Review of Patient Satisfaction with Case Management (사례관리 만족도 측정에 관한 고찰)

  • 연구 목적: 사례 관리(CM) 환자 만족도 측정에 관한 연구 결과를 고찰하여 사례관리 실무에서 활용할 수 있는 근거를 제공하고자 한다. 연구 방법: MedLine, CINAHL에서 주요용어로 case management, care management, 주제어로 patient satisfaction, client satisfaction을 이용해 문헌 검색을 실시하였다. 연구 결과: 만족도에 대한 개념은 관련 연구가 매우 부족하며, 서로 다른 이론적 접근을 하고 있으므로 그 측정에 어려움이 있다. 신뢰할 만한 만족도 측정을 위해서는 설문 문항을 서술하는 여러 가지 방법들, 측정 척도의 종류, 만족도에 영향을 미치는 혼란변수들을 주의 깊게 고려하여야 한다. 만족도는 사례관리 연구에서 흔히 측정되는 성과이지만 대부분의 관련 연구에서 사례관리의 특징이 반영된 측정 도구를 사용하지 않고 있었다. 따라서 본 논문은 다섯 개의 사례관리 만족도 측정 도구를 제시하고 그 특징을 소개하였다. 또한, 간호성과분류체계(The Nursing Outcomes Classification)에 제시된 만족도 측정항목들 중 사례관리와 관련이 높은 약 40%의 항목들을 발견하였다. 결론: 사례관리 만족도 측정 도구가 매우 부족하므로 표준 사례관리 측정도구 개발이 시급하다. 한편 사례관리사들은 만족도 측정에 대한 이해를 높이고 기존 연구 결과를 활용함으로써 보다 정확한 사례관리 만족도 평가가 이뤄지도록 노력해야 한다.

Client Experience Using Care Management - How to Access The Phenomenological Study of Giorgi - (사례관리를 이용한 클라이언트의 경험 연구 - Giorgi 현상학적 연구 접근 -)

  • This study aims at introducing the subjective experiences of the users who have been involved in care management, and finding out the meaning and essence of services. Total 16 interviewees who have experienced case management participated in a focus interview. The analysis of raw data brought about total 408 meaning units. Then, through restructuring and summarizing, 122 units of central meaning were revealed. After classifying 23 topics were found. Then, after consistent phenomenological comparison among the 23 topics, 7 essential topics were discovered. Based on the results and discussion above, this Study would like to suggest : approach to integrated care management of family, the meaning of social worker as the partner of life, the object of care management is the change from isolated being to understand the each other.

A Study on the Development of Case Management Program for Arthralgia in Customized Visiting Health Care (맞춤형 방문건강관리사업에서의 관절통증 사례관리 프로그램 개발 연구)

  • 본 연구는 2008년 맞춤형 방문건강관리사업에 사용되고 있는 관절통증을 중심으로 한 사례관리를 수정 보안하여 우리나라 실정에 맞는 관절통증 사례관리 프로그램을 개발, 제안하며, 맞춤형 방문건강 관리사업의 활성화와 완성도를 높이는데 있다. 연구방법으로는 2007년 전국 12주 관절통증 사례관리 결과자료 분석하고, 전국 253개 보건소의 맞춤형 방문건강관리사업 인력에 대한 자료 분석과 전국 보건소 전문가 자문회의와 토론 결과를 통해 설문지를 수정 보완하여 2008년도 충청남도 관절통증 12주 사례관리를 실시하였다. 자료분석은 SPSS 12.0 통계 프로그램을 이용하여, p-value가 0.05 미만과 0.01미만인 경우를 통계적으로 유의한 것으로 판정하였으며, 전국자료는 빈도분석, wilcoxon 부호순위 검정과 McNemar's 검정을 실시하였으며, 12주의 관절통증 사례관리의 연구기간동안 수집된 자료를 1주와 8주간, 1주와 12주간, 8주와 12주간을 paired t-test 검정과 McNemar's 검정을 실시하여 유의성 평가를 실시하였다. 연구결과는 다음과 같다. 12주 기간 동안 사전 사례관리 방문간호사의 교육을 통한 사례관리 서비스의 강도의 조절 및 매주로 서비스의 횟수를 조절하여 사례관리를 실시한 결과 총 109개 항목에서 1주와 8주간에 유의한 항목은 TG(mg/dl)를 비롯한 51개 항목, 1주와 12주간에는 콜레스테롤(mg/dl)을 비롯한 53개 항목, 8주와 12주간에는 지난 48시간동안 관절통증 점수를 비롯한 3개 항목으로 유의한 차이를 볼 수 있었으며, 1주와 8주간은 유의하나 1주와 12주간은 유의하지 않게 나타나는 항목은 TG(mg/dl)를 비롯한 3개 항목, 1주와 8주간은 유의하지 않다가 1주와 12주간은 유의하게 나타나는 항목은 콜레스테롤(mg/dl)를 비롯한 6개 항목, 1주, 8주, 12주간의 모든 기간에서 유의한 항목은 지난 48 시간동안 관절통증 점수를 비롯한 3개 항목으로 조사되었다. 결론적으로 현재 우리나라에서 추진되고 있는 맞춤형 방문건강관리 사업의 사업지침에 대한 보완을 위해 관절통증사례관리 프로그램에 있어 중재 서비스 또는 프로그램의 기간은 12주간에서 8주간으로 조정 되어야 하며, 추가가 필요한 항목으로는 교육, 자기역량 강화, 운동처방, 물리치료, 약물치료, 대체요법, 식이, 영양, 생활지도 등이며, 어골도 분석을 위한 기본 틀 및 주요 구성요소를 제시 및 기여 요인 및 결정요인을 위한 논리적 모형 제시가 필요하며, 개선목표를 위한 유지증진 및 관리능력, 지기 관리 수행도 개선과 대상자별 맞춤형 사례관리를 위한 표준화된 행동 체크리스트 제작 보급 및 사례별 운동, 물리치료 지도 방법 계획 수립에 대한 인력 충원이 필요하다.

