Development of Rule-based Expert System for Interpretative Report with Health Screening Tests (건강검진자를 대상으로 해석적 보고를 위한 전문가 시스템의 개발)

  • Background : Interpretative reporting is an important aspect of laboratory medicine. The large menu of laboratory tests available today makes it increasingly difficult for the non-specialist to order and interpret all laboratory tests. The aim of this study was to determine the usefulness of an expert system to interpret laboratory tests and help physicians order the appropriate tests. Materials and Methods : In order to interpret laboratory tests, a rules-based expert system was developed. In this module, if-then rules were used to interpret the given test result patterns (e.g. urinalysis, anemia, hepatitis B virus, hypercholesterolemia, glucose, syphilis, and tumor markers) and select matching text elements. The system was used to evaluate 535 subjects who visited a health-check program. Results : The overall abnormal rate was 50.5% in the expert system; 34% for cholesterol, 9.9% for urinalysis, 8.0% for anemia, 7.7% for thyroid function tests, 4.5% for tumor marker study, 4.7% for hepatitis virus antigen, 4.3% for serum glucose, and 1.1% for syphilis. Conclusion : These results indicate that the application of the expert system for the interpretation of laboratory tests may provide a useful method for the interpretation of reports. However more rules are needed for the application to in-patients.

Operating Report of a Hospice Cyber Counselling Program (호스피스 사이버상담 프로그램 운영에 대한 소고)

  • Purpose: This study is to report the operating result of the Hospice Cyber Counselling Program for improving quality of lift for terminal patients and their families. Methods: This study was performed followed by counsellor training, building strategy of public relations, management, and evaluation of the effect in order. Results: 72 subjects visited our cyber homepage and became members for approximately 9 months from Oct. 2004 to July 2005; 17 subjects in their 20s (23.6%), 12 subjects in their 30s (16.7%), 17 subjects in their 40s (23.6%), and 13 subjects above 50s (18%). Classified by diagnose related group, cancer related disease consist of 90%. As for the relationship between contents of counselling and counselling applicants, families consist of 90% and self-applicants only 10%. Only 30 members used on-line counselling and their counselling frequency was just 35 times. Although counselling frequency and the number of members who used the counselling site were not high, they tend to contact with a counsellor continuously and use the site actively after the first counselling. Conclusion: Cyber counselling program for hospice was practically managed to improve quality of life for terminal patients and their families as a basic activity for hospice business activation. Basic public relations with regard to hospice business by mass media and the press should be performed continuously and the hospice training program is required for medical personnel to be continue.

Factors Influencing on Intention of Dental Care Utilization after Annual Dental Examination for Workers in Daegu, Korea (대구시 일부 근로자의 구강검진후 치과진료 이용의사에 영향을 미치는 요인)

  • Prior to the start of the general oral examination, this study was performed using the theory of planned behavior to provide data that would help not only increase intention of industrial workers' dental care utilization after their dental examination, but develop the follow-up oral health programs. As the first research group, 608 workers were selected from the 1016 workers while excluding both 53 workers who were currently undergoing dental treatment and 355 workers who had an opportunity to visit the dental clinic over the previous six months. Among the general characteristics that would influence the 608 workers' intention to utilize the dental clinic within one (1) month, their financial state showed that the higher their financial abundance level, the stronger their intention to visit the dental clinic(p<0.01). It was shown that the attitude toward the dental care utilization related behavior and subjective norm except the perceived behavioral control among the TPB variables had a significant effect on their intention(p<0.01). Among TPB variables, attitude toward the dental care utilization and subjective norm except the perceived behavioral control, were significant correlation with behavioral intention(p<0.01). The structure model's R-square of the attitude toward the behavior, subjective norm and perceived behavioral control having and effect on the intention accounted for 21%. In order to encourage the workers to have positive thoughts about the attitude toward their dental care utilization and subjective norm, therefore, it is required to perform the continued oral health programs, in addi1ion to oral health experts' regular education.

