The Association between Mibyeong Symptoms and Quality of Life (미병 증상과 삶의 질 연관성 연구)

  • Objectives : The purpose of this study was to investigate the association between quality of life and Mibyeong index of abnormal symptoms by the Mibyeong state. Methods : A total of 1,100 people were recruited into this study and were collected by specialized research company. The data were collected by personal information using MBI(Mibyeong Index) and SF-12 (Short form-12) and were analyzed with SPSS (version 21.0) computer program, and included Pearson's chi-square test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis. Results : The Mibyeong Index which represents abnormal symptom type showed significant difference for Mibyeong state (p<.001). In Total subject, fatigue, pain, low sleep quality and indigestion were found to be significant predictors of PCS(Physical Component Summary). In Healthy group, pain was found to be significant predictor of PCS. In MI 1 group, fatigue, pain, indigestion and anxiety were found to be significant predictors of PCS. In MI 2 group, fatigue, pain and low sleep quality were found to be significant predictors of PCS. In Total subject, fatigue, depression, anger and anxiety were found to be significant predictors of MCS (Mental Component Summary). In Healthy group, indigestion and anxiety were found to be significant predictors of MCS. In MI 1 group, depression and anger were found to be significant predictors of MCS. In MI 2 group, fatigue and depression were found to be significant predictors of MCS. Conclusions : The relationship between Quality of life and MBI changes according to Mibyeong state. We expect that this result could be contribute to health management of people in Mibyeong state.

Report on Two Cases of Gilbert's Syndrome Found in the Process of Administering Herbs (단미 한약 복용중 발견한 Gilbert's syndrome 2예 임상고찰)

  • Gilbert's syndrome is one that shows a benign course with intermittent unconjugate hyperbilirubinemia without any evidence of hepatobiliary tract disease or hemolysis. It is often found in a health examination or blood laboratory test by chance. In particular, patients who are taking drugs, including herbal medicine should be careful for their medication due to the possibility of associations with changes in liver function because of drug metabolism, sometimes they have to quit the use of the medication for a certain period and often they should get an additional test. Two male patients increased serum total bilirubin level without other systemic symptoms in screening test for clinical herb medicine pharmacokinetics study. Therefor they was diagnosed with suspected Gilbert's syndrome. They had been calory deprivation test with 24 hours fasting state. They also performed liver function test and ultrasonogram for evaluation of hepatobiliary tract disease. Total serum bilirubin was markedly increased, especially unconjugate bilirubin level higher over the two times than base line after they had been calory deprivation for 24 hours, They was not found another abnormality all laboratory results and physical examination. This study is a report on two cases of hyperbilirubinemia, diagnosed as Gilbert's syndrome, which were found in the process of a clinical pharmacokinetic study of a decoction of medicinal herbs.

Improvement of Solubility and Dissolution of Ketoconazole by Inclusion with Cyclodextrin (시클로덱스트린과의 포접에 의한 케토코나졸의 용해성 및 용출 증가)

  • Inclusion complexes of ketoconazole (KT) with ${\alpha}-$, ${\beta}-cyclodextrin$ (CD) and dimethyl-${\beta}-cyclodextrin$ $(DM{\beta}CD)$ in a molar ratio of 1:2 were prepared by freeze-drying and solvent evaporation methods. The interactions of KT with ${\alpha}-CD$, ${\beta}-CD$ and $DM{\beta}CD$ in aqueous solution and in solid state were investigated by solubility study, infrared (lR) spectroscopy and differential scanning calorimetry (DSC). The stability constant of $KT-DM{\beta}CD$ inclusion complex (lC) was found to be the largest among three inclusion complexes. Clear differences in IR spectra and DSC curves were observed between inclusion complexes and physical mixtures (PM) of KT-CDs. It was also shown by IR spectra and DSC curves that solvent evaporation method might be. superior to the freeze-drying method in preparing the inclusion complexes of KT-CDs. The dissolution rate of KT was markedly increased by inclusion complex formation with CDs in the buffer solution at pH 4.0 and pH 6.8. The mean dissolution time (MDT,min), which represents the rapidity of dissolution, was in the order of $KT-DM{\beta}CD$ IC (3.20) < $KT-{\beta}-CD$ IC (4.36) < $KT-{\alpha}-CD$ IC (6.99) < $KT-{\alpha}-CD$ PM (17.46)< $KT-{\beta}-CD$ PM (19.36) < $KT-{\beta}-CD$ PM (28.53). The dissolution rates of KT-CD ICsprepared by solvent evaporation method were faster than those of KT-CD ICs prepared by freeze-drying method.

