추간관절 증후군의 치료 (Treatment of Facet Joint Syndrome)

  • The common disorder called facet syndrome exhibits lower back pain, with or without, radiating pain to buttock and thigh due to facet joint arthropathy. Many physicians have believed that the usual lesion of facet syndrome was an anatomical impairments of facet joint itself. So facet joint block has been known only as a therapeutic and diagnostic modality of facet syndrome. Based on clinical experience and anatomical study, we have concluded varying opinions from common sense about facet syndrome. Pain in the facet joint is supposedly the secondary effect of narrowing of joint space by sustained muscle contracture around joints. We therefore conclude that spasmolytic treatment of muscles connecting the two vertebral articular space would be better for treatment and diagnosis of facet syndrome rather than facet block with local anesthetic and steroid only.

좌측 하지의 관절구축을 동반한 작열통의 치험 (A Case of Causalgia with Contracture Deformity in Lower Extremity)

  • Causalgia is a symptom complex usually consisting of burning pain, hyperesthesia and atrophy of the involved extremity. The pain may be aggravated by contact, motion of extremity or emotional excitement. It occurs following incomplete nerve injury. The patient was a 58-year-old male with a 3-year history of causalgic pain of left lower extremity. He had multiple fractures with degloving injury of left lower extremity in an automobile accident. The pain was exacerbated by movement, and he experienced knee joint and ankle joint contracture. The patient's pain decreased after continuous epidural block with 1% lidocaine and 0.25% bupivacaine. He also received lumbar sympathetic ganglion alcohol block resulting in much improvement of level of pain and walking disturbance.

Case of late hematoma after breast augmentation

  • We present a case report of a patient who experienced a late, spontaneous breast hematoma 26 years after primary breast augmentation. Late hematomas are a rare complication of breast augmentation with uncertain etiology. In this case, there was no trauma, calcifications, or implant rupture. We believe the patient's hematoma was secondary to erosion of a capsular vessel due to capsular contracture.

  • Equinus contracture of the gastrocnemius-soleus complex is associated with many foot and ankle diseases. We introduce a new simple technique that can be performed using a minimal incision with simple devices. Briefly, 0.5 cm sized medial and lateral longitudinal skin incisions are made at the level of the distal one third of the calf. The musculotendinous junction of the gastrocnemius is gently transected using an Arachnoid blade. The skin is, then, closed with non-absorbable sutures.

Surgery Requiring Detailed Preoperative Simulation and Scar De-epithelialization to Repair Severe Postoperative Scarring from Gastroschisis

  • We present the case of a patient with severe postoperative scarring from surgical treatment for gastroschisis, with the intestine located immediately under the dermal scar. Although many patients are unsatisfied with the results of scar repair treatment, few reports exist regarding severe or difficult cases involving the surgical repair of postoperative scar contracture. We achieved an excellent result via simulation involving graph paper drawings that were generated using computed tomography images as a reference, followed by dermal scar deepithelialization. The strategy described here may be useful for other cases of severe postoperative scar contracture after primary surgery for gastroschisis.

Treatment of Quadriceps Contracture with Femoral Shortening Ostectomy, Rectus Femoris Muscle Transposition and Dynamic Stifle Flexion Apparatus in a Dog

  • A 13-month-old, 3.3 kg castrated male Shih-tzu presented with right hindlimb lameness. The physical examination revealed atrophy of the right thigh muscles, hyperextension of the stifle joint and external torsion of the tibia. On the radiographic examination, patella alta and genu recurvatum were observed. A biapical deformity of the tibia and external torsion of the distal tibia were detected by computed tomography (CT). A three-dimensional (3D) printed bone model was designed and constructed for the preoperative plan prior to surgery. Rectus femoris muscle transposition, femoral shortening ostectomy and open wedge osteotomy of the distal tibia were performed using hybrid external skele/t0al fixation (hybrid-ESF). A dynamic stifle flexion apparatus was used to prevent recurrence of a quadriceps contracture (QC). Intense physiotherapy was administered postoperatively. The dog began to use the affected limb one week after surgery. Functional improvement in the affected limb was observed, and full weight-bearing was possible at 3 months after surgery. Union of the osteotomy lines was observed at 3 months, and the stifle joint was fully movable at 7 months after surgery. Regarding the treatments for QC, these methods may be excellent candidates, as they do not lead to severe damage to the limb or amputation.

The efficacy of dermofat grafts from the groin for correction of acquired facial deformities

  • Background: Posttraumatic acquired facial deformities require surgical treatment, with options including scar revision, fat grafts, implant insertion, and flap coverage. However, each technique has specific advantages and disadvantages. Methods: From 2016 to 2018, 13 patients (eight with scar contracture and five with a depressed scar) were treated using dermofat grafts from the groin. The harvested dermofat was then inserted into the undermined dead space after the contracture was released, and a bolster suture was done for fixation considering the patient's contour and asymmetry. A modified version of the Vancouver Scar Scale and satisfaction survey were used to compare deformity improvements before and after surgery. Results: In most cases, effective volume correction and an aesthetically satisfactory contour were maintained well after dermofat grafting, without any major complications. In some cases, however, lipolysis proceeded rapidly when inflammation and infection were not completely eliminated. A significant difference was found in the modified Vancouver Scar Scale before and after surgery, with a p-value of 0.001. The average score on the satisfaction survey was 17.07 out of 20 points. Conclusion: A dermofat graft with the groin as the donor site can be considered as an effective surgical option that is the simplest and most cost-effective method for the treatment of acquired facial deformities with scar contracture.

