  • $^{99m}$Tc Ferric Hydroxide Macroaggregates for Lung Perfusion Studies were prepared from home made Na $^{99m}$TcO$_4$ which was extracted by methyl ethyl ketone from low activity $^{99m}$Mo. Particle size was in between 20 and 60$\mu$. Rabbit and human body tests gave excellent results.sults.

The Effects of Dead Time and Its Correction Methods for Thyroid Prode using $^{99m}Tc$, $^{123}I$, $^{131}I$ ($^{99m}Tc$, $^{123}I$, $^{131}I$을 이용한 갑상선 탐침의 계수 불능시간영향의 특성과 그의 보정 방법에 관한 연구)

  • The purpose of this study was to examine the dead time effects and derive the correction factor. Using the thyroid probe and lucite cylindrical phantom, $^{99m}Tc$ 10.50mCi and $^{123}I$ 2.08mCi were counted with medical spectrometer at intervals of 2 hours for 43hrs and 79 hours. respectively. $^{123}I$ 2.06mCi was counted at intervals of 6 hours for 910 hours. To measure the starting point of dead time effect, the radioactivity was measured with dose calibrator in each time. The dead time effects started at about 0.80mCi at all distances for $^{99m}Tc$, and about 1.00mCi for $^{123}I$. The radioactivity corresponding to 20% counts loss is 1.29(center), 1.28(2cm), 1.31(4cm), 1.13(6cm)mCi for $^{99m}Tc$ and 1.39mCi for $^{123}I$. The correction factors for 2mCi of radioactivity as an example were 1.52(center), 1.52(2cm), 1.50(4cm), 1.58(6cm) for $^{99m}Tc$ and 1.58 for $^{123}I$.

Studies on the Preparation of $^{99m}TC$ Labelled Antimony Sulfide Colloid and Hydroxyethyl Starch for Lymphoscintigraphy (림포신티그래피용 $^{99m}TC$를 표지황화안티몬 콜로이드 및 전분의 제조에 관한 연구)

  • For the development of $^{99m}Tc-labelled$ antimony sulfide colloid and hydroxyethyl starch, various experiments such as preparation of colloid, control of the distribution of particle size, establishment of labelling conditions, determination of labelling yield and radiochemical purity, examination of stability, and organ imagings of rabbits etc. were carried out. 1) Antimony sulfide colloid was readily prepared by the reaction of aqueous solution of antimony potassium tartrate with hydrogen sulfide generated by treating ferrous sulfide with dilute sulfuric acid. The colloid could be stabilized by adding small amount of polyvinylpyrrolidone. 2) Electron microscopy analysis exhibited the distribution of colloid size in the range of $1\sim15nm$ with a major portion of 9 m. The colloid solution was sterilized by membrane filtration $(0.2{\mu}m)$ and then stored at $4^{\circ}C$. This sterilized colloid was so stable that it was usable at least for one year. 3) The antimony sulfide colloid was labelled by adding sodium $pertechnetate-^{99m}Tc$ solution to the reaction vial, followed by adding hydrochloric acid and then boiled for 30 min. The optimal pH of the reaction mixture was found to be in the range of $1.3\sim1.4$. Instant thin layer chromatography (ITLC) analysis showed high labelling yield of above 99.5%. This labelled colloid maintained high radio-chemical purity of above 99% until 10 hours after labelling. 4) Animal studies showed high uptake of $^{99m}Tc-Sb_2S_3$ colloid at lymph vessels and nodes indicating a suitable agent for lymphoscintigraphy. Satisfactory results were also abtained in other clinical studies. 5) Hydroxyethyl starch (HES $0.6\sim1.0%$) was labelled with $Na^{99m}TcO_4$ in the presence of $SnCl_2$ with high labelling yield of above 99.5%. The optimal pH of the reaction mixture was in the range of $1.8\sim2.0$. $^{99m}Tc-HES$ maintained high radiochemical purity of above 99% until 10 hours after labelling. 6) Animal studies showed that $^{99m}Tc-HES$ migrated more rapidly from the injection sites into the lymph vessels than $^{99m}Tc-Sb_2S_3$ colloid while less amount of the former was uptaken at lymph nodes than that of the latter. Similar phenomenon was also observed in other clinical studies. As a result, $^{99m}Tc-Sb_2S_3$ colloid was found to be more effective lymphoscintigraphic agent than $^{99m}Tc-HES$.

