects who are the community-residing elderly living alone. Sociodemographic status, medical condition, cognition, mood disorder, sleep quality and levels of loneliness were collected using a self-administered questionnaire and a specific semi-structured interview conducted by trained nurses. Descriptive statistics were used to analyze data regarding sociodemographic variable and loneliness. Univariate and Multivariate regression analyses were applied to examine the association between loneliness and sleep quality after adjustment for multiple confounding variables. Results : The Pittsburgh Sleep Quality Index (PSQI) mean score of "lonely group" (9.2±4.2) was significantly higher than that of "not lonely group" (7.3±3.7) (student-t test, p<0.001). Loneliness was significantly associated with PSQI (standardized β=0.065, p=0.025), sleep disturbance (standardized β=0.086, p=0.005), use of sleep medication (standardized β=0.065, p=0.034) after adjustment for possible confounding variables including sex and mood disorder. Conclusions : Loneliness was associated with sleep disturbance and this finding implied negative impact of loneliness on sleep quality of older adults. Public health promotion efforts to reduce loneliness may improve sleep quality and mental health in the elderly living alone.

A Case Study on The Operation of On-Campus Practicum for Core Basic Nursing Skills Using a Mobile Based Reflective Log (모바일 기반의 성찰일지를 활용한 핵심기본간호술 교내실습 운영 사례 연구)

  • Basic nursing core skills are the essential skills required of nurses to effectively care for their patients. This study introduces an on-campus practicum using a mobile-based reflective journal, and attempts identify the challenges faced by students when performing core clinical nursing skills. The on-campus practicum was operated based on Kolb's experiential learning cycle. For each class, students used mobile devices to write an online reflective journal. Analyzing contents of the reflective log helped in identifying difficulties experienced in executing core skills, and classifying them in terms of knowledge, skill, and attitude. The level of difficulty, importance, and confidence in the core clinical nursing skills were also assessed. Students were found to be struggling with various aspects of performing core nursing skills, especially in the skill category. Students also showed a lack of confidence in items they perceived as "high" difficulty, such as IV injection and indwelling catheterization. Moreover, over 50% students considered IV injection and vital sign checking as the most important core clinical nursing skills. Our data suggests the necessity to develop various contents and apply instructional strategies to solve the core skills difficulties faced by nursing students, and to continuously generate evidence for the same.

The Relations of Metacognition, Learning Flow and Problem Solving Ability of Online Classes in Nursing Students (일 지역 간호대학생의 온라인 수업에서 메타인지, 학습몰입, 문제해결력 관계)

  • This is a narrative research study conducted on the subject in order to understand the relationship between the meta-cognition, learning flow, and problem solving ability of nursing students in online class, and to find out the factors that influence problem solving ability, 88 students enrolled in 2nd and 3rd grade students who participated in online classes at G city universities. The analysis method was analyzed by means, standard deviation, t-test, one-way ANOVA, Pearson correlation, and stepwise multiple regression using SPSS version WIN 23.0. As a result of the study, the difference between learning flow and problem-solving ability according to general characteristics was significantly different depending on the perception of nurses after learning and the motivation for admission to the department of nursing, and the problem-solving ability was positively correlated with metacognition and learning flow. There was a relationship, and metacognition and learning flow were significant variables that influenced problem solving ability. The conclusion of the study is considered to be the basic data for the development of a systematic program that can increase the learning flow and meta-cognition to increase problem solving ability amid the change from nursing education to online classes, and to change the teaching method.

A Delphi Study for Constructing a Suicide Prevention Protocol for Male Baby Boomers (베이비붐세대 남성의 자살 예방 프로토콜 구성을 위한 델파이 연구)

  • Purpose: The purpose of this study was to construct a protocol for the prevention of suicide in male baby boomers. Methods: In order to construct the protocol, a three-step expert consensus process was carried out. The first step was to compose questions based on various research data. For this purpose, two professors of psychiatric mental health nursing and two advanced practitioner nurse of psychiatric mental health in the composition. The second step was to conduct the first Delphi survey on expert panelists. The survey period was from October 13, 2015 to November 1, 2015. The third stage, the second Delphi investigation was conducted from November 10 to December 4, 2015. Results: As a result of the survey, a total of 7 areas of protocol configuration items were derived. The derived items include the characteristics of male baby boomers, suicidal signs, suicide risk factors, suicide protection factors, suicide measurement tools, related organizations, and mental health promotion programs. Conclusion: This study can be used as a guideline when intervening suicide prevention of male baby boomers in community mental health centers.

