Ultrasound-Guided Acupotomy Treatment for Frozen Shoulder: A Systematic Review and Meta-Analysis

Abstract

Objectives This study aimed to evaluate the effectiveness of ultrasound-guided acupotomy therapy for frozen shoulder by a systematic review manner. Methods Eleven domestic and international databases were systematically searched up to August 14, 2024. Randomized controlled trials (RCTs) of ultrasound-guided acupotomy therapy for frozen shoulder were selected manually according to the inclusion criteria. The risk of bias in the studies was assessed using the Cochrane risk of bias criteria risk of bias tool. Results Eight clinical studies were included. The meta-analysis showed positive results on the use of ultrasound-guided acupotomy therapy in terms of the efficacy rate, pain (measured by visual analog scale), and shoulder joint mobility (measured by Constant-Murley score), compared to conventional acupotomy (relative risk 1.23 [95% confidence interval, CI; 1.14 to 1.33, p<0.0001], standardized mean difference (SMD) -2.87 [95% CI, -4.02 to -1.71, p<0.0001], SMD 2.34 [95% CI, 0.86 to 3.82, p=0.002]). Conclusions Our systematic review found favorable results on the effectiveness of ultrasound-guided acupotomy therapy for efficacy rate, pain and shoulder joint mobility of frozen shoulder. However, evidence was limited due to the lack of well-designed RCTs. More qualified clinical trials are needed to obtain stronger evidence.
