Background: The aim of our present study was to compare quality of life (QoL) between intermediate-stage (BCLC-B) HCC patients who had undergone either liver resection or transcatheter arterial chemoembolization (TACE). Materials and Methods: A total of 102 intermediate-stage HCC patients participated in our study, including 58 who had undergone liver resection and 44 who had undergone TACE. Baseline demographic characteristics, tumor characteristics, and long-term outcomes, such as tumor recurrence, were compared and analyzed. QoL was assessed using the Short Form (SF)-36 health survey questionnaire with the mental and physical component scales (SF-36 MCS and PCS). This questionnaire was filled out at HCC diagnosis and 1, 3, 6, 12, 24 months after surgery. Results: For the preoperative QoL evaluation, the 8 domains related to QoL were comparable between the two groups. The PCS and MCS scores were significantly decreased in both the TACE and resection groups at1 month after surgery, and this decrease was greater in the resection group. These scores were significantly lower in the resection group compared with the TACE group (P<0.05). However, these differences disappeared at 3 and 6 months following surgery. One year after surgery, the resection group showed much higher PCS scores than the TACE patients (P=0.018), and at 2 years after surgery, the PCS and MCS scores for the resection group were significantly higher than those for the TACE group (P<0.05). Eleven patients (19.0%) in the resection group and 17 (38.6%) in the TACE group suffered HCC recurrence (P<0.05). Univariate and multivariate analyses indicated that tumor recurrence (HR=1.211, 95%CI: 1.086-1.415, P=0.012) was a significant risk factor for poorpostoperative QoL in the HCC patients.Conclusions: Due to its effects on reducing HCC recurrence and improving long-term QoL, liver resection should be the first choice for the treatment of patients with intermediate-stage HCC.
Objectives While Temporomandibular disorders (TMD) is highly prevalent in Korea, studies examining its Korean medical treatment are currently lacking. The aim of this study was to assess current Korean medical treatment practice patterns for TMD. Methods A preliminary questionnaire was developed to investigate current practice patterns of TMD treatment and underwent further revision through external review. The final questionnaire was distributed as a web-based survey to 18,289 potential respondents by email. Results The response rate was 2.23%. Most participants replied that they received multiple Korean medicine interventions for TMD treatment consisting of such methods as acupuncture, chuna manipulation, electroacupuncture. The percentage of treatment services among uncovered services, which need to be covered by national health insurance was high in the order of pharmacopuncture, chuna manipulation, and herbal medicine. The most commonly used diagnostic examination tools for TMD was Physical examination and next was Imaging diagnosis. The frequency of TMD treatment was the highest at 2~3 times per week (76%) and the period was from 4 to 12 weeks (63%). Conclusions This survey study helps determine current practice patterns of TMD, and recognizes the need for use of diagnostic devices in TMD treatment. These results are further anticipated to provide basic data for clinical practice guide lines (CPGs).
Protein malnutrition of children is one of the most serious nutritional deficiencies in developing country. Urea nitrogen excretion in ureotelic animals is the function most sensitive to dietary protein. The 24 hours excretion of creatinine in the urine of a given subject is remarkably constant from day to day. The creatinine excretion of different individuals of the same age and sex is also quite constant. Low ratios of urinary urea to creatinine are found children low protein intake. The foregiving world-wide investigations indicate that the urea nitrogen/creatinine ratios seems to be a good biochemical indicator to distinguish among group with different levels of protein intake. The purpose of this study is to evluate an indicator of protein intake on the elementary school children ranged from 6 to 8 years of age living in rural and urban areas. Each child measured for height and weight of body. weight measured by means of a plate from scale and height by a vertical measuring rod. Biochemical test were taken from a finger-tip and urine. Hemoglobin level in the blood was measured by cyanomethemoglobin method. From the urine samples, urea nitrogen and urea creatinie were determined by Folin-Wu method and: calculate the ratio. The following result were obtained: 1) Mean of the body weight and height in urban children(Seoul) was higher and heavier than rural children(Kyunggi, Kangwon). And 12% of boys, 18% of girls in Kyunggi and 25% of boys, 22% of girls in Kangwon area weight less than 80% of Korean Physical Standard weight level. 2) The mean hemoglobin values of boys and girls in Seoul are children were 13. 3g/100ml, 13.1g/100ml and the mean of hemoglobin values in Kyunggi 12.9g/100ml of boys, 12.4g/100ml of girls, and 12.4g/100ml of boys, 12.9g/100ml of girls in Kangwon children. It is found that 22% to 24% children inrural area (Kyunggi, Kangwon) had hemoglobin level less than 12g/100ml which means anemia. 3) The mean of hematocrit level of Seoul, boys and girls children were 33.5%, 34.1% and 33.4%, 33.1%, in Kyunggi area and 33.1%, 32.9% in Kangwon area. 4) Urea nitrogen/creatinine ratios in Seoul children were 9. 0, 10. 0 of boys and girls, the ratio were 8.2, 8.0 in Kyunggi boys and girls children, and 7.5 and 7.4 in Kangwon boys, girls children. Low-income rural and upper-income urban background large differences between two groups in the urea nitrogen/creatinine ratio(Seoul: Kangwon in male, female children. p<0.05, p<0.001). The urea nitrogen/creatinine ratio definetly seems to be a good indicator of the quantity of the protein intake. However, whether or not it is an indicator of the quality of the ingested protein ramains to be seen.

