An Empirical Study of User Perceptions on EMR Standardization Leading Medical & IT Convergence (의료·IT융합을 이끄는 EMR 표준화에 대한 이용자 인식 연구)

  • Electronic Medical Record(EMR) is medical record that has been saved electronically onto a computer. The standardization activities for EMR is actively underway as it may not only improve the overall quality of the medical services but as the value of medical big data (medical & IT convergence area) is being considered very important. One of the most important issues is ensuring the necessary and effectiveness of EMR standardization to the stakeholder. Researchers did an empirical study to find out how the doctors perceived the EMR standardization from both technical and economical perspective. The results of the empirical analyses showed that system quality and an economical value had a positive effect on perceived usefulness and intention to adopt EMR standardization, yet interoperability have only affected the perceived usefulness. Additionally, the economical value seemed to be the most important variable in forming a consensus in the need of EMR standardization.

Study for Improvement of the Doctor's Satisfaction and Completeness of the Medical Record in the EMR System (전자의무기록(EMR) 시스템하에서 의사의 만족도와 의무기록정보의 기재 충실도 향상 방안)

  • This study aims to present ways to enhance the stabilization of electronic medical records, ensure the commitment to filling in information of the medical record and improve the overall quality Electronic Medical Record(EMR) information. For that purpose, the present state of the incomplete record rate and the doctor's satisfaction in Electronic Medical Record(EMR) have been surveyed by comparing and analyzing Paper-based Medical Record(PMR) and Electronic Medical Record(EMR). The survey was conducted on 31 doctors in charge of EMR system and each PMR and EMR inpatients were collected for a period of 5 months and analyzed. The results showed that the doctor's satisfaction level was higher for EMR, and the rate of incomplete record appeared to be lower in EMR in departments of both internal and external medicine. In this context, it can be said that the higher efficiency of EMR helped accomplish the increase in commitment to completing medical record information and improve the quality of the data.

  • 전자의무기록(EMR)[1]을 도입한 이후 현재 의료장비의 EMR 연동상황이 극히 미비한 상태로 대두되고 있다. 종합병원 의료장비의 약 30%정도는 검사 결과물이 출력이 안되고 보여지기만 하는 구조로 되어 있어 직접 수기로 EMR 에 연동할 수 있는 소프트웨어에 입력하여 EMR 에 연동하고 있으며, 약 60%는 의료장비에서 프린터 혹은 시리얼통신으로 데이터를 출력할 수 있는 구조로 되어있다. 하지만 의료장비의 노후화 및 검사결과물의 인터페이스 특성이 다양하기 때문에 쉽게 연동하지 못하고 있으며, 대부분 종이 출력 결과물을 스캔을 통해 이미지를 저장하거나 받아서 EMR 에 연동하고 있다. 그 외 나머지 10%는 의료장비 자체의 저장장치 또는 네트워크를 통해 검사결과 데이터를 EMR 에 전송하는 구조로[2] 되어있다. 본 논문에서는 의료장비의 프린터를 통한 결과물을 EMR 에 연동하기 위한 인터페이스 솔루션을 구현하고, 검사결과의 출력 데이터를 이미지 복원 및 관리하는 방법을 통해 많은 수의 의료장비의 출력 데이터를 EMR 연동을 하여 인력 및 기타 사무용품의 소모를 줄이며, 검사결과를 실시간 진단할 수 있는 방법을 제시한다.

  • 다양한 의료서비스를 정형화되고 효율적인 방법으로 제공하기 위하여 최근 의료기관의 EMR(Electronic Medical Record; 전자의무기록) 시스템의 도입이 급속도로 확산되고 있다. 그러나 국내에서 구축되고 있는 EMR은 초기에 기대했던 EMR의 도입목적을 중분하게 달성하지 못하고 있다. 이는 근본적으로 EMR의 방향성과 구조에 한계를 지니고 있기 때문이다. 국내에서 구축되고 있는 대부분의 전자의무기록시스템은 자료수집원 지향 의무기록 (Source Oriented Medical Record; SOMR) 방식으로 개별 사실을 수집하고 제시하기 위한 관찰자 중심, 서식중심의 시스템이며 차트 이송과 종이를 없애는 것이 주목적이다. SOMR 방식은 EMR의 다양한 도입목적, 특히 임상자료의 효과적 활용을 충족시킬 수 없는 태생적인 한계를 지니고 있기 때문에 문제지향 의무기록 (Problem Oriented Medical Record; POMR) 방식의 EMR과 적절한 혼용전략 (Hybrid Strategy)이 절실히 요구된다. 본 연구의 목적은 SOMR 시스템과 POMR 시스템이 보완적으로 결합된 하이브리드 EMR 아키텍처를 제시하고 아키텍처를 구성하는 데 필요한 기술적 요소들과 그들간의 상호관계 틀 파악한 후 이를 기반으로 개발방법론을 도출하는 것이다. 또한 만성신부전증 환자 사례를 이용하여 앞서 제시된 개발방법론에 따라 프로토타입을 개발하였다. 본 연구는 EMR 시스템이 SOMR 구조에만 편향됨으로써 발생될 수 있는 다양한 문제들을 해결함으로써 진료의 품질 항상, 임상연구의 활성화, 조직적이고 과학적인 진료계획의 수립과 같은 소외되어 왔던 EMR의 목표를 실현할 수 있을 것으로 기대 된다.