Analysis on Stress and Dietary Attitudes of Male Employees (직장인 남성의 스트레스와 식생활 태도에 관한 분석)

  • This study was conducted to vitalize life through analysis on lifestyles, stress and dietary habits of male employees. For this study, questionnaire survey and physical measurements to examine BMI and waist circumference were performed with male employees in four companies located in Gyeongnam. Total 350 copies of the questionnaire were distributed and 327 ones were finally analyzed. The results of analysis on dietary liferelated problems and stress were as follows. First, according to BMI, 56.9% were overweight and waist circumference recorded less than 90 cm in 90.5%. For lifestyles, 54.1%, 79.2%, 60.9% and 52.3% smoked, drank alcohol, didn't exercise and didn't sleep sufficiently. Second, for the correlation with job stress, the subjects graduating junior college or lower academic institution, production employees and sleeping insufficiently showed higher compared to the others. Analysis on the correlation with life stress found that conjugal relation stress was higher in alcohol-drinkers, the males not taking dietary supplements, those with a normal weight or more compared to underweight ones. Stress from relation with a child was higher aged 35~49 years than in those aged 20~34 years, and stress from economic life was higher aged less than 50 years, in those graduating junior colleges compared to those graduating colleges, in those sleeping insufficiently. Health life stress was found to be higher in those without religion compared to those with it such as Protestant Christianity and Catholicism, in alcohol drinkers and in those sleeping insufficiently and having a disease. Based on this, in order to improve the lifestyles and dietary habits and release stress effectively, practical nutrition education program should be carried out.

Development of measurement scale for Korean scaling fear-1.1 (한국형 스케일링두려움 측정도구 개발(KSF-1.1))

  • Objectives : The purpose of this study was to develop an instrument for Korean Scaling Fear (KSF)-1.1 in scaling patients. Methods : 402 sample size for scaling patients was studied in Daegu city in July and August of 2011. Mean and standard deviation was calculated in 3 dimensions(FWS: fear while scaling, DDH: distrust on dental hygienist, FAS: fear after scaling). Results : Age of 402 subjects was 36.5 years. In analyzing reliability for item-level, a range of correlation coefficient(${\alpha}$) on item-internal consistency(FWS, DDH, and FAS) was 0.58~0.88(${\alpha}$=0.90), 0.40~0.71(${\alpha}$=0.82), and 0.54~0.63(${\alpha}$=0.82), respectively. Floor(%) and ceiling(%) value on 3 dimensions were also 9.2% and 4.0%, 12.4% and 0.5%, and 17.7% and 1.2%, respectively, therefore, we found statistically high reliability for those(p<0.001). With explanatory factor analysis, this study could generate 3 dimensions(factor 1, eigenvalue 5.41, proportion 0.49; factor 2, eigenvalue 1.50, proportion 0.14; factor 3, eigenvalue 1.04, proportion 0.09) and 11 sub-scales. Also confirmatory factor analysis results showed that the KSF1.1 model was fitted very well in analysis of model fit($x^2$=112.94, df=41, p=0.000; goodness of fit index=0.95; adjusted goodness of fit index=0.92; root mean square residual=0.057). Conclusions : In conclusion, The findings of this study showed that developed reliable and valid instrument for measuring the KSF1.1 in the scaling patients.

A Study on the Design of Relocatable Wall System for Office Environment (사무환경을 위한 조립형 벽체 시스템 디자인에 관한 연구)

  • The fact that office environment is a principal factor affecting work efficiency is widely accepted. Under such a circumstance, developing relocatable office wall systems is highly required. Psychological, physiological, and economical factors, along with physical factors, should be considered to develope a office wall system. More specifically, competitive price, relocatable efficiency, structural stability, fast installation, health and safety, and aesthetic satisfaction are typical determinants. The evaluation results of newly developed wall system are as below: 1) Panel structural system added with the merits of frame structural system can be studied to accomplish minimal disruption to workplace and better finish details. 2) To cover up the existing interior defects such as uneven floor and ceiling, flexible solutions must be studied especially when panel structural system is accepted. 3) More consideration must be given to finish materials, section details, sound blocking and absorbtion to enhance the satisfaction level.

The Monitoring for the Effects of Pesticide Exposure on Plasma Components and Nutrient Intakes in Female Agricultural Workers (농촌 여성의 농약 사용에 따른 혈액성분과 영양소 섭취에 관한 연구)

  • Since 1982, female agricultural workers are populous than male in Korea. Previous study was suggested that female workers are more sensitive to the agricultural medicine poisoning than male. This study was conducted with the aim of evaluating the impact on plasma components and nutrient intakes in female agricultural workers by the use of pesticides. Data were obtained from 44 females residing in Wonjoo, Kangwon-do area. Analysis for the general characteristics, nutrient intakes, and plasma components of the subjects were performed by physical examination, 24-hour recall method and venous blood sampling. They were divided into 3 groups according to the degree of the pesticide use: 8 in none(Non-expose group), 14 in low(<4/yr)(Low-expose group), and 22 in high$({\geq}4/yr)$(High-expose group). The results are summarized as follows. Serum glucose, SGOT, SGPT, triglyceride, HDL-cholesterol did not correlated with pesticide use and were not different among the groups. But serum total cholesterol was higher in pesticide use group. With increasing the number of pesticide use times, leukocyte count was tended to increased. Eighty two percent of the subjects were hemoglobin less than or equal to 12g/dl. Hematocrit, MCV, MCH, MCHC, serum Fe, Serum ferritin belonged to normal range and did not correlated with pesticide use. Intake of energy, Fe, vitamin $B_2$ were lower than RDA. Fe, K, and vitamin A intake were higher in don't use group than the other groups. There are not many differences according to using the pesticide in anthropometric measurements, nutrient intakes, and blood components, except for serum total cholesterol level and some nutrients intake. But these results suggest the need the systemic researches about the effects the pesticide using on nutritional status in Korean agricultural workers.