A Study on Comparative Analysis of Mammography and Tc-99m MIBI Scintimammography for Dense Breast (치밀형 유방에서 유방특이감마영상검사의 유용성 평가)

  • Purpose: This study was to evaluate usefulness of $^{99m}Tc$-MIBI scintimammography for dense breast by comparing concordance in test results between Tc-99m MIBI scintimammography and mammography whose effect was proved the most as an imaging tool depending on breast density and at the same time by examining limitation on evaluation depending on density of breast tissue. [Materials and Methods] In the period from December 2010 to July 2011, this study targeted 150 patients who took both of $^{99m}Tc$-MIBI scintimammography and mammography conducted by using breast gamma camera in this hospital. Breast density was classified to the four levels of pattern 1~4 based on the results of mammography. $^{99m}Tc$-MIBI scintimammography was conducted with the LCC, the RCC, the LMLO, and the RML one minute after intravenous injection of 99mTc-MIBI 7400 MBq (20 mCi) while analysis was made for concordance in test results of $^{99m}Tc$-MIBI scintimammography and mammography. [Results] Among the 150 patients, pattern 1 was found in 3 patients, pattern 2 in 44 patients, pattern 3 in 61 patients, and pattern 4 in 37 patients. There were 5 patients who showed the case where it was impossible to determine density of breast tissue due to foreign body inserted to breast. The concordance ratio of the results between $^{99m}Tc$-MIBI scintimammography and mammography was 95.5% for pattern 2, 95.1% for pattern 3 and 94.6% for pattern 4. This demonstrated that the concordance rate decreased according to the increase in breast density. [Conclusion] When there was limitation on evaluation of breast specific gamma imaging test results due to increased intake in breast tissue or surgical site, the concordance rate was 6.8% for pattern 2, 16.3% for pattern 3 and 18.9% for pattern 4. This demonstrated that the degree of limitation on evaluation of breast specific gamma imaging test results increased according to the increase in breast density.

Estimation of the Arterial Fraction of Total Hepatic Flow from Radionuclide Angiogram Using $^{99m}Tc-DISIDA$ ($^{99m}Tc-DISIDA$를 이용한 간 혈류 중 동맥 분획의 측정)

  • Arterial fraction of total hepatic blood flow was estimated by a new method, slope method, on radionuclid angiogram using $^{99m}Tc-DISIDA$ and was compared with that from $^{99m}Tc-Phytate$ radionuclide angiogram. This study included 11 of normal subjects, 37 of intermediate group with various liver diseases, and 25 patients with liver cirrhosis. We analyzed the datas with slope method from radionuclide angiograms and the results were compared with hepatic arterial fractions from uptake method, introduced by Lee et al. at 1986. The hepatic arterial fractions from radionuclide angiograms using $^{99m}Tc-DISIDA$ and $^{99m}Tc-Phytate$ were $0.32{\pm}0.09\;and\;0.31{\pm}0.11$ respectively in normal subjects, and $0.75{\pm}0.18\;and\;0.77{\pm}0.21$ respectively in patients with liver cirrhosis. The hepatic arterial fractions by the slope method was well correlated with those of the uptake method on $^{99m}Tc-DISIDA$ scan. There was high correlation between the hepatic arterial fractions from $^{99m}Tc-DISIDA$ and $^{99m}Tc-Phytate$ scans. Hepatic arterial fraction estimated by the slope method is a useful index for the diagnosis of liver cirrhosis and the evaluation of status of portal hypertension.

  • Avidin과 biotin의 높은 결합력을 이용하여 종양 영상을 개선하는 방법이 많이 연구되고 있다. 본 실험에서는 이러한 목적으로 쓰기 위하여 적당한 $^{99m}Tc$ 표지 avidin을 제조하였다. Avidin을 표지하기 위하여 우선 $^{99m}Tc$과 안정한 킬레이트를 형성할 수 있는 benzoylmercaptoacetyltriglycine (Bz-MAG3)과 biocytin을 화학적으로 결합시킨 Bz-MAG3-biocytin을 합성하였다. 이 화합물을 Tc-99m으로 표지시켜 avidin 또는 streptavidin을 1:1로 섞어 줌으로서 Tc-99m으로 표지된 avidin과 streptavidin을 제조하였다. 이들의 생쥐 생체내 분포를 조사한 결과 avidin의 경우 높은 간(56.6%, 10min)과 신장(28.5%, 10min) 축적을 보였고 streptavidin의 경우 높은 신장 축적 (28.9%, 21hr)을 보였다. Avidin의 높은 정상 조직 축적을 줄이기 위하여 succinic acid를 결합시켜 등전점(pI)을 낮춘 다음 같은 실험을 하여 본 결과 신장 축적율은 pI가 $7.0{\sim}9.3,\;5.5{\sim}6.2,\;4.0{\sim}4.8$로 낮아졌을 경우 19.0%, 3.1%, 1.7%로 각각 떨어졌지만 간에의 축적은 pI 변화에 따른 상관성을 찾아 볼 수가 없었다. 체내 제거율을 측정하여 본 결과 pI를 변화시킨 avidin과 변화시키지 않은 avdin들은 반감기가 13.5에서 16.0시간 사이로 큰 차이점을 보이지 않았는데 streptavidin은 반감기 61.5시간 정도로 느리게 제거된다는 것을 알았다. 이 실험의 결과 1. Avidin을 $^{99m}Tc$-MAG3-biocytin으로 안정하게 표지할 수 있었고, 2. pI가 낮아진 avidin은 신장에의 축적율이 크게 감소되었으며, 3. $^{99m}Tc$으로 표지된 avidin과 streptavidin은 먼저 간으로 흡수된 후 대사된 다음 신장으로 배설된다는 사실을 알았다.

