간호대학생이 인지한 노인요양시설에 대한 주관성: Q 방법론적 연구 (Subjectivity toward Nursing Homes as Perceived by Undergraduate Nursing Students: Q Methodology Study)

  • Purpose: Nursing homes are a type of residential care that provide diverse services such as planned activities, convalescent care, skilled nursing and memory care. Nursing homes are good facilities for old people with dementia to improve their quality of life. On the other hand, the subjectivity toward nursing homes can vary to greater or lesser degrees. The purpose of this study was to identify the subjective attitudes and viewpoints toward nursing homes in undergraduate nursing students. Methods: The Q methodology, which is a research method to study the people's subjectivity, was used. The 40 Q-statements selected from each of 43 participants were classified into the shape of a normal distribution. The data collected were analyzed using the QUANL program. Results: Three types of subjectivity toward nursing homes emerged: 'Type I, in terms of usage of nursing homes-hesitative perspectives;', 'Type II, in terms of running of nursing homes-progressive perspectives;', and 'Type III, in terms of admission to nursing homes-constructive perspectives'. Conclusion: The findings of this study suggest that nursing intervention programs should be developed for the three types. The study results will lead to further research.

  • Purpose: The study was performed to compare the contents of jobs and job satisfaction of resident nurses at free elderly homes with nursing homes in Korea. Method: The subjects consisted of 138 resident nurses at free elderly homes and nursing homes. Data were collected from the 1st of January to the 22nd of February in 2005 using a structured questionnaire containing questions on general characteristics. contents of the nurses' daily tasks, health program. job satisfaction and pay satisfaction. Collected data were analyzed using SPSS (version 11.5) through descriptive statistics, $X^2-test$, t-test and ANOVA. Result: For nurses at free elderly homes and nursing homes. their daily tasks consisted of health assessment. medication care and eating care. The general job satisfaction of the nurses was similar between those at free elderly homes and those at nursing homes, but working conditions and pay were significantly different according to the sub content of job satisfaction. Job satisfaction was significantly different according to marital status in resident nurses at the free elderly home, and according to the experience of living with the elderly and the experience of receiving special training or lectures in elderly nursing in resident nurses at free nursing homes. Conclusion: Resident nurses at free elderly homes and nursing homes had various types of tasks, and it is necessary to differentiate tasks and payment systems for them according to their job.

한·중 대규모 양로시설의 공간 특성 비교 분석 - 한국 경기도와 중국 안양시를 중심으로 - (Comparative Analysis of the Spatial Characteristics of Nursing Homes in Korea and China - Focused on Gyeonggi Province in Korea and Anyang City in China -)

  • As Korea and China rapidly enter the super-aged societies, their demand for nursing homes will also increase sharply. The residential cultures of the two countries have distinctive characteristics, which will certainly affect the planning and use of nursing homes. Accordingly, through the comparison of the characteristics of residential spaces in Korea and China, this paper aims to obtain differences in spatial planning for nursing homes. In the residential space in Korea, public spaces such as living rooms and dining rooms are placed at the center, while private spaces are decentralized. These characteristics illustrate differences from China in the layout of the residential spaces. As a result of comparing the characteristics of private spaces and public spaces in nursing homes in both countries using the space syntax, it is analyzed that nursing homes in Korea place more importance on privacy than those in China. When compared to private spaces in Chinese nursing homes, it is determined that private spaces in Korean nursing homes display relatively more independence. Also, in Korean nursing homes, public spaces are more accessible than private spaces, and it is easier to secure privacy in private spaces than in Chinese nursing homes. However, in China, private spaces have higher accessibility than that of public spaces, with more difficulty in securing privacy.

Nursing Homes of the Elderly in Asia: A comparison of Korea and Japan

  • Purpose - The purpose of this paper was to examine the views of nursing homes for the elderly in Korea and Japan. Research design, data, and Methodology - This study aims to review the past researches on the social welfare and nursing homes for the elderly in Korea and Japan. A comprehensive view is offered with comparisons on the differences between the two countries regarding the dramatic increase of the elderly population. Results - It could be seen that with the significant increase of the elderly populations, it is getting more difficult for families to take care of their elders. Thus, nursing homes offer an alternative to busy family members in the current times. Improvements are much needed in the nursing homes for the health of the elderly. Conclusions - In order to offer better quality of life for the elderly in the nursing homes, more measures need to be taken to engage the elderly into activities. The depression that can be experienced when the elderly move to the nursing homes might be alleviated with new social circles and activities in the nursing homes, as well as human interactions with the staff.

노인요양시설의 간호서비스 질 평가 지표 개발 및 적용 (Development and Application of Nursing Service Quality Indicators in Nursing Homes)

  • Purpose: This study was designed to develop Nursing Service Quality Indicators(NSQIs) in nursing homes that would lead to an appropriate evaluation and improvement of nursing service quality. Methods: The preliminary NSQIs were developed through literature reviews and analysis of existing quality indicators. A content validity testing was done twice by using a panel of experts who were from academia and the clinical areas. The final NSQIs were confirmed and applied in three nursing homes to test feasibility. Results: The preliminary NSQIs had 4 domains and 31 indicators. Two content validity testings were performed. The indicators scoring over.80 CVI for each testing were selected and modified by experts' opinions. The final NSQIs consisted of 7 domains and 33 indicators. They were applied in three nursing homes and it was revealed that all the indicators were applicable. Conclusion: In this study, it is shown that this new 'Nursing Service Quality Indicators in Nursing Homes' is suitable for a holistic evaluation of nursing service quality of elderly patients in nursing homes. This NSQIs will be able to provide a basis for establishing nursing care standards and improving the nursing care quality in nursing homes.

