Current Status of Interval of Heparin Flushing for Maintenance of an Implanted Port in Solid Tumor Patients (고형암 환자의 삽입형 포트 개방성 유지를 위한 헤파린 관류 주기 현황)

  • Purpose: Little is known about appropriate interval periods between the heparin flushing of implanted ports after completion of chemotherapy. The purpose of this study was to describe the current status of interval of heparin flushing for maintenance of an implanted port in solid tumor patients. Methods: We performed a retrospective review of all patients who had undergone implanted port removal in 2012 at the Seoul St. Mary's Hospital. The subjects were 90 patients who, after completion of chemotherapy, retained their ports for extended periods of time. Results: The mean number of flushes of heparin was 4. Compliance with visits for implanted port maintenance varied with the individual, and the mean accession times were in the range between 13 days and 243 days. The overall mean time between flushes was 66 days. One patient showed resistance during flushing. Conclusion: Our results demonstrate that extending the flushing interval to a maximum of 8 weeks remains medically safe. Less frequent heparin flushing of an implanted port decreases medical expenditure and the workload of medical professionals; it also improves the patient's satisfaction.

  • Hormone replacement therapy in menopausal women has recently been a hot issue in medical fields. The author reviewed the physiology of menopause and discussed the physiological mechanism and biological management of vascular flushing and menopausal depression. The above symptoms are hard to distinguish from those of psychiatric disorders.

Surgical Treatment of Aortoiliac Arterial Occlusion: Report of 2 Cases (대동맥하단부-장골동맥의 급,만성 폐쇄성 동맥질환 2례)

  • The recent development of cardiovascular surgery as well as aortoarteriogaphy has been established excellent operative result with great aid of limb-salvage. However, less consideration or less experience still exists on the regard of vascular accident and vascular disease, as well as vascular surgery in Korea. During the last 13 years, we experienced only two cases of aorto-iliac occlusion,acute and chronic, regardless of having had more than 300 cases of mitral valvotomy and gradual increasing tendency of arteriosclerosis and hypertension in Korea. Therefore it is noteworthy to report the cases in order to promote the consideration for vascular surgery. Case 1; 52 year old female who had 20 years history of mitral stenosis with uricular fibrillation and received medical treatment for recent 1 year in the medical department. 10 days before admission, acute saddle emboli developed and 15 days after the onset, embolectomy through both common femoral arteries on the groin and abdominal approach was made. The progression of emboll to the right popliteal bifurcation was found by arteriography on operating table and retrograde flushing with heparin solution by the polyethylene catheter inserted through posterior tibial artery. The operation was successful, but 9 hours after operation sudden death occurred. Considering this case, first, mitral valvotomy already before might prevent peripheral embolizatlon, secondarily, the more early detection and surgery might also prevent the progression of emboli. Thirdly, although preoperative or postoperatlve heparinization is controversial for mitraI stenosis, heparinization might prevent additional emboli to vital organs in this case Cases 2; 66 year old female who had 4 years history of left hip and calf intermittent claudication and has had rest pain, inability to walk and ischemic necrosis on the the left leg since last 3 months prior to admission to the orthopedic department under the suspicion of herniated disc. Absence of pulsation on the groin and aortography evidenced aortoillac occlusion predominantly on the left side. Thromboendarterectomy was made and the operative result was successful with absence of claudication, healing of ulcer and aortographic patency of occlusive site. This chronic occlusion is considered to result from arteriosclerosis in origin with the evidence of moderate hypertension, x-ray evidence of calcified plaque on the aortic knob and operative finding of palpable plaques.

  • This paper is for studying stress condition of apprentice doctors who were in Oriental Medical Hospital. This study was done on 35 apprentice doctors who were in Kyungsang University Oriental Medical Hospital and Bulgy Oriental Medical Hospital. The Seven-Minute Stress Test of Thomas E. Sttats and 10 questions which were made by the present writer shows the following results.1. The Body stress scale was the highest point. In order of high percent, this shows Whole scale, Mind scale, Situation scale. 2. In comparison of the stress scale of men and women doctors, all the stress scale of women doctors were higher than men doctors. 3. In comparison of the stress scale of Interne and Resident doctors, Whole and Situation stress scale of Resident doctors were higher than Interne doctors, Body and Mind stress scale of Interne doctors were higher than Resident doctors. 4. The personal relation with other people was harmonious, the worst relation of the other colleague was the administrative staff, the next were nurses, senior apprentice doctors orderly. 5. In order of high percent of stressors, this shows personal relation, many works and troubles of care, the lack of private life, unsatisfactory administration system and equipment, the class system and overbearing atmospheres, education and continuous stress, bad conditions and fatigue, economic problems. 6. In order of high percent of systemic stress diseases, this shows musculo- skeletal disease, nervous disease, digestive disease, neurosis, urinary and genital disorder, opthalomo-otolaryngo disease, immunity disorder and vascular disease. In order of high percent of stress symptom, this show headache, fatigue, shoulder pain, back and leg pain, pantalgia and abdominal pain, diarrhea and dismenorrhea, stiffness of neck dizziness indigestion languor after a meal insomnia, neurasthenia lacking interest constipation menorrhalgia bloodshot eyes otitis media allergy thirst flushing edema. 7. In order of the seven mode of emotions in relation to stress, this shows anger, anxiety, isolation, melancholy, fright, sorrow, terror, overjoy.

A Case of Bronchopulmonary Atypical Carcinoid Tumor with Liver Metastasis (간전이를 동반한 폐기관지 비정형 카르시노이드 종양 1예)

  • Bronchial carcinoid tumors are uncommon, constituting approximately 5% of all primary lung cancers. Carcinoid tumors belong to the calss of neuroendocrine tumors that consist of cells that can store and secrete neuramines and neuropeptides. Neuroendocrine tumors of the lung include three pathologic types : a low-grade malignancy, the so-called 'typical carcinoid', a more aggressive tumor, the "atypical carcinoid", and the most aggressive malignant neoplasm, the small-cell carcinoma. Atypical carcinoid tumor have a higher malignant potential, is more commonly peripheral than is the typical carcinoid tumor. Histologic features would characterize a carcinoid as hitologically atypical : increased mitotic activity, pleomorphism and irregularity of neuclei with promonent nucleoli, hyperchromatin, and abnormal nuclear-cytoplasmic ratio, areas of increased cellularity with disorganization of architecture, and areas of tumor necrosis. Metastatic involvement of regional lymph nodes and distant organ is common. The prognosis is related to size of the tumor, typical of atypical appearance, endoluminal of extraluminal growth, vascular invasion, node metastasis, Pulmonary resection is the treatement of choice for bronchial carcinoid. We experienced one case of bronchopulmonary atypical carcinoid tumor. In the case, radiologic study showed solitary lung mass with liver metastasis and the level of 5-HIAA was elevated. There was no history of cutaneous flushing, diarrhea, valvular heart disease. The authors reported a case of bronchopulmonary atypical carcinoid tumor with review of literatures.