A Study on the Stages of Exercise Behaviors in Patients with Rheumatoid Arthritis (류마티스 관절염 환자의 운동행동 변화단계에 관한 연구)

  • This study examined factors affecting the stages of exercise behavior among patients with rheumatoid arthritis. The analysis was based on data collected from 238 patients who enrolled at the medical center of D university. The participants showed 5 stages of exercise behavior: pre-contemplation 23.9%, contemplation 23.0%, preparation 33.2%, action 5.0%, and maintenance 29.0%. There were statistically significant differences in perceived barriers of exercise, exercise self-efficacy, and perceived health status according to the stage of exercise behavior of the subjects. It was found that among the barriers to exercise, physical and cognitive psychological factors significantly influenced the stages of exercise behavior. Also, exercise self-efficacy significantly affected the stages of exercise behavior. Thus, for rheumatoid arthritis patients to practice and maintain proper exercise behavior, the nursing staff should continue to assess and reduce the barriers to exercise. Exercise programs should enhance self-efficacy to achieve long-term exercise behavior.

Relationship between Health Literacy and Self-Management Adherence in Patients with Chronic Obstructive Pulmonary Disease (만성폐쇄성폐질환 환자의 의료정보 문해력, COPD 관련 건강문해력 및 자가관리 이행 정도와의 관계)

  • Purpose: The purpose of this study was to identify the level of health literacy (HL) and self-management adherence and to investigate the relationship between HL and self-management adherence in patients with chronic obstructive pulmonary disease (COPD). Methods: The participants of this study were 165 patients with COPD from a tertiary general hospital located in G city from February 10th to May 10th, 2021. The general HL was measured by the Korean health literacy assessment tool (KHLAT), COPD specific HL was measured by COPD-related HL tool, and COPD self-management adherence was measured by the COPD self-management tool. Results: The mean score of general HL was 60.45±9.42. The mean score of the COPD-related HL was 47.03±8.82. The mean score of the COPD self-management adherence was 87.58±14.47. There were significant differences in the level of COPD-related HL according to age (t=2.43, p=.016), and in the level of general HL (F=10.96, p<.001) and the level of COPD-related HL according to the level of education (F=19.14, p<.001). There were significant correlations between the level of general HL and the level of COPD-related HL (r=.75, p<.001) and the level of COPD-related HL and the level of self-management adherence (r=.219, p=.005). Conclusion: The HL of COPD patients was at the level of mild school 1-2 years in this study. Therefore, nurses who care for COPD patients should understand patients' level of HL and communicate self-management skills with patients at a level appropriate for patients' HL. In addition, it is necessary to develop strategies to easily learn COPD-related information when providing self management skills or training.

Factors affecting the turnover intention of hospital nurses quarantined in cohort due to the outbreak of COVID-19: Focused on a university hospital (코로나19 확진자 발생으로 코호트격리된 병원간호사의 이직의도 영향요인: 일 대학병원을 중심으로)

  • This study is a descriptive survey study conducted to identify the factors affecting the turnover intention of hospital nurses who were cohort-isolated due to the outbreak of COVID-19. The data collected from 191 university hospital nurses were analyzed using the IBM SPSS Statistics 23 program by descriptive statistics, t-test, Pearson's correlation analysis, ANOVA, and multiple regression analysis. Among the factors affecting the turnover intention, the higher the infection control job stress (𝛽=.40, p<.001), the lower the support of the boss among social support at work (𝛽=-.21, p=.004). The lower the resilience (𝛽=-.16, p=.016) the higher the turnover intention. The explanatory power of these variables was 25% (Adj. R2=.25, F=16.99, p<.001). In particular, the infection control job stress of hospital nurses who had the entire hospital under cohort-isolated due to the outbreak of COVID-19 was above average. It is necessary to find a way to reduce infection control job stress, and to reduce the negative aspects of the organization and work with the support of the superior to lower the turnover intention. In addition, it is necessary to seek to reduce turnover intention as a program to strengthen resilience to improve resilience.