Chloroform Body Burden From Swimming In Indoor Swimming Pools (실내 수영장에서 수영시 야기되는 클로로포름 인체부담)

  • The use of chlorinated water in swimming pools produces elevated chloroform levels in the water and air of the pools which can cause chloroform body burden of swimming individuals. Present study confirmed the chloroform body burdens from a 40-min swimming and evaluated the decay of chloroform breath concentration after the cessation of a 60-min swimming. Air and water concentrations were measured in the pools. The water and air chloroform concentrations ranged from 18.1 to 25.3 ${mu}g/l$ and from 30.9 to 60.7 ${\mu}g/m3$ for the confirmation study, respectively. The breath level after 40-min swimming was about 64 to 266 folds higher than the corresponding background breath. The breath concentration after the 40-min swimming ranged from 10.5 to 21.3 ${\mu}g/m3$, while that prior to the corresponding swimming ranged from 0.07 to 0.19 ${\mu}g/m3$. In addition, the post-exposure breath level varied with the subjects who swam in the pool on the same visiting day. Breath concentration increased gradually during 60-min swimming, then decreased rapidly within 5 minutes after the cessation of exposure, after that, decreased slowly, and finally approached to a background breath level at 1-2 hr after exposure.

Effects of a Health Diary Program on Fall-Related Outcomes in Low-Income Elderly Women with Osteoarthritis (건강다이어리 프로그램이 저소득 관절염 여성노인의 낙상관련 심리적 변수에 미치는 효과)

  • Purpose: This study was done to identify the effects of a health diary program on fall related outcomes the low-income elderly women. Methods: The study was carried out with a nonequivalent control group with pretest-posttest design. The study was composed of two groups, each made up of 24 subjects: experimental group and control group. The subjects were low-income women aged over 65 years with osteoarthritis and both the experimental and control groups were made up of subjects with the same age profiles. The independent variable was the health diary program, and the dependent variables were fall related outcomes (fear of falling, fall-efficacy, knowledge of fall) difficulty of performing activity, and mood state. The health diary program was performed for 50 minutes each session and twice a week for 8 weeks. Data were collected before the health diary program 10 weeks after the beginning of the program. Results: The experimental group showed significant differences in knowledge of fall, fear of falling, and mood state compared to the control group. However there was no significant differences in difficulty of performing activity and fall efficacy. Conclusion: The results of this study may be used as part of an education to prevent falls for low-income elderly women with osteoarthritis.

  • The rules and terms are described different meaning, in this results the research is accomplished for preventing practical workers from confusion. Atomic law are kept up modification and development in our situation by the ICRP's recommendation, on the other hand, the rules of diagnosis radiation equipment safety managements are modified partial, then resulted in confusion. The study was comparison between the rules of diagnosis radiation equipment safety management and atomic energy law, and the modification items obtained were as follows. 1. With each other different the terms and units are used. With the exception of special terms for affairs usage, it is needless to say that common term uniformity is standardized. The standardization of rules and guidance have not need to confusion radiological practical workers. 2. The following is omitted. 1) The radiation protection against tile patient and the hospital visitor. 2) Radiation dose limit of the woman patient who is in the process of becoming pregnant. 3) Radiation dose limit of the person who is not regarded as madical madical exposure. 4) The control of the exposure of pregnant of women at work.

Related Factors and Oral health Status of Some Manufacturering Workers (일부 제조업 생산직 남성근로자들의 구강건강상태 및 관련요인)

  • The Purpose of this study was to examine the factors affecting oral health status of some workers. Answer sheets for questionnarie for 178 workers at Changwon city, Korea, were collected and analyzed using SPSS 19.0. The number of respondents who have visited dental clinic is lower among group with dental caries than group without dental caries. The respondents who have scaling corresponds to 31.3% in the group with dental caries and a little bit lower percentage is shown in the group without dental cares(p<.05). Among those with dental caries, the respondents who experienced tooth pains are to 51.3% and those who had frequent blooding in gum 48.7% and those who had ordors inside the mouth 53.8%. The number of respondents who had scaling within last one year among the group with gingival inflammation is lower than those without it(p<.001). In the group with gingival inflammation, no smokers are corresponding to 13.9%(p<.01) and those with more than 3 times brush of tooth 4.7% and those with oral hygiene devices are to 5.9%. Important variables which have effects on dental caries are understood as self-evaluation for dental health, dental floss or interdental brush, ordors in the mouth. Also variables related with gingival inflammation are considered as tooth brush before bed time, tongue pain, orders in the mouth, the cold as ice tooth and tooth scaling experience. In summary, Oral health education is needed to increase the motivation of industrial workers to control their basic dental disease.

