Effects of Awareness of Good Death and End-of-Life Care Attitudes on End-of-Life Care Performance in Long-Term Care Hospital Nurses

  • Purpose: The purpose of this study was to determine the effects of awareness of good death and end-of-life care attitudes on end-of-life care performance in long-term care hospital nurses. Methods: This study used a cross-sectional study design. The participants were 147 nurses working at six long-term care hospitals with more than 200 beds in B city, South Korea. Data were collected using self-reported questionnaires, and analyzed with descriptive statistics, the t-test, analysis of variance, Pearson correlation coefficients, and multiple stepwise regression analysis using IBM/SPSS 26.0 for Windows. Results: The participants' awareness of good death, end-of-life care attitudes, and end-of-life care performance were positively correlated. The factors affecting end-of-life care performance were age, education level, awareness of good death, and end-of-life care attitudes; these variables explained 19.0% of end-of-life care performance. Conclusion: In order to improve long term care hospital nurses' end-of-life care performance, continuing education and training should be provided regarding awareness of good death and end-of-life care attitudes.

'Good life', 'good politics' and Mencius ('좋은 삶'과 맹자(孟子)의 인정론(仁政論))

  • Both in the East and the West, the most classical question in political philosophy was 'what is truly a good life?' 'Good life' and 'good politics' are thus essentially tied together. Mencius (孟子, B.C385-303/302) was not exception to this belief. It is not an exaggeration that his Mencius, the treatise that encapsulates his whole views, begins and ends with good life and good politics. He began with new definitions of good life and politics and the rest elaborates why they are so. This paper attempts to systematically approach to what Mencius thinks as a good life in terms of political thoughts. Confucius, his intellectual mentor, asserted that the most humane human life is a civilized life and it means to realize the value of 'Yin (仁: Grace)' and thus set his good life from previous ones. Mencius concurred that Confucius's explication of the good life was right. Moreover, he argued that to realize this, political practice should follow. The 'good life' for Mencius consists of 1) the life in which the ruler does not monopolize joy but shares it with people, that is '$Y{\breve{o}}mindongrak$(與民同樂)' or '$Y{\breve{o}}minhaerak$(與民偕樂)' and 2) the life, based on this political foundation, that pursues the life of 'Five Ethics (五倫)' in which each individual member of society has its share in it. Mencius discussed about 'Four Virtues (四德)', the essential goodness of human, confirmed by 'Four Clues (四端)' to talk about the possibility of realizing the good life. On the other hand, he devised the political device of '$YinJ{\breve{o}}ng$ (仁政: Gracious Politics)' as a practical tool for it. Furthermore, he asserted 'Good people theory (養民論)' as the first condition for the good politics, '$YinJ{\breve{o}}ng$' and 'Education of people theory (敎民論)' as the final one. As Mencius inherited Confucius effort for a good life with the theory of '$YinJ{\breve{o}}ng$', the so-called Zhuxi scholars inherited his as 'Sugichiin (修己治人: cultivate yourself and then order society)' after 1500.

We began with Platon and Confucianism's definitions of human virtue & good life, and 'good State'. This Essay attempts to systematically approach to what Confucianism thinks as a human virtue and a good life in contrast with Platon. Confucianism believe in reality of the human mind & it's nature. Confucian human mind & it's nature is comprised of benevolence, righteousness, propriety and wisdom. Confucianism proposed that we have to approach to other man as benevolence.

Awareness of good death, perception of life-sustaining treatment decision, and changes in nursing activities after decision to discontinue life-sustaining treatment among nurses in intensive care units at tertiary general hospitals (상급종합병원 중환자실 간호사의 좋은 죽음인식, 연명의료결정 인식 및 연명의료중단 결정 후 간호활동의 중요도 변화)

  • Purpose : This study aims to explore nursing activities after the decision to discontinue life-sustaining treatment, awareness of a good death, and perception of life-sustaining treatment decisions among nurses in intensive care units (ICUs) at tertiary general hospitals. Methods : Participants were 173 nurses working in two tertiary general hospitals. The data were collected using structured questionnaires and analyzed using an independent t-test, paired t-test, one-way ANOVA, Scheffé's test, and Pearson's correlation coefficient. Results : Participants were 173 nurses working in two tertiary hospitals. The nursing activity increase was the greatest in the spiritual domain, and the physical domain was where the activities decreased the most. There were significant associations between Awareness of good death (Clinical) and Perception of life-sustaining treatment decision(r=.26, p <.001), Awareness of good death (Closure) and Perception of life-sustaining treatment decision(r=.36, p <.001), and Awareness of good death (Personal control) and Perception of life-sustaining treatment decision(r=.49, p <.001). Conclusion : Based on the results, systematic education programs and job training are required to improve the awareness regarding good death and perception of life-sustaining treatment decision for nurses in ICUs where discontinuing life-sustaining treatment decisions are made.

