과제기반거울치료와 과제기반치료가 만성뇌졸중 환자의 상지 운동기능과 감각, 일상생활 기능에 미치는 영향

Effect of Task-based Mirror Therapy and Task-based Therapy on Motor Function and Sensory of the Upper Extremity and Daily Living Function in Chronic Stroke Patients

초록

연구배경 : 거울치료는 손상된 상지의 움직임 수행 능력을 증가시키기 위한 시각적 고유수용성 감각의 상호작용을 위해 사용 할 수 있는 간단한 치료적인 중재이다. 과제기반거울치료와 과제기반치료가 만성뇌졸중 환자의 팔의 운동기능과 감각, 일상생활 기능에 미치는 효과를 확인하기 위해 실시하였다. 연구방법 : 대상자들은 무작위로 과제기반거울치료군과 과제기반군에 배정되었다. 과제기반거울치료군(15명)은 거울을 통해서 비마비측 사지의 반사 환영을 보면서 마비측 사지의 움직임을 유도하는 반복적이고 대칭적인 운동연습의 상지훈련을 받았다. 과제기반군은 과제를 기반한 상지훈련을 받았다. 치료는 하루 60분, 주 5일, 4주간 실시하였다. 결과 측정은 퓨글메이어 평가, 골격근긴장도, 수정된 Nottingham 감각평가, ABILHAND 설문지로 측정하였다. 연구결과 : 과제기반거울치료군은 과제기반거울치료군은 퓨글메이어 평가점수에서 전체점수와 먼쪽 부분에서유의하게 높은 점수를 보였다. 수정된 Nottingham 감각평가의 모든 항목에서 과제기반거울치료군이 과제기반군보다 유의하게 향상되었다. 골격근 긴장도와 ADL 부분에서는 두 군간 유의한 차이가 없었다. 결론 : 과제기반거울치료군은 만성뇌졸중환자의 상지의 운동기능과 감각회복에 효과적인 치료로 적용될 수 있다.