  • This study was conducted to arrange the basic data for the ways to operate the effective nursing programs and reduce job stress by grasping the awareness, satisfaction and job stress of the nurses due to the introduction of EMR(EMR : Electronic Medical Record) system and clarifying the relationship among them. Methods: Of the hospitals which introduced EMR, the study was conducted for 356 nurses who used EMR in a university hospital in D city and the data was collected from June 1 to June 30. The collected data was analyzed with real number, percentage, T-test, ANOVA, and Pearson correlation coefficient. Results: The results of the study above, it could be certified that the higher the satisfaction and awareness of the users of EMR were, the more the job stress of them decreased. Therefore, the qualitative nursing should be provided to the patients by reducing job stress with the improvement of the awareness and satisfaction of the nurses of EMR, and shortening the time in keeping the records of patients, in order to enhance the satisfaction of EMR, there should be a proper management, such as a regular EMR education, and there must be the ways to reduce the job stress of the nurses and strengthen the satisfaction of EMR.

The Effect of Mobile EMR's Technological Characters on System Qualities and User Satisfaction (모바일 전자의무기록(EMR)의 기술적 특성이 시스템 품질과 사용자 만족에 미치는 영향)

  • We empirically show that the effect of mobile EMR's technological characters on system qualities and user satisfaction. For the study, the relationship among technological characters of mobile, EMR's system quality, EMR's information quality, and user satisfaction were modelled and validated with hypotheses. An empirical test was performed for a path model using structural equation modeling on samples of 148 nurses in hospitals from various size and region. This study find that, for the overall sample, there are positive relationships among the technological characters of mobile, EMR's system quality, EMR's information quality, and user satisfaction. And the results support that mobile is an effective method for EMR. Findings of this study have several theoretical and practical implications as follows : First, this study empirically tested the relationship among technological characters of mobile, EMR's system quality, EMR's information quality. Second, the study shows that the technological characters of mobile should be considered for advance of EMR system's quality in mobile environment. And lastly, this study can practically suggest mobile healthcare information system for process effectiveness.

  • The purpose of this study is to find out the most successful way for the protection of medical information focusing on the electronic medical record(EMR). In this study, every aspect of the EMR is reviewed in terms of the hospital management. In particular, definitions, major functions, strengths and weaknesses of the EMR are considered. This study also examines the general development of the EMR as well as the current situation of applying the EMR.   • EMR(Electronic Medical Record) is being broadly used in general medical institution, but it could be more efficient and convenient if patients could use it themselves. Because present EMR is the formula written by medical experts with professional words, the patient can not identify his detailed symptoms and even the name of disease. Otherwise, the patient should have many efforts for obtaining his medical records. To solve this problem, this study developed Patient-Accessible EMR system, which was founded as one of patient-centric medical services, and it shows that the patient can take his medical information without medical experts.

  • The purpose of the present study is to theoretically assess IT Implementation Model of Cooper and Zmud (1990) in a hospital IS use context. A case study was applied to analogical study by interview from several end-users of the information systems at a university hospital. This study presented an EMR(Electronic Medical Record) systems how is initially implemented at an initial stage, continually adopted, adapted, accepted at an adoption stage, and finally rountinized and infused into an organization. Our study also elaborated IT Implementation Model as defining EMR development and its impact on nature of IS use in a hospital. This case study explained the characteristics of EMR and hospital organization context conceptually.

Regularity and coupling correlation between acoustic emission and electromagnetic radiation during rock heating process

  • Real-time characterization of the rock thermal deformation and fracture process provides guidance for detecting and evaluating thermal stability of rocks. In this paper, time -frequency characteristics of acoustic emission (AE) and electromagnetic radiation (EMR) signals were studied by conducting experiments during rock continuous heating. The coupling correlation between AE and EMR during rock thermal deformation and failure was analyzed, and the microcosmic mechanism of AE and EMR was theoretically analyzed. During rock continuous heating process, rocks simultaneously produce significant AE and EMR signals. These AE and EMR signals are, however, not completely synchronized, with the AE signals showing obvious fluctuation and the EMR signals increasing gradually. The sliding friction between the cracks is the main mechanism of EMR during the rock thermal deformation and fracture, and the AE is produced while the thermal cracks expanding. Both the EMR and AE monitoring methods can be applied to evaluate the thermal stability of rock in underground mines, although the mechanisms by which these signals generated are different.
