Intravenous levetiracetam versus phenobarbital in children with status epilepticus or acute repetitive seizures

  • Purpose: This study compared the efficacy and tolerability of intravenous (i.v.) phenobarbital (PHB) and i.v. levetiracetam (LEV) in children with status epilepticus (SE) or acute repetitive seizure (ARS). Methods: The medical records of children (age range, 1 month to 15 years) treated with i.v. PHB or LEV for SE or ARS at our single tertiary center were retrospectively reviewed. Seizure termination was defined as seizure cessation within 30 minutes of infusion completion and no recurrence within 24 hours. Information on the demographic variables, electroencephalography and magnetic resonance imaging findings, previous antiepileptic medications, and adverse events after drug infusion was obtained. Results: The records of 88 patients with SE or ARS (median age, 18 months; 50 treated with PHB and 38 with LEV) were reviewed. The median initial dose of i.v. PHB was 20 mg/kg (range, 10-20 mg/kg) and that of i.v. LEV was 30 mg/kg (range, 20-30 mg/kg). Seizure termination occurred in 57.9% of patients treated with i.v. LEV (22 of 38) and 74.0% treated with i.v. PHB (37 of 50) (P=0.111). The factor associated with seizure termination was the type of event (SE vs. ARS) in each group. Adverse effects were reported in 13.2% of patients treated with i.v. LEV (5 of 38; n=4, aggressive behavior and n=1, vomiting), and 28.0% of patients treated with i.v. PHB (14 of 50). Conclusion: Intravenous LEV was efficacious and safe in children with ARS or SE. Further evaluation is needed to determine the most effective and best-tolerated loading dose of i.v. LEV.

X-RAY PROPERTIES OF THE PULSAR PSR J0205+6449 IN 3C 58

  • We report X-ray timing and spectral properties of the pulsar PSR J0205+6449 measured using NuSTAR and Chandra observatories. We measure the pulsar's rotation frequency ν = 15.20102357(9) s-1 and its derivative $\dot{\nu}=-4.5(1){\times}10^{-11}\;s^{-2}$ during the observation period, and model the 2-30 keV on-pulse spectrum of the pulsar with a power law having a photon index Γpsr = 1.07 ± 0.16 and a 2-30 keV flux F2-30 keV = 7.3±0.6 × 10-13 erg cm-2 s-1. The Chandra 0.5-10 keV data are analyzed for an investigation of the pulsar's thermal emission properties. We use thermal and non-thermal emission models to fit the Chandra spectra and infer the surface temperature T∞ and luminosity Lth of the neutron star to be T∞ = 0.5 - 0.8 MK and Lth = 1 - 5 × 1032 erg s-1. This agrees with previous results which indicated that PSR J0205+6449 has a low surface temperature and luminosity for its age of 800-5600 yrs.

Changes of Compound Muscle Action Potential in Short-term Steroid Therapy for Compression Injury of Rat Sciatic Nerve (백서 좌골신경의 압박손상에 대한 단기간 스테로이드 투여 시 복합근활동전위의 변화)

  • Purpose: Many surgical procedures in oral and maxillofacial area can induce trauma to the peripheral nerve. The aim of the study is to evaluate the effects of short-term steroid therapy on nerve recovery after crush injury. Methods: Sixteen rats were randomly divided into two groups. The right sciatic nerves were exposed, crushed, and sutured. The control group was not given steroids. The test group was injected with dexamethasone disodium phosphate (2 mg/kg body weight/day) for 7 days. In all animals, compound muscle action potential (CMAP) was recorded before and at 1, 7, 14, 21, and 28 days after injury. Results: The amplitude of the CMAP before and at 1, 7, 14, 21, and 28 days after injury were $53.20{\pm}4.80$ mV, $20.12{\pm}5.38$ mV, $30.01{\pm}14.15$ mV, $31.14{\pm}13.56$ mV, $31.73{\pm}16.33$ mV, and $37.23{\pm}16.98$ mV in the control group, and $55.25{\pm}6.72$ mV, $18.62{\pm}6.26$ mV, $29.50{\pm}13.06$ mV, $32.90{\pm}13.226$ mV, $30.17{\pm}11.80$ mV, and $38.41{\pm}12.27$ mV in the test group, respectively. The nerve conduction velocity was $18.82{\pm}3.94$ m/s, $16.73{\pm}3.48$ m/s, $19.60{\pm}2.45$ m/s, $18.68{\pm}3.94$ m/s, $18.02{\pm}3.51$ m/s, and $19.25{\pm}3.88$ m/s in the control group, and $18.94{\pm}3.48$ m/s, $17.28{\pm}2.53$ m/s, $7.57{\pm}2.54$ m/s, $18.77{\pm}2.12$ m/s, $19.48{\pm}1.55$ m/s, and $19.22{\pm}2.97$ m/s in the test group, respectively. There was no significant difference between both groups (P>0.05). Conclusion: This study did not show any therapeutic effect of short-term administration of steroids on injured rat sciatic nerve. Further studies are needed.

