Therapeutic Relationship Formation in Korean Older Adult Counseling: A Meta-Ethnographic Synthesis and a Tentative Proposal of the Relational Self-Actualization Model

Abstract

This study synthesizes factors shaping therapeutic relationship formation in Korean older adult counseling and proposes the Relational Self-Actualization Model. Methods: Following PRISMA 2020, 30 domestic studies on Korean older adults (2000?2025) were analyzed using seven-phase meta-ethnography. Findings: Inter-rater reliability was high (κ = .84, ICC = .92). Five main categories (developmental tasks, sociocultural context, counselor competence, therapeutic process, approach-specific effectiveness) and 21 subcategories were derived. The therapeutic relationship convergently developed through five stages: cautious assessment, tentative disclosure, deep connection, pursuit of reciprocity, and internalization. Korean cultural characteristics operated through three dimensions: relational values, spiritual resources, and communal solidarity. Application/Improvements: This study extends geriatric counseling theory developmentally and clinically, providing foundations for scale development.

본 연구는 한국 노인상담에서 치료적 관계 형성 요인을 체계적으로 합성하고 관계적 자아실현 모델을 제안하는 것을 목적으로 한다. PRISMA 2020에 따라 2000~2025년 8개 DB에서 한국 노인 대상 30편을 선정해 메타민족지학 7단계로 분석하였다. 분석 결과(κ = .84, ICC = .92) 5개 주요범주(발달적 과업, 사회문화적 맥락, 상담자 역량, 치료적 과정, 접근법별 효과성)와 21개 하위범주가 도출되었으며, 치료적 관계는 신중한 사정·시험적 개방·깊은 연결·상호성 추구·내재화의 5단계로 수렴적으로 발전하였다. 한국 노인의 문화적 특성은 관계적 가치, 영적 자원, 공동체 연대의 3차원으로 작용하였다. 이를 통합한 관계적 자아실현 모델을 잠정 제안하였다. 본 연구는 노인상담 이론의 발달적·임상적 확장에 기여하며 후속 척도 개발의 토대를 제공한다.
