Evaluation of Respiration Reproducibility of Chest General X-ray Examination using Self-made Respiratory Synchronization Device (자체 제작한 호흡 동기화 장치를 통한 흉부 일반촬영 검사의 호흡 재현성 평가)

  • The purpose of this study was to develop a respiratory synchronization device for X-ray (X-RSD) to increase the reproducibility of inspiration when examining the Chest X-ray of a patient who difficulty in breathing coordination. The X-RSD was self-made using an air pressure sensor and air was injected by connecting a ventilator to the mannequin for CPR. At this time, the amount of injected air was quantified using the SkillReporting device. After placing the X-RSD on the chest of the mannequin, the amount of air was tested in 6 steps from 200 to 700 cc by 100 cc increased. For the accuracy evaluation, the sensitivity of X-RSD was measured by repeating a total of 80 measurements, and the sensitivity was 100%, and very precise results were obtained. After that, the images examined while viewing the X-RSD of the chest lateral examination and the images obtained by the blind examination were compared and evaluated. The lung volume of X-RSD was larger than that of the blind test, and the deviation was smaller. Overall, the use of X-RSD can help with chest X-ray examination of patients who have difficulty in cooperating, and it is thought that it will be possible to contribute to the reduction of exposure dose by reducing the repeat rate of general X-ray examinations.

  • Paragonimiasis was one of the most common causes of cavitary lung lesions until the 1960s, but now it has become a very rare disease in Korea. A 16-year-old boy presented with hemoptysis and several days of cough. His plain chest radiograph showed nodular opacity with a cavity in the left upper lung region. Although his symptoms disappeared after taking antituberculous drugs, his plain chest radiograph and computed tomography images after completion of therapy showed a new nodule with a cavity in the left lung field. Through video-assisted thoracoscopic wedge resection, Paragonimus eggs were found in the lung tissues.

  • The Study studied the Inspection Method of Images to obtain a sharp image of the Costume among the bones Composed of Thoracic Using the Thoracic Cage Rando Phantom. At 80 cm of the phantom Distance at the X-ray tube focus, the position of the Phantom was Examined by Changing the Rt and Lt Posterior Oblique(LAO) and Rt and Lt Posterior Postero Oblique positions by 20°, 25° and 30°. The acquired images were Subjectively Evaluated by the Radiographer, and the Evaluation data were analyzed as SPSS ver. 3.0. The signal-to-noise ratio (SNR) was Calculated using the ImageJ Program. As a result, the Cronbach Alpha value was Significantly higher at 0.789. The results of the Signal-to-Noise Ratio (SNR) were high at 20° to 6.038 in the right posterior Transcription Direction at the time of Examination and 7.860, in the Supine Position, for images of Sternum bones. In conclusion, it is Believed that the patient position can be obtained from the Right Anterior Oblique(RAO) Position 20° if the X-ray technique is used to obtain the Sternum's advanced image, and the Left Anterior Oblique(LAO) Position 25° when filming in the Rght lying position.

  • Most of the spatial scattered dose caused by the scattered rays generated by the collision between the object and X-rays is relatively easily absorbed by the human body as electromagnetic waves in the low energy region, thereby increasing the degree of radiation exposure. Such spatial scattering dose is also used as an indicator of the degree of radiation exposure of radiation workers and patients, and there is a need for a method to reduce exposure by reducing the spatial scattered dose that occurs indirectly. Therefore, in this study, a lead-free radiation shielding sheet was proposed as a way to reduce the spatial scattering dose, and a Monte Carlo (MC) simulation was performed based on a chest X-ray examination. The absorbed dose was calculated and the measured value and the shielding rate were compared and evaluated.

  • Traumatic diaphragmatic rupture is uncommon, but requires a prompt diagnosis and repair. Diaphragmatic injury is most commonly associated with automobile accidents. The diagnosis is difficult and may be delayed because there are no specific symptoms, signs, or radiographic studies that are pathognomic for diaphragmatic injury. The most important factor in the diagnosis is a high suspicion and the use of proper diagnostic studies. We report a case involving the delayed presentation of diaphragmatic rupture in a 54 year old man, requiring surgical repair 12 days following multiple blunt trauma. It should be noted that early recognition for diaphragmatic injury is important in patients with multiple trauma to avoid the potential fatal complications.

  • Hemangiopericytoma of the lung is a very rare malignant tumor despite it's high vasculities of the lung, because, this tumor arises from the pericytes enveloping capillaries. A pulmonary hemangiopericytoma was diagnosed in a 63-year-old female who had complained of cough, sputum, and intermittent chest pain for about 8 years. She was admitted to our hospital, because of progressive severe dyspnea on exertion and orthopnea for about 3 months. She was taken explothoracotomy and left pneumonectomy including evacuation of lots of blood clots due to ruptured large tumor and ineffective drainage with closed thoracic tube. She was discharged at post-operative 7 days under very good condition. She has been treated with radiotherapy&chemotherapy since then.

