Ovarian Morphology and Menstrual Cycle of Reproductive Age Women Receiving Anti-Epilepsy Medication (항경련제 투여를 받는 가임기 간질 여성의 난소의 형태 및 월경주기에 관한 연구)

  • Background : An increased frequency of reproductive endocrine disorders and polycystic ovary(PCO) has been reported in women with epilepsy. A possible role of the seizure disorder or, of the use of antiepileptic drugs(AEDs) has been suggested as the pathogenic mechanism. The objective of the present study was to assess the prevalence of reproductive endocrine disorders, such as PCO or menstrual abnormalities, in a series of women with epilepsy, examining the possible relationships of these disturbances with different epilepsy syndromes and AED treatment. Methods : Thirty epileptic women, all of reproductive age and none pubertal, pregnant, or lactating, were evaluated by clinical endocrinological assessment, and pelvic ultrasonography. Seven patients were on valproic acid(VPA), nineteen on carbamazepine(CBZ), and four on diphenylhydantoin(DPH) treatment, respectively. Results : Menstrual irregularity was observed in 8 women(26.7%), dysmenorrhea in 7(23.3%), and premenstrual syndrome in 1(3.3%). Ultrasonographic examination revealed that one women(3.3%) showed polycystic ovary, 4(13.3%) had ovarian cyst(s), and 2(6.7%) had uterine myoma, respectively. There was no difference in the prevalence of menstrual abnormalities or polycystic ovary according to the different preparations of AEDs. Conclusions : Data from this investigation suggest that, in Korean reproductive age women, the treatment of AEDs and the kind of medication may not have a significant effect on the prevalence of menstrual abnormalities or ultrasonographic polycystic ovary.

Diagnostic Evaluation of Serum FSH and LH in Primary Amenorrhea by Radioimmunoassay (방사성 면역측정상의 혈청 FSH와 LH에 관한 일차성 무월경증 환자의 진단적 고찰)

  • The usefulness of single, random measurements of serum FSH and LH in the diagnosis of primary amenorrhea by radioimmunoassay was investigated. The 16 patients were divided into 3 groups by the level of serum FSH and LH. The first group with increased level of serum FSH and LH is five patients, all of these are related to the acquired or congenital abnormality of the ovary. Further studies indicated include buccal smear, chromosome analysis, gynecography and laparosocopy. The second group with normal serum FSH and LH is nine patients, four patients of these are related to the developmental anomaly of the Mullerian duct and five patients are undo etermined origin. Further studies indicated include laparoscopy and gynecography. The third group with decreased serum FSH and normal or decreased serum LH is two patients, one of these is related to the pituitary function, isolated FSH deficiency, the other is undetermined origin. Further studies indicated include the pituitary function test, LH-RH stimulation test, skull radiography. Determination of serum FSH and LH levels does not permit a specific etiologic diagnosis of primary amenorrhea. However the serum levels of FSH and LH can be used to differentiate the principal area of the investigation and can be of assistance in choosing more specific testing procedures.

A Clinical Study on Amenorrhoea(經閉) Patients (경폐(經閉)(무월경(無月經))에 관(關)한 임상적(臨床的) 고찰(考察))

  • An amenorrhoea means the pathologic condition that menstruation stops before the post menopausal period. Clinically, this is classified to both primary amenorrhoea and secondary amenorrhoea. Primary amenorrhoea indicate what has no first menstruation until 14 without secondary sexual sign or what has no first menstruation until 16 with secondary sexual sign before the time. Secondary amenorrhoea is diagnosised when a women with normal menstrual cycle before the onset doesn't have a menstruation over 6 months or appeals amenorrhoea for three times of her normal menstrual cycle. This clinical study was done on the patients who had come to gynecology department of oriental Hospital of Kyung Hee Medical Center from August 1, 1994 to July 31, 1995, complaining of amenorrhoea. The results obtained were as follows: 1. The amenorrhoea patient rate among outpatients who came to the deptment of gynecology was 3.4%. 2. The ratio between primary amenorrhoea and secondary amenorrhoea was 1 : 6.5. 33. The patient rate via other hospitals was 75% and Unremarkable finding(47.6%) was most numerous according to other hospital's diagnosis and hyperprolactinemia(11.9%), premature menopause(11.9%) were the second numerous diagnosis. 4. Unremarkable history(56.7%) was most numerous and among history, the fast(weight loss; 30%) was most numerous. 5. The most general symptom of amenorrhoea patient was indigestion(51.7%). 6. The most frequently used prescribtion for non-insurance was Onpojongoktang(溫胞種玉湯 ; 55%), for insurance was Gamisoyosan(加味逍遙散 ; 16.7%). 7. 25% patient show menstruation in their therapy and among this, 80% patient show menstruation within 40 days. 8. 26.9% secondary amenorrhoea patient show menstruation in their therapy and only one primary amenorrhoea patent(12.5%) shows the same result. 9. Among the effective used prescribtion, Onpojongoktang(溫胞種玉湯 ; 46.7%) is most numerous.