Factors Influencing Attitude toward Withdrawal of Life-Sustaining Tratment among Nursing Students (간호대학생의 연명치료중단 태도에 미치는 영향요인)

  • This study aimed to identify the integral factors influencing the attitudes of nursing students toward withdrawal of life-sustaining treatments. Methods: 139 nursing students were selected from the school of nursing of a single university. Questionnaires were used as measurement tools to measure their good death recognition, attitude towards death & towards withdrawal of life-sustaining treatment. The degree of good death recognition, attitude towards death & towards withdrawal of life-sustaining treatment were analyzed using descriptive statistics. Correlation between variables was analyzed using Pearson's correlation coefficient and factors influencing the attitude towards withdrawal of life-sustaining treatment using multiple linear regression. Results: Attitude towards withdrawal of life-sustaining treatment was significantly positively correlated with good death recognition(r=.312, p=.000). As a result of multiple linear regression, good death recognition significantly influenced (β=.312, p=.000), accounting for 8.5% of the variance in attitude towards withdrawal of life-sustaining treatment. Conclusions: The results from this study can be contribute to develop educational programs to foster positive attitudes towards withdrawal of life-sustaining treatment.

Social Support and Life Satisfaction of Living Alone elderly in Rural Area (농촌 독거노인들의 사회적 지원과 생활만족도)

  • This study was conducted to identify the social support to living alone elderly in some rural communities and the level of life satisfaction. For the purpose of this study, face-to-face interviews and questionnaire were performed with 315 old people(male 43 and female 272) aged over 65 living alone in rural communities covered by 14 community health posts within Gyeongsangbuk-do Province randomly selected. The followings are summaries of findings; The average score of support from their children was $4.29{\pm}2.73$ out of 8. Variables that showed a significant difference were religion, level of living, type of medical insurance, frequency of meeting with children, time taken from houses of children by usual means of transportation, and subjective health status. It was found that the score of support from children was high for the elderly who had a religion, a good level of living, benefit from medical insurance, a high frequency of meeting with children, or a good subjective health status, or who resided close to their children's houses. The level of the support from friends and relatives showed a significant difference depending on the subjective health status, of which the average score was $4.13{\pm}2.61$ out of 8. The average score of the level of life satisfaction was $6.83{\pm}4.24$(male $7.60{\pm}4.09$ and female $6.71{\pm}4.26$) out of 17. Male elderly showed the higher level of life satisfaction than female elderly. Variables that showed a statistically significant difference in the level of life satisfaction were religion, level of living, medical insurance, hobby, children, disease, subjective health status, and ADL. That is, the level of satisfaction with life was found to be higher for the elderly who had a religion, a good level of living, benefit from medical insurance, a hobby, children, no diseases, or a good ADL, or who thought that they are healthy. The regression analysis with support from children as a dependent variable showed that the level of support from children was higher for the elderly who had a good level of living, frequency of meeting with children, or a good subjective health status. The regression analysis with the level of support from friends or relatives was higher for old people who had a good level of living. The regression analysis with the level of life satisfaction as a dependent variable showed that the factors which related to the level of satisfaction were sex, religion, level of living, hobby, ADL, and subjective health status. That is, it was found that for male elderly who had a religion, a good level of living or a hobby, or who thought that they were healthy, the level of life satisfaction was higher.

Confucius' viewpoint of happiness: Focusing on Analects (공자의 행복관 : 『논어(論語)』를 중심으로)

  • The theme, 'Good Life', is one of the most classical questions all over the oriental and European societies in the history of politic ideas. Confucius (BC. 551 ~ BC. 479), a philosopher representing oriental ideas, also sought 'Good World' by proposing new world, and such 'Good World' cannot be separated from 'Good Life', which is 'Happy Life', so that his foundation of Confucius School means that he dreamt of 'Confucianism based Happiness' in another words. This study is aimed to look into Confucius' viewpoint of happiness using Analects that best shows his ideas, words and behaviors. In the Chapter 2, I checked good life and happy life dealt in the ancient European societies to get a general cognition of them. Then, I looked into the details written in the Chapter 1 Introduction, Section 1 'HakEe(學而)', Analects, in which Confucius thought that the genuine happiness of human was the process toward internal joy from learning, external joy from communicating with society and humanistic human that was the completion of oneself, which I used as the keys for this study. In the Chapter 3, 4 and 5, therefore, I investigated how such keys shown in the Chapter 1 of the Section 'HakEe' were achieved all through Analects.