Formaldehyde Exposure in the Plywood Manufacturing Factory (목재 가공업소에서의 Formaldehyde폭로)

  • In the plywood manufacturing factory established in 1979, female 3 workers who had exposed to adhesives containing formaldehyde for more than 48 months of duration suffered from eye, nasal, oral, throat and skin irritation and dyspnea as chief complaints. The actual level of the exposure to formaldehyde were not estimated when the exposed workers started to have above symptoms. The environmental monitoring of workplace was measured on April 25, 1990, and the concentration of formaldehyde revealed 0.2 ppm, however the exhaustive ventilatory system was already installed at that time. Twenty six subjects from entire factory were examined by questionnaire, physical examination and spirometry, etc. on August 22, 1990. Significant difference was observed in symptoms and signs of nasal, oral and throat irritation between the exposed group with longer duration and that with short duration. When the exposed group with longer duration was compared to the group of nonexposed, symptoms and signs of nasal, oral, throat and skin irritaion, chest tightness and dullness were significantly increased in the former group. The results of the spirometric test showed that the forced vital capacity and the forced expiratory volume at 1 second were decreased among the exposed group with longer duration but not significant.

The Clinical Features of Pertussis in Infancy (영아 백일해의 임상 양상)

  • Purpose : Pertussis was very common in the past, but reported cases have dramatically decreased. The improvement of vaccination programs and unreadiness of laboratory confirmation seems to have developed this situation. This study investigated the frequency of pertussis among infants with a paroxysmal cough and compared the clinical characteristics between infants with and without pertussis. Methods : Between June and November 2006, 27 infants admitted to the hospital that were 15-90 days old with a history of a cough for more than seven days were enrolled. The cough was described as: paroxysmal, whooping, and post-tussive vomiting. PCR and cultures for Bordetella pertussis with nasopharyngeal aspirates were obtained. The patients were divided into two groups: (1) the pertussis group that had positive results by PCR or culture; (2) the control group that had negative results by PCR and culture. Clinical and laboratory characteristics were compared between the two groups. Results : Among the 27 cases, five (18.5%) were finally diagnosed with pertussis. Only one out of the five pertussis cases was initially diagnosed with a pertussis-like syndrome on admission. Compared to the group without pertussis, the pertussis group had a significantly higher frequency of: no fever (P =0.043), a paroxysmal cough (P =0.040), cyanosis (P =0.001), non-immunized status for DTaP (P =0.047), normal auscultation (P =0.028), normal chest X-ray findings (P =0.027), high absolute lymphocyte count (P =0.039), and low CRP (P =0.046). The patients with the diagnosis of pertussis had a significantly longer duration of coughing (27.2${\pm}$10.6 vs. 12.6${\pm}$5.6 days, P =0.039). Conclusion : Pertussis should be suspected in any infant with typical symptoms of pertussis in addition to: a persistent cough without fever, accompanied by paroxysms or cyanosis prior to the age of DTaP immunization. Active laboratory confirmation should be carried out to confirm more cases with pertussis.

A Study on the Distribution of Microorganisms in Department of Radiography (영상의학과 촬영실의 미생물 분포에 관한 연구)

  • To prevent the secondary hospital-acquired infection (cross-infection) from occurring in the general radiographic room in the department of radiology, the microbial measurement was conducted at the points making direct close contact with radiologists and patients. For the case of radiologists, the microbial measurement and incubation were focused on the x-ray tube handle of the radiation generating device, and, for the case of patients, the microbial measurement and incubation were focused on the chin supporting device, chest-contact point, and handle. Once disinfected with Aniosurf, the sterilized media were gathered and identified, and the microorganisms were confirmed. Based on the identification results, it was confirmed that the points making direct close contact with radiologists showed a value of 103 CFU for Proteus mirabilis, Staphylococcus epidermidis, Bacillus spp. and Candida spp., and that the points making direct close contact with patients showed a value of 103~5 CFU for Proteus mirabilis, Enterococcu faecium, Pseudomonas aeruginosa, NTM(Non-Tuberculosis Mycobacteria) and Candida spp.. It was also confirmed that the types and number of microorganisms gathered from the points making direct close contact with patients were greater. Fortunately, most of the involved microorganisms were observed to be on the skin surface and are known to become extinct when disinfected in accordance with the hospital-acquired infection control rules. However, since even minor exposure to such microorganisms may be lethal for patients with reduced immunity, caution must be taken. In particular, since the points making contact with patients showed a high level of microbial measurement, it was thought that it would be necessary for radiologists and personnel having frequent access to strictly disinfect the parts, such as instruments and handles, making frequent contact with patients. The purpose of this study was to announce the importance of safe microbial control in the radiographic inspection room in hospital, and this study is expected to be used as the baseline data for preventing hospital-acquired secondary infection and Nth infectious diseases.
