Taxonomic Review of the Genus Saurida (Aulopiformes: Synodontidae) from Korea (한국산 매퉁이속(홍메치목, 매퉁이과) 어류의 분류학적 재검토)

  • Taxonomic review of the genus Saurida, belonging to the family Synodontidae, was conducted based on three species (S. macrolepis, S. microlepis and S. wanieso) collected from Korea between 1994 and 2017, and one species (S. elongata) loaned from Japan. S. wanieso can be readily distinguished from S. tumbil by following characteristics: outer palatine teeth (2 rows in S. wanieso vs. 3~4 rows in S. tumbil) and pectoral fin length (reaching to pelvic - dorsal line in S. wanieso vs. not reaching to pelvic - dorsal line in S. tumbil). In counts, the four Saurida spp. are well distinguished: number of lateral line scales (48~50 in S. macrolepis vs. 54~58 in S. wanieso vs. 61~62 in S. elongata vs. 63~70 in S. microlepis), vertebrae (47~49 in S. macrolepis vs. 51~53 in S. wanieso vs. 57~59 in S. elongata vs. 62~64 in S. microlepis). The present study suggests S. undosquamis and S. tumbil must be absent in Korean waters, and also S. elongata may be rare or absent in Korean waters. Here we use the Korean name, "tum-bil-mae-tung-i" for S. wanieso, following Yamada et al. (1995) who treated Korean S. tumbil as S. wanieso.

  • Backgrounds/Aims: Metastatic lesions of the pancreas (PMET) account for 1%-5% of all malignant solid pancreatic lesions (SPL). In this study we evaluated the utility of endoscopic ultrasonography with fine needle aspiration (EUS-FNA) in diagnosing PMET. Methods: Patients who underwent EUS-FNA at a community referral center between 2011-2017 for SPL were identified. Clinical, radiologic, and EUS-FNA features of those with PMET were compared to those with primary solid tumors of the pancreas: pancreatic adenocarcinoma (PDAC) and neuroendocrine tumors (PNET). Results: A total of 191 patients were diagnosed with solid pancreatic malignancy using EUS-FNA: 156 PDAC, 27 PNET, and eight (4.2%) had PMET. Patients with PMET were less likely to have abdominal pain (25.0% vs. 76.3% vs. 48.2%; p < 0.01) or obstructive jaundice (37.5% vs. 58.3% vs. 0%; p < 0.01) compared to PDAC and PNET. Those with PMET were more likely to have mass lesions with/without biliary or pancreatic ductal dilatations (100% vs. 86.5% vs. 85.2%; p < 0.01) and lower CA19-9 (82.5 ± 43.21 U/mL vs. 4,639.30 ± 11,489.68 U/mL vs. 10.50 ± 10.89 U/mL; p < 0.01) compared to PDAC and PNET. Endosonographic features were similar among all groups. Seven (87.5%) patients with PMET had a personal history of malignancy prior to PMET diagnosis. The primary malignancy was renal cell carcinoma in five PMET. Conclusions: PMET are exceedingly rare, comprising less than 5% of SLP. Patients with PMET are less likely to present with symptoms and mostly identified by surveillance imaging for the primary malignancy.

Paraoxonase 1 (PON1) Q192R Gene Polymorphism and Cancer Risk: A Meta-Analysis Based on 30 Publications