This study aimed to evaluate the effects of stretching management on musculoskeletal joint pain in community indwelling elderly farmers. The study included 28 residents who lived in a rural community. All participants were assigned randomly to either the stretching group (n=13, $59.67{\pm}4.77$ year-old) or the control group (n=15, $61.44{\pm}10.41$ year-old). Respondents were interviewed by means of a structured questionnaire. Pain severity of 6 body areas caused by symptoms of work-related musculoskeletal disorders and pain severity on day time variations were measured by the visual analogue scale on a self-assessed questionnaire. The stretching group was administered stretching therapy (45 min, 2 sets, warming up and cool down, main exercise; total 19 stretching kinds of subset 5 fields) for 12 times for 4 weeks. There were no significant difference between the two groups in general social and pain characteristics. After 4 weeks of stretching, the stretching group showed significant improvements at almost all joint pain scores except arm/elbow, and day time variation scores of pain compared to both baseline scores, and with control group scores. These results showed that stretching therapy is one of the most useful modalities to manage musculoskeletal pain in community-based elderly farmers.

This study was performed to determine the levels of depressive symptoms and to reveal its relationships between ADL and IADL among rural elderlies. The interviews were performed, during the period from April 1st, to June 30th, 2010, to 412 elderlies in rural areas. As a results, the distribution of ADL among all subjects were 94.7% in normal-range group, and 5.3% in impaired group. In terms of IADL, 87.4% of the subjects were normal-range group, 12.6% were impaired group. The distribution of depression group among all subjects were 38.2% in normal-range group of ADL, and 90.9% in impaired group of ADL, but it was 35.8% in normal-range group of IADL, and 76.9% in impaired group of IADL. In logistic regression analysis, the adjusted odds ratio of the depression was significantly increased in impaired group of ADL than in normal-range group of ADL(OR=23.21, 95% CI=4.38~123.05). Also it was significantly increased in impaired group of IADL than in normal-range group of IADL(OR=7.76, 95% CI=2.99~20.08). In conclusion, the depression of rural elderlies was significantly increased in impaired group than in normal-range group of ADL and IADL. Thus, strategic effort needs to be prevented to depression in impaired group of ADL and IADL.
Objectives : Loneliness is associated with negative mental and physical health. However, little is known about the risk factors of loneliness in the Korean elderly living alone. The aim of this study was to examine sociodemographic and social network related risks for loneliness among the elderly living alone. Methods : This is a cross-sectional study that enrolled 1,091 subjects who are the community-residing elderly living alone. Sociodemographic status, medical condition, cognition, mood disorder and levels of loneliness were collected using a self-administered questionnaire and a specific semi-structured interview conducted by a trained nurse. Descriptive statistics were used to analyze data regarding sociodemographic variable and loneliness. Univariate and Multivariate regression analyses were applied to examine factors associated with loneliness. Results : The mean score of loneliness was 3.8 (SD=1.7). No family contact (standardized β=0.115, p<0.001), no religious attendance (standardized β=0.057, p=0.028), no gathering with friends (standardized β=0.088, p=0.001) and high score of Short for of Geriatric Depression Scale (standardized β=0.502, p<0.001) were significantly associated with high loneliness in the elderly living alone. Conclusions : Family function, social network and depressive mood could be significant risk factors for high loneliness in the elderly living alone. Public health promotion efforts to reduce loneliness should focus on improving family function, social network and decreasing depression.
The purpose of this study was to investigate the effects of weight control program on dietary habits and blood composition in obese middle-aged women. The program consisted of nutrition education, physical exercise and nutrition counseling was conducted for 12 weeks. Participants of the preliminary study were 255 obese women, among which 82 women participated in the nutrition education program. We evaluated the anthropometry, blood lipid profiles, dietary habits of obese women before and after the nutrition education. Dietary habits of the subjects were significantly improved in regularity of having breakfast (p < .05), the frequency of eating snack after dinner (p < .001), eating out (p < .01) and the speed of meal (p < .05) after the program. There was significant decrease in total cholesterol, triglyceride, LDL-cholesterol and systolic pressure of the subjects after nutrition education. Changes of atherogenic index and diastolic pressure were not significant, but had a tendency to decrease. All of the anthropometric outcomes except lean body mass such as BMI, percent body fat, fat mass, and WHR were significantly decreased after weight control program. These findings show that weight control program including nutrition education and counseling can improve dietary habits and weight control pattern, and lead the authors to recommend that public health organizations and dietetic professionals should educate obese women on practical strategies for an effective weight management.
