Bone Density and Related Factors of University Students in the Seoul Area

서울지역 대학신입생의 골밀도에 미치는 영향요인에 관한 연구

Abstract

This study was conducted to investigate factors affecting the bone density of university students in the Seoul area. Data for food habits, and dietary and health-related behavior was obtained by self-administered questionnaires. BQI (bone quality index) of the subjects was measured by a Quantitative Ultrasound (QUS). The results are summarized as follows. The average height, weight, BMI and osteopenia percentage were 175.4cm, 69.3kg, 22.5 and 15.6% for male students, and 161.5cm, 55.9kg, 21.7 and 34.1%, for female students, respectively. The mean BQI of the subjects was 110.25 (range 60.7 ~ 176.8) in male students and 90.64 (range 52.9 ~ 137.5) in female students. Height and weight were significantly related with BQI in the female group but the relationship with BMI was not significantly related with bone density in either group. BQI was positively affected by nutrition supplement in the male student group. One-side eating, diet, and intake of milk and instant food were not significantly related with BQI in males or females. The results of this study revealed that desirable food habits, dietary behavior and health-related lifestyle may have a beneficial effect on bone density. There should be established a practically and systematically organized nutritional education on optimum body weight, good eating habits, weight bearing exercise and intakes of good quality nutrient for higher bone density level.

본 연구에서는 남ㆍ여 대학신입생 177명을 대상으로 골밀도를 측정하고 신체계측, 식습관, 운동 등의 요인이 골밀도에 미치는 영향을 조사하였으며 연구 결과를 요약하면 다음과 같다. 1. 조사대상자들의 평균연령은 남학생 20.1세, 여학생 20.4세이었고, BMI는 남학생 22.5, 여학생 21.7, 체지방율은 남학생 17.4%, 여학생 28.2%이었다. 2. 조사대상자들의 골밀도는 BQI값은 남학생 110.25, 여학생 90.64, T-score는 각각 0.18, -0.68, Z-score는 각각 0.38, -0.69이었다. 신체측정치와 골밀도 BQI값의 상관관계를 살펴본 결과 여학생의 경우 신장, 체중과는 유의적인 양의 상관관계(p<0.05)를 나타내었으며 남학생의 경우 유의적 상관관계가 나타나지 않았다. 3. 조사대상자의 골밀도 상태를 정상, 골감소증, 골다공증 3군으로 분류하였을 때 골밀도가 정상인 경우는 남학생 84.4%, 여학생은 65.2%이었으며, 골감소증은 남학생 15.6%, 여학생 34.1%로 여학생군이 남학생군과 비교하였을 때 골감소증이 높게 나타났으며 골다공증은 여학생군에서 1명(0.7%)이었다. 4. 조사대상자의 식습관을 조사한 결과 남ㆍ여학생간에 유의성이 나타나지 않았다. 규칙적으로 식사를 하는 경우는 남학생 55.6%, 여학생은 39.4%이었으며, 육식 위주 식사는 남학생 15.6%, 여학생 9.1%이었다. 아침을 매일 먹는 남학생은 44.4%, 여학생은 43.9%이었고, 과식한다는 응답이 남학생 37.8%, 여학생 36.4%이었다. 5. 조사대상자의 식품의 섭취빈도를 나타낸 결과 두부 및 대두 식품을 매일 섭취하는 경우는 남학생 33.3%, 여학생이 22.0%이었다. 우유 및 유제품 섭취는 매일 섭취한다고 하는 경우가 남학생 24.4%, 여학생은 15.9%이었고, 해조류를 일주일에 0~2회 섭취한다는 응답율은 남학생 62.2%, 여학생 56.8%로 나타났다. 육류 및 육가공품 섭취는 거의 섭취하지 않는 경우가 남학생 31.1%, 여학생 19.0%로 나타났으며 유의적인 차이를 나타내었다(p<0.05). 6. 조사대상자의 운동 빈도를 보면 항상 운동을 하는 경우는 남학생 33.3%, 여학생 20.5%로 나타났으며(p<0.001) 여학생의 경우 남학생에 비하여 운동량과 운동시간이 짧게 나타났다. 체중에 대한 만족도를 보면 남학생 40.0%, 여학생은 13.6%가 만족한다고 하였고(p<0.001) 체중조절의 방법에 있어서는 운동으로 한다는 응답이 남학생은 64.4%, 여학생은 43.2%로 유의적인 차이가 없었다. 7. 조사대상자의 골밀도 BQI간과 제요인과의 상관관계를 살펴보았을 때, 운동시간, 식사의 규칙성, 콩식품, 육류, 차 등의 섭취빈도, 편식과 다이어트 및 우유, 해조류, 인스턴트 식품 섭취 등의 요인과는 유의적인 상관관계를 보이지 않았으며, 영양 보충제의 경우 남학생군에서 유의적인 양의 상관관계(p<0.05)를 나타내었다. 본 연구에서 대학신입생의 골밀도에 미치는 영향 요인은 뚜렷하게 나타나지 않았다. 그러나 대학생의 시기는 여전히 골질량의 형성이 가능하므로 건강한 골격을 형성하고 유지하기 위해서 균형된 식사, 적정 체중의 유지 및 골격에 하중을 가하는 운동을 지속적으로 하는 개개인의 노력과 관심이 중요하다 하겠다.
