  • Recognizing the importance of early childhood education, the government is preparing measures at the national level, including expanding the scope of free education for infants, linking early childhood education to elementary schools, unifying the system in charge of early childhood education, printing the cost of operating a full-time system for double-income couples, and policies for low-income infants. In Korea's childcare policy, the need for childcare has increased due to the increase of free double-income couples, the increase of nuclear families, and the increase of divorce rates. Despite the overall increase in the child care budget, Korea's child care policy shows many problems in its efficiency due to the burden of childcare facilities and childcare fees for caregivers. After looking into the theoretical contents of the free childcare policy and the corresponding child care support policy, we would like to consider ways to improve the free childcare policy.

  • The current study explored the relative effects of reciprocal/empathic type on messages appealing to help patients with Lou Gehrig's disease and their family. By this, we attempted to examine the applicable scope of the reciprocal type, a way of appealing rationally in the delivery of helping messages. The focus was on the moderation of empathic concern trait, a representative altruistic personality. Conducted as the online experiment, empathic concern trait was measured in the first stage, and the helping messages presented each by reciprocal/empathic type were shown and evaluated in the second stage after about 7 days. The data of 134 people were finally analyzed, excluding insincere participants. As a result, the relative advantage of empathic type was shown, but it was only limited to the subject attitude. In the result of the interaction, the reciprocal type were as much or more influential as the empathic type for the individuals high in empathic concern trait.

  • Objectives: This survey was intended to provide basic data which can be available as a baseline in the set up of dietary guidelines for assuring community-based self-support of the rural elderly, through investigation of the relationship of the various dietary consumptions with their ADL and IADL. Methods: The study subjects, 439 rural residents(male: 196, female:243) aged over 65 in Kumsan Kun and Chongyang Kun, Chungchongnamdo Province were interviewed, in June of 2004, about their sociodemographic characteristics, daily life styles, the variety of dietary consumption, ADL and IADL with the following major findings: Results: In terms of the scores' distribution to show variety of food consumption among all subjects, 68.3% got 1~3 points, 23.2% 4~6 points, and 8.4% 7~10 points with a decreasing proportion of subjects in higher points. In terms of their functional status, normal-range groups showed 93.2% of ADL and 72.9% of IADL whereas, impaired ADL group 6.8% of ADL and 27.1% of IADL, respectively. Concerning the relation of ADL and IADL with the variety of their consumed food, the greater scores for food variety was associated with the significantly higher proportion of normal ADL group and the lower proportion of impaired ADL group. Multiple logistic regression analysis with ADL and IADL as dependent variables, and food variety scores as explanatory variables, the relative risk of impaired-ADL group was 0.84 in the food variety group of 4~6 points, 0.63 in 7~10 points with no statistical significance. The relative risk of impaired- IADL group was 0.52(p<0.01) in the food variety group of 4~6 points, 0.41(p<0.05) in 7~10 points with statistical significance. Conclusions: These study results suggest that the lower dietary variety, the lower functional capacity of daily living, and the variety of dietary is associated with the functional capacity of daily living in rural elderly.

  • Purpose: In terminal cancer patients, pain control with narcotic analgesics and supportive care by hospice are very useful treatment modality. However, many patients and their caregivers are poorly compliant in using narcotic analgesics for fear of addiction and tolerance. And also many patients and family caregivers are reluctant to accept hospice, presuming that hospice means patient's condition is no longer reversible and progressively deteriorating. The purpose of this study was to evaluate and analyze the perception of using narcotic analgesics and hospice by family caregivers of terminal cancer patients who play a critical role in health care in Korean culture. Methods: A total of 54 terminal ranter patient's family caregivers participated in this study. Questionnaire consisted of 15 questions about narcotic analgesics and hospice. Results: The study revealed following results. 1) family caregivers who are not aware of hospice are more than half (56.7%). 2) 81.8% of family caregivers agreed that hospice care is beneficial to terminal cancer patients. 3) 85.1% of family caregivers were under financial burden. 4) 83.2% of patient complained pain in 24 hours. 3) while 88.5% of family caregivers believed that narcotic analgesics can control pair, 79.1% and 79.6% of them also believed that use of narcotic analgesics would result in addiction and tolerance, respectively. Conclusion: There still exist barriers to family caregivers in using narcotic analgesics for pain control. And also, terminal cancer patient's family caregivers have poor information about hospice. Therefore, educational intervention about narcotic analgesics by pharmacist and doctors are needed for proper pain control for terminal ranter patients. In addition, more precise information about hospice care should be provided for terminal cancer patients and their family caregivers.

  • Purpose: This study was to identify the role of general hospice palliative nurse through job analysis (duties, task, and task elements). Methods: The sample consisted of 136 nurses or professors who were performing duties related to hospice care areas in Korea. A survey method was used, and the questionnaire included frequencies, criticality, and difficulties of task elements in job description by the DACUM method. Descriptive statistics were performed by using SPSS WIN 17.0. Results: The job description of general hospice palliative nurse was identified 8 duties, 36 tasks, and 137 task elements. As for the 8 duties, the average scores of frequency, criticality, and difficulty were 2.94, 3.66, and 2.80, respectively. The role of ‘pain assessment’ was the most important task element among frequency and criticality. The lowest score at the frequency and criticality were ‘manage public finance’ and ‘collect datum through diagnostic test & lab', respectively. Furthermore, the role of 'identify spiritual needs of patients and family' was the most difficult task, whereas the role of 'manage documents and information' was the least. Conclusion: In this study, we could recognize the reality of general hospice palliative nurse's performances. For general hospice palliative nurse, therefore, concrete practice guide lines of psychosocial and spiritual care, communication skills, and bereavement care with qualifying system are critically needed.

  • Introduction: There has been an increasing interest in the relationship between sleep and suicidality. In addition, suicidal patients habitually report their sleep problems. Although sleep-related complaints and electroencephalographic changes are generally encountered in psychiatric disorders, sleep complaints such as insomnia, hypersomnia and nightmares are more common in suicidal patients. In current study, we aimed at investigating the relationship between self-reported sleep duration and suicidality in general population. Methods: One thousand general population (male:female=500:500, mean age=$39.6{\pm}11.6$ years, ranged age=20-77 years) completed Center for Epidemiologic Study-Depression (CES-D), Beck Suicide Intent scale (BSI), Spielberger State-Trait Anger Expression Inventory (STAXI), Barratt Impulsiveness Scale (BIS), Morningness-Eveningness Scale (MES) and brief questionnaire of sleep habits. Results: After controlling for age and sex, score of BSI was correlated positively with the score of CES-D, STAXI and BIS on partial correlation analysis ($r_p$=0.251; p<0.001, $r_p$=0.352; p<0.001, and $r_p$=0.175; p<0.001, respectively). In addition, score of BSI was inversely correlated with the score of MES (rp=-0.066; p=0.037). However, score of BSI showed no significant correlation with sleep duration. However, regression analysis revealed that short (<6 hrs) or long (>10 hrs) sleep duration, the family history of psychiatric illness, the score of CES-D, and the score of STAXI predicted higher score of BSI significantly in total subjects (F=17.837, adjusted $R^2$=0.166; p=0.003, p=0.003, p<0.001, and p=0.003, respectively). This model was explained better in depressed subjects with 16 or higher score of CES-D (F=9.920, adjusted $R^2$=0.298). Conclusion: Current result suggested that not only short sleep duration (<6 hrs) but also long sleep duration (>10 hrs) might be related to suicidality.