Factors for Completing Case Management of Suicide Attempters: A Coihort Follow-Up Study Based on Data From Case Management of Emergency Room-Based Suicide Attempters (자살시도 환자의 지속적 관리 완수 요인: 응급실 기반 자살시도자 사후관리 사업 자료를 기반으로 한 코호트 추적 연구)

  • Objectives : The purpose of this study was to find out how demographic factors, suicide attempt patterns, psychiatric history and management of suicide attempters affect the completion of emergency department (ED) based case management program. Methods : Among the patients who attempted suicide and visited the emergency department of Chung-Ang University Hospital from June 1, 2018 to May 31, 2021, 661 patients who agreed to case management were studied. After being discharged from the emergency department, subjects were registered for an eight-week follow-up service program. Hierarchical logistic regression analysis was conducted with demographic factors, suicide attempt patterns, psychiatric history and management as independent variables, and completion of case management program as dependent variables. Results : Suicide attempt pattern had the most significant influence on the completion of case management program, followed by demographic factors, psychiatric history and management. Those who completed the case management program were significantly more likely to have suicide plans in the future, more authentic in suicide attempts, and had higher proportion of past suicide attempts than those who did not complete the program. Conclusions : To ensure that the subjects complete the follow-up project program and get connected to community services, an individualized approach with consideration of suicide attempt patterns, demographic factors, and psychiatric history is needed.

Subjective Symptoms according to Fitting Test in Soft Contact Lens Wearers (소프트콘택트렌즈 착용자의 피팅 상태 확인 유무에 따른 자각적 증상)

  • Purpose : The purpose of this study was to investigate subjective symptoms according to the wearing, purchase and fitting status of soft contact lens wearers, and consumer perception of base curve. Methods : A survey was conducted for those who visited optical shops and lens shops in Seoul from June to August 2018. A total of 98 answer sheets they submitted were used for the analysis. The chi-square test and Fisher's exact test were used to compare subjective symptoms (dryness, glare, uncomfortable fitting, and decreased vision) according to whether fitting condition is screened (case history and push-up test, ect relevant to wearing sensation), and the odds ratio (OR) was obtained by the logistic regression analysis. Results : When the contact lenses were purchased, the rate of which the visual acuity test was performed was 86.6% at optical shop and 64.7% at lens shop. When purchasing contact lens and the contact lens was not tested for fitting, they more experienced dryness (OR 4.41, 95% CI 1.25-15.62) and uncomfortable fitting (OR 2.68, 95% CI 1.08-6.64) than testing for fitting. In addition, it was investigated that 87.8% of contact lens wearers did not know about the term base curve, 92.9% did not listen to an explanation related to the base curve when purchasing contact lenses, and 96.9% did not experience with base curve test. Conclusion : It would be thought that satisfaction of the contact lens fitting of existing consumers should be improved by changing a proper base curve by confirming the fitting condition when prescribing soft contact lens.

Psychological Characteristics of Suicide Attempters with Major Depressive Disorder using the Minnesota Multiphasic Personality Inventory-2 Restructured Form (다면적 인성검사 II 재구성판으로 살펴본 주요우울장애 자살 시도자의 심리적 특성)

  • Objectives : This study was conducted to investigate differences in psychological characteristics between major depressive disorder (MDD) patients with and without suicide attempt using MMPI-2-RF. Methods : Subjects were 107 MDD patients who had visited the department of psychiatry of hallym university hospital and met the DSM-IV diagnostic criteria of MDD by the korean version of MINI-Plus 5.0.0. The patients were divided into suicidal attempters (n=43) and non-suicidal attempters (n=64) using C-SSRS. The one-way ANOVA was used to compare MMPI-2-RF scale scores between two groups. Additionally, ANCOVA was conducted considering the severity of depressive symptom and comorbidity as covariate. Results : Our results showed that Suicide/Death Ideation (SUI), Inefficacy (NFC) and Interpersonal Passivity (IPP) scales were significantly higher in the MDD patients with suicidal attempt compared to MDD patients without suicidal attempt (p<0.05). However, after controlling for the severity of depressive symptom and comorbidity, SUI scale showed a significant tendency (p<0.10). Conclusions : The result suggests that MMPI-2-RF scales could be a useful tool for identifying patients transitioning to actual suicidal attempts in the moderate or severe major depressive disorder group. Limitations of this study and directions for further research are also discussed.
