In this part the writing group will cover strategies, techniques, and endpoints of atrial fibrillation (AF) ablation. Prior to all, electrical isolation of the pulmonary veins is recommended during all AF ablation procedures. In addition, techniques to be used for ablation of persistent and long-standing persistent AF, adjunctive ablation strategies, nonablative strategies to improve outcomes of AF ablation, and endpoints for ablation of paroxysmal, persistent, and long-standing persistent AF will be reviewed. Currently many technologies and tools are employed for AF ablation procedures. Radiofrequency energy, cryoablation, and other energy sources and tools are in various stages of development and/or clinical investigation. Finally, anticoagulation strategies pre-, during, and postcatheter ablation of AF and technical aspects of ablation to maximize safety are discussed in this section.

본 논문에서는 OR게이트를 적용한 순차전압제어방식의 24V 태양광 단위 모듈 확장형 태양광-ESS 시스템을 나타내고 있다. 저압연계방식으로 용량확대에 따른 문제점을 가지고 있었던 기존 태양광 시스템에 고압의 아날로그 방식의 순차전압 제어 방식을 적용함으로써 고효율, 저가격이 가능하다. 본 논문은 기존 24V 태양광 단위 모듈의 직렬연결 확장형 고압 배터리의 출력전압 384V을 DC-AC 인버터의 입력 전압으로 하여 인버터의 출력 전압과 출력 전력을 AC380[V]@60[Hz]과 10kW로 구성하여 실험을 통해 OR게이트를 적용한 순차전압제어시스템의 동작 특성을 확인하였다.

본 논문은 친환경 전기차용에 적용되는 있는 고밀도 LDC(Low-Voltage DC-DC Converter) 전력변환장치의 PCB구조와 스위칭소자에 따른 PCB의 발열특성을 해석한다. 전력변환장치 PCB사이에 알루미늄 플레이트를 적용하여 다면 방열경로를 통한 PCB방열특성을 비교하고, 또한 기존 Si-FET와 낮은 온 상태 도통저항을 가지는 GaN-FET 반도체디바이스를 적용한 전력변환장치의 PCB 방열특성을 시뮬레이션을 통해 비교 및 검토하였다.

Malignant salivary gland tumors only represent 0.08% of all childhood tumors. Especially, nasopharyngeal mucoepidermoid carcinoma(MEC) in pediatric age is an extremely rare malignancy. We hereby report a case of nasopharyngeal MEC in 5 year-old female patient. The patient underwent the complete removal of the tumor by endonasal endoscopic approach. Adjuvant postoperative radiotherapy was not considered. After 3.5 years of follow-up, there are no sign of recurrence and metastasis. Minor salivary gland tumor must be considered as a differential diagnosis of angiofibroma in nasopharynx in pediatric age. To our knowledge, the case we describe is the third case of nasopharyngeal MEC in pediatric age reported in literature.

Chondroid choristomais a rare tumor like lesion of normal tissue in an unusual location. Oral cavity chondroid choristoma is exceedingly uncommon. This lesion is commonly covered by normal oral mucosa and can develop during a whole lifetime. We experienced a case of 57-year-old man who presented as 6-months history of asymptomatic mass on the dorsal surface of the tongue. We performed surgical excision under local anesthesia, and the pathological diagnosis was chondroid choristoma. After surgery, patient was followed up without any recurrence and discomfort. Therefore, we report this case with a review of literature.

Parathyroid carcinoma is very rare malignant neoplasm, accounting for less than 0.005% of all cancers. Most parathyroid carcinoma is a functioning tumor that causes hyperparathyroidism, leading to hypercalcemia. We report a parathyroid carcinoma case that was suspicious for spontaneous infarction of cancer, leading to resolution of hypercalcemia. A 29-year-old male visited our hospital presenting with right neck swelling and pain. He has been experiencing frequent urolithiasis for four years but laboratory tests showed normal serum calcium level. Right vocal cord paresis was identified with laryngoscopy. Ultrasonography revealed a 3.7 × 3.5 cm mass in the right thyroid containing a focal cystic portion. Computed tomography confirmed the presence of a low-density right thyroid mass. Right thyroid lobectomy was performed and pathological evaluation revealed parathyroid carcinoma with central necrosis. We report this very rare case with a literature review.

본 논문에서는 운동 관련 빅데이터를 적용한 인공지능을 활용하여 개개인에게 알맞은 운동 루틴을 추천해 주는 비대면 방식 PT를 제안한다. 이 정책은 '건강한 사람이 앱을 만나 더 건강해진다.'는 모토를 중심으로, 홈 트레이닝을 하고 싶지만 운동방법을 모르는 사람들로 하여금 자신에게 맞추어진 루틴 구성을 통해 운동 수행능력의 효율성을 높이고, 잘못된 자세로 인한 부상 등을 최소화한다. 또한 이 정책은 기존의 일일이 사용자가 입력해야 했던 시스템들에서 머신러닝을 통한 AI 알고리즘을 통한 추천을 통해 비대면 방식의 수동적인 운동 방식에서 AI가 트레이너 역할을 해주는 방식으로 사용자와 상호작용하고, 정확한 운동 목표를 추천함으로써 운동 지속성과 동기성을 부여한다. 본 논문에서는 프로토타입을 통해 제안하는 AI를 적용한 맞춤 헬스 정책이 기존의 헬스 앱 업계에서 시장성을 보일 수 있다는 가능성에 의의를 둔다.

본 논문에서는 효율적인 환자 이송 및 치료를 위한 응급구조사와 의료기관 간의 의사소통 및 질 관리를 위한 EC&M (Efficient Communication and Management) 시스템을 제안한다. 이는 관련 앱을 효율적으로 사용하여 응급차 배치부터 병원 도착 시 환자 수속 작업 등을 최소화하고, 각 작업의 수행시간을 최소화 한다. 또한 이 시스템의 사용자는 소방청, 의료기관, 119 응급 구조사이고, 이 중 응급구조사에 초점을 두었다. 이는 PRE-KTAS를 소방청과 의료기관에 동시전달하여 전달된 PRE-KTAS를 통한 적절한 의료기관을 배치하고 선정된 응급의학 전문의에게 직접의료지도를 요청할 수 있다. 그리고 응급구조사는 의무기록활동지를 작성하여 의료기관에 전달하여 내용전달의 효율성을 높인다. 본 논문에서는 시뮬레이션을 통하여 제안하는 응급구조사와 의료기관 간의 정보 공유 시스템은 기존에 제안하는 앱을 사용하지 않고 발생했던 많은 문제를 해결 할 수 있을 것이므로 기존의 앱보다 우수할 것으로 기대된다.