Understanding the Nurturing Theory of Lee-Jema : Focusing on Taoist Books (이제마의 양생론에 대한 이해 - 도교서적을 중심으로 -)

  • Objective : We compare the nurturing theories of Between the Nurturing Theory of Dogyogwonseonseo(道敎勸善書) JeungjungGyeongsinrok(增訂敬信錄), Gyeongsinrokeonseok(敬信錄諺釋), Taisanggauempyeondosul(太上感應篇圖說) and Guahwajonsin(過化存神) vs Dongui-Susebowon-Sasang-Chobongwon(東醫壽世保元四象草本卷) and Dongui-Susebowon(東醫壽世保元) of Lee-Jema. And the influences that Dogyogwonseonseo had on Nurturing Theory of Lee-Jema. Method : We sort out the identical or similar vocabularies and abstract the important concepts from all four kinds of Dogyogwonseonseo and two kinds of Lee-Jema's books mentioned above and compare them respectively. The prehistories of vocabularies and concepts needed such as Bok(福, good fortune) and Su (壽, longevity) will be traced. And the synchronic understanding needed such as 'Good deed leads to longevity and good fortune whereas evil deed leads to short life' in late Choseon dynasty will be surveyed by investigating the controversy of the time. Results : The Nurturing Theory of Lee-Jema was influenced by all four kinds of Dogyogwonseonseo mentioned above. We can ascertain the influence from Lee-Jema's Concept of Myeong(命), Nurturing Life through everyday life, 'Good deed leads to longevity and good fortune whereas evil deed leads to short life', deceiving mind(詐心), betraying mind(欺負心) and the jealousy of Man and Woman. Conclusion : The influences Dogyogwonseonseo had on Lee-Jema's nurturing theory were key concept words such as Myeong(命), the ways of thinking like 'Good deed leads to longevity and good fortune whereas evil deed leads to short life' and vocabularies such as deceiving mind(詐心).

The Quality of Life of Patients with Good Outcomes after Anterior Circulation Aneurysm Surgery Assessed by the World Health Organization Quality of Life Instrument-Korean Version

  • Objective : Even in the patients with neurologically good outcome after intracranial aneurysm surgery, their perception of health is an important outcome issue. This study aimed to investigate the quality of life (QOL) and its predictors of patients who had a good outcome following anterior circulation aneurysm surgery as using the World Health Organization Quality of Life instrument-Korean version. Methods : We treated 280 patients with 290 intracranial aneurysms for 2 years. This questionnaire was taken and validated by 99 patients whose Glasgow Outcome Scale score was 4 and more and Global deterioration scale 3 and less at 6 months after the operation, and 85 normal persons. Each domain and facet was compared between the two groups, and a subgroup analysis was performed on the QOL values and hospital expenses of the aneurysm patients according to the type of craniotomy, approach, bleeding of the aneurysm and brain injury. Results : Aneurysm patients showed a lower quality of life compared with control patients in level of independence, psychological, environmental, and spiritual domains. In the environmental domain, there were significant intergroup differences according to the type of craniotomy and the surgical approach used on the patients (p<0.05). The hospital charges were also significantly different according to the type of craniotomy (p<0.05). Conclusion : Despite good neurological status, patients surgically treated for anterior circulation aneurysm have a low quality of life. The craniotomy size may affect the QOL of patients who underwent an anterior circulation aneurysm surgery and exhibited a good outcome.

The Effect of Nursing Students' Consciousness of Biomedical Ethics, Good Death Recognition, and Self-Esteem on the Attitude toward Withdrawal of Life-Sustaining Treatment (간호대학생의 생명의료윤리 의식, 좋은 죽음 인식, 자아존중감이 연명치료 중단에 대한 태도에 미치는 영향)

  • This study was a descriptive study to investigate the effects of nursing students' consciousness of biomedical ethics, good death recognition, and self-esteem on attitudes toward withdrawal of life-sustaining treatment. The subjects of this study were 204 nursing students attending university. The data were collected from October 24 to October 31, 2017 and analyzed using the SPSS Win. 22.0 program. Attitudes toward withdrawal of life-sustaining treatment scored $2.97{\pm}0.29$ out of 4, $3.01{\pm}0.31$ for biomedical ethics, $3.24{\pm}0.38$ for good death recognition and $3.23{\pm}0.41$ for self-esteem. There was a significant positive correlation between attitudes toward withdrawal of life-sustaining treatment and good death recognition, and there was a significant positive correlation between attitudes toward withdrawal of life-sustaining treatment and self-esteem. As a result of multiple regression analysis, it was found that good death perception affected nursing students' attitudes toward the withdrawal of life-sustaining treatment. In other words, elevated perception of good death was associated with more positives attitudes toward withdrawal of life-sustaining treatment. Based on the above results, it is necessary to develop a systematic education program for nursing college students. In addition, this researcher proposes an in-depth study to explore the variables that influence nursing students' attitudes toward withdrawal of life-sustaining treatment.
