Clinical Practice Guideline for the Diagnosis and Treatment of Pediatric Obesity: Recommendations from the Committee on Pediatric Obesity of the Korean Society of Pediatric Gastroenterology Hepatology and Nutrition

  • The Committee on Pediatric Obesity of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition newly developed the first Korean Guideline on the Diagnosis and Treatment of Obesity in Children and Adolescents to deliver an evidence-based systematic approach to childhood obesity in South Korea. The following areas were systematically reviewed, especially on the basis of all available references published in South Korea and worldwide, and new guidelines were established in each area with the strength of recommendations based on the levels of evidence: 1) definition and diagnosis of overweight and obesity in children and adolescents; 2) principles of treatment of pediatric obesity; 3) behavioral interventions for children and adolescents with obesity, including diet, exercise, lifestyle, and mental health; 4) pharmacotherapy; and 5) bariatric surgery.

  • Various types of functional bedding for inducing and maintaining sleep, are developed and launched with the importance of improving health through sleep emphasized currently. The purpose of this study is to examine development status and direction of functional bedding in the $4^{th}$ Industrial Revolution era, through systematic classification of elements of IoT-based smart bedding cases actively developed as functional bedding at home and abroad. Through previous research, literature and Internet data, characteristics and functional extension of smart bedding and the background of smart bed development was analyzed. And it was analyzed that smart bedding pursues recent functionalism and convergence of physical and digital concept such as IoT or AI, and also mental value to improve sleep quality. As bedroom where smart bedding place in has the private and limited characteristics and users are in sleep-conscious, that hard to ensure power and discomfort in carrying are moderated and the aesthetic elements are not very important, and that the smart bedding performance while sleeping were affected on developmental background. Based on CES case study and analysis on how smart beds are functionally expanded from conventional bedding, smart beds have gained information through digital sensing, and common properties that can be controlled anytime, anywhere, using a smart phone. Some set up the right environment and pose, while others stimulate nerves directly as active intervention. It is expected that smart bedding will be developed to cure user's body and mind, through active intervention when sleeping.

Depression and aggression according to health lifestyle of coronary artery disease patients (관상동맥 질환자의 건강생활 습관에 따른 우울과 공격성)

  • This study is to investigate the difference of depression and aggression according to lifestyle habits in 135 patients with coronary artery disease. The survey was conducted from May 13 to May 15, 2019. In order to investigate the difference of depression and aggression according to lifestyle, independent sample t-test and one-way ANOVA were conducted. Post-hoc analysis was conducted when there was a difference between depression and aggression. The results of the analysis showed that the group with relatively high smoking and drinking was high in physiological depression and the group with active participation in exercise and dietary control showed low physiological depression. The group with more alcohol intake showed higher level of verbal aggression, physiological aggression, and aggressive attitude than the group without alcohol. The group that participated actively in physical activity showed lower physiological aggression and aggressive attitude than the group without active aggression. It was found that the group that participates hard in dietary control is less aggressive than the group that does not.

An Evaluation of the Reliability and Validity of the Automatic Pennation Angle Measuring Program (깃각 자동측정 프로그램의 신뢰도와 타당도 평가)

  • Purpose: Ultrasound imaging is commonly used to measure the pennation angle of human skeletal muscles in vivo. However, manual assessment of the pennation angle using ultrasound images is subjective and time-consuming and requires a high level of examiner skill. The architectural analysis of human skeletal muscles is thus challenging. Automated approaches using image processing techniques are therefore required to estimate the pennation angle in ultrasound images. The purpose of this study was thus to assess the intra-tester and inter-tester reliability and validity of the pennation angle using an automatic measurement program. Methods: Twenty-two healthy young adults (mean age = 22.55 years) with no medical history of neurological or musculoskeletal disorders voluntarily participated in this study. Ultrasound imaging was used to measure the pennation angle of the gastrocnemius muscle at rest. One examiner acquired images from all the participants. The intra-tester and inter-tester reliability were evaluated using the intraclass correlation coefficient (ICC) to estimate reliability. Validity was measured using Pearson's correlation coefficient. Results: The intra-rater reliability was excellent for the automatic pennation angle measuring program and the manual pennation angle assessment method (ICC>0.95). The inter-rater reliability was also excellent for both methods (ICC>0.93). All the correlation coefficients for the automatic pennation angle measuring program and the manual pennation angle assessment method were 0.79, which indicated a significantly positive correlation (p<0.05). Conclusion: Pennation angle measurement using the automatic pennation angle measuring program showed acceptable reliability and validity. This study therefore demonstrated that the automatic measuring program was able to automatically measure the pennation angle of skeletal muscles using ultrasound images, and thus made it easy to investigate skeletal muscle architecture.