Public Reporting on the Quality Ratings of Nursing Homes in the Republic of Korea

  • Background: Quality ratings could provide vital information to help people in choosing a nursing home. Purpose: This study investigated factors aligned with quality ratings of nursing homes. Methods: We employed a cross-sectional descriptive design to assess publicly available data on 1,354 nursing homes with 30 or more beds in the Republic of Korea. After excluding 289 nursing homes with no reported quality-evaluation ratings, we analyzed the 2015 data of 1,065 nursing homes. To prevent multicollinearity among independent variables, we carefully selected the final set of variables based on clinical and theoretical meaningfulness to direct nursing care. Quality, the ordinal outcome, was scored from 1 to 5 with a higher score indicating higher quality of the organization. We constructed a multivariate ordered logistic regression model. Results: Higher quality ratings of nursing homes was significantly related to the number of unoccupied beds (OR=0.99, p=.024), registered nurses (RNs) (OR=1.30, p=.003), qualified care workers (OR=1.03, p=.011), cognitive-improvement programs (OR=1.05, p=.024), and other programs for residents' activities (OR=1.09, p<.001). Conclusion: The number of RNs had the strongest influence on the publicly reported quality rating, while the rating of qualified care workers demonstrated little effect and that of nursing assistants had no effect. The number of RNs could be used as a crucial indicator for high-quality homes; more resident-engaging programs also demonstrated better quality of nursing home care.

  • With its rapid modernization and unparalleled speed of aging, South Korea faces the need for dramatic increase in its supply of nursing homes. Among the spacial components of nursing homes, residential units are considered as an essential part because the older person in nursing homes usually spend most of their time in residential units and the nursing homes are mainly composed of various residential units. It is necessary, therefore, to prepare a guideline for the plan of residential units in order to design the healthy and efficient nursing homes. This paper has first identified the concept of residential unit and analyzed 66 cases in the 62 nursing homes from 4 countries. And 4 nursing homes in Korea have been surveyed for the in-depth study. On the basis of this analysis, architectural prototypes of residential units have been proposed and the architectural characteristics of them have been compared analytically.

요양병원과 요양시설 노인의 건강상태, 사회적 지지 및 거주만족도 (Comparison of Perceived Health Status, Social Support and Residential Satisfaction in Longterm Care Hospital and Nursing Homes)

  • Purpose: The purpose of this study was to identify and compare the differences on perceived health status, Activities of Daily Livings (ADL), social support, and residential satisfaction between long-term care hospitals and nursing homes. Methods: Data was collected through questionnaires and interviews conducted from March 29 to April 22, 2011. The subjects were 66 old adults in 3 long-term care hospitals and 53 old adults in 6 nursing homes. Data were analyzed by Pearson's correlation analysis and t-tests. Results: ADL and subjective health status in nursing homes were worse than those in long-term hospitals, but it was not statistically significant (p>.05). Old adults in nursing homes received more emotional support from other residents and staff, and received more instrumental support from staff than those in long-term care hospitals (p<.001). The mean scores of resident satisfaction in long-term care hospitals and nursing homes were 3.53 ($SD={\pm}0.36$) and 3.97 ($SD={\pm}0.44$), respectively. Resident satisfaction in nursing homes significantly was higher than long-term care hospitals (p<.001). Conclusion: Health care personnels in long-term care hospitals should enhance resident satisfaction and social support and need to coordinate long-stay patients with nursing homes.

  • This study analyzed some of nursing homes in various domestic and foreign regions in order to find solutions to reduce suicidal incidents from the loneliness. To analyze community space of nursing homes, elderly community is analyzed into three categories based on their characteristics (morphological characteristics, Social characteristics, Environmental characteristics). With those three characteristics above, architectural spaces are analyzed the relation with them. Elderly nursing homes are divided with living, nursing, management, and public spaces. The space of elderly nursing homes (classification of function)-elderly community (classification of elderly characteristics) is approached in an architectural way related to the space of elderly nursing homes and elderly characteristics. Through some case studies, specific space relation of elderly nursing homes and elderly community is analyzed. Also, with the building analysis for summary, plans, wards(hospital rooms), cafeteria, program rooms and alley, some of general information and elderly community space is understood. In this study, there will be an analysis of relation of elderly community(morphological, social, environmental characteristics) which is separated into some spaces of elderly nursing homes(living, nursing, public spaces and facilities) written above. Furthermore, if people can recognize the importance of community space for elderly facilities, there will be a new guide line for that which can have a play on catalyst.

한국 노인요양시설 실무종사자들이 인식하는 환자안전문화와 환자안전도 (Perception of Workers on Patient Safety Culture and Degree of Patient Safety in Nursing Homes in Korea)

  • Purpose: The purpose of this study was to investigate provide basic data for improving patient safety in nursing homes in Korea by measuring the patient safety culture of nursing homes and understanding its influencing factors. Methods: This study is a secondary analysis of a descriptive research study using data from development and validation of the Korean patient safety culture scale for nursing homes. A total of 982 cases were analyzed using the SPSS Statistics 20 program. Results: For the safety culture of the patient, there was a significant difference based on the size and location of the facility. For the degree of patient safety, age, education, occupation, marital status, and the size of the nursing home were significant factors. Patient safety culture and the degree of patient safety had a positive correlation. The regression model of the degree of patient safety was significant (F=20.73, p<.001) and the explanatory power of the model was 27.4%. Conclusion: The study results indicate that patient safety culture is a factor influencing safety of elders in nursing homes. To improve patient safety for nursing homes in Korea, continuous evaluation and improvement projects need to be done at a national level.
