정부는 글로벌 헬스케어를 신성장동력 산업 중 5대 고부가 서비스 분야에 포함시키는 한편, 의료관광의 활성화를 위해 외국인 환자유치를 허용하는 의료법을 개정했다. 또 의료 서비스산업 선진화 방안도 발표하는 등 오는 2013년까지 외국인 환자 20만명을 유치해 의료 서비스를 차세대 성장동력으로 키우겠다는 목표를 내세웠다. 이처럼 정부가 의료관광에 관심을 기울이는 것은 황금알을 낳을 것이라는 기대 때문이다. 세계의 의료관광 현황과 우리나라의 발전 과제를 짚어본다.

한국가톨릭의료협회(회장 최영식)는 "너희는 온 세상에 가서 모든 피조물에게 복음을 선포하라(마르 16.15) 하신 예수님의 말씀에 따라 온 인류의 복음화를 위한 노력의 일환으로 6월 15일부터 23일까지 9일간의 일정으로 보건 의료환경이 열악한 모골다르항에서 지역 주민과 어린이를 대상으로 의료봉사를 펼치고, 7월 3일엔 평가함을 가졌다.

The purpose of this study was to examine the relationship between the awareness of health care workers on the protection of patient health information and their practice of it in an attempt to provide some information on the policy setting of health care institutions about medical information protection. As a result, the awareness of the health care workers on the protection of patient health information and their practice of it were both the best in the communication area, followed by the patient health information management area and the area of direct contact with health information. As for the variables linked to their awareness and practice of patient health information protection, the type of the health care institutions, job satisfaction, religion and the departments in which they worked were significantly related. To determine what factors affected the patient health information management area, a multiple regression analysis was carried out by selecting the area of direct contact with patient health information and the communication area as independent variables and by selecting the patient health information management area as a dependent variable. And it's found that the patient health information management area became better when the area of direct contact with the information and the communication area were better.

CRM의 장점은 원하는 고객에 대해서 마케팅 노력을 기울일 수 있고 그 결과로 마케팅 비용을 줄이고 효율을 높일 수 있는 것이다. 그리고 마케팅 전략 수립에 대한 방법의 변화와 그 수행방법의 변화가 CRM에 있기 때문에 여기에 따른 절차와 문화, 조직구조까지 변화 시킬 수 있다. 이를 방지하기 위하여 CRM교육이 필요하며, 그 중요성을 인식하고 실행하여야 한다. 그리고 경영환경에 적합한 마케팅 전략 수립이 필수적이다. CRM의 정확한 이해와 적합한 수행이 있어야만 병원경영의 목표달성에 도움이 된다. 그러나 CRM자체의 문제가 없는 것도 아니다. 무엇보다 전문 인력이 부족하면 시스템 구축과 마케팅이나 컨설팅 인력을 확보하여 프로젝트의 부실화를 방지하여야 한다. 다음은 실업자들의 업무패턴을 파악하고 사용자의 편의를 고려한 적용이 있어야 한다. 고가의 비용을 투입하여 CRM을 실행하였다 해도 사용자 교육의 미비가 생기면 효율이 감소될 것이다. 역시 CRM의 적용에 있어서 호환이 되지 않으면 통합비용으로 별도의 비용이 지출해야 하므로 비용의 증가를 감수하여야 한다. 환자의 의료의 질에 대한 만족은 환자의 주관적 개념으로 동일한 의료서비스에 대하여 환자들은 저마다 다르게 의료의 질을 평가하게 된다. 대부분의 환자들은 자신의 욕구와 의료의 질에 대한 기대를 가지고 병원을 방문하여 의료의 질을 평가하게 된다. 대부분의 환자들은 자신의 욕구와 의료의 질에 대한 기대를 가지고 병원을 방문하여, 그것을 의료제공자가 충족시키는 정도가 환자의 만족도를 결정하게 된다. 의료의 질은 의료서비스에 대한 만족과 밀접한 관련이 있다. 의료의 질과 만족은 강하게 연관이 있으면서도 양자는 분명히 다르다. 의료의 질에 대한 평가와 만족도는 매우 일치도가 높다고 볼 수 있다. 이것은 의료의 질적 수준이 높으면 환자만족도도 높다는 것을 의미한다. 따라서 본 연구는 병원의 환자관리를 위한 CRM 마케팅의 전기를 실증적으로 역설하고 이의 도입을 통한 효과와 문제점, 이에 대한 대응방안을 제시하고, 모범적인 CRM운영 사례를 제시함과 동시에 효과적인 의료의 질 경영을 구현하고 이것이 환자의 의료서비스 만족도를 높이는 결과인 것을 모색하는 것이 본 연구의 목적이다.