Complexity Comparison of Center of Pressure between Fallers and Non-fallers during Gait (보행 시 낙상 유무에 따른 압력중심점의 복잡성 비교)

  • Objective: The purpose of this study was to investigate the effect of the falls on the center of pressure (CoP) complexity during gait using non-linear approximate entropy (ApEn). Method: 20 elderly women with experience of falling ($age=72.55{\pm}5.42yrs$; $height=154.40{\pm}4.26cm$; $body\;weight=57.40{\pm}6.21kg$; $preferred\;gait\;speed=0.52{\pm}0.17m/s$) and 20 elderly women with no experience of falling ($age=71.90{\pm}2.90yrs$; $height=155.28{\pm}4.73cm$; $body\;weight=56.70{\pm}5.241kg$; $preferred\;gait\;speed=0.56{\pm}0.13m/s$) were recruited for the study. While they were walking at their preferred gait speed on a treadmill (instrumented dual belt treadmills, Bertec, USA) with a force plate CoP data were collected for the 20 strides. The complexity of the CoP was analyzed using the ApEn technique. Results: The ApEn of the medial-lateral CoP in the fallers showed smaller about 16% compared to the non-fallers (p<.05). The ApEn of the antero-posterior CoP of the fallers showed smaller about 12% compared to the non-fallers, but the difference was not statistically significant. Conclusion: Based on the results of this study, the reduction of the medio-lateral CoP complexity in the elderly gait would be an index to determine the potential fall.

Utilization of Korean Medicine among Children with Cerebral Palsy - Qualitative Study by Grounded Theory (뇌성마비 아동의 한방의료 이용행태 - 근거이론에 의한 질적연구)

  • Objectives : The purpose of this study was to identify factors that affect the utilization of Korean medicine (KM) among children with cerebral palsy (CP) and explore a processing model of decision-making by their parents. Methods : We conducted an individual in-depth interview with 21 mothers of children with CP who were recruited during the observational study. A grounded theory of Strauss and Corbin was used for qualitative analysis. Thus, collected data were classified into open coding including 121 concepts, 27 sub-categories and 7 categories. Results : The central phenomenon of the process of using KM for children with CP is 'KM as an optional treatment not essential'. Therefore there are many obstacles to starting KM treatment and it is easily interrupted for various reasons, unlike general rehabilitation treatment. However, if the patient experiences the therapeutic effect, the parents want to continue KM treatment. They try to provide treatment as much as possible if they can afford it. The parents give priority to general rehabilitation treatment and value the information and experience provided by the caregivers in similar situation as well as expert opinions. Conclusions : To expand and generalize KM treatment for children with CP, efforts to change interventional conditions such as treatment effect, treatment cost, treatment compliance, and convenience of treatment based on the understanding of the strategy used by the parents in KM utilization.

Effect of increasing nitric oxide and dihydrotestosterone by Taraxacum coreanum extract (포공영(Taraxacum coreanum) 추출물에 의한 산화 질소 및 dihydrotestosterone 증가 효과)

  • Men's climactic syndrome, andropause, or testosterone deficit syndrome, is one of the new problems with the health of older men in the age of aging. This phenomenon is a natural phenomenon occurring in men as they age, clinically characterized by a decrease in blood testosterone levels and a marked decrease in physical and mental activity. The purpose of this study was to investigate the effect of hydrothermal extract of Taraxacum coreanum by comparing the levels of nitric oxide (NO) in the cavernosum and the levels of male hormone in the blood. Taraxacum coreanum extract increased NO production in vitro and in vivo in a dose-dependent manner. Levels of dihydrotestosterone and 17-hydroxyysteroid dehydrogenases, as well as levels of neurogenic nitric oxide synthase and cGMP, increased significantly in elderly rats (22 weeks) after 4 weeks of daily intake of Taraxacum coreanum extract. However, prostaglandin $E_2$, testosterone, and sexually-hormone-binding globulin levels were not different among all groups. Furthermore, total sperm and motile sperm counts were also no significant difference. Overall, these results suggest the possibility of Taraxacum coreanum extract as a safe and effective natural substance for enhancing NO, cGMP and free testosterone.

