Effects of Horticultural Occupational Therapy on the Physical and Psychological Rehabilitation of Patients with Hemiplegia after Stroke (반신마비 뇌졸중 환자의 심리와 정서적 재활에 대한 원예작업치료 효과)

  • To examine the effects of horticultural occupational therapy (HOT) on the physical and psychological rehabilitation of stroke patients with paralysis on one side of the body, a horticultural therapy (HT) program was implemented along with occupational therapy (OT) in 20 patients with hemiplegia after stroke (treatment group). In the control group, another 20 patients with hemiplegia after stroke received OT but no HT. The HOT program consisted of various indoor horticultural activities that proceeded stepwise on a weekly basis over the course of four weeks. The selected horticultural occupations were organized into four-phases: motivation, adaptation, sociality, and interpersonal relationships and communication. The grooved pegboard test (GPT), geriatric depression scale (GDS), and functional independent measure (FIM) were tested to the patients in this study. Unlike control group, the treatment group showed statistically significant differences in GPT, GDS, and FIM ($p$<0.001). In addition, communication, social cognition, and self-care scores as FIM subordinate factors were improved significantly by HOT program which motivated patients to engage in rehabilitation therapy. This finding suggests that HOT has the potential to be used as an OT program for stroke patients with hemiplegia.

Horticultural Activity Interventions and Outcomes: A Review

  • The objective of the present study was to review the current research about the outcomes of horticultural activity interventions in order to determine research trends and analyze the characteristics of horticultural activity interventions. We reviewed the literature using search engines such as the Web of Science, Proquest Dissertation and Theses, Academic Search Premier, Research Information sharing Service, and Korean National Assembly Digital Library to locate journal articles that include relevant search terms (e.g., gardening activity, gardening program, allotment garden, urban agriculture, horticultural therapy, and horticultural activity). We initially identified 14,414 journal articles that were published before April 2014 and selected 509 of the papers for review. We found that studies investigating treating children and adults 8 to 64 years old were the most common, and horticultural activities such as gardening outdoors, planting indoors, making crafts with live plants, arranging flowers, making crafts with artificial or pressed flowers, and other activities were usually combined. Short/medium term (11 to 20 sessions) horticultural activity programs were the most frequent, and most interventions were of medium duration (> 60 min to 120 min). Most of the studies focused on the psychological or emotional effects of horticultural intervention, such as its effects on emotional intelligence, self-esteem, stress, and depression. Further studies are needed to analyze the research methodology, specific outcomes, and strengths or weaknesses of studies investigating horticultural activity interventions.

Analysis of Horticultural Therapy Programs for the Mentally Disabled (정신적 장애인을 위한 원예치료 프로그램 분석)

  • In order to analyze the horticultural-therapy program, which was carried out targeting the mentally disabled, relevant 559 copies of 'Confirmation Note of horticultural activity' submitted for to be used the use in license examination to Korean Horticultural Therapy and Wellbeing Association were used as a tool. It contains 64 horticultural therapists for level 1 and 524 horticultural therapists for level 2 from May in 2000 to February in 2008. With the aim of examining difference depending on people covered by the program, license kind and horticultural therapy activity period in horticulture therapists, ${\chi}^2$ test was conducted on the basis of frequency in each. Data was analyzed by using SPSS (Statistical Package for the Social Science) Win 13.0 program, which was carried out targeting the mentally disabled, the 'art and craft activity' was the largest with 46.0%. In terms of 'growing activity', the 'normal growing' showed the highest ratio with 74.7%. In the 'art and craft activity', the 'flower decoration' showed the highest ratio with 37.5%. In the result of 'Cooking activity', the activity related to 'tea' was the largest ratio with 33.6%. As a result of 'learning activity', 'orientation' was the largest ratio with 47.6%. In the 'outdoor activity', 'excursion' was the largest ratio with 36.7%.

Effects of Horticultural Therapy Programs on Depression and Self-esteem in Released-Convicts

  • This study carried out to identify effect of horticultural therapy programs (HTP) on the changes of depression and self-esteem in released-convicts. HTP were conducted once a week total 4 times (each time 60 minutes) from 7 November to 28, 2016 on 13 released-convicts coming Korea rehabilitation agency Gwangju Jeonnam branch. Measurement methods were used scale of depression, scale of self-esteem, Form for evaluation of HTP. As a result, the mean value of depression was decreased to 11.2 after HTP compared with 17.4 before HTP and there was statistically significant difference (p=.013). And HTP in grade of self esteem was increased to 16.1 after HTP compared with 12.1 before HTP and there was statistically significant difference (p=.001). The result of this study indicated that the horticultural therapy programs has positive effects on depression and self-esteem in released-convicts.

  • As a result of inquiring into the analysis consequent on a symptom by research subject, it was found that there were 1 paper (1.5%) which did research for the purpose of rehabilitation of the general public, 26 papers (38.8%) targeting diagnosis-related groups (DRG), and 40 papers (59.7%) targeting social consideration subject. Also, as a result of the inquiry about the frequency & number of times of horticultural therapy program implementation, it was found that 49 papers (73.1%) implemented one time a week, and 45 papers (67.2%) were surveyed as the highest by conducting a total of 11~20 sessions of horticultural therapy programs, In the analysis of horticultural therapy activities by type, plant cultivation activity was found to be the most 506 times, accounting for 41.5%, followed by 297 time crafts activity (24.4%), 213 time floral decoration activity (17.5%), and 203time others activities (16.7%). In cultivation activity, soil-using cultivation activity (25.1%) was found to be lower than the proportion (74.9%) of soiless cultivation (16.4%), crafts activity (24.4%), floral decoration activity (17.5%) and other activities (16.7%). The most used plants in a restricted place like a hospital were found to be in the order of Hedera helix, Chamaedorea elegans, Succulent plant, Syngonium podophyllum, Neofinetia falcata HU, Hoya carnosa (L.f.) R.Br.), Rosmarinus officinalis, and Spathiphyllum spp.