  • Common genetic variation Q192R in the paraoxonase 1 (PON1) gene has been considered to be implicated in the development of many cancers. Nevertheless, results from the related studies were inconsistent. To elucidate the association, we performed a meta-analysis for 8,112 cases and 10,037 controls from 32 published case-control studies. Odds ratios (ORs) with 95% confidence intervals (CIs) were used to assess the strength of the association by STATA 12.0 software. Overall, we revealed that the PON1-192R allele was associated with a reduced risk of the overall cancers. Moreover, in the stratified analysis by cancer types (breast cancer, prostate cancer, brain cancer etc.), the results showed that PON1-192R allele was associated with a decreased risk in breast cancer (R vs Q: OR=0.605, 95% CI=0.378-0.967, $P_{heterogeneity}=0.000$; RR vs QQ: OR=0.494, 95% CI=0.275-0.888, $P_{heterogeneity}=0.002$; RQ vs QQ: OR=0.465, 95% CI=0.259-0.835, $P_{heterogeneity}=0.000$; and RR+RQ vs QQ: OR=0.485, 95% CI=0.274-0.857, $P_{heterogeneity}=0.000$), and associated with prostate cancer in homozygote (RR vs QQ: OR=0.475, 95% CI=0.251-0.897, $P_{heterogeneity}=0.001$) and recessive models (RR vs RQ+QQ: OR=0.379, 95% CI=0.169-0.853, $P_{heterogeneity}=0.000$), while an increased risk was identified in lymphoma (R vs Q: OR=1.537, 95% CI=1.246-1.896, $P_{heterogeneity}=0.944$; RR vs QQ: OR=2.987, 95% CI=1.861-4.795, $P_{heterogeneity}=0.350$; RR+RQ vs QQ: OR=1.354, 95% CI=1.021-1.796, $P_{heterogeneity}=0.824$; and RR vs RQ+QQ: OR=2.934, 95% CI=1.869-4.605, $P_{heterogeneity}=0.433$), and an increased risk in prostate cancer under heterozygote comparison (RQ vs QQ: OR=1.782, 95% CI=1.077-2.950, $P_{heterogeneity}=0.000$) and dominant models (RR+RQ vs QQ: OR=1.281, 95% CI=1.044-1.573, $P_{heterogeneity}=0.056$). When subgroup analysis that performed by the control source (hospital based or population based), a decreased risk of the overall cancers was revealed by homozygote (RR vs QQ: OR=0.601, 95% CI=0.366-0.987, $P_{heterogeneity}=0.000$) and dominant models (RR vs RQ+QQ: OR= 0.611, 95% CI=0.384-0.973, $P_{heterogeneity}=0.000$) in hospital based group. Stratifying by ethnicity, a significantly reduced risk of the overall cancers under allele contrast model (R vs Q: OR=0.788, 95% CI=0.626-0.993, $P_{heterogeneity}=0.000$) was uncovered in Caucasian. In summary, these findings suggested that PON1 Q192R polymorphism was associated with a reduced risk of the overall cancers, nevertheless, it might increase cancer susceptibility of prostate and lymphoma risk. Large well-designed epidemiological studies will be continued on this issue of interest.

A Clinical Observation on 55 Cases of Neonatal Sepsis (신생아 패혈증에 관한 임상적 고찰)

  • Neonatal sepsis is a frequent and important cause of morbidity and mortality in the neonatal period. This study was undertaken to observe the 55 cases of proven neonatal sepsis among 6,717 newborn infants under 4 weeks of age, admitted to the nursery of Yeungnam University Hospital from May 1, 1983 to April 30, 1988. We observed following results : 1. The morbidity rate of male(1.12%) was higher than that of female(0.44%) significantly (p<0.05). 2. The morbidity rate of prematurity(2.65%) was higher than that of full term(0.76%) significantly (p<0.05). 3. The incidence of perinatal obstetric complications in early onset neonatal sepsis was higher than that of late onset neonatal sepsis (38.5% vs 10.3%, p<0.05). 4. The common clinical manifestations were poor feeding(52.7%), jaundice(45.5%), diarrhea(30.9%) and irritability (30.9%). 5. Among the causative organisms, gram positive organisms were predominated and Staphylococcus epidermidis was the most common organism. There was no difference in the causative organisms between early onset and late onset sepsis.