Using Artificial Neural Networks for Forecasting Algae Counts in a Surface Water System

  • Algal blooms in potable water supplies are becoming an increasingly prevalent and serious water quality problem around the world. In addition to precipitating taste and odor problems, blooms damage the environment, and some classes like cyanobacteria (blue-green algae) release toxins that can threaten human health, even causing death. There is a recognized need in the water industry for models that can accurately forecast in real-time algal bloom events for planning and mitigation purposes. In this study, using data for an interconnected system of rivers and reservoirs operated by a New Jersey water utility, various ANN models, including both discrete prediction and classification models, were developed and tested for forecasting counts of three different algal classes for one-week and two-weeks ahead periods. Predictor model inputs included physical, meteorological, chemical, and biological variables, and two different temporal schemes for processing inputs relative to the prediction event were used. Despite relatively limited historical data, the discrete prediction ANN models generally performed well during validation, achieving relatively high correlation coefficients, and often predicting the formation and dissipation of high algae count periods. The ANN classification models also performed well, with average classification percentages averaging 94 percent accuracy. Despite relatively limited data events, this study demonstrates that with adequate data collection, both in terms of the number of historical events and availability of important predictor variables, ANNs can provide accurate real-time forecasts of algal population counts, as well as foster increased understanding of important cause and effect relationships, which can be used to both improve monitoring programs and forecasting efforts.

Comparison of the Graduate Medical School Student's Perception Structure about 'Happy Doctor' by Clerkship Experience (임상실습 경험에 따른 의학전문대학원생들의 '행복한 의사' 개념 인식 비교)

  • The purpose of this study is to provide direction of medical education by analysing medical school student's perception structure about 'happy doctor'. In particular, this study compared perception structure between two groups of students before clerkship and after clerkship. The subject of this study were 1~4 academic year students in medical school. Students' text about 'happy doctor' were collected by open-ended questionnaire and analyzed by using sematic network analysis. Based on the result of network analysis, perception structure of each groups were confirmed. The network of each groups have 'Professionalism' group including words such as 'patient', 'treatment', 'worthwhile' in common. Three groups, 'Professionalism', 'Quality of life' and 'Self-realization' constituted the before clerkship network. And five groups, 'Professionalism', 'Time with family', 'Balance between work and household', 'Interpersonal relationship', 'Physical and psychological health' constituted the after clerkship network. The results of this study is expected to contribute for developing the basic medical education curriculum for 'happy doctor'.

Effects of cranberry powder on biomarkers of oxidative stress and glucose control in db/db mice

  • Increased oxidative stress in obese diabetes may have causal effects on diabetic complications, including dyslipidemia. Lipopolysccharides (LPS) along with an atherogenic diet have been found to increase oxidative stress and insulin resistance. Cranberry has been recognized as having beneficial effects on diseases related to oxidative stress. Therefore, we employed obese diabetic animals treated with an atherogenic diet and LPS, with the aim of examining the effects of cranberry powder (CP) on diabetic related metabolic conditions, including lipid profiles, serum insulin and glucose, and biomarkers of oxidative stress. Forty C57BL/KsJ-db/db mice were divided into the following five groups: normal diet + saline, atherogenic diet + saline, atherogenic diet + LPS, atherogenic diet + 5% CP + LPS, and atherogenic diet + 10% CP + LPS. Consumption of an atherogenic diet resulted in elevation of serum total cholesterol and atherogenic index (AI) and reduction of high density lipoprotein (HDL)-cholesterol. However, with 10% CP, the increase in mean HDL-cholesterol level was close to that of the group with a normal diet, whereas AI was maintained at a higher level than that of the group with a normal diet. LPS induced elevated serum insulin level was lowered by greater than 60% with CP (P < 0.05), and mean serum glucose level was reduced by approximately 19% with 5% CP (P > 0.05). Mean activity of liver cytosolic glutathione peroxidase was significantly increased by LPS injection, however it was reduced back to the value without LPS when the diet was fortified with 10% CP (P < 0.05). In groups with CP, a reduction in mean levels of serum protein carbonyl tended to occur in a dose dependent manner. Particularly with 10% CP, a reduction of approximately 89% was observed (P > 0.05). Overall results suggest that fortification of the atherogenic diet with CP may have potential health benefits for obese diabetes with high oxidative stress, by modulation of physical conditions, including some biomarkers of oxidative stress.