Dosimetric comparison between modulated arc therapy and static intensity modulated radiotherapy in thoracic esophageal cancer: a single institutional experience

  • Purpose: The objective of this study was to compare dosimetric characteristics of three-dimensional conformal radiotherapy (3D-CRT) and two types of intensity-modulated radiotherapy (IMRT) which are step-and-shoot intensity modulated radiotherapy (s-IMRT) and modulated arc therapy (mARC) for thoracic esophageal cancer and analyze whether IMRT could reduce organ-at-risk (OAR) dose. Materials and Methods: We performed 3D-CRT, s-IMRT, and mARC planning for ten patients with thoracic esophageal cancer. The dose-volume histogram for each plan was extracted and the mean dose and clinically significant parameters were analyzed. Results: Analysis of target coverage showed that the conformity index (CI) and conformation number (CN) in mARC were superior to the other two plans (CI, p = 0.050; CN, p = 0.042). For the comparison of OAR, lung V5 was lowest in s-IMRT, followed by 3D-CRT, and mARC (p = 0.033). s-IMRT and mARC had lower values than 3D-CRT for heart $V_{30}$ (p = 0.039), $V_{40}$ (p = 0.040), and $V_{50}$ (p = 0.032). Conclusion: Effective conservation of the lung and heart in thoracic esophageal cancer could be expected when using s-IMRT. The mARC was lower in lung $V_{10}$, $V_{20}$, and $V_{30}$ than in 3D-CRT, but could not be proven superior in lung $V_5$. In conclusion, low-dose exposure to the lung and heart were expected to be lower in s-IMRT, reducing complications such as radiation pneumonitis or heart-related toxicities.

  • The time division multiplexing PCM encoder which is constructed for an airborne telemetering system is investigated. Selected by program switch, the PCM encoder has 0~64 words/framd($\pm$5V full scale) of allowable analog input channels, 0~30bits(5V$\pm$1V or 0V$\pm$1V dc) of discrete channels, 70 and 140K bits/sec of bit rate and 8~12bits/word of resolution. And filtered output PCM code is NRZ-L and Bi-S through the 5 pole Bessel LPF(f=100kHz), and the maximum accuracy of PCM encoder is $\pm$0.2% of its full scale.

  • The Odd-Even graceful labeling of a graph G with $q$ edges means that there is an injection $f:V (G)$ to $\{1,3,5,{\cdots},2q+1\}$ such that, when each edge $uv$ is assigned the label ${\mid}f(u)-f(v){\mid}$, the resulting edge labels are $\{2,4,6,{\cdots},2q\}$. A graph which admits an odd-even graceful labeling is called an odd-even graceful graph. In this paper, we prove that some well known graphs namely $P_n$, $P_n^+$, $K_{1,n}$, $K_{1,2,n}$, $K_{m,n}$, $B_{m,n}$ are Odd-Even graceful.

S-Wave Velocities Beneath Jeju Island, Korea, Using Inversion of Receiver Functions and the H-κ Stacking Method (수신함수 역산 및 H-κ 중합법을 이용한 제주도 하부의 S파 지각 속도)

  • Shear-wave velocity ($v_s$) structures beneath two seismic stations, JJU and JJB on the flanks of the volcano Halla on Jeju island, Korea, were estimated by receiver-function inversion and H-${\kappa}$ stacking applied to 150 teleseismic events ($M_W{\geq}5.5$) recorded since 2007. $P_S$ waves converted at the Moho discontinuity does not appear clearly for northwesterly back-azimuths ($207{\sim}409^{\circ}$, average $308^{\circ}$) at station JJU and southeasterly back-azimuths ($119{\sim}207^{\circ}C$, average $163^{\circ}$) at station JJB. This may be due to a gradual velocity increase at Moho or heterogeneity within the crust. The $v_s$ models derived by inversion of receiver functions indicate a distinct low velocity layer ($v_s{\leq}3.5km/s$; LVL) within the crust and a gradual increase in $v_s$ in the depth interval of 30 to 40 km. Within the radius of 18 km beneath station JJB, the LVL occurs at depths of 14 ~ 26 km and the 'Moho' ($v_s{\geq}4.3km/s$) is at 34 km depth. Ten kilometers to the west, within the radius of 16 km beneath station JJU, both the LVL and the Moho are significantly shallower, at depths of 14 to 24 km and 30 km, respectively. H-${\kappa}$ analyses for stations JJU and JJB yield estimated crustal thickness of 29 and 33 km and $v_p/v_s$ ratios of 1.64 and 1.75, respectively. The lesser $v_p/v_s$ ratio was derived for rocks nearest to th peak of the volcano.