Validity of the dietary reference intakes for determining energy requirements in older adults

  • BACKGROUND/OBJECTIVES: The objectives of this study were to evaluate the accuracy of the Dietary Reference Intakes (DRI) for estimating the energy requirements of older adults, and to develop and validate new equations for predicting the energy requirements of this population group. MATERIALS/METHODS: The study subjects were 25 men and 23 women with a mean age of $72.2{\pm}3.9\;years$ and $70.0{\pm}3.3\;years$, and mean BMI of $24.0{\pm}2.1$ and $23.9{\pm}2.7$, respectively. The total energy expenditure (TEE) was measured by using the doubly labeled water (DLW) method, and used to validate the DRI predictive equations for estimated energy requirements (EER) and to develop new EER predictive equations. These developed equations were cross-validated by using the leave-one-out technique. RESULTS: In men, the DRI equation had a -7.2% bias and accurately predicted the EER (meaning EER values within ${\pm}10%$ of the measured TEE) for 64% of the subjects, whereas our developed equation had a bias of -0.1% and an accuracy rate of 84%. In women, the bias was -6.6% for the DRI equation and 0.2% for our developed equation, and the accuracy rate was 74% and 83%, respectively. The predicted EER was strongly correlated with the measured TEE, for both the DRI equations and our developed equations (Pearson's r = 0.915 and 0.908, respectively). CONCLUSIONS: The DRI equations provided an acceptable prediction of EER in older adults and these study results therefore support the use of these equations in this population group. Our developed equations had a better predictive accuracy than the DRI equations, but more studies need to be performed to assess the performance of these new equations when applied to an independent sample of older adults.

Will CFD ever Replace Wind Tunnels for Building Wind Simulations?

  • The use of computational fluid dynamics (CFD) is becoming an increasingly popular means to model wind flows in and around buildings. The first published application of CFD to both indoor and outdoor building airflows was in the 1970's. Since then, CFD usage has expanded to include different aspects of building design. Wind tunnel testing (WTT) on buildings for wind loads goes back as far as 1908. Gustave Eiffel built a pair of wind tunnels in 1908 and 1912. Using these he published wind loads on an aircraft hangar in 1919 as cited in Hoerner (1965 - page 74). The second of these wind tunnels is still in use today for tests including building design ($Damljanovi{\acute{c}}$, 2012). The Empire State Building was tested in 1933 in smooth flow - see Baskaran (1993). The World Trade Center Twin Towers in New York City were wind tunnel tested in the mid-sixties for both wind loads, at Colorado State University (CSU) and the [US] National Physical Laboratory (NPL), as well as pedestrian level winds (PLW) at the University of Western Ontario (UWO) - Baskaran (1993). Since then, the understanding of the planetary boundary layer, recognition of the structures of turbulent wakes, instrumentation, methodologies and analysis have been continuously refined. There is a drive to replace WTT with computational methods, with the rationale that CFD is quicker, less expensive and gives more information and control to the architects. However, there is little information available to building owners and architects on the limitations of CFD for flows around buildings and communities. Hence building owners, developers, engineers and architects are not aware of the risks they incur by using CFD for different studies, traditionally conducted using wind tunnels. This paper will explain what needs to happen for CFD to replace wind tunnels. Ultimately, we anticipate the reader will come to the same conclusion that we have drawn: both WTT and CFD will continue to play important roles in building and infrastructure design. The most pressing challenge for the design and engineering community is to understand the strengths and limitations of each tool so that they can leverage and exploit the benefits that each offers while adhering to our moral and professional obligation to hold paramount the safety, health, and welfare of the public.