Continuous Mesophilic-Dry Anaerobic Digestion of Organic Solid Waste (유기성고형폐기물의 연속 중온 건식혐기성소화)

  • Continuous dry anaerobic digestion of organic solid wastes (30% TS, Total Solids) comprised of food waste and paper was performed under mesophilic condition. During the operation, hydraulic retention time (HRT) was decreased as follows: 150 d, 100 d, 60 d, and 40 d, which corresponded to the solid loading rate of 2.0, 3.0, 5.0, and 7.5 kg TS/$m^3$/d, respectively. Volumetric biogas production rate ($m^3$/$m^3$/d) increased as HRT decreased, and the highest biogas production rate of 3.49${\pm}$0.31 $m^3$/$m^3$/d was achieved at 40 d of HRT. At this HRT, high volatile solids (VS) reduction of 76% was maintained, and methane production yield of 0.25 $m^3$/kg $TS_{added}$ was achieved, indicating 67.4% conversion of organic solid waste to bioenergy. The highest biogas production yield of 0.52 $m^3$/kg $TS_{added}$ was achieved at 100 d of HRT, but it did not change much with respect to HRT. For the ease feed pumping, some amount of digester sludge was recycled and mixed with fresh feed to decrease the solid content. Recirculation volume of 5Q was found to be the optimal in this experimental condition. Specific methanogenic activity (SMA) of microorganisms at mesophilic-dry condition was 2.66, 1.94, and 1.20 mL $CH_4$/g VS/d using acetate, butyrate, and propionate as a substrate, respectively.

Nonsurgical transfer of fresh and frozen embryos of dairy cattle (젖소의 신성수정란의 동결수정란의 비외과적 이식에 관하여)

  • Sixty Four fresh and 142 frozen embryos of dairy cattle were transferred to synchronized dairy, beef or Korean Native Cattle nonsurgically at National Animal Breeding Institute from 1985 to 1990. The results obtained were as follows ; 1. The pregnancy rate of fresh embryos(39.1%) was higher than that of frozen embryos(32.4%) and average pregnancy rate was 34.5%. 2. The pregnancy rate of grade 1 embryos was higher than that of grade 2 embryos for both fresh(41.3% vs 33.3%) and frozen embryos(35.4% vs 25.6%). 3. The pregnancy rate according to development stage of fresh embryos was increased with maturity as 29.2%, 33.3%, 50.0% and 54.5% for morula, early blastocyst, blastocyst and expanded blastocyst, respectively. For frozen embryos, the pregnancy rate of blastocyst(44.4%) was higher than those of morula(31.3%) and early blastocyst(28.0%). 4. The pregnancy rate according go recipient-donor synchrony for fresh embryos was higher when the recipients exhibited estrus 1 day earlier than the donors(43.8%) than when the recipients exhibited estrus 1 day later than the donors(38.1%) or when the recipients and donors exhibited estrus at the same time(37.0%). For forzen embryos, the pregnancy rate was decreased when the recipients and donors exhibited estrus at the same time(37.9%), when the recipients exhibited estrus 1 day later than the donors(32.0%) and when the recipients exhibited estrus 1 day earlier than the donors(23.5%), in sequence. 5. The pregnancy rate of heifers was higher than that of cows for both fresh(50.5% vs 37.9%) and frozen embryos(39.7% vs 25.7%). 6. The pregnancy rate according to recipient breed for fresh embryos was higher in dairy cattle(42.1%) and beef cattle(40.%) than in Korean Native Cattle(33.3%). For frozen embryos, the pregnancy rate was decreased beef cattle(39.1%), dairy cattle(30.3%) and Korean Native Cattle(14.3%), in sequence. 7. The pregnancy rate according to equilibrium steps of glycerol and freezing rate was higher when transferred after 3-steps equilibrium and freezing by the rate of $0.3^{\circ}C$/min from $-6^{\circ}C$ to $-35^{\circ}C$ and $0.1^{\circ}C$/min to $-38^{\circ}C$(39.4%) than when transferred after 6-steps equilibrium and freezing by the rate of $0.5^{\circ}C/min$ from $-6^{\circ}C$ to $-30^{\circ}C$(30.3%).