In 2026, in ten years from now, Korea is expected to enter into a super aged society. By facing this social phenomenon, analyzing the elderlies and preparing measures are needed among society. In the transportation field, the traffic accidents which are related to the elderly pedestrians have drastically increased so that R&D projects and policy supplementations are introduced. However very few base studies on which focused on the behavior, capability, impact factors analysis of for elderly pedestrians are conducted. To determine the walking capability, this study divided the walking capability into 3 three categories - general, health and exercise. It carries out the comprehensive survey targeting 52 elderlies with the average age of 72.6 years and this survey is made up of total 12 factors in the 3 three categories. This survey also is conducted with various measuring devices and methods such as interview, National Fitness Award, SPBB, Cybex and the like for. From the 12 detailed factors, the general factors such as age, physical shape and so forth have little impact on the walking speed. However the factors that have the greatest impact on the walking speed are extensor muscle on lower limbs in the health factors and balance, coordination, and SPPB in the exercise factors. With these results, this study develops the independent walking model which can measure the walking capability of the elderly people. The developed model is expected to be utilized as the base study for elderly's walking patterns in the transportation field by examining the walking capability of the older people.

Objectives: The aim of this study was to investigate the correlation between fatigue degree and Comprehensive diagnosis of Qui Xui Shui in patients with functional dyspepsia. There again, we investigated an association between scores obtained from Comprehensive diagnosis of Qui Xui Shui and state of skin sympathetic tone or cardiovascular reactivity. Methods: Subjects were 56 patients with Functional dyspepsia and 8 healthy People. Fatigue degree was assessed by questionnaires consisting of subjective complaints of fatigue. Comprehensive diagnoais of Qui Xue Shui was investigated by questionnaire and physical examination, total score and composition score were calculated from the symptom score. Skin sympathetic tone was measured by Ryodoraku Score(RS) and Cardiovascular Reactivity(CV Reactivity) was checked by Pulse diagnostic apparatus. All patients were divided into two groups by RS $40{\mu}A$(below and above) or by CV Reactivity(decreased and increased or not decreased). Results: There was highly significant correlation between Fatigue Degree and total score and each composition score of Comprehensive diagnosis of Qui Xui Shui. And total score of patients was significantly higher than that of health controls : in the patients, females than males. Significance related with the level of RS $40{\mu}Aor$ the state of CV Reactivity were not remarkable in both patients and health control group. Conclusions: Fatigue degree seems to have a special feature reflecting the state of Comprehensive Diagnosis of Qui Xui Shui in patients with functional dyspepsia.

Sounds and music have been used in various ways in the East and the West, as it is well known that they influence the human mind and health a lot. Recently, there have been many studies regarding the remedy through sounds and music in the West and they have made good effects in the actual therapy. In the East, people knew that sounds and music would make great effects on human mind long time ago and they placed importance on sounds and music. Although there is a Sound Therapy in Oriental Medicine, it is not studied sufficiently in modern Oriental Medicine. It does not have clear standards for the clinical application and has not been frequently used. Accordingly, Yukjagyeol which has long been used in Korean Medicine and Younggamudo which was made by Kim Il-bu in late Chosun were compared from the perspective of acoustics and demonstrative treatment theory of Korean Medicine. Younggamudo by Kim il-bu allot the five sounds such as 'Eum, Ah, Eo, Ih and Uh' to 'spleen, lung, liver, heart and kidney' respectively. As the five sounds are all vowels and vibrate the vocal band as much as possible, they vibrate and resonant the respective organs to help them to function and promote circulation. In oriental medicine, there is 'Singing and Dancing Treatment'. They say songs and dances nurtures personality, supplement blood stream and comfort the mind. As the five sounds of Younggamudo take the melody while vibrating the vocal band sufficiently, when it passes from Youngto Ga(Songs) we may dance moving our bodies according to the rhythm. Therefore Gongbeop of Younggamudo helps the functionality of human organs and promotes blood circulation. As Yukjagyeol is Sabeop(discharging method), it should be used for those who have surplus Sagi and should not use for Heojeung. As Younggamudo is Bobeop(supplementing method), it is not for Shiljeung but for Heojeung which lacks of Jeonggi. It is considered that healthy people without specific disease can use it as a preventive method of 'Chimijeong' to maintain their physical and mental health. It is considered that we can have useful effects if we apply Yukjagyeol and Younggamudo dialectically with the concept of Bosa. It is considered that concrete application to Sound Gigong can be possible by searching for the methodology for empirical further study and clinical experiments and clinical application in the future.