This study was aimed to identify factors affecting the purchase of indemnity private health insurance and analyze its effect on healthcare utilization by gender. This study analyzed data of the 2010-2016 Korea Health Panel Survey using panel logit model and panel regression model. Men were less likely to purchase indemnity private health insurance than women. Self-rated health and unmet need for healthcare in women were related to insurance purchases. In men, married people and residents of the metropolitan area were more likely to purchase. The indemnity private health insurance did not affect the women's outpatient expenses per visit, but for men it increased. The indemnity private health insurance significantly increased women's inpatient expenses per admission, but it did not affect men's inpatient expenses. This study confirmed gender differences in the factors affecting the purchase of indemnity health insurance and the impact of indemnity health insurance on healthcare utilization. This suggests that gender differences should be taken into account in private health insurance research and institutional operation.

Despite of advanced plans from Korean government to encourage Korean medical tourism, the medical tourism in Korea is still under competitive compared to the countries including Thailand, Singapore, and India, in which the government has been driving the development in industries of medical tourism, Moreover, the studies identifying the factors affecting tourists' satisfaction has not been actively performed up to date. Therefore, this study attempts to investigate the influences of quality of medical service, accessibility, cost adequacy, and quality of tourism of Korean medical tourism on Chinese tourists' satisfaction and revisit intention. For the purpose of empirical study, a survey has been conducted to Chinese medical tourists. Findings are as follows. First, the factors of medical tourism attributes including quality of medical service, cost adequacy, and quality of tourism have turned out to have positive effects on medical tourists' satisfaction. Second, tourists' satisfaction has a positive effect on revisit intention.

본 연구의 목적은 환자가 거주지가 아닌 타 지역에서 입원의료를 이용하는 요인을 규명하여 주민들의 의료이용에 대한 지역적 접근성을 향상시킬 수 있는 방안을 제시하는데 있다. 2005년 환자조사 입원자료 523,782건을 연구대상으로 하였으며, 2004년 의료기관평가 자료, 2005년 인구센서스, 2006년 보건의료자원실태조사를 이용하였다. 자료분석은 기술통계, 카이제곱 검정, 로지스틱 회귀분석을 실시하였다. 타 지역 입원의료 이용의 가장 큰 요인은 의료기관 소재지의 의료서비스 수준으로, 인구 10만명당 의료기관평가 점수가 9.5점 이상인 지역이 9.5점 미만인 지역에 비해 타지역 입원의료 이용확률이 8.3배가 높았다. 반면 인구 10만명당 병상수가 910병상 이상인 지역이 910병상 미만인 지역에 비해 타 지역 입원의료 이용확률이 2.0배 높았다. 정부는 지역주민의 의료의 접근성을 높이기 위해 의료공급량의 확충과 분배보다는 지역의료의 질적 수준을 향상시키는 방향으로 정책을 펼쳐야 한다.

본고(本稿)의 목적(目的)은 우리나라의 국민의료비추계치(國民醫療費推計値)를 제시하고 OECD제국(諸國)의 자료(資料)를 이용하여 국제비교(國際比較)에 의한 추이분석(推移分析)을 행함에 있다. 이를 위하여 우선 각국 의료비(醫療費)의 결정요인(決定要因)에 대한 최근의 논쟁(論爭)을 재조명(再照明)하고 "각국간(各國間)의 소득격차(所得隔差)로서 의료비변동(醫療費變動)을 충분히 설명할 수 있으며 의료비의 소득탄력도(所得彈力度)는 1보다 크다"는 Newhouse의 주장을 검증(檢證)하였다. 그 결과 소득수준(所得水準)이 각국의 의료비 변동을 설명하는 중요한 변수(變數)임을 확인할 수 있었다. 그러나 소득수준이 높다고 해서 반드시 국민의료비(國民醫療費)의 GNP비율(比率)이 제고(提高)되지는 않는다는 점이 국제비교결과(國際比較結果) 밝혀졌다. 이는 소득 이외에도 의료비지불제도(醫療費支拂制度)등 제도적(制度的) 요인(要因)이 의료비 변동에 영향을 준다는 점을 시사해 준다. 한국의 의료비(醫療費)는 비교적 급속히 증가하고 있는데 우리나라의 의료비동향(醫療費動向)은 소득(所得) 이외에도 고령인구비율(高齡人口比率) 및 의료보험제도(醫療保險制度)의 실시(實施) 등을 가지고 유의(有意)하게 설명할 수 있었다. 국민의료비(國民醫療費)의 지속적인 고율성장(高率成長)은 국민개보험(國民皆保險)을 앞두고 국민의료비(國民醫療費)의 안정화노력(安定化努力)을 요청하고 있는바 이 점에 있어서는 호주, 덴마크, 노르웨이 등 최근 의료비증가율(醫療費增加率)이 진정되고 있는 국가들로부터 지불제도(支佛制度)의 개선을 중심으로 유익한 교훈을 얻을 수 있다.

The aim of the research is to lead to suggestions on public health policy, to suggest the direction of accessibility to public health services in rural area through analyzing various health programs and health service system, health care facilities. Nepal's public health policy needs improvement in the quality of the health services as well as accessibility to the services to improve population quality.