The Influence of Diet, Body Fat, Menstrual Function, and Activity upon the Bone Density of Female Gymnasts (신체구성성분, 영양상태 및 월경기능이 여자체조선수의 골밀도에 미치는 영향(제2보))

  • This study was conducted with 20 female gymnasts and 23 age-matched controls to examine the relationship of diet, menstrual function and bone mineral density (BMD). The results obtained are summarized as follows : Energy intake of gymnasts was 968.9$\pm$421.4kcal, and energy expenditure was 2091.4$\pm$361kcal showing negative energy balance(-1,122.5$\pm$534.6kcal). The average intakes of calcium, iron, vitamin A, thiamin, riboflavin and niacin did not meet the Recommended Dietary Allowances for their age groups. Mean age at menarche in gymnasts is 15.8$\pm$1.2 years compared with 11.8$\pm$2.8 years in age-matched controls. The profile of estradiol, progesterone, and luteinizing hormone was lower than age-matched controls but not significant. Athletic amenorrheic gymnasts(n=12) have the menstrual irregularity(n=10) and amenorrhea(n=2). A number of variables as such nutritional deficiency in diet, negative energy blasnce and hypogonadotropic hormonal status were included. The bone mineral density (BMD) of female gymnasts were significantly higher than controls for the lumbar neck(p<0.001), trochanter(p<0.01), and Ward's triangle(p<0.001), but there were no significant differences for the lumbar spine and forearm. The lumbar spine BMD had a positive correlation with age and lean body weight. The femoral neck BMD was significantly associated with age, group and lean body mass. The trochanter BMD had significant relationship with group, body mass index, energy expenditure and follicular stimulating hormone. Ward's triangle BMD were related to body mass index and follicular stimulating hormone. The significant association was deterced between forearm BMD and age and lean body weight. The major finding of this investigation is that the BMD of gymnasts were higher than age-matched controls despite the fact that gymnasts as a group had inadequate dietary calcium and a higher propensity to have an interruption of their menstrual cycle. These data indicate that grymnsts involved in sports producing significant impact loading on the skeleton had greater femoral neck, trochanter and Ward's triangle bone density than age-matched controls.

The Clinical Trial of $SP_6$(Sanyinjiao) Moxibustion on the Primary Dysmenorrhea (원발성 월경곤란증에 대한 삼음교($SP_6$) 애구의 임상적 연구)

  • Objectives : The aim of this study was to evaluate the effect of $SP_6$ moxibustion on the primary dysmenorrhea. Methods : The data was collected from subjects who was suffering for dysmenorrhea. The subjects were employed using Measure of Menstrual Pain(MMP) and Menstrual Symptom Severity List(MSSL) questionnaire. We treated the subjects with $SP_6$ moxibustion and measured Skin Resistance Variability(SRV) before and after treatment. The method of analysing data was paired t-test. Results : 1. $SP_6$ moxibustion produced a significant decrease in symptoms of primary dysmenorrhea. Results : 2. The SRV of 2, 3 area was significanltly changed after $SP_6$ moxibustion. Conclusions : $SP_6$ moxibustion was effective in decreasing the symptoms of primary dysmenorrhea. The SRV results suggest that women with primary dysmenorrhea may have organic energy deficiency and functional abnormality of the hypothalamus-hypophysis-ovarian axis.

Zoophthora phalloides Batko (Zygomycetes: Entomophthoraceae), a Fungal Parasite of the Aphid Dactynotus species in Korea (Dactynotus 진딧물의 국내 미기록 곤충병원성 곰팡이, Zoophthora phalloides에 관한 보고)

  • An entomophthoraceous fungus, Zoophthora phalloides, was found in populations of the aphid Dactynotus species, at National Institute of Agricultural Science and Technology, and Jeonnam Provincial RDA during June, 1998. Occurrence of the fungus and aphid has never been recorded in Korea. Microscopic observations of this fungus are described, and illustrated. The fungus has sausage-shaped primary conidia with $28.98\;{\mu}m$ in length, and two types of secondary conidiophores. Our specimens exhibit closer relationship with the North American isolates than with the European isolates on the basis of conidial length described by other study.

