Approaches to Teaching Health Policy : Moving Students into the Political Process (보건정책 교육을 위한 접근법 -간호학생의 정책참여 과정을 중심으로-)

  • 이 연구는 간호사의 보건정책과 정치에 대한 미래지도력을 향상시키기 위한 방법으로써 한국의 4년제 간호학과에서 가르쳐야할 보건정책과목을 상세하게 제시한다. 새로운 보건정책과목 개설을 위한 기초작업으로써, 미국 간호사들의 정치와 정책참여에 대한 사례를 열거하고 어떻게 간호사들이 정책에 참여하고 영향력을 발휘 했는지를 서술한다. 특히 Oregon plan과 지역사회 중심으로 형성된 연합체제와 간호정책 분석가의 역할이 소개된다. 간호사들의 사회 정치 진출의 한 방법으로써 간호교육에서의 보건정책과목의 필요성과 중요성이 언급되며, 미국의 4년제 간호대학 학부과정, 석사과정, 박사과정에서 가르쳐지는 보건정책 과목이 소개된다. 간호정책 전문가로써의 등용문으로 알려 진 Robert Wood Johnson Health Policy Internship과 Fellowship이 소개한다. 미국간호사들의 보건정책 참여방법과 현재 가르쳐지는 보건정책과목의 장점을 토대로 하여 우리의 실정에 맞는 간호학생을 위한 보건정책 과목이 제시되어진다. 보건정책과목의 목적과 내용이 자세히 서술되어지고 역동적인 실습과정이 소개되어진다. 또한 학과목으로 정착되기 위해서 필요한 새로운 전략이 서술되어 진다.

  • 오늘날의 대부분 의료기관이 성취해야 할 가장 중요한 과제는 의료 관리비용의 절감과 의료 서비스품질의 향상이다. 최근 병원의 대형화 및 병원간의 경쟁 과다로 인하여 환자에 대한 서비스는 호전되었지만, 병원 자체 인력에 대한 효율적 배분 및 인력 운용에 있어서의 형평성 문제는 아직까지 해결되지 못한 부분이 많다. 특히 모든 일을 경제 원리로 풀려고 하는 사회적 풍토 및 현대화, 대형화에 따른 병원 재정의 압박은 병원 운용 인력의 $30{\sim}40\%$를 차지하는 간호 인력에 대한 관심을 소홀히 하고 있다. 이에 본 연구에서는 국내 대형 병원을 사례기관으로 선정하여 기존의 간호사 근무 편성 기준을 분석하고, 이를 토대로 간호사들의 요일별, 시간별 선호도를 고려한 근무 편성표를 작성하여 간호사 근무에 형평성을 기하고자 한다. 또한 기존 휴리스틱에 의거한 근무표와 수리계획법을 활용하여 작성한 근무표 간의 비교를 통하여, 간호사 개인이 특정 근무 기간동안 얻게 되는 만족도를 분석하고자 한다.

  • Emergency room nurses whom exposed to more confused environment, violence, and medical disputes feel anxiety, due to increasing of demands and needs of emergency room services. Emergency room nurse anxiety needs to intervention in order to relieve emergency room nurses from anxiety, because it is an important factor that decreases job skill and satisfaction. In this paper, we examine whether to improve the effect of music intervention on emergency room nurse anxiety through appreciation of music. We used vocal music and a SAI(State Anxiety Inventory) tool for measuring the effect of music on emergency room nurse anxiety and physiological responses. Experimental results showed that vocal music intervention reduces emergency room nurse anxiety and physiological responses.

Korean Nurses' Judgments of Child Sexual Abuse Situations: The Use of Vignettes (아동 성폭력 상황에 대한 한국 간호사의 판단)

  • 본 연구의 목적은 아동 성폭력 상황 자체와 성폭력 가해자에 대한 간호사의 태도가 상황 판단에 미치는 영향을 규명하는데 초점을 둔다. 503명의 병원 간호사와 526명의 보건교사를 대상으로 vignette 디자인 설문조사를 실시하였다. 아동성폭력 상황 vignettes은 7가지의 특성 (성폭력 행위, 성폭력 빈도, 피해자의 연령, 피해자의 저항, 가해자의 연령, 가해자와 피해자의 교차성별, 가해자와 피해자와의 관계)을 조합하여 구성하였다. 1/4 factorial design의 결과로서 64개의 vignettes 이 도출되었으며 이 중 16개 vignettes을 무작위로 추출하여 그 vignettes에 나타난 각 성폭력 상황들의 심각정도를 표시하도록 하였다. 성폭력 가해자에 대한 문항들은 1) 가해자가 아동 성폭력을 하는 이유 2) 전형적인 성폭력 가해자3) 아동성폭력 가해자의 처벌방법에 관한 간호사의 태도를 묻는 것이었다. 자료분석은 Logistic Regression을 실시하였다. 연구결과에서 간호사들은 아동 성폭력 상황의 심각도를 판단할 때 주로 성폭력 상황에 의해 영향을 받고 아동 성폭력 가해자에 대한 개인적 태도는 거의 영향을 미치지 않는 것으로 나타났으며, 아동 성폭력 상황 특성 중 피해자의 저항, 성폭력 행위, 성폭력 빈도, 가해자의 연령, 가해자와 피해자의 교차성별 순으로 아동 성폭력상황의 심각도에 영향을 미치는 것으로 나타났다. 결론적으로, 간호사들은 아동 성폭력 상황에 직면하여 사례를 판단할 때 아동 성폭력 문제에 대한 개인의 태도에 영향을 받기보다는 사례 그 자체에 근거를 두고 판단하는 경향을 나타낸다. 본 연구 결과는 법적, 전문가적 의미에서 아동성폭력 신고자로서의 의무와 사례중재에 개입된 의료인으로서의 역할 수행에 긍정적인 면을 시사한다.