Evaluation of Pharmacotherapy for the Patients with Depression in Alzheimer's Disease, Parkinson's Disease or Vascular Dementia (알츠하이머병, 파킨슨병 및 혈관성치매 환자들의 우울증에 대한 약물 치료 평가)

  • Background: Prevalence of depression comorbid with neurologic disorders such as Alzheimer' disease (AD), Parkinson's disease (PD) and vascular dementia (VD) is higher than that of primary depression. Antidepressant medications, suggested by many researches for depression comorbid with neurologic disorders such as AD, PD and VD, are mainly selective serotonin reuptake inhibitors (SSRI). Objective: The primary objective of this study is the evaluation of antidepressant drug therapy for AD, PD and VD. Method: This study was a retrospective study based on medical records, carried out for 3 years and 6 months (Jan. 2007~Jul. 2010). Patients, diagnosed as comorbid depression through Beck Depression Inventory (BDI), Cornell Depression Scale (CDS), Geriatric Depression Scale (GDS) among neurologic out-patients of Chungnam National University Hospital because of AD, PD and VD, were selected. The results were evaluated by efficacy and safety of antidepressant drug therapy. Results: In result, the prescribing rates of antidepressants were 30%, 55% and 40% for each AD, PD and VD. Depression cure rates of patients receiving antidepressants vs patients not receiving antidepressants were 40% vs 39%, 33% vs 23% and 38% vs 30% for AD, PD and VD. The frequencies of prescriptoin of SSRI were 21%, 11% and 27% for each AD, PD and VD. The frequencies of prescriptoin of benzodiazepine (BZD) was 61%, 82% and 61% for each AD, PD and VD. The ratio of single BZD prescription was more than that of combination prescription of antidepressants. Tricyclic antidepressants (TCA) were rarely prescribed. The rate of patients with BZD-related side effects was 54%. The most frequent side effects of BZD were dizziness (30%), drowsiness (21%) and headache (16%). Side effects of SSRI were rare. Conclusion: In conclusion, the frequencies of prescription of antidepressants were not common for AD, PD and VD. There was little difference in depression cure rate between patient receiving antidepressants and not receiving. Even though SSRI has to be the highest priority of usage, the frequencies of prescription of SSRI were lower than those of BZD. Additional researches and efforts are required to improve antidepressant drug therapy for neurologic disorders such as AD, PD and VD.

Survivorship Analysis in Asymptomatic COVID-19+ Hip Fracture Patients: Is There an Increase in Mortality?

  • Purpose: Mortality rates following hip fracture surgery have been well-studied. This study was conducted to examine mortality rates in asymptomatic patients presenting for treatment of acute hip fractures with concurrent positive COVID-19(+) tests compared to those with negative COVID-19(-) tests. Materials and Methods: A total of 149 consecutive patients undergoing hip fracture surgery during the COVID-19 pandemic at two academic medical centers were reviewed retrospectively. Patients were divided into two groups for comparative analysis: one group included asymptomatic patients with COVID-19+ tests versus COVID-19- tests. The primary outcome was mortality at 30-days and 90-days. Results: COVID-19+ patients had a higher mortality rate than COVID-19- patients at 30-days (26.7% vs 6.0%, P=0.005) and 90-days (41.7% vs 17.2%, P=0.046) and trended towards an increased length of hospital stay (10.1±6.2 vs 6.8±3.8 days, P=0.06). COVID-19+ patients had more pre-existing respiratory disease (46.7% vs 11.2%, P=0.0002). Results of a Cox regression analysis showed an increased risk of mortality at 30-days and 90-days from COVID-19+ status alone without an increased risk of death in patients with pre-existing chronic respiratory disease. Conclusion: Factors including time to surgery, age, preexisting comorbidities, and postoperative ambulatory status have been proven to affect mortality and complications in hip fracture patients; however, a positive COVID-19 test result adds another variable to this process. Implementation of protocols that will promote prompt orthogeriatric assessments, expedite patient transfer, limit operating room traffic, and optimize anesthesia time can preserve the standard of care in this unique patient population.