Menstruation and the Variability of Food Intake in Female College Students (월경 전.후 여대생의 식사섭취 행태 변동성 분석)

  • This study aimed to elicit the variability of appetite and food intake patterns in female college students during different menstrual phases. The craving for certain foods and physiological and psychological symptoms of menstrual phase (pre- and post-menstrual) were investigated by self-administered questionnaire. Three hundred and sixty six students who were 20.9 years old and had $19.8kg/m^2$ of BMI volunteered to participate in this study. Most of the subjects (89.5%) experienced the premenstrual syndrome (PMS) such as irritation, bodily fatigue, nervosity. Symptoms such as tiredness, stomachache, changes in taste and increased appetite were pointed out to be experienced at the onset of the menstrual cycle. The variability of food intake in premenstrual phase, 11.0% of subjects had decreased food intake where as 68.8% had experienced increased intake. The postmenstrual phase, 20.1% had decreased food intake while 45.2% had experienced increased intake due to changes in the appetite. Before starting menstruation, most of the subjects craved for sweets. The group who had experienced abnormal appetite during menstrual phase was significantly high ratio in overweight and obese students (p<0.05). We also observed an association between the PMS score and the variability of eating patterns during the menstrual phase. The students who experienced changing appetite and food intake had significantly high PMS score in the premenstrual phase (p<0.01) and postmenstrual phase (p<0.05). These results suggested a need for future study related to changes in the actual nutrient intake and activity level during the menstrual phase.

  • Herlyn-Werner-Wunderlich syndrome(HWWs) is a rare variant of Mullerian ductal anomalies characterized by the presence of a hemivaginal septum, a didelphic uterus, and ipsilateral renal agenesis. It usually presents after menarche with progressive pelvic pain, and palpable mass due to hemihematocolpos. If a cystic mass is detected behind the urinary bladder in children, in association with the absence of a kidney, the diagnosis of uterus didelphys with imperforate vagina and hydrocolpos should be considered. When renal agenesis is found in asymptomatic children, the small size and the tubular shape of the uterus makes it almost impossible to evaluate uterine anomalies, so follow-up should be performed until the end of puberty. Appropriate preoperative diagnosis and treatment will prevent unnecessary procedures and offer relief of symptoms. We report one case of didelphic uterus with blind hemivagina and ipsilateral renal agenesis with biopsy- proven thin glomerular basement membrane disease which is not related to the above syndrome.

An Analysis of Groundwater Flow at Bugok Area Using MODFLOW (MODFLOW 모형을 이용한 부곡온천지역 지하수 유동해석)

  • This study was conducted to analyse groundwater flow in the Bugok hot spring area using the MODFLOW model which can simulate three dimensional groundwater flow both in confined and unconfined aquifers. Based on this study the following conclusions were obtained: 1) The hydraulic conductivity and the specific storage of the aquifer were 0.0135 m/day and 0.020, respectively, and the model-predicted groundwater elevation agreed well with the observed one. 2) Simulation results showed that the groundwater level declines at the end of the one-year simulation period when the annual recharge rate is small and the annual pumping rate high, which is the worst combination. Except that combination, the groundwater level does not decline at the end of one-year simulation period indication the pumping rates used were allowable. 3) The safe yield depends upon the magnitudes of the recharge and pumping rates. The pumping rate should not produce excess decline of groundwater level around April when the water level is the lowest in a year.

The Effects of Menopause on the Metabolic Syndrome in Korean Women (폐경이 대사증후군에 미치는 영향)

  • This study was the secondary analysis using the Korea National Health and Nutrition Examination Survey of 2012 and purpose was to investigate the effects of menopause on the metabolic syndrome(MetS) in Korean women. In raw data, total 2,746 women aged ${\geq}19$ years and provided the correct data about menstruation and menopause were analysed. The results were as follows: First, components of MetS were significantly different according to menopausal status. Second, the prevalence of MetS was 13.2% in premenopausal women and 42.7% in postmenopausal women. Third, the Odds ratio of postmenopausal women was 4.88(95% CI=3.888-6.126) compared with premenopausal women for MetS. As 5 years from 40 years increased, the Odds ratios of postmenopausal women were 3.15(95% Confidence Interval=1.862-5.331), 4.159(2.558-6.761), 5.971(3.955-9.016), 9.52(6.591-13.749) compared with premenopausal women for MetS, respectively. Based on the results, the intervention should be developed and provided for preventing and managing the MetS in postmenopausal women.