  • In patients with chronic liver disease, hepatopulmonary syndrome, the right to left shunt without previous pulmonary and cardiac disease, can develope and cause respiratory distress. Tools to prove shunt are contrast echocardiography, pulmonary angiography, and $^{99m}Tc-MAA$ perfusion lung scan. Among them, $^{99m}Tc-MAA$ scan is a simple and safe method detecting the right to left shunt. At the same time, quantitation of shunt amount is possible by this method. We report a case of hepatopulmonary syndrome confirmed by $^{99m}Tc-MAA$ scan and contrast echocardiography with review of literlatures.

  • Tetrofosmin is a ligand that forms a lipophilic, cationic complex with Tc-99m. Tc-99m tetrofosmin was developed as a myocardial perfusion imaging agent and also used to depict tumors. Mediastinal tumors is also detected by Tc-99m tetrofosmin. We report a case of extracardiac mediastinal activity detected by Tc-99m tetrofosmin scintigraphy, which revealed thymoma.

Influence of Iodinated Magnetic Resonance Contrast Media and Isotope 99mTc on Changes of Computed Tomography Number

  • The purpose of the study was to identify how isotope and magnetic resonance imaging (MRI) contrast media impact on noise to computed tomography (CT) examination. For the study, divide the phantoms to two groups: 1) saline, saline + different kinds of contrast agent without $^{99m}Tc$ administration; 2) $^{99m}Tc$ administration: saline, saline + different kinds of contrast agent with $^{99m}Tc$ administration. CT contrast agent was used for Iopamidol$^{(R)}$ and Dotarem. And MRI contrast agent was used for Primovist$^{(R)}$ and Gadovist$^{(R)}$. To obtain an image, we used CT scanner. With an obtained image, we set the $1cm^2$ region of interest in the middle of bottle to measure the noise and CT number. As a result, there was no difference in CT number before and after inserting $^{99m}Tc$ into all contrast media including Normal Saline. However, when it comes to Noise, there was a difference before and after inserting $^{99m}Tc$ into every contrast media except MRI contrast media such as Primovist$^{(R)}$ and Gadovist$^{(R)}$.

[ $^{99m}Tc-DISIDA$ ] Scintigraphic Findings of Hepatocellular Carcinoma (간세포암의 $^{99m}Tc-DISIDA$ 신티그램 소견)

  • It is well known that $^{99m}Tc-sulfur$ colloid or phytate hepatic scintigraphy is highly sensitive but not specific. Both $^{99m}Tc-DISIDA$ and bilirubin have been shown to share the same anionic transport pathway in the liver. Hepatocellular carcinoma(HCC) retains the ability to produce bile but has marked limitation to excreting it resulting in accumulation of bile within the tumor cells. Based upon such a fact, $^{99m}Tc-DISIDA$ hepatobiliary scintigraphy is used for the diagnosis of HCC. The present communication deals with our experience of DISIDA scintigraphic exploration of 9 cases of HCC in a retrospective way. We have made an observation on intensity of positive radio nuclide accumulation in the cold area of HCC as it is demonstrated by phytate scintigraphy. In addition we have semi quantitatively analyzed time-activity pattern and the following results were obtained. (1) All of 9 cases showed an increased uptake of $^{99m}Tc-DISIDA$ in delayed scintigrams. Of these 5 cases showed accumulation less than, 3 equal to, 1 more than the surrounding liver tissue. (2) The mean of the first appearing time of $^{99m}Tc-DISIDA$ activity in tumoral region was 2 hours and 20 minutes. (3) DISIDA scintigraphy provides us with positive informations of diagnostic value.