The Effects of Horticultural Activity with Reminiscence Materials and Singing Time on the Ability to Recall Words and Depression in the Elderly with Mild Dementia

  • The purpose of this study was to investigate the effects of horticultural activities with reminiscence materials and singing songs on the ability to recall words and depression in the elderly with mild dementia. A total of 20 sessions of a horticultural therapy program was held once or twice a week from July 14, 2015 to November 3, 2015 for the elderly with mind dementia at the social welfare center in D city. The subjects of this study were 13 women and the mean age was 81.2 years. The ability to recall words significantly improved from $8.6{\pm}1.8$ before the program to $9.4{\pm}0.6$ after the program (p=.012). However, there was no significant difference in the score of depression after the program. Synergistic effects were generated by applying horticultural activities, singing and reminiscence therapy activities simultaneously, and it was effective in recalling memories. As emotional functions were associated with hormones in vivo, there were emotional ups and downs at each session and short-term alternative therapies were not sufficient to completely eliminate neurotoxic substances caused by dementia.

Development of Elements of Horticultural Therapy Evaluation Indices (HTEI) through Delphi Method (전문가 델파이 조사를 통한 원예치료 평가지표 구성요소 개발)

  • The aim of this study is to develop and construct the elements of a new evaluation tool to assess the effectiveness of a horticultural therapy. Delphi method was applied to classify the realms of evaluation and specify their components and constituting items. Delphi process was performed through three rounds and 24 professionals attended all the rounds as panel members. Some of the items were revised or removed according to the results of such tests as content validity ratio, agreement rate, convergency and stability of each item. On the contrary, a few new items were added by accepting the opinions of the professional panels. Previous to the Delphi, the realms of evaluation were classified into physical, cognitional, psycho-emotional and social ones on the basis of literature review, professionals' consultation and in-depth interview with practitioners. At the first round, 112 items constituting four realms were reviewed. The numbers of items to be reviewed were reduced to 107 at the second round and to 102 at the third round. As a result, 98 items in 4 realms were decided as proper evaluation points of horticultural therapy. The compositions of the four respective realms are as follows: 5 components and 27 items in physical realm; 4 components and 25 items in cognitional realm; 2 components and 24 items in psycho-emotional realm; and 4 components and 22 items in social realm. These realms, components and specific items derived from this study constitute indices to evaluate the effectiveness of horticultural therapy. In addition, they might be utilized as good guidelines for planning horticultural therapy programs.

A Study on the Effectiveness of Horticultural Therapy on the Emotional Stability of Children From Broken Homes (원예심리치료가 결손가정아동의 정서적 안정에 미치는 효과성에 관한 연구)

  • The purpose of this study is to analyze the effectiveness of horticultural therapy on children from broken homes: this treatment program, implemented in the social welfare field, targets the children's depression, self-esteem, and social and emotional stress. Its object was composed of 13 lower grade students enrolled in special facilities according to the life-cycle method, and its data were collected as making use of the structured questionnaire. The horticultural Therapy Program was practiced 14 sessions in total from September 2nd to December 16th, 2010, and its results in summary are as follows; The sociality in horticultural therapy was higher by .53 point in 2.76 after practice than in 2.23 before practice, and its value of the ability to cope with stress was higher by 0.52 point in 2.82 after coping with stress than in 2.30 before that. That was meaningful (t=-1.4398, p<.05). The value of anxiety was lower by .15 in 1.45 post-anxiety than in 1.60 pre-anxiety, and that was meaningful (t=-2.3842, p<.05). The value of self-esteem is higher by .42 in 2.92 post-self-esteem than in 2.50 pre-self-esteem, and that was strategically meaningful (t=-1.1871, p<.05). The value of emotional assessment was higher by 2.05 in 5.25 post-emotional assessment than in 3.20 pre-emotional assessment, and that was strategically meaningful (t=-2.4599, p<.05). Based on these results, this study suggests that this horticultural therapy program should apply to the issues of children in social welfare field as one of various approaches.

The Effect of Horticulture Therapy Program for Behavior Problems and Subjective Mood and Energy of Childhood Cancer Patients

  • This study was conducted to apply a horticultural therapy program to childhood cancer patients, to analyze their psychosocial changes and thus to examine its possibility as a way to manage the recovery of childhood cancer patients. This study was conducted on patients who were diagnosed with childhood cancer, and they were divided into the experimental group (8 patients who visited H association offices located in 3 cities and participated in the horticultural program) and the control group (5 patients who did not participate in the program). The homogeneity between the two groups before conducting the program was compared, and there was no statistically significant difference, which proved that the two groups were homogeneous. After conducting the program, the score of social immaturity, one of the sub-categories of problem behaviors, of the experimental group was statistically significantly lower than that of the control group (p=.019). Changes before and after the program within the groups were compared, and the somatization score (p=.039) of the experimental group (p=.039) after the program was 1.13, down from 2.13 before the program. The social immaturity score (p=.017) of the experimental group after the program was 1.00, down from 2.63 before the program, and the attention problem score (p=.026) of the group after the program was 1.25, down from 2.50 before the program. The total problem behavior score (p=.034) of the experimental group after the program was 9.63, down from 18.50 before the program, showing a statistically significant decrease. The cumulative subjective mood score (p=.000) of the experimental group throughout the entire sessions after the program was 7.82, up from 6.99 before the program, showing a statistically significant increase. Therefore, it can be concluded that the horticultural therapy program developed based on the 'relaxation-social support-cognitive behavior' strategy was effective to reduce the negative psychosocial state of childhood cancer patients.