A Study on the Effectiveness of Team Approach In Case Management Service for the Elderly in Puchon : A Pretest-Posttest Study (재가 노인을 위한 사례관리 서비스의 효과성 연구 - 팀접근 모델을 중심으로 -)

  • The outcome of study has shown that team approach in case management in the community welfare center has shown a fair amount of effectiveness in increasing the quality of life for the elderly. 30 elderly people who had complex difficulties in life were selected and given case management for a 6 month period. The student volunteers supervised by a case manager visited the clients once a week and arranged a series of community resources following assessment of the clients' needs. A group of professionals active in the community was invited for supervision. Compared to the clients in the comparison group, those clients who case management showed the increase in the quality of life and life satisfaction scale. The results of the study suggest that face-to-face service for the elderly is one important factor for successful case management intervention. Also, a team approach in case management utilizing professionals and volunteers can be useful for the case manager in the community welfare center who is overwhelmed with too many case loads.

The Genealogical Analysis of Case Management in the Modern Society (Foucault의 계보학을 활용한 사례관리에 대한 비판적 고찰)

  • The main focus of the study is to find out the origin of the case management in social work and relevant background and context around it, by using the method of genealogy, an historical framework or perspective developed by Foucault, French social philosopher. The reasons of the study are two fold : First, why the case management can be established as a major strategy of social work field in such a very short time. Second, since the case management has been accepted, why it has been reported ceaselessly that a lot of case managers have experienced the severe stress and sufferings in carrying out the case management practice. The results of the study shows that the case management is partly a very social device needed in the late 20th century to manage the social work resources and manpower efficiently, which might be against the genuine social work's mission and idealism. In a conclusion, to retrieve the genuine identities of social work profession, not only the practice case management should be cautious to tendency of management and control inherited in it but also be balanced by the traditional ideals of human emancipation and liberation.

Development of Case Management Activities Inventory in Community Welfare Centers (사회복지관 사례관리 활동목록 개발)

  • The purpose of this study is to develop case management activities inventory and analyze the perception on case management practice using importance-performance analysis(IPA) in community welfare centers. The comprehensive literature review and the content analysis from the professional scholar and social workers produced 89 items of case management activities items. A survey was conducted to 277 case managers in community welfare centers. The exploratory factor analysis, reliability and validity analysis developed the case management activities inventory comprised of 11 factors and 61 items. Suggestions were made to improve case management practice in community welfare centers.

  • This study aims to develop the case management activities inventory relevant to community-based mental health settings. The comprehensive literature review was conducted to define the concepts and activities for the case management process. After the content analysis from the professional scholar and the mental health-related worker, 83 items were constructed. A survey was conducted to 265 case managers in community mental health centers. Through exploratory factor analysis, reliability and validity analysis, finally 8 factors and 43 items were identified.

  • The purpose of this study was to confirm the effect by case management program and durability of the effect, which targeted on the 250(101 men and 149 women) people who had undergone hypertensive patients case management program through the National Health Insurance Corporation from May 1. 2007 to June 30. 2007. In order to evaluate the effect of case management and durability, the study was examined just before the case management, right after case management, after 6 months and after 12 months through classifying case management 1(Group I) and case management 2(Group II). As a result, the participants' blood pressure level in the hypertensive patients case management improved just after the case management compare to just before one. but it had a tendency to increase once more after 6 months and 12 months case management. In addition, it was confirmed to maintain the effect by the case management in Group II compare to Group I.

Effectiveness of Medical Aid Case Management in Excessive Healthcare User by Interventions based on the Number of Accesss (의료급여 과다이용자의 중재방법별 접근횟수에 따른 사례관리 효과)

  • This study analyzed medical aid case management effects based on the number of access by visits, phone calls, letters, internal investigation, resource links, and requests, in medical aid case management business that is carried out to improve the quality of life of medical care beneficiaries and reduce financial cost. Secondary analysis study using data of 564 high-risk group in the medical aid case management and their healthcare utilization data. Letters had positive correlations with the scores of all case management domains. The higher visits was, the lower the score of self-health care ability was, and the higher the number of phone calls was, the higher the score for reasonable medical use was. While there was no significant difference in medical cost according to aid management by interventions, the higher visits and resource links were, the lower the subjects' total number of payment days was. There is a difference in the various areas of the quality of health -related life and medical use depending on case management by intervention method and its number. It is necessary to carry out the efficient number of access to case management by intervention method.