Experience of the Role Conflict of Nurses in Long-term Care Hospitals (요양병원 간호사의 역할갈등 경험)

  • Purpose: The purpose of this study was to describe the experience of the role conflict among nurses in long-term care hospitals. Method: Data were collected through in-depth interviews with nine nurses in long-term care hospitals. Colaizzi's phenomenological method was used for data analysis. Result: In this study, five categories and twelve theme clusters were generated. The five categories consisted of 'Heavy workload,' 'Nursing job delegation of unclear regulations,' 'Unprotected doctor's job performance,' 'Non-systematic nursing assistants management' and 'Limit of competency.' Conclusion: These results will contribute to the role establishment of nurse in long-term care hospitals and the qualitative improvement of work by providing the grounds for the strategy development which improve working environment and job satisfaction of nurses in long-term care hospitals apposite to the case of Korea.

A Study on the Nurse's Medical Malpractice Liability (간호사의 의료과오 책임에 관한 연구)

  • Nurses are medical care providers most closely associated with the national health. Their works are subdivided and specialized, and it is such a factor making nurse's role more important, and with the appearance of specialized nurses, they have secured a position as an independent medical care provider. As the domain of nurse's service becomes broader, there are more accidents and disputes related to nurses. However, there are not many studies conducted on such problems, and even when medical disputes take place related to nurses, the court does not make consistent judgments as a matter of fact. Besides, as the ambiguity of nurse's range of service and the lack of nursing workforce work as a factor causing nurse's medical malpractice, more legal discussions and studies are required to seek proper solutions to such problems. Thus, as a plan to clarify legal issues likely to occur due to nurse's medical practice, this study classified nurse's work into medical assistance practice and other jobs based on their own independent judgments, and proposed establishing concrete regulations on the range of their work, while reviewing common problems extracted from precedents related nurse's medical malpractice. Moreover, while examining Japanese precedents related to the Act of Medical Service Personnel, Nurses and Midwives, which is the sole act of nurses in Japan, this study reviewed the necessity of revising the present nurse-related regulations in Medical Service Act, or enacting a sole act of nurses.

Influences of Rotating Shift Work and Quality of Sleep on Depression in Nurses (간호사의 순환 교대근무 및 수면의 질이 우울에 미치는 영향)

  • The aim of this study was to investigate the level of depression among female hospital nurses and the influence of rotating shift work and quality of sleep on depression. A total of 295 nurses in Seoul were recruited and the data were analyzed using descriptive statistics and multiple regression analysis. The number of depressed nurses was 71.2%(CES-D total score ≥ 16) and factors influenced on nurses' depression were quality of sleep, rotating shift work, and work experience in nursing(1-2.9 years, more than 10 years). Based on the result of this study, nurse managers and hospital organization should use appropriate interventions to improve quality of sleep and negative effects of rotating shift work for nurses.

  • This study examined the perceptions of health literacy on inpatient and outpatient patients among nurses currently in the field. The study participants consist of ten nurses working for general hospitals in Seoul and Busan. The data was collected from focus group interviews and all interviews were recorded and transcribed. Data were analyzed by using the content analysis method. Nurses recognized that there was a difference in the patient's health literacy level according to the patient's age, education, income level, underlying disease, and emotional state. This study suggests implementing 1) education programs to increase nurses' awareness of the importance of health literacy, to make accurate assessments of patent level of health literacy and communicate with them accordingly, and 2) support system for patients with poor health literacy.

An Exploratory Study on Anxiety and Depression of Corona Virus 19 among Hospital Nurses (병원 간호사의 코로나 바이러스 감염증(COVID-19) 질병 이환에 대한 불안과 우울에 관한 탐색적 연구)

  • The purpose of this study is to identify anxiety and depression level for corona virus 19 among hospital nurses to the spread of COVID-19. Data were collected from 232 nurses in 5 hospitals from August to September in 2020 using questionnaire. Characteristics relative to variables were measured, and analyzed using SPSS 26.0 program for descriptive statistics, t-test, ANOVA, and pearson's correlation. The depression level differed according to gender. The anxiety about COVID-19 infection showed a significantly positive correlation to depression (r=.26, p<.001). Based on these results, several roles and coping strategies were suggested to help nurses recover and maintain their psychological stability and mental health.

Concept Analysis of Tae-um in Nurses (간호사의 태움 개념분석)

  • This study was conducted to identify the conceptual definition and attributes of nurses' Tae-um based on the conceptual analysis method of Walker and Avant. The attributes of nurses' Tae-um were derived from the organizational culture, inexperienced new nurses that are victims, senior nurses that are perpetrators, alteration of teaching, verbal and physical violence experiences, and psychological and physical symptoms. The antecedents were due to excessive work, high work intensity, job stress, lack of workforce, poor working environment, lack of job ability of new nurses, organizational hierarchy, and power imbalance. Moreover, the consequences were physical and psychological symptoms, decreased efficiency, increased distrust, increased turnover intention, and decreased quality of patient care. Improvements of working conditions and social supports for expanding nursing staff and building a culture in which communication and mutual cooperation are used to solve the antecedents of Tae-um among nurses is necessary. This study suggests the need for more qualitative research on Tae-um in nurses, the development of tools that reflect the attributes of Tae-um in nurses, and the development of a program to improve the culture of Tae-um in nurses. This study is significant in that it provides a rationale for development of an adaptation program for new nurses by suggesting the antecedents of Tae-um and solutions for it.