Antimicrobial Activities of Sword Bean (Canavalia gladiata) Extracts against Food Poisoning Bacteria (식중독원인균에 대한 작두콩 추출물의 항균활성)

  • Various solvents (chloroform, hexane, ethyl acetate, ethanol, methanol, and hot water) were tested to investigate the antimicrobial activities of sword bean (Canavalia gladiata) against 12 food poisoning bacteria. Chloroform, hexane, ethyl acetate and hot water extracts had no antimicrobial activities, but ethanol extract showed V. parahemolyticus 10 mm, S. sonnei 9 mm, and methanol extract showed strong activities in order of V. parahemolyticus 22 mm, S. sonnei 21 mm, L. monocytogenes 20 mm by disk diffusion. The minimal inhibitory concentrations (MICs) were also determined. The methanol extract had MIC values of 50 mg/mL against S. Typhimurium, V. parahemolyticus, and S. sonnei and values of 100 mg/mL against other 7 food poisoning bacteria and values of 200 mg/mL against Y. enterocolitica and MRSA. The inhibitory effect of methanol sword bean extract on the growth of V. parahemolyticus was investigated. Growth of the strain occurred at the concentration of 0.5% extract and was inhibited continuously at 1.0 and 1.5% for 30hours after inoculation, whereas the strain was completely inhibited at 2.0% after 9hours of inoculation.

Characterization of Lactobacillus brevis JBE 30 as a Starter for the Brewing of Traditional Liquor (전통주 양조에 적합한 Lactobacillus brevis JBE 30 균주의 특성)

  • For the collection of starters suitable for the brewing of traditional liquor, an alcohol-resistant strain of lactic acid bacteria with low level of acid production was isolated from traditional fermented soybean lumps. The strain named as JBE 30 was identified as Lactobacillus brevis by 16S rRNA sequence analysis and additional biochemical tests. The strain could grow well at a MRS medium containing 8% (v/v) ethanol for 96 h of cultivation at $30^{\circ}C$. The final pH after cultivation was 4.5. It also inhibited the growth of food spoilage and pathogenic bacteria including Escherichia coli, Bacillus cereus, Listeria monocytogenes, Staphylococcus aureus, Micrococcus luteus, and Pseudomonas aeruginosa. These results showed that Lactobacillus brevis JBE 30 could be used as a promising starter in brewing process of traditional liquor.

Structure and physical properties of Earth Crust material in the Middle of Korean Peninsula(2) : Comparison between elastic Velocity and point-load of core specimen of sedimentary rocks. (한반도 중부권 지각물질의 구조와 물성연구(2) : 퇴적암류 코아시료의 탄성파 속도와 점재하 강도 비교)

  • In order to investigate the correlation of sedimentary rock properties. specific gravity, porosity, water content, sonic wave velodty, and point4oad strength index of core samples of limestones, sandstones and shales were measured. The relationships between density and velocity show $V_p=16300d-38719.3,{\;}V_s1896.4d-29225.1$ of regression equation for sandstones and $Vp=4085d-10264.8,{\;}V_s=3519d-7841.3$ for shales and <$Vp=4085d^2-20747d+303,{\;}V_s=3899d^2-21442d+318$ for limestones. Seismic wave velocity of shales which have high density is lower than that of sandstones, and this seems to be an effect of bedding in shale. P-wave velocity and S-wave velocity of limestones, sandstones and shales show the linear relationships as a whole. The regression equations are respectively calculated V_s=0.26V_p+1041.6m/sec,{\;}V_s=0.43V_p+424.2m/sec,{\;}and{\;}Vs=0.51V_p+261.9m/sec$ and the correlation coefficients of the velocity show r= 0.86 in sandstones, r= 0.75 in limestones and r=0.86 in shales. According to the point4oad strength test for limestones, point4ord strength anisotropy was not so dear even though the specimens show generally the banded structure. Variations of dip angle of bedding whihin the range $30^{\circ}-60^{\circ}$ does not have much influence upon the diametral strength index and axial strength index. From the result of point load test, P-wave velocity increases with point4ord strength index but the regression equations are $V_p=98.5lI{s_d}+4082.1m/sec,{\;}V_p=106.41{s_a}+3954m/sec$ and their correlation coefficient is low.