안면마비 후유증 및 평가 방법에 대한 고찰 (A Study of Facial Palsy Sequelae and Evaluating Scale)

  • Objectives : The purpose of this study is research on facial palsy sequelae and evaluating scale that have studied insufficiently until now. Methods : We researched on the symptoms, epidemiology and evaluating scale of facial palsy sequelae. For this, we searched the research papers on facial palsy sequelae and the clinical papers that find out the effect of treatment by evaluating facial palsy sequelae. Results : The symptoms of facial palsy sequelae are synkinesis, contracture, spasm, crocodile tears syndrome, tearing decrease, gustation impairment, hearing impairment, tinnitus, hyperacusis, etc. Among these, synkinesis, contracture, spasm and crocodile tears syndrome are the most frequently observed broadly. The poor prognosis factor of facial palsy can be the risk factor of facial palsy sequelae. For example, severe degeneration of facial nerve can be the risk factor of facial palsy sequelae. Most of clinical papers on facial palsy sequelae have used NRS(numeric rating scale) as evaluating scale. But NRS is very subjective scale. The scales of Stennert, Peitersen, Murata et al. can evaluate facial palsy sequelae grossly. Sunnybrook scale, Sydney scale, SAQ(synkinesis assessment questionnaire), the scale of Kim, the scale of Scott, HFS-7(hemi facial spasm), HFS-36 and Schirmer's test can evaluate the respective symptoms of facial palsy sequelae. Conclusions : The symptoms of facial palsy sequelae are synkinesis, contracture, spasm, crocodile tears syndrome, etc. Most of clinical papers on facial palsy sequelae have used NRS as evaluating scale. There were some scales that can evaluate facial palsy sequelae grossly and respectively. In future, we will need more progressed study of facial palsy sequelae and evaluating scale.

토끼 심방근 및 혈관 평활근에서의 $Na^{+}/Ca^{2+}$ 교환기전에 관한 연구 ($Na^{+}/Ca^{2+}$ Exchange System in Atrial Trabeculae and Vascular Smooth Muscle of the Rabbit)

  • In order to elucidate the regulatory mechanism of intracellular calcium ion concentrations, contractions or contractures induced by $Na^{+}-removal$, calcium-application or ouabain-treatment as an index of $Na^+/Ca^{2+}$ exchange activity were studied in atrial muscle or vascular smooth muscle (aorta and renal artery) of the rabbit. The magnitude of low sodium contractures in atrial trabeculae increased with sigmoid shape when external sodium concentrations were reduced to sodium-free condition, whereas that of calcium contracture intensified in a parabolic pattern when external calcium concentrations were elevated to 8 mM. $Na^{+}-removal$ contractures were induced in a duration-dependent manner to $K^{+}-free$ exposure and same findings were observed with ouabain treatment. $Na^{+}-free$ contractures were not affected by verapamil treatment, but stimulated by $100{\mu}M\;Mn^{2+}$ and inhibited by high concentrations of $Mn^{2+}\;(2{\sim}8mM)$ in a dose-dependent manner. Ryanodine which is known to suppress the release of calcium from internal store abolished spontaneous twitch contractions induced by $K^{+}-free$ solution, but had no effect on the development $Na^{+}-free$ contractures. Na-free contractures were not always induced in vascular smooth muscle preparations. Contractures by $O\;mM\;Na^+$ were usually seen in aorta, but not often in renal artery.$50\;mM\;K^+$, noradrenaline (NA) and angiotensin II (AII) always evoked very large contraction in all preparations of vascular smooth muscle. Contractures developed by $O\;mM\;Na^+$ were not sensitive to verapamil treatment as in atrial trabeculae, but were abolished by $100{\mu}M\;Mn^{2+}$. In contrast to $Na^{+}-free$ contractures, $Mn^{2+}(100{\mu}M)$ had no effect on the contractures induced by NA or 50 mM$K^+$. Caffeine in the concentration of 10 mM evoked transient contracture in the distal renal artery. The rate of spontaneous relaxation in caffeine contracture was dependent upon the concentrations of external sodium, and had double component of relaxation when the rate of relaxation was plotted in the semilogarithmic scale of relative tension versus time. Especially late components of relaxation had more direct relation to $Na^+$ concentrations. It could be concluded that $Na^+/Ca^{2+}$ exchange mechanism in the heart has a large capacity, inhibited by $Mn^{2+}$ but not by verapamil and ryanodine, while $Na^+/Ca^{2+}$ exchange system in vascular smooth muscle has a very low capacity especially in small artery, inhibited by low concentration of $Mn^{2+}\;(100{\mu}M)$ but not affected by verapamil and ryanodine.

장무지 굴곡근의 혈관성 기형에 동반된 발가락의 굴곡 변형: 증례 보고 (Vascular Malformation of Flexor Hallucis Longus Muscle Associated with a Flexion Deformities of Toes: A Case Report)

  • 혈관성 기형은 주로 무통성 종괴나 종창과 통증을 수반한 종괴의 형태로 나타난다. 드문 경우에서, 근육에 발생한 혈관성 기형은 침범한 근육의 위축을 유발하여 관절의 변형을 초래한다. 혈관성 기형이 하퇴부의 굴곡근에 발생한 경우 발목의 첨족 변형을 유발한다. 그러나, 발가락의 장족지 또는 장무지 굴곡근에 혈관성 기형이 발생하여 발가락의 굴곡 변형을 초래한 것에 대한 문헌 보고의 예는 없다. 이에 저자들은 40세 여자 환자에서 장무지 굴곡근에 발생한 혈관성 기형으로 인해 유발된 발가락의 굴곡 변형을 경험하고 수술적 절제술을 통해 치료하였기에 문헌 고찰과 함께 보고하는 바이다.