Effect of Dang Gui Bo Hyul-tang on fatigue types of Mibyeong (피로형 미병에 대한 당귀보혈탕의 효과)

  • Objectives : Mibyeong is a korea medicine have original concept of the disease. However, no previous report has effect of mibyeong herbal medicine in fatigue types of mibyeong. This study investigated the question of whether Dang Gui Bo Hyul-tang(DGBHT) of effect on fatigue types of Mibyeong. Methods : C57BL/6 mice were randomaly divided into three group (n=10). The mice were then group (1) Nocontrol, (2) Restraunt stress(veh), (3) Dang Gui Bo Hyul-tang 200mg/kg. The administered Dang Gui Bo Hyul-tang 200mg/kg or distilled water (orally) 1 hr prior to daily exposure to repeated restraint stress (2 h) for 15 days. The performed behavior test (Mechanical hyperalgesia test,, Open-field test, Forced swimming test, Sucrose preference test and immunostaining and biochemical measured in serum. Results : Stress fatigue induced mices significantly increased lethargic, hyperalgesia through behavior test (Mechanical hyperalgesia test (decrease 43%), Open-field test ($4,809{\pm} 226.13cm$ vs. $3121{\pm}226.64cm$), Forced swimming test ($11.45{\pm}3.96$ vs. $79.10{\pm}8.12sec$), Sucrose preference test (decrease 58%)). In addition, chronic fatigue model evidently increased corticosterone level ($122.54{\pm}18.88$ vs. $186.94{\pm}18.26ng/ml$), AST level ($46.22{\pm}3.23$ vs. $31.40{\pm}3.86U/L$), ALT level ($38.78{\pm}5.72$ vs. $17.60{\pm}1.30$), liver necrosis, lateral ventricle size. These alterations were significantly ameliorated by DGBHT. DGBHT significantly attend the elevated serum concentrations of corticosterone ($155.90{\pm}6.29ng/ml$), AST ($31.40{\pm}3.86U/L$), ALT ($17.60{\pm}1.30U/L$). Moreover, DGBHT improved lethargic, hyperalgesia when compared the stress fatigue (Mechanical hyperalgesia test (improve 28%), Open-field test ($4,038{\pm}615.81cm$), Forced swimming test ($7.56{\pm}1.88sec$), Sucrose preference test (increase 21%) Conclusions : Theses result suggest that DGBHT have improved lethargic, hyperalgesia and fatigue-associated hormone and liver protective on stress fatigue model. It will be necessary to research to present evidences on benefits and effects of Korean medical treatment for Mibyeong through clinical researches based on benefits and effects of those animal models.

The Study on Comparison of Clinical Outcomes of Intracytoplasmic Sperm Injection in Patients with Epididymal Sperm and Testicular Sperm (난자 세포질 내 정자 주입술시 부고환 및 고환 정자의 체외수정능력에 관한 비교 연구)

  • Objective: This study was carried out to compare the clinical outcomes of intracytoplasmic sperm injection (ICSI) in patients with obstructive azoospermia according to sperm retrieval site and technique; microsurgical epididymal sperm aspiration (MESA), percutaneous epididymal sperm aspiration (PESA), testicular sperm extraction by open biopsy (TESE). Methods: The outcomes of ICSI and IVF-ET were evaluated and compared among 3 groups. Seventy three men suffering from infertility due to obstructive azoospermia had 107 ICSI cycles using MESA (21 cycles in 15 patients), PESA (26 cycles in 17 patients) and TESE (60 cycles in 41 patients). Results: In the clinical outcomes in patients undergoing ICSI with epididymal or testicular sperm, there were no significant differences in fertilization rate (66.1% vs. 60.5%), cleavage rate (94.9% vs. 97.6%), cumulative embryo score (CES) (51.3 vs. 58.8), implantation rate (7.9% vs. 6.1), and clinical pregnancy rate per ET (30.4% (14/46) vs. 25.4% (15/59)) between both groups. Also, in the clinical outcomes in ICSI patients using MESA, PESA, TESE, there were no significant differences in fertilization rate (61.8%, 69.4%, 60.5%), cleavage rate (92.1%, 97.3%, 97.6%), CES (38.1, 52.0, 58.8), implantation rate (9.5%, 6.6%, 6.1%), and clinical pregnancy rate per ET (35% (7/20), 26.9% (7/26), 25.4% (15/59)) among 3 groups. Conclusion: When compared with MESA or TESE, PESA, the clinical outcomes were similar in ICSI patients with obstructive azoospermia whatever the origin or the technique of sperm retrieval. However, we considered PESA is more time-saving and cost effective for ICSI in patients with obstructive azoospermia.
